A Study on Effective Source-Skin Distance using Phantom in Electron Beam Therapy

  • In this study, for 6-20 MeV electron beam energy occurring in a linear accelerator, the authors attempted to investigate the relation between the effective source-skin distance and the relation between the radiation field and the effective source-skin distance. The equipment used included a 6-20 MeV electron beam from a linear accelerator, and the distance was measured by a ionization chamber targeting the solid phantom. The measurement method for the effective source-skin distance according to the size of the radiation field changes the source-skin distance (100, 105, 110, 115 cm) for the electron beam energy (6, 9, 12, 16, 20 MeV). The effective source-skin distance was measured using the method proposed by Faiz Khan, measuring the dose according to each radiation field ($6{\times}6$, $10{\times}10$, $15{\times}150$, $20{\times}20cm^2$) at the maximum dose depth (1.3, 2.05, 2.7, 2.45, 1.8 cm, respectively) of each energy. In addition, the effective source-skin distance when cut-out blocks ($6{\times}6$, $10{\times}10$, $15{\times}15cm^2$) were used and the effective source-skin distance when they were not used, was measured and compared. The research results showed that the effective source-skin distance was increased according to the increase of the radiation field at the same amount of energy. In addition, the minimum distance was 60.4 cm when the 6 MeV electron beams were used with $6{\times}6$ cut-out blocks and the maximum distance was 87.2 cm when the 6 MeV electron beams were used with $20{\times}20$ cut-out blocks; thus, the largest difference between both of these was 26.8 cm. When comparing the before and after the using the $6{\times}6$ cut-out block, the difference between both was 8.2 cm in 6 MeV electron beam energy and was 2.1 cm in 20 MeV. Thus, the results showed that the difference was reduced according to an increase in the energy. In addition, in the comparative experiments performed by changing the size of the cut-out block at 6 MeV, the results showed that the source-skin distance was 8.2 cm when the size of the cut-out block was $6{\times}6$, 2.5 cm when the size of the cut-out block was $10{\times}10$, and 21.4 cm when the size of the cut-out block $15{\times}15$. In conclusion, it is recommended that the actual measurement is used for each energy and radiation field in the clinical dose measurement and for the measurement of the effective source-skin distance using cut-out blocks.

아유르베다 체질에 따른 피부 유형 분석 (Quantitative Evaluation of Skin Condition According to Ayurvedic Constitution Classification)

  • 본 논문은 89명의 여대생을 대상으로 아유르베다의 세 가지 체질인 바타, 피타, 카파에 따른 얼굴 피부의 수분, 유분, pH의 값을 측정하여 비교하였다. 아유르베다 체질에 따른 피부 상태를 알아보기 위하여 분석한 결과 수분의 경우 피타, 카파, 바타의 순으로(p<0.001) 유분의 경우 카파, 피타, 바타의 순으로 (p<0.001, p<0.01) pH의 경우 카파, 피타, 바타의 순으로 (p<0.01) 통계적으로 유의한 차이가 있었다. 앞으로의 연구에서는 피부미용학적 관점에서 아유르베다 체질에 대한 보다 체계적이고 과학적인 연구가 진행되어야 할 것이다.

  • 얼굴 피부는 신체 건강을 표현할 뿐만 아니라 매력을 결정하는 요소 중 하나이다. 특히 윤기 나는 피부는 대부분의 사람들이 선호하며 윤기 나는 피부 표현을 위해 다양한 화장품들이 만들어져왔다. 하지만 윤기 있는 피부는 주관적 평가이기 때문에 기름진 피부의 경우 기존의 피부 윤기 평가 방법으로는 명확한 윤기 있는 피부로 평가하기가 어렵다. 더욱이 윤기와 유사하게 광이 있지만 부정적으로 평가되는 번들거리는 피부와 명확한 분류 기준 또한 없는 상황이다. 본 연구에서는 피부측정 파라미터 5개[피부 광채, 피부 보습, 피부 투명도, 피부 피지, 안면 이미지]를 측정하였고, 연구대상자들이 촬영된 안면 이미지를 근거하여 각각의 윤기 있는 피부와 번들거리는 피부의 이미지를 선별하여 피부 측정의 정량화를 통해 비교 분석하였다. 그 결과 번들거리는 피부에서 피부 광채 및 피지 증가, 투명도 감소, 보습 감소를 확인하였다. 그러나 윤기 있는 피부의 경우 피부 광채, 피지, 투명도는 유사한 패턴을 보였지만 보습이 유의차 있게 증가하는 결과를 보였다. 본 연구에서는 기기측정 및 정량적 평가를 통해서 윤기 있는 피부와 번들거리는 피부를 분류하는 객관적 평가 기준을 제시하고자 한다.

A study on the traceability and uncertainty of skin hydration measurement on the sole of the foot

  • Objectives: In order to accurately measure skin moisture and accumulate normal skin moisture data, analyzing the uncertainty of the skin moisture measurement and establishing standard reference data is essential. It helps to evaluate the skin's condition and has great significance in diagnosis and treatment. In this study, the traceability of the equipment and the uncertainty of the results were analyzed for their potential use in the field of Korean medicine. The data was collected measuring skin moisture on the sole of the foot. Methods: One professional measured the moisture of the adult male's foot 10 times following a determined protocol. The standard uncertainty of repeated measurements was calculated from the mean value of 10 repeated measurements. Ten trainee participants also measured the adult male's foot following a determined protocol. The standard uncertainty by tester was calculated from the value of repeated measurements. Results: As a result of analyzing the difference between the mean and standard uncertainties in both the expert and trainees, it was confirmed that the variance in this study did not satisfy the normal distribution. In addition, the Mann-Whitney U-test was carried out, and it was found that there was no significant difference in the measured values of the two groups. The authorized uncertainty of measurements and traceability of all the equipment was not confirmed. Conclusions: This paper establishes the basis for later measurement-equipment research to provide the objective indicators to approach the dryness of plantar skin from dehydration, the Korean medicine perspective.

왼쪽 유방암 세기변조방사선 치료시 Skin Flash 적용에 대한 유용성 평가 (Evaluating efficiency of application the skin flash for left breast IMRT.)

  • 목 적 : 왼쪽 유방암 세기변조방사선 치료 시 Skin flash를 적용함에 있어 치료계획의 변화를 알아보고, 적용하지 않았을 경우와 적용 했을 시 피부선량의 변화를 비교해 보고자 하였다. 또한 Skin flash를 다양한 두께별로 적용하여 두께별 피부선량의 변화를 비교, 분석해 보고, 적절한 Skin flash의 적용에 대해 알아보고자 한다. 대상 및 방법 : 실험대상은 Anthropomorphic phantom을 이용하였고, 치료계획을 위한 영상획득은 전산화 단층촬영(CT)으로 하였다. 전산화치료계획은 Eclipse(ver 13.7.16, Varian, USA)의 RTP 시스템을 이용하여 2 field hybrid IMRT와 6 field static IMRT의 두 가지 치료계획을 하였다. 위의 치료계획을 바탕으로 각 치료계획마다 Skin flash 두께를 0.5 cm, 1.0 cm, 1.5 cm, 2.0 cm, 2.5 cm으로 변경하여 추가치료계획을 수립하였고, 추가치료계획에 대해 총 MU값과 최대선량의 변화를 알아보았다. 치료 장비는 $VitalBeam^{TM}$(Varian Medical System, USA)의 6 MV를 이용하였다. 측정 장비는 MOSFET(metal oxide semiconductor field-effect transistor)을 이용하였고, 측정지점은 phantom의 왼쪽 유방의 센터를 기준으로 위(1번), 중간(2번), 아래(3번) 세 부위에 위치시켜 피부선량을 측정하였다. 그리고 인위적으로 좌표를 0.5cm 내측(medial), 외측(lateral)으로 이동시켜 피부선량의 변화를 비교 분석하였다. 결 과 : Skin flash를 적용하지 않은 치료계획 2F-hIMRT, 6F-sIMRT의 측정값을 기준선량으로 하였다. 2F-hIMRT의 1번 측정점 206.7 cGy, 2번 186.7 cGy, 3번 222 cGy였고, 6F-sIMRT의 기준값은 1번 192 cGy, 2번 213 cGy, 3번 215 cGy로 측정되었다. 이 기준값들과 비교하여 2F-hIMRT에서 1번 측정점은 Skin flash 2.0 cm, 2.5 cm 적용시 평균 261.3 cGy로 26.1 %diff로 가장 많은 선량차를 보였고, 2번 측정점은 Skin flash 2.0 cm일 때 평균 197.3 cGy, 5.6 %diff의 선량차가 났고, 3번 측정점은 Skin flash 2.5 cm일 때 최대 245.3 cGy, 10.5 %diff였다. 6F-sIMRT에서는 1번 측정지점이 Skin flash 2.0 cm 적용시에 216.3 cGy, 12.7 %diff로 가장 큰 선량차이를 보였고 측정지점별로 Skin flash 2.5 cm 적용시가 아닌 2.0 cm 적용시 선량차가 가장 크게 나타났다. 2F-hIMRT, 6F-sIMRT에서 각각 내측 0.5 cm 이동 후 Skin flash를 사용하지 않았을 때 측정치는 2F는 기준값과 비교시 1번, 3번 측정지점에서 -75.2 %diff, -70.1 %diff 차이였고, 6F는 1번, 2번, 3번 측정 지점에서 -14.8, -12.5, -21.0 %diff로 처방선량에 미치지 못하는 저선량이 측정되었다. 두 치료계획 모두 Skin flash 두께가 커질수록 총 MU와 최대선량이 증가하는 것으로 나타났고, 몇몇 결과를 제외하면 전체적으로 %diff도 증가했다. 모든 조건에서 0.5 cm의 Skin flash 두께 사용이 기준값의 피부선량과 약 20 % 이하의 차이로 가장 적은 것으로 나타났다. 결 론 : 결과에 따라 Skin flash의 두께를 0.5 cm으로 최소화 하는 것이 최대선량이나 MU의 증가를 최소화할 수 있었고, 기준이 되는 치료계획과도 가장 적은 피부선량차이를 보였기에 0.5 cm으로 최소화 하여 적용하는 것이 가장 이상적이라 할 수 있을 것이다. 또한 Skin flash 두께를 무한정 크게 한다고 해서 MU나 최대선량, 피부선량차가 증가하지 않고 수렴한다는 것을 알 수 있었다. 그러므로 유방암 환자의 호흡에 의한 PTV의 변화와 여러 오차의 인자들을 고려한다면 0.5~1.0 cm Skin flash 범위에서 Skin flash를 사용한다면 적용하지 않는 것보다는 더 많은 이점이 있을 것으로 사료된다.

Color Texture Analysis as a Tool for Quantitative Evaluation of Radiation-Induced Skin Injuries

  • Background: Color texture analysis was applied as a tool for quantitative evaluation of radiation-induced skin injuries. Materials and Methods: We prospectively selected 20 breast cancer patients who underwent whole-breast radiotherapy after breast-conserving surgery. Color images of skin surfaces for irradiated breasts were obtained by using a mobile skin analyzer. The first skin measurement was performed before the first fraction of radiotherapy, and the subsequent measurement was conducted approximately 10 days after the completion of the entire series of radiotherapy sessions. For comparison, color images of the skin surface for the unirradiated breasts were measured similarly. For each color image, six co-occurrence matrices (red-green [RG], red-blue [RB], and green-blue [GB] from color channels, red [R], green [G], blue [B] from gray channels) can be generated. Four textural features (contrast, correlation, energy, and homogeneity) were calculated for each co-occurrence matrix. Finally, several statistical analyses were used to investigate the performance of the color textural parameters to objectively evaluate the radiation-induced skin damage. Results and Discussion: For the R channel from the gray channel, the differences in the values between the irradiated and unirradiated skin were larger than those of the G and B channels. In addition, for the RG and RB channels, where R was considered in the color channel, the differences were larger than those in the GB channel. When comparing the relative values between gray and color channels, the 'contrast' values for the RG and RB channels were approximately two times greater than those for the R channel for irradiated skin. In contrast, there were no noticeable differences for unirradiated skin. Conclusion: The utilization of color texture analysis has shown promising results in evaluating the severity of skin damage caused by radiation. All textural parameters of the RG and RB co-occurrence matrices could be potential indicators of the extent of skin damage caused by radiation.

밀착형 셔츠 설계를 위한 피부변형의 3차원 측정 (3D Measurement of Skin Deformation for the Design of a Tight-fitting Torso Pattern)

  • This study develops tight-fitting torso patterns for performance garments by taking into account the skin deformation generated directly from a 3D scan during arm movements. The skin deformation caused during the arm movements was scanned after scanning the skin surface stamped with a circle. To create a torso pattern in response to skin deformation, the ratio and direction of the skin deformation were first measured and analyzed so that the 3D human body could be segmented. After translating, the 3D skin surface was segmented into 2D flat patterns, designing nude patterns and reducing them as well as tight-fitting shirts: the skin deformation segment shirts were made in response to the skin deformation. The features of the fabric deformation and the garment pressure were analyzed and evaluated. In comparison with a clothing construction segment shirt, the diameter of the skin deformation segment shirt was smaller as well the ratios of extension and reduction was less. The garment pressure of the skin deformation segment shirt was higher. The skin deformation segment shirt fitted more tightly compared to a clothing construction segment shirt as it covered the body more thoroughly and was as comfortable as the other shirts with less fabric deformation made as the body moved.

유방암환자의 세기조절방사선치료에서 피부선량 평가 (Evaluation of Skin Dose of Intensity Modulated Radiation Therapy in Breast Cancer Patients)

  • 유방암 환자의 유방보존절제술 후 방사선치료를 시행하는 경우에서 비껴방향치료방법과 세기조절방사선치료를 시행한 두 경우에 피부에 조사되는 표피 흡수선량을 비교, 분석하여 유방암 환자에서 세기조절방사선치료시 피부선량의 특성을 살펴보았다. 비껴방향치료방법과 세기조절방사선치료에 대한 두 가지 방법으로 치료계획을 시행하여 유방 피부에 조사되는 계산선량과 TLD로 측정한 측정선량을 비교하였다. 선량측정은 유두가 있는 지점을 지나도록 몸의 중심에서 가장자리 방향으로 1 cm 간격으로 피부 흡수선량을 측정하였다. 비껴방향방사선치료에서 PTV에 180 cGy 방사선량을 계획할 때, 유방 표피에서 계산선량은 103.6 cGy에서 155.2 cGy 사이를 나타내었고, 측정선량은 107.5 cGy에서 156.2 cGy 사이를 나타내었으며, 중심부위의 피부선량이 가장자리 피부선량보다 최대 1.45배 많이 조사되었다. 세기조절방사선치료에서는 PTV에 180 cGy 방사선량을 계획할 때, 유방 표피에서 계산선량은 9.8 cGy에서 80.2 cGy사이를 나타내었고, 측정선량은 8.9 cGy에서 77.2 cGy사이를 나타내었으며, 중심부위의 피부선량이 가장자리 피부선량보다 최대 0.23배 적게 조사되었다. 비껴방향방사선치료에서 보다 세기조절방사선치료에서 피부에 조사되는 방사선량이 평균적으로 3.5배 적어서 방사선 치료에 의한 피부 위해를 줄일 수 있다.

전통 한방처방을 이용한 기초 한방화장품에 대한 임상적 연구 (The Clinical Study on Cosmetics by Using Traditional Herbal Medicine)

  • During a clinical test with Korean herbal cosmetics, significant data and related results are summarized as following: 1. The questionnaire resulted the subjects who used Korean herbal cosmetics showed significant subjective improvement. 2. $11{\%}$ of the subjects who used Korean herbal skin lotion replied "very good" , $60{\%}$ "good" , and $29{\%}$ "so so". 3. In the questionnaire about how often the subjects who used Korean herbal skin lotion experienced skin irritation, $91{\%}$ of the subjects replied they didn't experience any skin irritation 4. The subjects who used oily & combinational cosmetics showed more significant result than the placebo group in the questionnaire about how they feel when using emulsion. 5. In the questionnaire about how they feel when using emulsion. $11{\%}$ of all the subjects replied "very good" , 60{\%}$ "good" , and $29{\%}$ "so so". 6. In the questionnaire about how they feel when using essence $18{\%}$ of all the subjects replied "very good" , $56{\%}$ "good" , and $27{\%}$ "so so". 7. In the questionnaire about how often the subjects who used skin lotion experienced skin irritation, $100{\%}$ of the subjects replied there was no skin irritation. 8. the subjects who used Korean herbal cosmetics showed significant result in the questionnaire about how much moisture they feel when using cream. 9. The subjects who used neutral & dry cosmetics showed more significant result than the placebo group in the questionnaire about if the cosmetics spread well when using emulsion. 10. The subjects who used oily & combinational cosmetics showed more significant result than the placebo group in the questionnaire about how much moisture they feel when using cosmetics. 11. In the questionnaire about how they feel when using cream $13{\%}$ of all the subjects replied "very good", $49{\%}$ "good", and $2{\%}$ "so so". 12. In the questionnaire about how often the subjects who used cream lotion experienced skin irritation, $96{\%}$ of the subjects replied there was no skin irritation. 13. In the measurement of coreometer, the subjects who used oily & combinational cosmetics showed significant result(measurement area : indang). Especially in the area of sungjang there is significant difference between the subjects group and placebo group. 14. In the measurement of skin-ph-meter, there was no significant change in all group. 15. in the measurement of melanin-erythema index meter, the subjects who used cosmetics which doesn't contain Korean herbs showed significant change and made significant difference in comparison to the subjects who used Korean herbal cosmetics.

Fundamental Study of nanoDot OSL Dosimeters for Entrance Skin Dose Measurement in Diagnostic X-ray Examinations

  • Background: In order to manage the patient exposure dose in X-ray diagnosis, it is preferred to evaluate the entrance skin dose; although there are some evaluations about entrance skin dose, a small number of report has been published for direct measurement of patient. We think that a small-type optically stimulated luminescence (OSL) dosimeter, named nanoDot, can achieve a direct measurement. For evaluations, the corrections of angular and energy dependences play an important role. In this study, we aimed to evaluate the angular and the energy dependences of nanoDot. Materials and Methods: We used commercially available X-ray diagnostic equipment. For angular dependence measurement, a relative response of every 15 degrees of nanoDot was measured in 40-140 kV X-ray. And for energy dependence measurement, mono-energetic characteristic X-rays were generated using several materials by irradiating the diagnostic X-rays, and the nanoDot was irradiated by the characteristic X-rays. We evaluated the measured response in an energy range of 8.1-75.5 keV. In addition, we performed Monte-Carlo simulation to compare experimental results. Results and Discussion: The experimental results were in good agreement with those of Monte-Carlo simulation. The angular dependence of nanoDot was almost steady with the response of 0 degrees except for 90 and 270 degrees. Furthermore, we found that difference of the response of nanoDot, where the nanoDot was irradiated from the randomly set directions, was estimated to be at most 5%. On the other hand, the response of nanoDot varies with the energy of incident X-rays; slightly increased to 20 keV and gradually decreased to 80 keV. These results are valuable to perform the precise evaluation of entrance skin dose with nanoDot in X-ray diagnosis. Conclusion: The influence of angular dependence and energy dependence in X-ray diagnosis is not so large, and the nanoDot OSL dosimeter is considered to be suitable dosimeter for direct measurement of entrance surface dose of patient.
