Non-small cell lung cancer (NSCLC) is the leading cause of cancer related deaths. Most patients were presented with advanced disease at the time of diagnosis. In advanced NSCLC, it is almost impossible to anticipate complete remission by using only cytotoxic chemotherapy or molecularly targeted agents. In our case, two patients were diagnosed as advanced NSCLC and received chemotherapy. They achieved complete response (CR). After finishing treatment, disease recurred. They were retreated with the same regimens and achieved second CR. Until now, they have received each regimen, continuously, and the CR state has been maintained.
고형암에서의 종양용해증후군은 드물지만 발생하면 높은 사망률을 보이는 치명적인 합병증으로 외국에서는 지금까지 약 50여명의 증례가 보고되어 왔다. 우리 나라에서도 종종 발생한다고 알려져 있지만 실제로 뒷받침 할만한 증례를 찾아 보기가 힘든 실정이었다. 이에 저자들은 소세포폐암에서 항암화학요법 도중 발생한 치명적 종양 용해증후군 1예를 경험하였기에 문헌 고찰과 함께 이를 보고하는 바이다.
Background: To evaluate the value of combined detection of serum carcinoembryonic antigen (CEA), cytokeratin 19 fragment (CYFRA21-1), and carbohydrateantigen 125 (CA125) for the clinical diagnosis of nonsmall cell lung cancer (NSCLC). Materials and Methods: Serum CEA, CYFRA21-1 and CA125 were assessed in 140 patients with NSCLC, 90 patients with benign lung disease and 90 normal control subjects, and differences of expression were compared in each group, and joint effects of these tumor markers in the diagnosis of NSCLC were analyzed. Results: Serum CEA, CYFRA21-1 and CA125 in patients with NSCLC were significantly higher than those with benign lung disease and normal controls (P<0.05). The sensitivity of CEA, CYFRA21-1 and CA125 were 49.45%, 59.67%, and 44.87% respectively. As expected, combinations of these tumor markers improved their sensitivity for NSCLC. The combined detection of CEA + CYFRA21-1 was the most cost-effective combination which had higher sensitivity and specificity in NSCLC. Elevation of serum CEA and CYFRA21-1 was significantly associated with pathological types (P<0.05) and elevation of serum CEA, CYFRA21-1 and CA125 was significantly associated with TNM staging (P<0.05). Conclusions: Single measurement of CEA, CYFRA21-1 and CA125 is of diagnostic value in the diagnosis of lung cancer, and a joint detection of these three tumor markers, could greatly improve the sensitivity of diagnosis on NSCLC. Combined detection of CEA + CYFRA21-1 proved to be the most economic and practical strategy in diagnosis of NSCLC, which can be used to screen the high-risk group.
Metformin has been widely used as an antidiabetic drug, and reported to inhibit cell proliferation in many cancers including non-small cell lung cancer (NSCLC). In NSCLC cells, metformin suppresses PI3K/AKT/mTOR signaling pathway, but effect of metformin on RAS/RAF/MEK/ERK signaling pathway is controversial; several studies showed the inhibition of ERK activity, while others demonstrated the activation of ERK in response to metformin exposure. Metformin-induced activation of ERK is therapeutically important, since metformin could enhance cell proliferation through RAS/RAF/MEK/ERK pathway and lead to impairment of its anticancer activity suppressing PI3K/AKT/mTOR pathway, requiring blockade of both signaling pathways for more efficient antitumor effect. The present study tested the combination therapy of metformin and trametinib by monitoring the alterations of regulatory effector proteins of cell signaling pathways and the effect of the combination on cell viability in NCI-H2087 NSCLC cells with NRAS and BRAF mutations. We show that metformin alone blocks PI3K/AKT/mTOR signaling pathway but induces the activation and phosphorylation of ERK. The combination therapy synergistically decreased cell viability in treatment with low doses of two drugs, while it gave antagonistic effect with high doses. These findings suggest that the efficacy of metformin and trametinib combination therapy may depend on the alteration of ERK activity induced by metformin and specific cellular context of cancer cells.
131 Patients of non·small cell lung cancer treated with irradiation at Yonsei Canter Center from Jan. 1971 to Dec. 1980 were retrospectively analysed. Overall 5 year survival rate was $7\%$ in 117 cases, treated with radiotherapy alone and $33\%$ in 14 Cases, treated with surgery & postoperative ratiotherapy. Their median survival was 9.6 months in the former, while 11.1 months in the latter. The patients treated with radical aim achieved $10\%$ in 5 YSR and with palliative aim, $0\%$. Also, survival according to histolocial classification and staging was obtained. Treatment failure was mostly distant failure (40 cases/49 cases), and within 6 months (34cases/49cases).
본 증례에서 환자는 폐렴 치료 중 갑자기 발생한 복통을 주소로 급성 충수염을 진단받고 수술을 시행 받았다. 그 후 조직 검사 결과 소세포암으로 진단되었고 폐병변에 대해 추가적인 검사 결과 소세포폐암이 진단된 경우이다. 저자들은 소세포폐암의 충수 전이를 국내에서 처음으로 경험하여 문헌고찰과 함께 보고하는 바이다.

One of the major limitations in using adenoviral vector for gene therapy is inefficient infection of host cells. The presence of coxsackievirus and adenovirus receptor (CAR) and ${\alpha}$-integrin on cell surfaces is required for efficient adenovirus infection. In this study, we investigated the effect of trichostatin A, a histone deacetylase inhibitor, on transfection efficiency after transduction of adenovirus mediated p16$\^$INK4a/ gene transfer. In our previous study, p16$\^$INK4a/ tumor suppressor gene transfer in the non-small cell lung cancer cells (A549 cells) by transduction of recombinant adenovirus (Ad5CMV-p16) resulted in significant inhibition of cancer cell proliferation. Objective: The aim of this study was to determine a method of dose prescription that minimizes normal tissue irradiation outside the planning target volume (PTV) during stereotactic body radiotherapy (SBRT) for patients with non-small cell lung cancer. Methods: Previous research and patients with typical T1 lung tumors with peripheral lesions in the lung were selected for analysis. A PTV and several organs at risk (OARs) were constructed for the dose calculated; six treatment plans employing intensity modulated radiotherapy (IMRT) were produced, in which the dose was prescribed to encompass the PTV, with the prescription isodose level (PIL) set at 50, 60, 70, 80, 90 or 95% of the isocenter dose. Additionally, four OARs around the PTV were constructed to evaluate the dose received in adjacent tissues. Results: The use of higher PILs for SBRT resulted in improved sparing of OARs, with the exception of the volume of lung treated with a lower dose. Conclusions: The use of lower PILs is likely to create significant inhomogeneity of the dose delivered to the target, which may be beneficial for the control of tumors with poor conformity indices.

The technique of serological analysis of antigens by recombinant cDNA expression library (SEREX) uses autologous patient sera as a screening probe to isolate tumor-associated antigens for various tumor types. Isolation of tumor-associated antigens that are specifically reactive with patient sera, but not with normal sera, is important to avoid false-positive and autoimmunogenic antigens for the cancer immunotherapy. Here, we describe a selection methodology to isolate patient sera-specific antigens from a yeast surface-expressed cDNA library constructed from 15 patient lung tissues with non-small cell lung cancer (NSCLC). Several rounds of positive selection using patient sera alone as a screening probe isolated clones exhibiting comparable reactivity with both patient and normal sera. However, the combination of negative selection with allogeneic normal sera to remove antigens reactive with normal sera and subsequent positive selection with patient sera efficiently enriched patient sera-specific antigens. Using the selection methodology described here, we isolated 3 known and 5 unknown proteins, which have not been isolated previously, but and potentially associated with NSCLC.
그동안 폐암의 발생에 관여하는 여러가지 유전자의 이상이 보고되어 왔으나 모든 종류의 폐암에서 보이는 비억제적 성장(unconkolle6 growth)을 대변할 수 있는 유전자적 종양 표시자(molecular tumor marker)는 보고된 바 없었다. 최근 유전자(chromosome)의 말단부에 위치한 특이한 구조인 telomere가 세포의 노화나 증식정도 (proliferative activity)에 따라 그 구조, 특히 TrAGGG 반복 구조가 변한다는 것이 알려진 바 있다. 따라서 telomere의 반복 구조를 만드는 효소인 telomerase의 활성도는 대상 세포의 증식 상황이나 증식 가능성을 나타내는 표시 자로서의 기능이 있다고 추정된다. 따라서 이는 종양의 진단은 물론 대상 환자의 향후 예후를 판 단하는 데도 좋은 지표로 이용될 수 있다. 12개의 다양한 비소세포폐암 세포주와 수술로 적출된 41명의 비세포계암 환자의 종양 조직에 대해 nROolymerase chain reaction)을 기초로 한 TRAP assay를 이용하여 telomeiase활성도를 측정하였다. 대조군으로는 동시에 적출된, 종양으로부터 가장 먼 위치의 정상 폐조직을 이용하였다. 12개 전 종양 세포주는 물론 대부분의 종양 조직(94%)에서, 성별, 연령, 세포 병리학적 subtype, 암기(stage) 등과 관련이 언이, telomerase의 활성도가 측정되었으며 정상이라고 간주된 조직에서는 5명으로 부터 채취한 조직에서만 미약하게 telomerase활성도가 관찰되었다. 예후에 연관지어 telomerase활성도의 의미는 telmerase활성도가 보이지 않는 종글이 극히 적었고 또한 추적 관찰 기간이 짧아 판정할 수 없었다. Telomere의 길이의 변화는의미를 판정키 어려웠다. 이상의 결과는 비소세포폐암에 있어 telomerase활성도의 변화가 암발생에 아주 중요한 과정일 수 있다는 추정을 가능하게 하며 telomerase활성도의 측정이 종양의 진 단에 유용하게 이용될 수 있음을 확인시켜 주는 것이라 사료된다.
