위암에시 PET의 임상적 역할 (Role of PET Scan in Gastric Cancer as a Diagnostic Tool)

  • Clinical application of positron emission tomography (PET) is rapidly increasing for the detection and staging of cancer at whole-body studies performed with the glucose analogue tracer 2-[fluorine-18]fluoro-2-deoxy-D-glucose (FDG). Although FDG PET cannot match the anatomic resolution of conventional imaging techniques in gastrointestinal and abdominal organs, it is particularly useful for identification and characterization of whole body at the same time. FDG PET can show foci of metastatic disease that may not be apparent at conventional anatomic imaging and can aid in the characterization of indeterminate soft-tissue masses. Most gastrointestinal cancer need to surgical management. FDG PET can improve the selection of patients for surgical treatment and thereby reduce the morbidity and mortality associated with inappropriate surgery. FDG PET is also useful for the early detection of recurrence and the monitoring of therapeutic effect. The gastrointestinal cancers, such as gastroeso-phageal cancer, colorectal cancer, liver cancer and pancreatic cancer, are common malignancies in Korea. PET is one of the most promising and useful methodology for the management of gastric cancer as well as other gastrointestinal cancers.

Imaging Manifestations and Misdiagnosis Analysis of Six Cases of Bone Hydatid Disease

  • We retrospectively evaluated the clinical and imaging features of 6 patients with bone hydatid disease confirmed by surgery and pathological examination. Among the 6 patients, 2 were infected with Echinococcosis granulosus metacestode and 4 were infected with E. multilocularis metacestode. The 2 cases with cystic echinococcosis were diagnosed by computed tomographic (CT) examination, and other 4 cases were diagnosed by magnetic resonance (MR) imaging. On the initial evaluation, 1 case each was misdiagnosed as a giant cell tumor or neurogenic tumor, and 2 were misdiagnosed as tuberculosis. The imaging manifestations of bone hydatid disease are complex, but most common findings include expansive osteolytic bone destruction, which may be associated with sclerosing edges or dead bone formation, localized soft tissue masses, and vertebral lesions with wedge-shaped changes and spinal stenosis. Combining imaging findings with the patient's epidemiological history and immunological examinations is of great help in improving the diagnosis and differential diagnosis of bone hydatid disease.

  • Myoepithelioma is composed exclusively of myoepithelial cells. Myoepithelial cells are ectodermally derived contractile cells that can be routinely identified in many normal tissues having secretory function such as major and minor salivary glands, lacrimal gland and sweat gland. Tumors composed exclusively of myoepithelial cells, so-called myoepitheliomas, are rare-less than 1% of all salivary gland tumors. Grossly, these tumors are well demarcated. The external surface is smooth and may be bosselated. The cut surface is white and homogenous. Microscopically, these tumors are surrounded by a thin fibrous capsule. They are composed of benign-appearing spindle- shaped and/or polygonal cells. Mitoses are rare. Frequently these tumors contain myxomatous stroma which is susceptible to alcial blue stain. Clinically, myoepitheliomas present as slow-glowing, painless masses and can not distinguished from pleomorphic adenomas. Treatment is the same as for pleomorphic adenoma, and the surgical excision should include a margin of normal tissue. Although the majority of myoepitheliomas have behaved in a beingn manner, pleomorphism and mitotic activity have been associated with local aggressiveness.

Micromorphological and Chemical Characteristics of Cengal (Neobalanocarpus heimii) Heartwood Decayed by Soft Rot Fungi

  • The heartwood of cengal (Neobalanocarpus heimii) is known to have a high degree of decay resistance by virtue of its high extractive content. After 30 years in ground contact an utility pole of this tropical hardwood was found to be degraded only in the surface layers by cavity-forming soft rot fungi. The present work was undertaken 1) to characterize the degradation of cengal heartwood from the aspect of ultrastructure and chemistry and 2) to investigate the correlation between soft rot decay and its extractive microdistribution in wood tissues. The chemical analysis of cengal heartwood revealed the presence of a high amount of extractives as well as lignin. The wood contained a relatively high amount of condensed lignin and the guaiacyl units. Microscopic observations revealed that vessels, fibers and parenchyma cells (both ray and axial parenchyma) all contained extractives in their lumina, but in variable amounts. The lumina of fibers and most axial parenchyma were completely or almost completely filled with the extractives. TEM micrographs showed that cell walls were also impregnated with extractives and that pit membranes connecting parenchyma cells were well coated and impregnated with extractives. However, fungal hyphae were present in the extractive masses localized in cell lumina, and indications were that the extractives did not completely inhibit fungal growth. The extent of cell wall degradation varied with tissue types. The fibers appeared to be more susceptible to decay than vessels and parenchyma. Middle lamella was the only cell wall region which remained intact in all cell types which were severely degraded. The microscopic observations suggested a close correlation between extractive microdistribution and the pattern and extent of cell wall degradation. In addition to the toxicity to fungi, the physical constraint of the extractive material present in cengal heartwood cells is likely to have a profound effect on the growth and path of invasion of colonizing fungi, thus conferring protection to wood by restricting fungal entry into cell walls. The presence of relatively high amount of condensed lignin is also likely to be a factor in the resistance of cengal heartwood to soft rot decay.

흉부에서 발생한 IgG4 연관 질환: 영상 소견 및 감별진단 (Immunoglobulin G4-Related Disease in the Thorax: Imaging Findings and Differential Diagnosis)

  • 면역글로불린G4 연관 질환(immunoglobulin G4-related disease; 이하 IgG4-RD)는 IgG4를 생산하는 면역세포에 의한 만성 염증성질환으로, 주로 타액선, 누액선, 안와, 췌장, 담도, 간, 신장, 후복막, 대동맥, 폐, 림프절 등 다양한 장기를 침범하고, 조직학적으로 IgG4 양성 형질세포와 림프구의 침윤 및 나선형의 섬유화(storiform fibrosis), 폐색정맥염(obliterative phlebitis)을 특징으로 한다. IgG4-RD의 흉부 침범에서 가장 흔한 소견은 종격동 림프절 비대와 폐의 림프관주위 간질 비후이다. 폐의 기관지혈관주위 간질 비후와 우측 척추곁 밴드형 연부조직은 IgG4-RD의 특징적 소견이고, 그 외에도 폐결절 혹은 종괴, 간유리음영, 폐포 간질비후, 흉막삼출 및 비후, 흉벽이나 종격동 종괴, 대동맥과 관상동맥의 혈관염이 발생할 수 있다. 영상의학적으로는 악성 종양이나 감염 및 다양한 염증성질환과의 감별진단이 필요하다. 본 연구에서는 흉부에서 발생하는 IgG4-RD의 영상 소견과 감별진단에 대해 기술하였다.

구강 및 악골에 생긴 전이 암종 (METASTATIC CARCINOMA OF ORAL CAVITY)

  • 구강내에 발생하는 암종은 원발성으로 나타나는 것이 보통이지만 신체내 다른 부위의 원발성 암종에 의해 이차적으로 전이되기도 한다. Clausen 과 Poulsen에 의하면 원발성 암종중에서 breast, lung, kidney, thyroid의 순으로 구강내 전이암종을 많이 나타내는 것으로 알려져 있다. 문헌에 의하면 구강 및 악골에 발생한 전이암종 중에서 약 70%가 adenocarcinoma 이며 주로 혈행적(hematogenous) 전이를 하는 것으로 알려져 있다. 이때 전이는 일차적으로 악골내에, 이차적으로 연조직에 전이되며, 이로인해 발생한 악성 전이 암종은 구강내에 발생하는 악성종양의 약 1%에 해당한다. 그러나, 실제로 이의 발견이 어려운 것은 악골에 대한 일상의 검사가 이루어지지 않으며 환자 자신이 특별한 증상을 호소하지 않기 때문이다. 본 교실에서는 문헌상 빈도수가 적은 gall bladder cancer 와 pancreatic tumor 에서 전이된 암종이 특이하게 악골내에서는 나타나지 않고 연조직에만 발생하였기에 이에 보고하는 바이다.

폐전이를 보인 경-흉추 이행부 악성 신경초종 1례 - 증 례 보 고 - (A Case of Cervico-Thoracic Malignant Schwannoma with Lung Metastasis)

  • Acase of malignant schwannoma in the cervico-thoracic region with lung metastasis is presented. The patient was 55-year-old man presented with right arm weakness. Magnetic resonance image demonstrated lobulated enhancing soft tissue masses in spinal canal, neural foramen and right paraspinal space at C7-T1 level compressing the dural sac and spinal cord. Subtotal removal was performed and histological diagnosis of malignant schwannoma was made. Reoperation due to recurrence was done but subsequent metastasis to lung was observed.

상완 원위부에 발생한 기무라씨 병 - 증례 보고 - (Kimura's Disease of the Distal Arm - A Case Report -)

  • Kimura씨 병은 원인이 알려지지 않은 만성 염증성 질환으로 주로 동양의 젊은 남자에게 호발되는 드문 질환이다. 주로 두경부의 피하 조직이나 타액선에 무통성의 종괴로 발생하며, 주변의 국소 림프 병증을 동반하기도 한다. 말초 혈액에 호산구 증가증과 혈청 IgE 증가를 나타내고, 조직학적으로 이환된 림프절의 과증식된 배중심과 호산구 등의 염증세포 침윤의 병리 소견을 보인다. 치료 방법으로는 스테로이드 치료, 방사선 치료 및 수술적 치료 등이 있으며 예후는 양호하다. 저자들은 28세 남자 환자의 좌측 상완 원위부 내측에 발생한 종괴를 변연부 절제술을 시행하여, 병리 조직학적으로 Kimura씨 병으로 확진한 1예를 문헌 고찰과 함께 보고하는 바이다.

다발전이성 위장관 간질종양을 동반한 제 1형 신경섬유종증 1예 (A Case of Type 1 Neurofibromatosis Associated with Multiple Metastatic Gastrointestinal Stromal Tumors)

  • Type 1 neurofibromatosis (von Recklinghausen's disease, NF-1) is an autosomal-dominant neurocutaneous-disorder characterized by systemic cafe'-au-lait spots, multiple cutaneous neurofibromas, axillary or inguinal freckling, and Lisch nodules (pigmented iris hamartomas). Approximately 10-25% of NF1 patients have gastrointestinal neoplasms. Gastrointestinal stromal tumor (GIST) in patients with neurofibromatosis is most commonly found in the small bowel and the stomach, and approximately 60% of such patients have multiple tumors or multiple tumor sites. Although, the increased incidence of GIST in patients with neurofibromatosis is well documented in pathology literature in English, but has rarely been documented in Korea. Here, we report a case of multiple GISTs in a 48-year-old woman accompanied by NF1. She was admitted to Yeung-nam University Hospital with complaints of melena and dyspnea. A contrast-enhanced computed tomography (CT) scan revealed that multiple soft tissue masses were occupying the entire peritoneal cavity. An ultrasonogram- guided biopsy was performed and the tumors were found to have been composed of tumor cells that were positive for c-kit protein. The patient was put on Imatinib mesylate treatment, and further follow-up will be carried out.
