The U-Net architecture-based segmentation models attained remarkable performance in numerous medical image segmentation missions like skin lesion segmentation. Nevertheless, the resolution gradually decreases and the loss of spatial information increases with deeper network. The fusion of adjacent layers is not enough to make up for the lost spatial information, thus resulting in errors of segmentation boundary so as to decline the accuracy of segmentation. To tackle the issue, we propose a new deep learning-based segmentation model. In the decoding stage, the feature channels of each decoding unit are concatenated with all the feature channels of the upper coding unit. Which is done in order to ensure the segmentation effect by integrating spatial and semantic information, and promotes the robustness and generalization of our model by combining the atrous spatial pyramid pooling (ASPP) module and channel attention module (CAM). Extensive experiments on ISIC2016 and ISIC2017 common datasets proved that our model implements well and outperforms compared segmentation models for skin lesion segmentation.
The purpose of this study was to investigate usefulness of the pine needles extract on Atopic Dermatitis(AD)-like skin lesions. To investigate the effect of pine needles extract in vivo, atopic dermatitis (NC/Nga) mice using DNCB (2.4-Dinitrochlorbenzene) was used. NC/Nga mice were challenged with DNCB during 2 weeks to develope AD-like skin lesion. After that, pine needles extract was applied to AD-like skin lesion on the backs of the NC/Nga mice during 3 weeks. The efficacy of pine needles extract in the NC/Nga mice was evaluated by measurement of the skin lesion severity(NC mouse score), the serum IgE level, epidermal thickness changes, and mast cell number. Blood was collected from the retro-orbital area and the level of IgE in the blood was measured. The epidermal thickness and mast cell number were observed by microscopic method after H&E stain. The serum IgE levels were decreased after treatment with pine needles extract. The epidermal thickness and mast cell number were decreased after treatment with pine needles extract. To conclude, the topical application of pine needles extract suppressed the progression of AD-like skin lesion.
A 66-year-old man visited author's institute complaining of the swelling on the submandibular gland area. Clinically, the exophytic mass penetrated the skin of the submandibular area. On MRI, the lesion occupied the left submandibular space and extended downward, protruding exterior to the subcutaneous fat layer, but the center of the lesion was located on the side of the skin and the growth exterior to the skin was prominent. Demarcation of the lesion and the submandibular gland was unclear. Histopathologically the epithelial nests and keratin production were seen, then the biopsy result was squamous cell carcinoma. The stroma of lesion showed a myxoid characteristic and some ducts showed metaplasia of the ductal cells, which suggested the gland-origin carcinoma. However, lots of keratin production and carcinomatous change of cells continuous to the normal epithelium of the skin, the skin-origin carcinoma invading into the submandibular gland area could not be excluded.
A Morel-Lavallée lesion is a post-traumatic closed degloving soft tissue injury after blunt trauma. Infection and skin necrosis frequently occur if it is not treated properly in the early stages. However, there is no clearly established treatment algorithm. In the acute stage, it is mainly treated with aspiration, simple compression, and incisional drainage. In the chronic stage, sclerotherapy is usually performed. If skin necrosis develops, the necrotic tissue is resected and a skin graft is needed. We describe a case of acute Morel-Lavallée lesion in the buttock region that was treated with limited incisional drainage and negative-pressure wound therapy, and also present a review of the literature.

A 7-year-old intact male Maltese dog was presented with firm, raised, erythematous, alopecic, and pruritic skin lesions on the right dorsal distal metatarsal region and the tail. The skin lesion started a year ago with a crust-like lesion on the right dorsal distal metatarsal region which gradually swelled as the patient continued to lick and chew without healing. Recently, similar lesions occurred on the tail. Based on the licking history and ruling out other diseases through skin examination, acral lick dermatitis (ALD) was diagnosed. Treatment included antidepressants, preventive antibiotics, topical corticosteroid, and wearing Elizabethan collar as a physical barrier. After three weeks, the lesion on the hind limb healed entirely. However, the tail lesion recurred as it was able to lick the tail again with a type of an E-collar. This case shows that it is essential to block the contact lesion and provide medical treatment until the lesion has been completely resolved for successful management of ALD.
Porocarcinoma (PC) and basal cell carcinoma (BCC) are distinct skin cancers. Few studies have documented the occurrence of two concurrent types of skin cancers, and to the best of our knowledge, this represents the inaugural report of such a coexisting lesion arising from a capillary malformation. Herein, we report a case of concurrent PC and BCC presenting with capillary malformation. A 93-year-old woman visited our hospital with a protruding mass in her right nasal ala that appeared as a capillary malformation. A biopsy was performed on the skin lesion, and BCC was diagnosed. A wide excision was performed. Permanent biopsy revealed that the skin lesion was a PC with basal cells and squamous differentiation. The safety margin of the deep tissue margin was < 0.1 cm; however, considering the advanced age of the patient, further excision was deemed to not possess any benefits. This case illustrates the importance of recognizing the possibility of multiple skin cancers, even in patients with benign lesions such as capillary malformations. The rarity of this presentation highlights the importance of thorough investigation and histopathological examination of skin lesions in guiding appropriate surgical excision.
The malignant melanoma accounts for about 1 to 3% of the total malignant tumor in the West, especially in the US, it is a disease that causes more than 9,000 deaths each year. Generally, skin lesions are difficult to detect the features through photography. In this paper, we propose a computer-aided diagnosis algorithm based on deep learning for classification of malignant melanoma and benign skin tumor in RGB channel skin images. The proposed deep learning model configures the tumor lesion segmentation model and a classification model of malignant melanoma. First, U-Net was used to segment a skin lesion area in the dermoscopic image. We could implement algorithms to classify malignant melanoma and benign tumor using skin lesion image and results of expert's labeling in ResNet. The U-Net model obtained a dice similarity coefficient of 83.45% compared with results of expert's labeling. The classification accuracy of malignant melanoma obtained the 83.06%. As the result, it is expected that the proposed artificial intelligence algorithm will utilize as a computer-aided diagnosis algorithm and help to detect malignant melanoma at an early stage.
Porcine juvenile pustular psoriasiform dermatitis (PJPPD) is a disease of young pigs and characterized by nonpruritic round eruption of skin. The cause of this disease is yet undetermined but is presumed to be genetic predisposition. There may be few opportunities for veterinarian to detect this disease compared with actual situation in field because these lesions resolve spontaneously in two months. The authors detected spontaneous PJPPD case and performed clinical and pathological studies on three pigs from one farm. The specific skin lesions were observed in the forty-day old pigs of mixed breed, which were produced by the sows received semen from the same boar, restrictively. However, there was no skin lesion of pigs in suckling or fattening periods. Grossly, lesions were commonly found on the ventral abdominal part as a papule and were spreaded to the skin of whole body. With the spreading of lesions centrifugally, skin was showed as a umbilicated plaques or mosaic pattern with a few pustules or crusts. Microscopically, the most prominent lesion was the psoriasiform hyperplasia with acanthosis, down growth of rete ridges, exocytosis of eosinophils and neutrophils, ballooning degeneration of superficial epidermis, and koilocytic degeneration of keratinocytes. Additionally, there were moderate dermal edema and severe mixed cellular infiltration, especially eosinophils. No infectious agent which can cause the skin lesion, was detected or cultured, and no lesion caused by infectious agents was also observed, pathologically. With pathological results of this study, it is supposed that pathogenesis or severity of PJPPD may be related to the infiltration of eosinophil or hypersensitivity.
Usually, the skin pigmentation detection and diagnosis are made by clinicians. In this process it is subjective and non-quantitative. We develop an approach to detect and measure the different pigmentation lesions base on computer vision technology. In the paper we study several usually used skin-detecting color space like HSV, YCbCr and normalized RGB. We compare their performance with illumination influence for detecting the pigmentation lesions better. Base on a relatively stable color space, we propose an approach which is RGB channels vector difference characteristic for the detection. After the object region detection, we also use the difference to measure the difference between the lesion and the surrounding normal skin. From the experiment results, our approach can effectively detect the pigmentation lesion, and perform robustness with different illumination.
Three cases of cutaneous larva migrans (CLM) were diagnosed in a returnee from a trip to Thailand and in 2 domestic farmers during July and September, 2003. The linear and serpiginous skin lesions on the lower extremities were presented in all 3 cases. Routine laboratory findings were normal. In the imported case, a $650\times30{\mu}m$ sized filariform nematode larva, presumably a species of hookworm, was detected in the lesion. All cases were treat-ed with 400 mg albendazole once daily for 3-5 days, and their skin lesions gradually improved. In the present study, a causative agent of CLM was isolated for the first time in the Republic of Korea. Moreover, we speculate that CLM is prevalent in farmers who are in frequent contact with soil in the Republic of Korea.
