Esophageal Leiomyoma with intense FDG uptake on $^{18}F-FDG$ PET/CT ($^{18}F-FDG$ PET/CT에서 강한 FDG 섭취를 보인 식도근종)

  • A 56 years old woman referred to our hospital with dysphagia and epigastric soreness. Gastroendoscopy revealed huge submucosal tumor with ulceration extending from distal esophagus to lesser curvature of stomach. Subsequent computed tomography(CT) demonstrated soft tissue mass encircling distal esophagus, and 18F-FDG PET/CT demonstrated intense $^{18}F-FDG$ accumulation in it. Finally this case was diagnosed as esophageal leiomyoma based on pathologic evaluation of the surgical specimen.

  • Objectives : The purpose of this report was to examine the relation between ocular fatigue and dry eye symptoms in patients with ocular fatigue. Methods : 115 patients with ocular fatigue disease were included in this analyses. The study examination included a symptom interview that assessed stiff sense, foreign body sense, pain, soreness, redness, etc. and ocular fatigue. Results : The survey showed in subjective ocular fatigue in all subjects. All patients(115) reported symptoms of ocular fatigue and 25.2% (29/115) were diagnosed as having dry eye with symptoms. Conclusions : These findings suggest that there is a strong between ocular fatigue and dry eye.

  • Objectives : The prevalence of peptic ulcer disease is estimated about 10% in Korea, and has declined due to Helicobacter pylori eradication therapy. Symptoms of peptic ulcer are postprandial epigastric pain, hunger pain, anorexia, nauea, vomiting, soreness, postprandial fullness, postparandial discomfort. The purpose of the study was to evaluate the effectiveness of the Anjungsan in the improvement of the symptoms in patients with peptic ulcer. Methods : After taking Anjungsan to patient with peptic ulcer, Examined to assess the improvemnet by Ordinal Scale VI grade and VAS. Results : Patients with symptoms of the appeal was taking Anjungsan gets better after 3 dyas appeared to be more than half. And 11 days after treatment all symptoms were improved under the VAS 1-2 except for epigastric lump sensation. Conclusions : After treatment with Anjungsan, patient showed improvement in all symptoms associated with peptic ulcer.

Gastric Lipomatosis

  • Gastric lipomatosis is an extremely rare condition. We present a case of a 69-year-old woman admitted with epigastric soreness. Computerized tomography (CT) revealed extrinsically compressing, fat-containing mass lesions on the entire gastric wall of the antrum and body except for the lesser curvature. A subtotal gastrectomy was performed. Pathology findings confirmed a gastric lipomatosis with multiple gastric ulcerations and extensive disruptions of the muscular layers. This case and reports of other gastric lipomatosis cases indicate that CT should be used to characterize large submucosal masses because CT can show the specific nature and extent of the disease. We believe that surgical treatment is the most appropriate treatment for symptomatic gastric lipomatosis that shows extensive gastric involvement, or when there are multiple gastric lipomas.

  • Vaccines can cause adverse reactions, such as soreness, swelling, or redness at the injection site. Some reactions are associated with fever and rash, which are usually mild and transient, and serious side effects are rare. In particular, there are no reports of systemic infection following a COVID-19 vaccination. The authors present a case report of a patient who developed multiple abscesses caused by Staphylococcus aureus after a COVID-19 vaccination. The patient had no previous symptoms or signs of infection. The patient was controlled successfully after surgical and antibiotics treatment.

Analysis of Physical Factors and Obesity Condition in Cascade Stomach Group (폭포상위를 가진 검진자의 신체적 요인 및 비만상태 분석)

  • Cascade Stomach is commonly found in obese patients. This study examined the physical factors, such as gender, age, height, weight and level of obesity, associated with a cascade stomach. Over a 12 month period between 2007 a series upper gastrointestinal examinations were carried out on 3,320 people, which included 564 patients with a Cascade Stomach. The results are as follows : 1. A cascade stomach was observed in 564(16.9%) out of 3,320 subjects. 2. A body mass index(BMI) > 120% was observed in 226 of the 564 cascade stomach patients. 3. Among those with a BMI > 120, 63.7% were female, 45.6% were aged between 50-59 years, 49.1% were between 151-160 cm in height and 31.0% were above 76 kg in weight. 4. The symptoms observed in the cascade stomach and obesity groups were subclinical(62.6% and 57.5%, respectively), dyspepsia(16.8% and 17.3%, respectively) and gastric soreness(10.8% and 15.0%, respectively). The incidence of a cascade stomach is higher in those with obesity. In addition, there is a higher incidence in females, those aged between 50 and 59, those between 151 and 160 cm in height and those weighing more than 76 kg. The main symptoms observed were subclinical, followed in order by dyspepsia and gastric soreness. This study showed a strong association between a cascade stomach and obesity. However, more studies will be needed to confirm this.

  • The purpose of this study was to investigate the spontaneous therapy, TENS(transcutaneous electrical nerve stimulation) and ice therapy after induction of DOMS(delayed onset muscle soreness) through application of eccentrically exercise in ralation to biceps brachii muscle of opposite-dominance. And to explain this, we analyzed VAS(visual analogue scales), peak torque, RMS(roots mean square) when exercising elbow joint. The subjects were 30 adults without any physical defect. Subjects were divided into three groups that spontaneous therapy, TENS therapy and ice therapy after DOMS induction, choosen movement could get the maximal peak torque in angular velocity of $60^{\circ}$/sec, and mearsured peak torque, RMS and VAS. 1. The VAS was significantly decreased after 3 days application of TENS than other groups(p<.05). 2. The peak torque was significantly increased after 3 days application of TENS than other groups(p<.05). 3. The RMS was significantly increased after 3 days application of TENS than other groups(p<.05). This study showed that TENS therapy application were effective treatment strategy on DOMS than spontaneous therapy and ice therapy. Therefore, it could be considered as a treatment method in the patients with DOMS.

Stiffness and Elasticity of the Masticatory and Facial Expression Muscles in Patients with the Masticatory Muscle Pain (저작근통 환자에서 저작근 및 안면표정근의 경도와 탄성도 평가)

  • This study aimed to assess stiffness and elasticity of the masticatory muscle in the patients with the masticatory muscle pain using a tactile sensor and to investigate whether the masticatory muscle pain affects the facial expression muscles. From those who visited Department of Oral Medicine in Dankook University Dental Hospital, 27 patients presenting with unilateral muscle pain and tenderness in the masseter muscle (Ms) were selected (mean age: $36.4{\pm}13.8$ years). Exclusion criterion was those who also had temporomandibular joint (TMJ) disorders or any neurological pain. Muscle stiffness and elasticity for the muscles of mastication and facial expression was investigated with the tactile sensor (Venustron, Axiom Co., JAPAN) and the muscles measured were the Ms, anterior temporal muscle (Ta), frontalis (Fr), inferior orbicularis oculi (Ooci), zygomaticus major (Zm), superior and inferior orbicularis oris (Oors, Oori) and mentalis (Mn). t-tests was used to compare side difference in muscle stiffness and elasticity. Side differences were also compared between diagnostic groups (local muscle soreness (LMS) vs myofascial pain syndrome (MPS) and between acute (< 6M) and chronic ($\geq$ 6M) groups. This study showed that Ms and Zm at affected side exhibited significantly increased stiffness and decreased elasticity as compared to the unaffected side.(p<0.05) There was no significant difference between local muscle soreness and myofascial pain syndrome groups and between acute and chronic groups. The results of this study suggests that masticatory muscle pain in Ms can affect muscle stiffness and elasticity not only for Ms but also for Zm, the facial expression muscle.

A study on the treatment of muscle pain using a high-frequency pain treatment device (고주파 통증치료기를 이용한 근육통 치료에 대한 연구)

  • After the recent COVID-19 outbreak, more and more people are paying attention to health care. Many people feel uncomfortable in life due to muscle pain after sports or exercise as a hobby and often take a break from exercise. After exercising for a long time or exercising excessively, pain appears throughout the body within 24 to 48 hours after exercise. This pain is called delayed onset muscle soreness (DOMS). In this study, the effectiveness of the pain relief of delayed onset myalgia was verified by using a high-frequency pain therapy device that generates deep heat. EMG was measured before and after pain treatment at ordinary times, RMS values were obtained through analysis, statistical analysis was performed using the SPSS program, and it was judged to be statistically significant. In addition, statistical analysis of the RMS value between normal and after pain treatment was conducted to confirm that there was no significance, and to confirm how much it returned to normal after pain treatment. In addition, since the standards for pain are different for each person even if the size of the same pain is the same, the NRS questionnaire for the size of pain was conducted, and the size of pain felt by the subjects was analyzed using the SPSS program and statistical significance was obtained. Therefore, as a result of this study, the high-frequency pain therapy device generated deep heat in the pain area to raise the temperature, and expanded the arteries and capillaries to increase blood flow, thereby increasing blood circulation and metabolism, and alleviating the pain of delayed onset muscle pain.

A prevalence of clinical sign and symptom in temporomandibular disorders patients (측두하악장애 환자의 임상적 양태에 대한 연구)

  • A prevalence of temporomandibular disorders(TMD) based on the clinical sign and symptom in 155 patients were investigated. History taking with interview chart and clinical examination were performed. Age and gender of the patients, duration of TMD, location of pain, joint sound, limitation of mouth opening and more detailed diagnostic name were identified. The results of this study were as follows: 1. TMD was more prevalent in female than in male. TMD was the most prevalent at the age of 20s and decreased with age. 2. Acute TMD was more prevalent than chronic one. 3. About ninety percent of TMD patients had pain. Pain had mainly a unilateral origin. Muscle pain was mainly related with the masseter muscle. 4. Joint sound was identified in about fifty-six percent of the TMD patients and unilateral joint sound was more prevalent than bilateral one. 5. Limitation of mouth opening was observed in about forty percent of the TMD patients. 6. In TMD patients, muscle dysfunction was the most prevalent one. In muscle disorders local muscle soreness was the most prevalent one. In joint disorders, capsulitis was the most prevalent one. In muscle-joint disorders, trigger point pain with retrodiscitis was the most prevalent one.
