감옥 모티프를 통한 도덕적 성숙 (The moral growth through prison motif)

  • This study attempts to survey the moral growth through prison motif. Malamud deals with the theme of suffering deeply. To learn love accompanied by suffering is the way to be moral and therefore suffering becomes Malamud's main concern. The hardship from suffering and love by trial leads Frank and Yakov to guest of selves. Through this their erotic impulses are changed into caritas. Malamud portrays Frank as a schlemiel. Like Malamud's other heroes he is also born to suffering, conditioned by it, and ultimately finds the meaning of his life in learning to deal with it. At last he carries the burden of supporting Helen and Ida. As a result, Frank becomes a social failure but a moral success as traditional American Adam. The prison motif is a rather strong function of setting symbolism in The Fixer. Yakov is imprisoned for a heinous crime that he did not commit. Yakov experiences hard physical and spiritual pains in prison for two and half years. His bitter suffering has matured him, changed him into a man with love, sacrifice and responsibility for others. At last he returns a true Jew and good man with perfect spirit.

생기능자기조절훈련(뉴로피드백)과 한약 치료를 병행한 사회공포증 치험 1예 (A Clinical Report about a Patient Suffering from Social Phobia and Treated by Herbal Medicine with Neurofeedback)

  • Objectives: In this case report, we will show the effectiveness of herbal medicine with neurofeedback treatment on social phobia. Methods: First, we diagnosed the patient by interview and questionnaire as having social phobia and suffering from anxiety, shaking and palpitation in public places and fear to meet new people. Beside, we examined the pattern of brainwaves by QEEG-8 system. Second, we treated the patient by herbal medicine with neurofeedback treatment for 4 months. Then we conducted new assessments by SCL-90-R, BDI, BAI and QEEG-8 system. Results: The patient's psychological and physiological symptoms were significantly improved. Conclusions: Herbal medicine with neurofeedback can be effective in the treatment of social phobia. Especially, it can be helpful to improve the patient's sensitivity to its physiological symptoms.

대리모 여성의 심리사회적 고통 체험 연구: Parse의 인간되어감 방법을 적용하여 (Lived Experience of Psycho-Social Suffering for Surrogate Mother in South Korea: Applied to Parse's Human Becoming Methodology)

  • 본 연구는 한국사회의 대리모 여성의 심리사회적 고통 체험의 구조를 심층적으로 이해하기 위하여 실행되었다. 자료는 미디어를 통해 수집된 질적 2차 자료를 상호주관적 삶으로서의 역동성과 의미를 잘 나타내주는 파시(Parse)의 현상학적-해석학적 연구방법에 적용하여 분석하였다. 분석 결과 대리모 여성의 체험은 경제적 고통으로 선택한 최선의 대안이었으나 출산과 양육이 분리된 모성은 일방적 관계로의 위험에 노출되나 보상과 입양의 맞물림으로 잊혀 질 모성의 현실을 수용하는 과정이라는 구조로 나타났다. 그러한 구조를 전환시키면 대리모 여성의 체험은 생의 마지막 선택이라 여기나 죄책감과 혼란스러움으로 드러낼 수 없는 존재로서 모성을 도구화하는 것으로 파악되었다. 이러한 해석은 파시의 추상성 높은 개념으로 통합하여 대리모 여성의 체험은 가치화를 가능-제한, 노출-은폐하여 독특성을 강화하고 변형시켜가는 과정으로 재해석 되었다. 대리모 여성에 대한 경제적·의료적·법적 차원에서의 논의와 함의를 다루었다.

The Effect of Logotherapy on the Suffering, Finding Meaning, and Suffering, Well-being of Adolescents with Terminal Cancer

  • Purpose: To evaluate the effects of a logotherapy education program. Methods: A nonequivalent control group, non-synchronized design was conducted with a convenience sample of 29 children with cancer. The experimental group (n=17) participated in the logotherapy education program which consisted of 5 daily sessions for one week. The control group (n=12) received the usual nursing care. The effects were measured using suffering, adolescent meaning in life (AMIL), and spiritual well-being (SWBS) scales. Results: There were significant differences in suffering (W=153.00, p<.05) and meaning in life (W=78.00, p<.05) between the experimental and control groups. However, there was no significant difference in spiritual well-being (W=136.50, p>.05). Conclusions: Logotherapy was effective in reducing suffering and improving the meaning in life. Logotherapy can be utilized for adolescents with terminal cancer to prevent existential distress and improve their quality of life.

말기 암환자 가족의 고통 측정도구의 신뢰도와 타당도 검증 (Reliability and Validity of the Suffering Scale of Family of Patients with Terminal Cancer)

  • Purpose: The purpose of this study was to test the reliability and validity of an instrument to measure suffering of family of patients with terminal cancer. Methods: This research was a methodological study processed as follows: 1) The preliminary 32 items were developed based a conceptual framework of suffering for patients with cancer. 2) The content was validated by an expert panel (n=24) and the family of patients with terminal cancers (n=33). 3) The instrument was validated by survey (n=92). Finally, 29 items were selected in developing final version of the scale. Results: Seven factors were extracted through factor analysis: 'economical distress', 'loss of meaning in life', 'feeling of social isolation', 'emotional distress', 'burnout', 'guilty feeling', and 'physical discomfort'. These factors explained 66.94% of the total variance. Cronbach's alpha and Guttman Split-half coefficient of the 29 items were .93 and .92. respectively. Conclusion: This scale identified as a valid and reliable tool with a high degree of reliability and validity. It can be effectively utilized to assess the degree of suffering of family in palliative care settings.

  • 본 연구는 외상후 스트레스 장애(Posttraumatic stress disorder, PTSD)가 등장한 이후 현대 사회에서 외상 및 고통을 인식하고 이에 대응하는 방법을 조명하였고, 한국 사회의 외상 인식과 대응 방식을 검토하면서 외상 회복을 위한 대안 패러다임의 원칙을 제시하였다. 외상은 기억을 매개로 만성적인 고통을 야기하는 외부 스트레스 사건으로서, 미국정신의학회는 1980년 정신장애 진단 및 통계 편람에서 외상후 스트레스 장애를 공식 인정하였다. 외상후 스트레스 장애 진단의 개발은 피해자에게 도덕적 정당성을 부여하였고, 성공적인 치료 개입의 길을 열었으며, 신경생리학과 인지신경과학 분야에서 연구 성과 축적에 기여하였다. 그러나 이와 동시에, 고통에 대한 인식이 협소해졌고, 기술 개입의 한계를 넘어서는 대응 방안의 중요성이 간과되고 있다. 특히 한국사회의 분리와 부인의 역사적 맥락, 의료 및 전문가 중심 관료주의 기저에는 외상을 개인의 문제로 치환하려는 전략이 발견된다. 따라서 사회적 고통으로서 외상을 인식하고 대안 패러다임을 모색할 필요가 있다. 이 글은 진실 규명과 정의 회복, 회복 주체로서 생존자와 공동체의 책임, 최신 생물-심리학적 성과의 생태학적 적용, 그리고 회복이란 무엇인가에 관한 지속적인 논의를 중시하는 대안 패러다임의 원칙을 제시하였다.

고문폭력 생존자가 반추한 고문의 고통 체험 : 군사정권시대 간첩혐의 희생자를 중심으로 (Lived Experience of Suffering for Victims of Torture : among the suspected espionage agents under the military government)

  • 본 연구는 군사정권시대에 간첩으로 몰려 고문폭력에 희생되었던 개인이 반추한 고문의 고통 체험의 구조를 심층적으로 이해하기 위하여 실행되었다. 자료는 본 연구대상의 체험이 담긴 텍스트에서 수집한 자료를 상호주관적 삶으로서의 역동성과 의미를 잘 나타내주는 파시(Parse)의 현상학적-해석학적 연구방법에 적용하여 분석하였다. 분석 결과 군사정권시대 간첩혐의 고문폭력생존자가 반추한 고문의 고통 체험은 자기(self)를 파괴시키는 가혹성에 주저앉았고, 신(神)이 내린 '죽음에 이르는 형벌' 로 억압받아 그늘진 세월에 묶여 생존하였으나 존재로서의 고유한 인간성을 회복해가는 구조로 나타났다. 그러한 구조를 전환시키면 온갖 폭력과 왜곡으로 간첩이 되었고 가족의 낙인과 시달림을 지켜보는 고통을 받아 억울하나 가려진 존재로 남아 견디어 희생자였던 과거를 보듬으며 응집된 자기(self)를 향한 통합을 추구하는 것으로 파악되었다. 이러한 해석은 파시의 추상성 높은 개념으로 통합하여 상상화, 언어화, 가치화를 가능-제한, 노출-은폐시켜 강화성으로 변형되어가는 과정으로 재해석되었다. 끝으로 고문폭력에 대한 인권적 접근과 치료적 이해를 논의하고 실천적 함의를 다루었다.

고혈압환자의 폭염 적응력 증진을 위한 프로그램 효과 (Effects of an Extreme Heat Adaptation Program in Hypertensive Patients)

  • Purpose: The purpose of this study was to identify the effects of an extreme heat Adaptation Program on the blood pressure, stress response, self-efficacy, and knowledge of management of hypertension and extreme heat of patients who suffered from hypertension. Methods: A quasi-experimental study with a non-equivalent control group pretest-posttest design was used. The data collection period was between July 2 and August 20, 2012. Thirty-seven patients participated in the study (18 in the experimental group and 19 in the control group). Data were analyzed using $X^2$-test, t-test, and Cronbach's alpha coefficients with SPSS/WIN 19.0. Results: Patients who participated in the program showed statistically significant improvements in systolic blood pressure (SBP), self-efficacy, and knowledge of management of hypertension and extreme heat. Conclusion: The results indicate that this extreme heat adaptation program can be utilized for patients suffering from hypertension in order to reduce their SBP and to increase self-efficacy and knowledge of management of hypertension and extreme heat. Therefore, it is recommended that this program be used for elderly patients suffering from chronic disease.

애도 개념개발 - 배우자 사별과정을 중심으로 - (Concept Development of Grief-Focusing on the Process of Spousal Bereavement)

  • Purpose: This study was done to develop the concept of grief focusing on the process of spousal bereavement in Korea. Methods: The Hybrid model was used for analysis according to the 3 phases. An extensive literature review was done for the Theoretical phase. In-depth interviews were conducted with 15 participants whose spouses died within the past 3 years in the Field phase. In the Final analytic phase, the results in the Theoretical and the Field phases were compared, analyzed, and integrated according to the process of grief. Results: The antecedent of the concept of spousal grief was spousal death. The dimensions of grief were classified to inner dimensions related to oneself, relational dimensions related to family and others, and existential dimensions related to the meaning of being. The attributes of grief were physical suffering, decline of cognitive ability, heartbreaking sorrow, expectations and conflicts of a new life, social stigma, dependence on or resentment towards God, etc. The empirical referent of grief was physical, psychological, social, and spiritual health status. The grieving progressed through 3 phases-shock-emancipation, suffering, and integration. Conclusion: Nurses should recognize the importance of their unique position as supporters for grievers, and try to assess individual characteristics and to provide tailored nursing interventions.
