The Comorbidity of Anxiety Disorder in Depressed Patients : A CRESCEND(Clinical Research Center for Depression in Korea) Cohort Study (우울증 환자에서 불안장애의 동반이환 : 우울증 임상연구센터 코호트연구)

  • Purpose : Anxiety disorder and depressive disorder are often comorbid with each other, and the comorbidity is associated with poorer psychiatric outcome, resistance to treatment, increased risk for suicide, greater chance for recurrence. We aimed to investigate the comorbidity of anxiety disorder in Korea. Method : Subjects were total of 867 depressed patients recruited CRESCEND-K multicenter trial. We used SCID (Structured Clinical Interview for DSM - IV) to find comorbidity of anxiety disorders in depressed patient. Results : Of 867 patients, total 8.2% had anxiety disorder. Proportion of anxiety disorder Not Otherwise Specified was 3.5%, panic disorder was 1.7%, generalized anxiety disorder was 1.1%, post traumatic stress disorder was 0.9%, obsessive compulsive disorder was 0.6%, social phobia was 0.4%. Conclusion : In this study, anxiety disorder in depression were measured at a low comorbidity rate in compare to previous studies. Selection bias, use of antidepressants at registration, severity of depression symptoms, and point of SICD administration seems to have affected these results. It is probable that comorbidity evaluation would be more precise if shorter, structured interviews such as M. I.N.I.-Plus were used during first clinical interview for depression diagnosis.

Inner and Outer Resources of Coping in Newly Diagnosed Breast Cancer Patients : Attachment Security and Social Support

  • Objectives The purpose of this study is to evaluate the effects of attachment security, social support and health-related burden in the prediction of psychological distress and the mediation effects of social support and health-related burden in relationship between attachment security and psychological distress. Methods Finally, 161 patients were included for the analysis. Chi-square test and independent samples t-test were used for comparing differences between depressive/anxious group and non-depressive/non-anxious group. For evaluating the relationship among attachment security, social support, psychological distress and health-related burden, structural equation modeling analysis were performed. Results 40.7% and 32.0% of the patients have significant depressive symptoms and anxiety symptoms, respectively. In the analysis for testing the differences between groups who have psychological distress and who have not, there were no significant differences of sociodemographic factors and medical characteristics between groups, except for association between depressive symptoms and type of surgery (p = 0.01). Contrary to sociodemographic and medical characteristics, there were significant differences of health-related burden and two coping resources (attachment security and social support) between groups (all p < 0.01), except for the support from medical team in between anxious group and non-anxious group (p = 0.20). In the structural equation model analysis (Model fit : chi-square/df ratio = 0.8, root mean square error of approximation = 0.000, comparative fit index = 1.000, non-normed fit index =0.991), attachment security and social support emerged as an important predictor of psychopathology. Conclusions Attachment security and social support are important factors affecting the psychological distress. We suggest that individual attachment style and the social support state must be considered to approach the newly diagnosed breast cancer patients with psychological distress.

Factors Relating to Social Physique Anxiety in Adolescent Girls (청소년기 여학생의 사회적 체격불안 관련 요인)

  • Purpose: This descriptive study was to explore the factors relating to Social Physique Anxiety(SPA) in the adolescent girls(N=700). These factors included the biological(level of school[age], period of menstruation, and BMI), the sociocultural(Influence of Mass Media, [IMM], Stereotype of Ideal Female Body[SIFB], type of school, perceived figure, and ideal figure) and the BW control-related factors(experience of bodyweight [BW] control, satisfaction at the result of BW control, and motive of BW control). Method: The sample was 700 girls of the middle and high schools in Jeju-do. The data was collected by the self-reported questionnaire and processed with the SPSS Win 12.0 program. Result: 26% of the variance in SPA was explained by the biological factors(level of school) and the sociocultural factors (perceived figure, ideal figure, type of school, and IMM). SPA, IMM, and SIFB were significantly correlated with each others(r=1,65-2.92, p<.05). The mean SPA scores according to BW control-related factors were significantly different. Conclusion: This results suggest that media education is necessary to reduce or eliminate the adolescent girl's SPA. In addition, SPA needs to be considered as the important concept for the future nursing researches and interventions related to BW control.

A Study on Differences of the Effects of Firefighter's Traumatic Experiences on Their Burnout, Depression, Anxiety and Social Relationships (소방공무원이 지각한 외상경험이 직무소진과 우울, 불안 및 대인관계에 대한 차이 비교 연구)

  • The goal of this study is to investigate the factors affecting firefighter's traumatic experiences and to examine and compare the differences of how those factors contribute to firefighter's burnout, depression, anxiety and social relationships. This study suggests that it is imperative for firefighters to have mindfulness, which brings mental and emotional healing, for their job-satisfaction. To empirically examine the research model of this study, the questionnaire survey was carried out to firefighters in InCheon Metropolitan City for two weeks; from October 19thto November 3rd, 2017. Out of 180 questionnaires, 170were retrieved. Total number of 160 questionnaires are empirically examined this study, excluding10bad survey responses. The t-test was conducted to determine whether there exist differences by gender among traumatic experienced firefighters, and a meaningful difference was found from perceived intrusiveness, a sub factor to traumatic experiences. Male firefighters showed higher mean than that of female firefighters in perceived angle, evasion and intrusiveness. Second, the t-test was conducted to determine whether there exist differences by gender in firefighter's burnout, but no meaningful differences were found. Female firefighters showed higher mean in emotional exhaustion, impersonalization and in decrease in self-achievements. Third, the t-test was conducted to determine whether there exist differences by gender in firefighter's depression and relationships, but no meaningful differences were found.

  • This study is on theoretical foundation for orthodoxy korean style Environmental Education based on oriental ecological thought. The main theme is that various kinds of all the ideas related with ecological thought based on oriental thoughts for EE should be focused into Moderation, the core-concept of oriental confuscian thoughts. The Moderation has not meant just a personal enlightenment, as it has known to general people, it can be developed into social status core-consciousness for regulating the natural resource. So we should focus on this conceptual developing mechanism personal enlightenment into social status for our orthodoxy konean style EE. It can be infered from Taoism, Confuscionism Buddism, especially, Anxiety Consciousness (WOOHWAN) for grand-scale anxiety to public society and universal cosmos. And also I drawed Harbermas-philosophy to provide good means for fortifying this research. His communication theory and restoration of real life for public people communication can be connected to oriental thoughts in the aspect of reciprocative response among the community people. So our orthodoxy korean style EE can be erected on the community-based regionality and their public communication through reciprocative response process. We should try to make this surrounding conditions nature-familiar oriental philosophy has provided from ancient times in the aspect all the traditional thought has been eco-deterministic.

Effect of Participation in Life Sports on Life Satisfaction of the Elderly (생활체육에 참여하는 노인의 삶의 만족도의 영향요인)

  • Purpose: This study examined the effects of physical fitness and aging anxiety on life satisfaction among the elderly. Methods: The participants were 119 elderly. Data collection was conducted from July 1 to 10, 2018. Data were analyzed using descriptive statistics, t-tests, ANOVA, Pearson's correlation coefficients, and multiple linear regression analysis with SPSS software. Results: The mean life satisfaction score was 4.10. Life satisfaction differed significantly according to several general characteristics: education level (p=.014), income (p<.001), living with partner (p=.041), perceived health status (p<.001), and number of chronic diseases (p<.001). Statistically significant positive relationships were observed between life satisfaction with muscle strength (r=.20) and, flexibility (r=.21), and a negative relationship was observed with aging anxiety (r=-.66). A total of 60.0% of life satisfaction was explained by income, perceived health status, muscle strength, flexibility, and aging anxiety. Conclusion: These results can be used to develop life sports programs to improve physical, social, and emotional health promotion with consideration for support to alleviate economic burden. Effect of life sports are evident as a way for healthy and energetic elderly. It should not be a simple exercise program but an integrated plan for improving physical, mental, and social health of the elderly.

A Preliminary Study on Anxiety Inventory for Middle Graders at Elementary School (초등학교 중학년용 불안 검사 개발을 위한 예비 연구)

  • The purpose of this study is to have a preliminary study in order to develop a test which can comprehensively measure anxiety of elementary schoolchildren in middle grade. Following things are conducted to meet this task; first, to look into compositions of the anxiety of elementary schoolchildren in middle grade, to prepare proper questions to measure it and to develop a test; second, to look into whether such questions are fully understood by schoolchildren; third, to look into whether reliability and adequacy thereof are proper or not. Conclusion of the study is as follows; anxiety test of elementary schoolchildren in middle grade showed all the things related to general anxiety, social anxiety, study anxiety, examination anxiety, announcement anxiety. However, there were differences as per subordinate factor and detailed area. And reliability and adequacy of anxiety test for elementary schoolchildren in middle grade were appropriate. and such data could be valuably used for preparing anxiety test. To summarize the study result above, the test developed in this study supplements weak point of existing test instrument and reveals anxiety experienced by elementary schoolchildren in middle grade, and harmoniously composes all the kinds. It is meaningful that the test lowered applicable object to schoolchildren in middle grade who are in anxiety formation stage and also feel anxiety most strongly. In addition, it may be used usefully for living guidance and consultation to efficiently cope with anxiety by measuring base line regarding anxiety of individual schoolchildren in middle grade.

The Crime Effect on Societal Anxiety (범죄발생이 사회불안 지각에 미치는 영향)

  • The study aims to examine the effects of crime on societal anxiety. For this purpose, two studies were conducted. In study 1, the data were collected from 286 students(male 160, female 126) regarding perceived seriousness of crime, estimation of crime occurrence, societal anxiety, and estimation of change in these 3 variables every 3 years from 1993 to 2011. The means of the 3 variables were higher than the midpoint of the scale. And the means of all the 3 variables gradually increased from 1993 to 2011 and the change patterns were very similar. Furthermore, the estimation of crime occurrence and perceived seriousness of crime were significant predictors of societal anxiety. Study 2 was conducted to explore the effect of frequency estimation of the criminal acts on the societal anxiety. The data from 259 subjects(male 141, female 118) were analysed. The occurrence of frequency of 7 types of crimes including 25 criminal acts were estimated and societal anxiety was measured. The results of study 2 showed that only the major crime was significant a predictor of societal anxiety, and among the major crimes, arson was a significant predictor of the dependent variable. Implications and limits of the studies are discussed.

Latent Profile Analysis of Anxiety and Anger Symptoms among Korean Adults during the COVID-19 Pandemic (잠재프로파일분석(LPA)을 통한 코로나 대유행 시기 성인의 불안 및 분노 증상 유형 분석)

  • This study aimed to identify latent profiles of anxiety and anger symptoms experienced by Korean adults during the COVID-19 pandemic and factors that influence group classification. To this end, a latent profile analysis(LPA) was conducted on 1,434 Korean adults from February to March 2021 after the pandemic was announced. First, as a result of confirming the suitability of the latent model, a 5-class-model was found to have the best model fit with 'low anxiety/high anger group(Group 1)', 'low anxiety/low anger group(Group 2)', 'middle anxiety/middle anger group(Group 3)', 'middle anxiety/high anger group(Group 4)', 'high anxiety/high anger group(Group 5)'. Second, gender, economic status, emotion-focused coping, and dysfunctional coping were significant predictors of group classification. Third, there were differences between latent groups in PTSD symptom and suicidal ideation. Finally, discussions and implications of the study were discussed.

Factors Related to Depressive Symptoms Among the Elderly in Urban and Rural Areas (일부 지역사회 노인들의 우울수준과 관련요인)

  • Objectives: This study was performed to determine the levels of depression symptoms among the community elderlies and to reveal its related factors, specifically aimed at revealing psycho-social factors such as social support net-work, sense of self-esteem, state anxiety, interpersonal behavior trait and locus of control. Methods: The interviews were performed during the period from April 1st to June 30th, 2007 to the 790 elderlies in urban and rural areas. Results: The mean scores of depression (CES-D) among all subjects were 16.65±10.97(urban: 18.81±10.31, rural: 14.49±11.20) and they were higher in urban than rural. The level of depression symptoms was influenced by the variables of educational level, with or without spouse, bear for living expenses, sense of satisfaction in daily life. As for health related behaviors, they were influenced by the variables of with or without regular exercise. As for health status, they were influenced by the subjective health status, with or without disability of body, visual acuity, hearing ability, ability of food mastication, urinary incontinence, with or without help in activity of daily living (ADL) and instrumental activity of daily living (IADL). As for social support net-work and social activities, they were influenced by number of offspring, frequency of offspring contents, frequency of going out and activity of hobbies. As for psycho-social factors, they were influenced by sense of self-esteem, state anxiety and social support, especially, the level of depression symptoms was higher influenced by the variables of psycho-social factors than other variables. Conclusions: Lower of the level of depression symptoms among the community elderlies would require development and application of programs to keep under management psycho-social factors as well as looking for ways to promoting the present health status.
