지자체의 나트륨 섭취 감소 계획 평가 및 근거 기반 목표 재설정 : 서울시 사례를 중심으로 (Evaluation of the sodium intake reduction plan for a local government and evidence-based reestablishment of objectives: Case of the Seoul Metropolitan Government)

  • 문헌 고찰을 통한 국내외 나트륨 정책 현황 및 나트륨 섭취량, 서울시에서 진행되고 있는 나트륨 섭취 감소 프로그램의 분석, 국민건강영양조사에서의 서울 시민을 대상으로 한 나트륨 섭취 추이, 인구학 사회경제적 특성 및 나트륨 섭취와 관련된 식행동에 따른 나트륨 섭취량 자료분석, 전문가 안면 타당도 평가 결과를 통한 서울시 나트륨 섭취 저감화 계획 목표 재설정 및 프로그램 우선 순위와 대상자 선정은 다음과 같다. 나트륨 저감화 전략에 따른 각 국가의 나트륨 섭취 감소율을 분석한 결과 전년도 대비 약 1.0 ~ 2.0%씩 감소하였고 우리나라의 경우 5.3%의 감소율을 보였다. 따라서 본 연구에서 제시하는 서울시 나트륨 섭취 저감화 정책 목표는 연간 나트륨 섭취량 감소율 2.7%와 서울시 나트륨 저감화 사업의 확대 계획을 고려하여 2.0%로 연간 감소율을 적용하는 것으로 하여 2020년에는 약 3,600 mg의 나트륨 섭취량을 목표로 재설정하였다. 서울시에서 수행하고 있는 나트륨 감소를 위한 프로그램을 생태학적 프레임에 적용하여 분석한 결과, 전반적으로 생태학적 프레임에서 제안하고 있는 개인적, 사회적 환경, 물리적 환경, 거시적 환경 수준들이 프로그램에 포함되고 있었으나 각 프로그램들은 대상자에 따라 수준별로 연계가 되어 있지 못하였고, 주로 단편적이고 산발적으로 진행되고 있었다. 또한 각 자치구별로 공통되고 표준화된 프로그램은 없었다. 따라서 선택과 집중을 기반으로 하여 대상자별로 각 수준들이 연계되어 표준화된 프로그램이 필요한 것으로 나타났다. 프로그램 우선순위 대상자는 성별에서는 여자보다 남자에서, 연령대에서는 30 ~ 50대 성인, 소득수준에서는 저소득층이었고, 나트륨 섭취와 관련된 식행동 중에서는 과일 섭취량이 적은 군이 전반적으로 나트륨 섭취량이 높은 특성을 가져 과일 섭취에 대한 식행동 변화가 중요한 것으로 나타났다. 나트륨 섭취 감소를 위한 프로그램 분석, 서울시 나트륨 섭취 현황 및 고 나트륨 섭취 대상자 특성 분석, 전문가 대상 안면 타당도 평가 결과를 바탕으로 서울시 나트륨 감소를 위한 프로그램 우선순위 대상자는 성인 남성으로서 프로그램의 전략 방향은 생태학적 프레임을 적용하여 이들이 주로 생활하는 생활 터인 직장, 가정, 음식점에서의 나트륨 섭취 환경 개선을 위한 프로그램들을 수준별로 연계하여 진행하는 것으로 제안하였다. 따라서 가정과 음식점에서는 성인 남성의 나트륨 섭취에 영향을 줄 수 있는 주부와 조리 종사자도 프로그램 주요 대상자로 선정 되었다. 본 연구에서는 제안된 나트륨 섭취량의 목표치와 중재 프로그램에 대한 우선순위는 지속적인 모니터링과 효과평가를 통하여 업데이트가 되어야 할 것이다. 향후 생태학적 프레임에 적용된 나트륨 섭취 감소 프로그램의 효과에 대한 전반적인 평가가 필요할 것으로 사료되며 나트륨 섭취 저감화 정책 및 프로그램의 지속성과 효과적인 수행을 위해서는 타 기관 및 부서에서 수행되고 있는 관련 프로그램과의 연계와 협의 체계가 마련되어야 할 것이다.

Effects of Sodium Intake on the Association between the Salt-Sensitive Gene, Alpha-Adducin 1 (ADD1), and Inflammatory Cytokines in the Prevalence of Children Obesity

  • Objective: To examine the effects of sodium intake on the correlations between the saltsensitive gene ${\alpha}$-adducin 1 (ADD1) and inflammatory cytokines in Korean childhood obesity. Methods: A total of 2,070 students aged 8-9 years old participated in this study. The anthropometrics, serum biochemistry profile, inflammatory cytokines, and three-day dietary assessment were analyzed according to sex, obesity degree, and ADD1 polymorphism. Results: The obesity prevalence was higher in boys (15.6%) than in girls (11.9%). Boys also showed higher values in anthropometrics; lipid, glucose, and insulin profiles; total calorie intakes, as well as those of sodium and calcium compared with those of the girls. The more obese were boys and girls, the higher were the anthropometrics and the blood levels (total cholesterol, triglyceride, low-density lipoprotein cholesterol, fasting blood sugar, and insulin), but the lower was high-density lipoprotein cholesterol. The obese boys had significantly higher sodium and Na/K intakes, while the obese girls had higher visfatin level and Na/K intake. In addition, an increase in the risk factors for blood pressure and obesity in ADD1 variants was identified. Serum tumor necrosis $factor-{\alpha}$($TNF-{\alpha}$) significantly increased with increasing sodium intake in the ADD1 W allele carriers, regardless of sex. The presence of obesity with the ADD1 W allele induced inflammatory accelerators such as $TNF-{\alpha}$ or C-reactive protein(CRP) with higher sodium intake. Conclusion: Obese children with an ADD1w allele can experience a more complex form of obesity than non-obese when exposed to an obesity-inducing environment and need to be controlled sodium intake in the diet.

일부 산업체 근로자의 Sodium섭취에 관한 연구 (Sodium Intakes of Some Industrial Workers)

  • 본 연구는 산업체 근로자의 Sodium섭취 실태를 파악하므로서 효율적인 식사관리와 건강관리의 기초자료를 얻고자 서울 시내 H타이어 공장에 근무하는 건강한 성인남자 40명을 대상으로 3일간의 식사분석과 뇨분석을 행한 것이다. 열량섭취는 1일 평균 3,185$\pm$813(mean$\pm$SD)kcal 였으며 영양소 섭취량은 당질이 541.8$\pm$144.5g, 단백질이 127.2$\pm$61.0g, 지방질이 56.8$\pm$42.7g이었고 총 열량의 68%, 16%, 16%를 차지했다. 식사분석에 의한 Sodium섭취량은 225.3$\pm$75.2mEq(13.2$\pm$4.4g Nacl)였으며 뇨중의 Sodium 배설량은 232.8$\pm$63.8mEq(13.7$\pm$3.5g NaCl)였다. 주요 음식의 1회 순섭취량에 대한 Sodium섭취는 국종류에서 가장 많았으며 1일 섭취하는 식사의 Sodium분포는 밥에서 6.9%, 국 35.7%, 반찬류 49.4%, 식탁에서 사용하는 조미료로서 5.1%, 간식에서 2.9%이었다. 당질섭취와 Sodium섭취, 단백질 섭취와 Sodium섭취 사이에는 강한 양의 상관을 보였다.

24시간 소변분석과 음식섭취빈도지를 사용한 우리나라 성인들의 나트륨 섭취량과 지역별, 음식군별 나트륨 섭취량의 비교 (Sodium Intakes of Korean Adults with 24-hour Urine Analysis and Dish Frequency Questionnaire and Comparison of Sodium Intakes According to the Regional Area and Dish Group)

  • This study was performed to assess the sodium intakes of Korean adults using a 24-hr urine analysis and dish frequency questionnaire (DFQ) according to each dish group and the regional area. The subjects of this study were comprised of 522 adults (male : 267, female : 285), aged 20-59yr residing in the metropolitan area (N=200), Chungcheng-Do (N=117), Jeolla-Do(N=117), and Gueongsang-Do provinces (N=118). The subjects were recruited from the residents who once participated or are participating in the various health programs offered by the public health center. The number of subjects who completed the 24-hr urine collection was 205 (male : 110, female : 95). The mean age and BMI of the subjects were $39.0{\pm}$11.7y and $23.1{\pm}2.9 kg/m^2$, respectively. The mean systolic and diastolic blood pressure was $119.5{\pm}15.4 mmHg$, and $77.1{\pm}11.1 mmHg$, respectively. Eighteen percent of the subjects responded that they are currently smoking, 36% drinking and 50.4% exercising. Twenty point six percent of the subjects were assessed as having hypertension according to their systolic or diastolic blood pressure($SBP{\ge}140mmHg$ or $DBP{\ge}90mmHg$) measurements in the present study. Salt intake of the subjects estimated with 24-hr sodium excretion was 12.7g/d (male : 13.4g/d, female : 12.1g/d) based on the sodium excretion rate as 82%. Salt intake estimated with DFQ was 14.7g/d (male : 16.2g/d, female : 13.4g/d), 2 g more than the salt intake estimated with 24-hr urine analysis. The four dish groups that contributed most to the sodium intake in order were kimchi (11571.4mg), soup and stew (1260.5mg), fish and shellfish(706.3mg) and noodle and ramyeon(644.3mg). Salt intake estimated with DFQ was the highest in the subjects of Gueongsang-Do(17.0g/d), second highest Chungcheong-Do (16.4g/d) and the lowest in the metropolitan area (13.0g/d). Subjects of Gueongsang-Do showed the highest sodium intakes in most of the dish group, whereas subjects of the metropolitan area showed the lowest. Residents of Chungcheong-Do revealed the highest sodium intake with kimchi and of Jeolla-Do the higher sodium intake with main dish (meat, fish and beans). The highest salt percentage of kimchi ($3.0{\pm}0.8%$) and soybean paste ($14.5{\pm}5.1%$) were observed in Gueongsang-Do, whereas individuals of the metropolitan area were observed as having kimchi ($1.6{\pm}0.5%$) and soybean paste ($7.4{\pm}1.6%$) with the lowest salt percenage. Men were observed as having more salty kimchi ($2.4{\pm}0.1%$) than women ($2.1{\pm}0.1%$).

Comparison of salty taste acuity and salty taste preference with sodium intake and blood pressure based on zinc nutritional status in two rural populations in Korea

  • This study examined salty taste acuity and salty taste preference and sodium intake in relation to zinc nutritional status in 2 rural populations in Korea. And we also examined the main food contributors of their sodium intakes. We enrolled 218 adults (66 men and 152 women) from the Kangneung and Samcheok regions in Korea's Kangwon province in our study conducted from December 2011 to February 2012. Participants from each region were divided into 3 groups based on their serum zinc level (T1: lowest, T2: intermediate, T3: highest). We compared the salty taste acuity and preference, Na index (Dish Frequency Questionnaire for estimation of habitual sodium intake), blood pressure, and intakes of nutrients including sodium by 3 groups of serum zinc level. The results were as follows: a higher serum zinc level indicated a lower sodium intake and Na index (P<0.05). The salty taste acuity was considerably higher for participants from the Kangneung region than those from the Samcheok region (P<0.05). And the serum zinc level was significantly higher in participants from the Kangneung region than those from the Samcheok region (P<0.05). We further divided the participants into 2 groups: those who consumed more zinc than the recommended intake (RI) and the others. We compared salty taste acuity and salty taste preference in the 2 groups. The salty taste threshold and palatable salty taste concentrations were lower for the group with a zinc intake above RI than for the group with zinc intake below the RI. However, the difference was not significant. This study confirms that taste function differs depending on zinc nutritional status. In future, it is required to a large-scale, long-term, prospective study on the correlation between zinc intake, serum zinc levels, and taste perception function and blood pressure.

일부 대학생들의 라면 섭취 관련 식행동과 국물 섭취 실태 조사 (Survey on Dietary Behaviors and Intakes of Instant Noodle (Ramyeon) Soup among College Students)

  • High intakes of sodium may increase the risk of hypertension or cardiovascular diseases. According to the 2010 Korea National Health and Nutrition Survey, the average intake of sodium was 4,878 mg/day with salt, kimchi, soy sauce, fermented soybean paste and Ramyeon being the five main sources of sodium. In order to identify solutions to reduce the intake of sodium, we investigated the intake patterns and eating behaviors of Ramyeon among 347 college students (male 146, female 201) using survey questionnaires. The average age of study subjects was 23.7 years for males and 20.5 years for females. The average Body Mass Index ($kg/m^2$) was 21.9 for males and 20.1 for females. The average frequency of Ramyeon intake was 2.0 times/week. The main reason for eating Ramyeon was convenience (56%), followed by good taste (27%), low price (11%) and other reasons (9%). The criteria for choosing Ramyeon were taste (72%), convenience (14%), price (7%), nutrition (1%), and the other factors (2%). Males' average intake of Ramyeon soup (61%) was higher than that of the females (36%). The estimated intake of Ramyeon soup by survey showed a positive correlation with the measured intake of Ramyeon soup. Sodium contents of Ramyeon were measured separately for the noodles and the soup, which were 1,185 mg/serving and 1,148 mg/serving each. Therefore, the amount of sodium intake can be reduced if students eat less Ramyeon soup. Also, we observed that dietary behaviors and soup intakes of Ramyeon between the sexes were different. Appropriate nutritional education for proper eating habits may help decrease the intake of sodium.

아동 및 청소년의 라면 섭취에 따른 식품 및 영양소 섭취 실태 비교 - 2005년 국민건강영양조사 심층분석 결과 - (A Comparison of Food and Nutrient Intakes between Instant Noodle Consumers and Non-Consumers among Korean Children and Adolescents)

  • Instant noodle is one of the most popular foods in Korea. The objective of this study was to examine the association of instant noodle consumption and food and nutrient intake among children and teenagers in Korea. We used dietary data from 24-h recall of 1,748 subjects aged 7-19 years who participated in the 2005 Korea National Health and Nutrition Examination Survey. Those who consumed instant noodle once or more during the survey period were categorized as "instant noodle consumer (INC)" and were compared for food and nutrient intakes with the others (non-INC). The average age of the INC was 13.4 and that of the non-INC was 12.4; girls consumed more instant noodles than boys (p < 0.05). There was no significant difference in BMI between the two groups. With the exception of grains, as compared to the non-INC, the INC consumed significantly less amounts of potatoes, vegetables, mush-rooms, fruits, spices, seafood, and milk and dairy products. With respect to nutrient intakes, the INC-group showed significantly higher nutrient intakes of energy, fat, carbohydrate, sodium, thiamine, and riboflavin; however, as compared to the non-INC-group, the INC-group showed significantly less intakes of calcium, niacin, and vitamin C. The overall result suggests that consuming instant noodles may lead to excessive intake of fat and sodium, but it may cause an increased intake of thiamine and riboflavin. Therefore, nutrition education that helps children and teenagers choose a balanced meal while consuming instant noodle should be carried out. In addition, manufactures of instant noodles should consider nutritional aspects in product development processes.

대구.경북지역 위암환자의 식품과 영양섭취상태 (The Nutritional Intakes of the Stomach Cancer Patients in the Daegu and Gyeongbuk Areas, Korea)

  • The present study was conducted to evaluate the food and nutrients intakes of stomach cancer patients in the Daegu and Gyeongbuk areas in order to find out the dietary risk factors for stomach cancer. The subjects of the study were 102 patients who had recently been diagnosed with stomach cancer at the Gyeongbuk National University Hospital. The control subjects were 105 people including patients from the Department of Orthopedic Surgery and healthy volunteers who did not have any gastrointestinal diseases. Estimates of nutrients intakes were determined from the food intake frequency data obtained by individual interviews using questionnaires. The mean daily calorie intakes of the control and the case groups were not significantly different. However the energy intake from protein was significantly higher in the case group as compared to the control group. With regard to the nutrients intakes, the case subjects consumed significantly higher amounts of nutrients such as protein, calcium, sodium, phosphorus and niacin than the control group. The case group showed a tendency to consume higher amount of protein, fat, calcium and iron from animal food sources. In the present study dietary factors which were suspected as being risk factors for stomach cancer included high consumption of animal foods, specific nutrients such as protein, sodium and niacin, specific food groups such as meat, spices, and low consumption of fruits and mushrooms. Therefore, it is recommended that more extensive and systematic surveys be conducted to confirm the risk factors for stomach cancer, taking into consideration the dietary cultural characteristics of this region. 도시지역 미취학 어린이의 Na 섭취 및 배설에 관한 연구 (Soldium Intake & Excretion of Preschool Children in Urban)

  • This study was designed to estimate the sodium intake of preschool children . To determine the sodium intake & excretion of preschool children in Korea, dietary behaviors, anthropometry, intakes of dietary nutrients, urinary sodium excretion and preference for salty foods were measured in 42 preschool children (male 26 , female 16, average6.5 years old) and their mothers. The results are summarized as follows. Mean daily urinary sodium excretion was 52.7 mEq(1,212.1mg). This value did not show remarkable change compared with the other studied that were accomplished in the similarage group for about the last ten years. And the subjects showed lower preference for salty taste than those of elementary school children and adults. Mean daily lower preference for salty taste than those of elementary school children and adults. Mean daily urinary sodium excretion were significantly correlated with the frequency of eating out (p<0.01), potassium intake(p<0.001) and urinary sodium to potassium excretion ratio(p<0.001). But there weren't any correlations with mean daily sodium intake, blood pressure, dietary nutrients intake and the preference for salty taste.

서울지역 일부 여대생의 나트륨과 칼륨평형에 관한 연구 (A Study on Sodium and Potassium Balance of College Women in Seoul)

  • The purpose of this study was to investigate Na and K balances in healthy adult women. Anthropometric assessments, biochemical analysis of blood, 3-day dietary flood records and collections of 3-day food, 24-hr urine and faces were performed to evaluate intakes and excretions of Na and K in 20 college women living in Seoul. The mean BMI and blood pressure of the subjects were 21.08 and 110.25/67.50mmHg, respectively. Mean daily intake of energy was 1578.84kcal, 79% of Korean RDA. Also, daily intakes of Na and K ware 120.86mEq and 44.20mEq. The urinary and fecal excretions of Na were 99.88 and 4.45mEq/day, and those of K were 30.41 and 8.66mEq/day, respectively. The body retention, retention rate, and apparent absorption of Na were 17.11mEq, 13.23%, and 96.31%, and those of K were 5.82mEq, 8.69%, and 80.12%, respectively. The urinary and fecal Na/K ratio were 3.48 and 0.52. There were significantly positive correlations between 1) urinary Na, K excretions and intakes of Na or K, 2) urinary K and BMI, 3) serum K and serum globulin, and 4) urinary Na excretion and serum haptoglobin level, respectively. The results of this study show that Na intake was higher and K intake was lower than those of other advanced nations. Therefore, nutrition education show instruct people to reduce Na intake and to increase K intake.
