The effect of smoking and drinking habit on the health status of lead workers (흡연과 음주가 연취급 근로자들의 건강수준에 미치는 영향)

  • To investigate the effect of smoking and drinking habit on the health status in lead using industries, 2,785 male workers in lead using industries (7 storage battery industries, 7 secondary smelting and related industries, and 4 primary metal and other manufacturing industries) were selected for this study. This study was carried out as a part of periodic health examination. Selected study variables were zinc protoporphyrin in whole blood (ZPP), SGOT and SGPT for laboratory test. Questionnaire for lead related symptoms and smoking and drinking habit was provided to all the workers and filled up by themselves and reconfirmed by physician. The results obtained were as follows; 1. The overall smoking and drinking rate of study population were 69.8% and 73.6%, respectively. While the smoking and drinking rate of storage battery workers were 68.8% and 72.3%, those of secondary smelting industries and other industries were 66.0% & 66.4% and 74.6 & 80.3% respectively. 2. While the mean values of blood ZPP of lead exposed workers were significantly higher than other group, those of SGOT of storage battery workers were significant higher than other worker. But there were no differences of mean values of other variables. 3. Smoking habit did not affect on the mean value of blood ZPP of workers in special health examination group, but there were significant differences of blood ZPP and SGOT between drinker and non-drinker. 4. Symptom prevalence of lead exposure were higher in drinking and smoking group than non-drinking and non-smoking group. 5. In multiple regression analysis of the total lead related symptoms, blood ZPP, SGOT, and SGPT as dependent variable, respectively, and age, work duration, blood ZPP, pack year and amount of alcohol drinking as independent variables, work duration, pack year, amount of alcohol drinking, age contributed to total symptoms; and age, work duration, pack year contributed to blood ZPP; and age, amount of alcohol drinking, work duration contributed to SGOT; and pack year contributed to SGPT.

Noise Exposure Levels of Workplaces Exposed to Noise and Rate of Exceedance of Exposure Limits (소음 노출 사업장의 소음 노출수준과 노출기준 초과율 현황)

  • Objectives: The purpose of this study was to analyze noise exposure levels and the rate of exceedance of exposure limits in workplaces from a 2015 measurement of working environments according to area, industry, and scale of workplace and to determine changes compared to the past. Methods: Among the 408,875 measurements of noise in working environments from 27,030 workplaces in 2015, 16,359 workplaces that were linked to special health examination data were selected as the subjects of this study. The eight-hour corrected measurements and geometric mean values of the individual noise measurements of the workplaces were used to calculate noise exposure levels and the exceedance rate of exposure limits. Results: The average noise exposure level of the overall workplaces making up the subjects of this study was 83.6 dBA, and the exceedance rate of exposure limits was 15.1%. At least half of the noise measurements exceeded the exposure limits in 13.7% of the workplaces. Noise exposure levels were higher in the manufacturing industry and in smaller-scale workplaces. The exceedance rate of noise exposure limits was higher in the mining and manufacturing industries and in smaller-scale workplaces. Conclusions: Noise exposure has shown improvements compared to the past, but the exceedance rate of exposure limits was still high, and more than half of the workers were being exposed to noise of 85 dBA or higher. Therefore, it is necessary to make more active improvements in working environments in terms of noise exposure.

  • Being a medium to help understand the status of the whole body including the Five Vital Organs and the Six Viscera such as the heart, the mac is considered to represent the elasticity of the blood vessel and the condition of the bloodstream in the Oriental medicine. A doctor makes a subjective judgment on a patient's conditions based on the information derived from the diagnosis and the medical examination of the mac wave. Thus he or she needs objective records and make a decision based on them in order to prevent misdiagnosis. This paper set out to suggest the ways to detect the particular parameters of the mac wave with the methods to visualize the mac wave interpretation to detect basic documents(causes) of health status judgment in three-dimension and with the simulation.

Recommendations of the Korean Society for Health Education and Promotion for Developing the Korean Credentialing Policy of Health Education Specialist (보건교육사 제도정립의 방향)

  • Objectives: This research was conducted to suggest a recommendation for the Korean credentialing policy of health education specialist as the primary human resource in community health promotion activities from the special group perspective of the Korean Society for Health Education and Promotion. Methods: This research was conducted by the professional focus group discussion and descriptive literature review on health education and promotion. Results: This draft recommendation for Korean credentialing system development of health education specialist was based on the four background reasons for modifying health promotion related acts, for developing better policy of health education credentialing, for keeping the public and ethical responsibilities as the competitive professional society, and for improving health promotion activities in Korea. Theoretical background of the four reasons was Ottawa Charter. We classified three credentialing levels of health education specialist based on health education own competencies, coordiating competencies with environmental factors, and research competencies. Furthermore, we developed 10 major roles and categorized 53 sub-roles based on these competencies above. We recommended 10 classes required to take to become Health Education Specialist. These 10 classes were developed based on the credentialing systems in the United States and Japan. These 10 classes were about health education and promotion methods and strategies not health intervention topics. We also built the draft plan for continuing education to keep KCHES based on the NCHEC in the United States. Conclusions: Further research should be conducted to build better health education specialist credentialing systems modifing current communtiy-based health promotion activities in terms of modifying public regulation, developing KCHEC examination system, protecting job security both in public and private sectors, and creating professionalism in KCHEC.

Factors Affecting the Use of Medical Services by Workers with Respiratory Diseases (제조업 종사 근로자의 호흡기질환으로 인한 의료이용에 영향을 미치는 요인)

  • Objectives : To identify the relationship between the use of medical services by workers with three types of respiratory diseases(total respiratory diseases, acute upper respiratory infections and chronic lower respiratory diseases) and exposure to hazardous agents after controlling for other factors affecting medical services use, such as characteristics of the enterprises(scale, industry type) and employee demographics (sex, age). Methods : The study population comprised 28,882 workers who had undergone general or special medical examinations at the industrial health center at least once between Jan 1995 and Dec 1997 and had possessed medical insurance during the period. We combined medical examination data with medical insurance data in order to analyze the relationship between exposure to hazardous agents and respiratory diseases, Results : Among the 28,882 study subjects, 17,454 employees(60.4%) used medical services more than once during 3-year study period, owing to more than one kind of respiratory diseases. In logistic regression analysis, sex, age and the size of the enterprises proved to be significant variables on the use of medical services for all three types of respiratory disease; The use of medical services increased with employee age. Women used more medical services than men and the employees in the large-scale enterprises used more services than employees in small-scale enterprises. However, exposure to dust or organic solvents did not affect medical service use due to total respiratory diseases or acute upper respiratory infections. Only in the case of chronic lower respiratory diseases did workers exposed to dusts(OR=1.12, 95% CI=1.01-1.24) or organic solvents(OR=1.19, 95% CI=1.05-1.35) use more medical services than those not exposed. Conclusions : Workers exposed to dusts or organic solvents are particularly apt to suffer from chronic lower respiratory diseases and use medical services more often than those not exposed. That is, chronic exposure to hazardous agents such as dusts and organic solvents is believed to be harmful and to cause respiratory symptoms and diseases.

Understanding the Occurrence of Lung Cancer in Foundry Workers through Health Insurance Data (의료보험 전산자료 주상병명으로 파악한 주물공장 근로자들의 폐암)

  • Objectives : To investigate the difference in the occurrence of lung cancer between foundry workers and non-foundry workers by comparing the number of workers diagnosed with lung cancer through health insurance data. Methods : The study population was comprised of 28,884 workers who had undergone at least one general or special medical examination between January 1995 and December 1997 at the occupational health center. All of the subjects had health insurance during this period. We combined the medical examination data with the health insurance data to compare the number of foundry workers diagnosed with lung cancer and the number of non-foundry workers diagnosed with lung cancer. Results : Seven workers were diagnosed with lung cancer among the 1,591 foundry workers, compared to twelve workers among the 27,293 non-foundry workers (odds ratio: 10.04, 95% confidence interval: 3.95-25.55). The seven foundry workers diagnosed with lung cancer were all exposed to dust, and six out these seven workers were engaged in finishing or shake-out processes. Conclusions : Although the information for this study was obtained from health insurance data, which has limitations such as accuracy and completeness, the number of foundry workers diagnosed with lung cancer was significantly higher than that of non-foundry workers. Therefore, a well-designed cohort study should be followed to confirm the higher lung cancer rates in foundry workers.

A Study on the Uterus in Korean Medical Literature based on its Meaning and Function (한의학(韓醫學) 문헌(文獻)에 나타난 자궁(子宮)에 대(對)한 고찰(考察) - 자궁(子宮)의 개념(槪念)과 기능(機能)을 중심(中心)으로 -)

  • Objectives : The uterus plays an important role in the woman's body. In Korean Medical literature, the uterus is mentioned in various contexts according to different perspectives on its meaning and function. An examination of these various contexts is crucial in understanding the meaning of the uterus and to better understand and approach woman's body. Methods : Aside from the most widely used term Jagung(子宮), there were various terms used to refer to the uterus. Based on a list of these terms, the Siku Qianshu collection of medical literatures was investigated. Contents related to the definition, shape, location, function were extracted and examined. Results : Among the findings, first, there were various terms referring to the uterus similar to contemporary understanding. Some of them referred to the uterus as a whole, while others referred to specific parts, one of which is the placenta. Some reflected a broader perspective on the meaning of the uterus. Second, the functions of the uterus could be summarized as gate keeping, and the maintenance of uterine environment that is related to menstruation, pregnancy, and childbirth. Third, based on the examination of the meaning of the uterus and its functions, perspectives on the uterus in Korean Medicine could be summarized into two. Conclusions : One viewed uterus as an organ dedicated to reproduction, similar to today's common understanding. According to this view, uterus is a special organ specific to women, which functions as an incubator for the fetus. The other viewed the uterus as an intangible source of life in the woman's body. As a general source of life to all human beings, it is not a passive organ but functions as an active source in woman's life phenomena. The two perspectives are not in conflict, but rather reflect the broad range of thought on the concept of the uterus in Korean Medicine. In today's biomedical society, the diversity and flexibility of these perspectives could shed light on medical practices that have resulted from extreme views on the woman's body.

The effect of change of mandatory referral system in an ophthalmology of tertiary care medical institution (의료전달체계 변경이 3차 의료기관 안과에 미친 영향)

  • According to the change of mandatory referral system in July 1, 2000, the effect to the medical utilization of outpatient clinic and medical income in ophthalmology of tertiary care medical institute, S Hospital in Seoul was evaluated for 6 months before(1999. 12$\sim$2000. 5) and after(2000. 12$\sim$2001. 5). The results were as follows: 1. The number of outpatients was reduced by 16.6%. The number of patient with blindness low vision, retina, glaucoma increased and that of patient with accommodation refractive error, cataract decreased. 2. The number of cataract patients was reduced by 36.6%. The major location of patient's address was changed to nearer to the hospital. The number of cataract surgery reduced in 4.1%, the waiting time reduced in 42.2%, however surgery time increased in 20.2% and number of postoperative complications increased in 11.4%. 3. The income of outpatient clinic and cataract surgery reduced. Among items of outpatient clinic income, the most increased was ocular examination and the most reduced was injection and drugs. Among items of cataract surgery income, the most increased was operation fee and the most decreased was doctor's fee. In conclusion, for the patient, due to the lowered density of outpatient population more space was provided to the patients with more severe disease entity such as blindness' low vision, retina and glaucoma. For the hospital, the need for the expansion of ophthalmology was not found, however that for creation of the special clinics dealing with more severe disease entity was found. Due to reduced income and increased need of financial investment for the equipment and manpower for the more severe disease entity, the ophthalmology of tertiary care medical institute is faced with financial disaster. It is strongly suggested that the cost of medical practice of more severe disease entity be raised to achieve the success after change of mandatory referral system in ophthalmology.

The Present Situation and Tasks of University Nursing Education in Korea (한국 간호교육의 현황과 과제)

  • This study was intended to grasp the history of nursing education from the beginning to the present in Korea, and grip and look-out current diversified systems of nursing education on basis of February, 2000 through literatural review and investigation by close telephone interviews. The basic nursing educational institutions in the whole country were total 113, namely, 3 years course, 65 junior colleges of nursing, and 4 years course, 48 colleges of nursing. And there were 3 types of continuing nursing educational system: two of three were transferring to another college for gain bachelor's degree in nursing; RN-BSN programs and university of broadcasting, and the other was the system of independent learning and then examination for BSN. Total nursing graduates from junior college of nursing courses and college of nursing courses were 7,564 on February, 1999. In general graduate school, the number of master courses were 21 and Ph.D courses were 13. And the number of special graduate schools were 21, i.e., graduate school of education were 7, graduate school of administration were 2, graduate school of public health were 11 and graduate school of industry was 1. As the perspective on nursing education, we overviewed changing nursing organizational environment, increasing the system of continuing education, making standards in nursing education and systemization of nursing educational accreditation, specialization of nursing, information system in nursing education and education of graduate school. The summary of the above overviewed subjects were as follows; Every nursing educational institution needed to educate by educational criteria and standard and characteristically run BSN and graduate courses. Specialization in nursing has to develop more and more, therefore advanced education and law should be prepared appropriately. According to the age's and social needs, we have to establish counter-plan for fundamental educational environment. We have to sensitive to rapidly changing information in the era of globalization. In the level of university education, each university needs characterization of educational objectives, goals and contents, and has to replace the shortage of the number of professor. And the regulation of thesis and dissertation examinations need to be reinforced. Education in nursing should consists with specialization. Collaboration among universities will bring efficiency in the nursing education.
