측두두정근막 자유피판술을 이용한 수부재건의 장기추적조사 (Long-term Follow-up of Reconstruction of the Hand with a Temporoparietal Fascial Free Flap)

  • Purpose: Soft tissue defect of the hand, which cannot be covered with skin graft or local flap, is usually reconstructed using a free flap. Temporoparietal fascial free flap is one of the best alternatives for functional reconstruction of the hand with exposed tendons, bones, and joints. Materials and Methods: We have experienced four cases of reconstruction using a temporoparietal fascial flap with a skin graft and followed up for 20 years. We conducted a retrospective review of the patients' clinical charts and photos. Results: At the time of initial injury, the average age of patients was 50.3 (39~62) years. The radial artery was used for reconstruction of the dorsal side of the hand, whereas the ulnar artery was used for that of the volar side of the wrist. Short term complication such as skin graft loss and donor site alopecia occurred. However, during the long term follow-up period, no change of flap volume was noted, and full range of motion in the adjacent joint was maintained. In addition, hyperpigmentation of the grafted skin on the flap disappeared gradually. Conclusion: Selection of the optimal flap is important for reconstruction of the hand without functional limitation. We obtained satisfactory soft tissue coverage and functional outcomes using a temporoparietal fascial free flap and followed up for 20 years.

초미세수술을 이용한 하지재건의 기술적 고찰 (Techniques in Lower Extremity Reconstruction with Supermicrosurgery)

  • Purpose: The concept and development of perforator free flaps have led to significant advances in microsurgery. Ongoing developments in perforator free flap surgery are aimed at reducing complications and improving surgical outcomes. The aim of this study was to evaluate the effectiveness and application of supermicrosurgery in free flap surgery. Materials and Methods: A total of 267 patients with soft tissue defects of the lower extremity due to various etiologies from January, 2007 to January, 2013. The patients received either an anterolateral thigh free flap (n=83), a superficial circumflex iliac artery free flap (n=152), an upper medial thigh free flap (n=19), or a superior gluteal artery perforator free flap (n=13). Microanastomosis was performed using a perforator-to-perforator technique, either end-to-end or end-to-side. Results: The mean postoperative follow up period was eight months (range: one to 16 months) and flap loss occurred in 11 cases out of 267. All cases of flap loss occurred within two weeks of surgery due to either arterial insufficiency (n=5) or venous congestion (n=6). Conclusion: Supermicrosurgery enables the selection of the most efficient perforator for microanastomosis at the defect site. It also reduces the time required for dissection of recipient vessels, and reduces the possibility of injury to major vessels. Microsurgery using a vessel of less than 1 mm has been reported to increase the risk of flap failure; however, using the most advanced surgical tools and developing experience in the technique can produce success rates similar to those found in the literature.

감시정찰 센서네트워크에서 환경요소와 노드수량을 고려한 노드 배치 전략 (A Node Deployment Strategy Considering Environmental Factors and the Number of Nodes in Surveillance and Reconnaissance Sensor Network)

  • 무선 센서네트워크에서 센서 커버리지와 네트워크 연결성 문제는 노드의 제한된 탐지거리와 통신거리로 인해 발생한다. 커버리지와 연결성 문제를 해결하기 위해 많은 연구가 진행되었지만 대부분의 연구가 무선 센서네트워크 배치에 영향을 주는 다양한 환경요소를 고려하고 있지 않기 때문에 실환경에 적용되는데 한계가 있다. 따라서 본 논문에서는 감시정찰 센서네트워크에서 지형, 식생, 기상 등 환경요소와 제한된 노드 수량을 고려하여 노드를 배치하는 방법을 제안한다. 제안방법은 감시정찰 센서네트워크 시스템의 설치 절차를 IPB분석을 통한 배치영향 요소파악, 센서 탐지범위 기반 센서노드 배치, 모니터링 장소 선정, RF 통신범위 기반 중계노드 배치와 같이 4단계로 구분하고, 감시정찰 센서네트워크 시스템 특정과 환경 영향요소를 고려하여 센서노드와 중계노드를 배치하는 것이다. 시뮬레이션을 통해 제안방법을 검증하였으며, 커버리지와 네트워크 연결성에서 성능이 향상됨을 확인하였다.

환경관리인의 교육.훈련 현황 및 개선 방안 연구 (Regimes for the Improvement on Education and Training for Environmental Mangers on site)

  • The environmental quality in Korea was significantly deteriorated though the past economic development period. Even though the government has spent huge sum of resources on the policies and projects to improve the environmental quality, it is much removed from what the people hope for. To solve the environmental problems effectively, pollutants generated at source should be kept as minimum as possible, and possible, and pollutants from source be controlled before leaving the sources, and most of all, environmental strategies and systems be established and enforced properly. So, it is not too much to say that nation's environmental quality is entirely dependent on the education and training of the environmental managers such as environmental employees for air, water, and noise discharge facilities, technicians at environmental plants, and environmental officers of central and local governments. Regimes for the improvement of the education and training for environmental managers are suggested into four categories-educational institute, circular of the study, training contents and trainees. Referring to the improvement of integrated institute, and to improvement on educational institute, it is necessary to investigate the feasibility of integrated institute, and to recommend the inclusion of the local colleges and universities, and environmental societies. For the improvement on training courses, it is necessary that the non-obligatory course be expanded, and the courses be developed in such a way to meet the needs from the field. As for the improvement on training contents. It is suggested that the practical training on size be strengthened, and also full time environmental specialist and integrated committee be required to develop the effective circular and selection of proper lectures. Finally, environmental education for high ranked officers of autonomies, and international courses for developing countries are suggested.

Functional Analysis of the Invariant Residue G791 of Escherichia coli 16S rRNA

  • The nucleotide at position 791(G791) of E. coli 16S rRNA was previously identified as an invariant residue for ribosomal function. In order to characterize the functional role of G791, base substitutions were introduced at this position, and mutant ribosomes were analyzed with regard to their protein synthesis ability, via the use of a specialized ribosome system. These ribosomal RNA mutations attenuated the ability of ribosomes to conduct protein synthesis by more than 65%. A transition mutation (G to A) exerted a moderate effect on ribosomal function, whereas a transversion mutation (G to C or U) resulted in a loss of protein synthesis ability of more than 90%. The sucrose gradient profiles of ribosomes and primer extension analysis showed that the loss of protein-synthesis ability of mutant ribosomes harboring a base substitution from G to U at position 791 stems partially from its inability to form 70S ribosomes. These findings show the involvement of the nucleotide at position 791 in the association of ribosomal subunits and protein synthesis steps after 70S formation, as well as the possibility of using 16S rRNA mutated at position 791 for the selection of second-site revertants in order to identify ligands that interact with G791 in protein synthesis.

A Novel Integrative Expression Vector for Sulfolobus Species

  • With the purpose of facilitating the process of stable strain generation, a shuttle vector for integration of genes via a double recombination event into two ectopic sites on the Sulfolobus acidocaldarius chromosome was constructed. The novel chromosomal integration and expression vector pINEX contains a pyrE gene from S. solfataricus P2 ($pyrE_{sso}$) as an auxotrophic selection marker, a multiple cloning site with histidine tag, the internal sequences of malE and malG for homologous recombination, and the entire region of pGEM-T vector, except for the multiple cloning region, for propagation in E. coli. For stable expression of the target gene, an ${\alpha}$-glucosidase-producing strain of S. acidocaldarius was generated employing this vector. The malA gene (saci_1160) encoding an ${\alpha}$-glucosidase from S. acidocaldarius fused with the glutamate dehydrogenase ($gdhA_{saci}$) promoter and leader sequence was ligated to pINEX to generate pINEX_malA. Using the "pop-in" and "pop-out" method, the malA gene was inserted into the genome of MR31 and correct insertion was verified by colony PCR and sequencing. This strain was grown in YT medium without uracil and purified by His-tag affinity chromatography. The ${\alpha}$-glucosidase activity was confirmed by the hydrolysis of $pNP{\alpha}G$. The pINEX vector should be applicable in delineating gene functions in this organism.

Development of a Genome-Wide Random Mutagenesis System Using Proofreading-Deficient DNA Polymerase ${\delta}$ in the Methylotrophic Yeast Hansenula polymorpha

  • The thermotolerant methylotrophic yeast Hansenula polymorpha is attracting interest as a potential strain for the production of recombinant proteins and biofuels. However, only limited numbers of genome engineering tools are currently available for H. polymorpha. In the present study, we identified the HpPOL3 gene encoding the catalytic subunit of DNA polymerase ${\delta}$ of H. polymorpha and mutated the sequence encoding conserved amino acid residues that are important for its proofreading 3'${\rightarrow}$5' exonuclease activity. The resulting $HpPOL3^*$ gene encoding the error-prone proofreading-deficient DNA polymerase ${\delta}$ was cloned under a methanol oxidase promoter to construct the mutator plasmid pHIF8, which also contains additional elements for site-specific chromosomal integration, selection, and excision. In a H. polymorpha mutator strain chromosomally integrated with pHIF8, a $URA3^-$ mutant resistant to 5-fluoroorotic acid was generated at a 50-fold higher frequency than in the wild-type strain, due to the dominant negative expression of $HpPOL3^*$. Moreover, after obtaining the desired mutant, the mutator allele was readily removed from the chromosome by homologous recombination to avoid the uncontrolled accumulation of additional mutations. Our mutator system, which depends on the accumulation of random mutations that are incorporated during DNA replication, will be useful to generate strains with mutant phenotypes, especially those related to unknown or multiple genes on the chromosome.

  • 이산화탄소 포집 및 저장 / 격리 (CCS) 기술은 많은 이산화탄소 저감 방법 중 이상적인 방법으로 주목 받고 있다. 이산화탄소를 포집해서 파이프라인을 통해 저장소까지 수송할 때, 저장소가 가까운 경우 직접 수송할 수도 있지만, 중간 저장의 역할을 하는 허브를 거쳐 수송할 수도 있다. 허브의 수와 위치를 결정하는 것은 중요한 문제이다. 다목적 의사 결정을 위한 수학 모델은 많은 제약식과 목적식을 수반하는데, 문제의 계산 복잡도가 증가하지만 항상 최적을 보장하지 않는다. 본 연구에서는, 이산화탄소 수송망에서 중간 저장 허브의 위치와 수를 결정하는 알고리즘을 제안하고, 이를 활용하여 이산화탄소 발생지의 연결 네트워크 시뮬레이터를 개발한다. 시뮬레이터에서는 또한 이산화탄소의 수송 경로를 제공한다. 사례 연구로 한국에 모델을 적용한다.

자가치아이식술의 장기적인 임상적 평가 (Evaluation of Autotransplantation)

  • Purpose: Many clinical studies have reported that higher success rates are achieved with teeth that have immature roots than other autotransplanted teeth that have more immature root. However, based on date published recently, the success rate of autotransplantation of teeth with complete root formation was higher. The purpose of this study was to examine the long term(2 to 6 years follow-up) success rate of autotransplantation of third molar with complete root formation and to discuss some conditions and prerequisites for success. Materials and Methods: 26 sites of 24 patients aged 26 to 55 (mean age 40.8) were autotransplanted with third molars with complete root formation. These cases were followed for 2 to 6 years after surgery. The success criteria included (1) no discomfort during functioning (2) absence of progressive root resorption and alveolar bone resorption. Result: Of 26 teeth 5 teeth were failed, therefore success rate is 81%(21/26 teeth). The results suggested that higher success rate is acquired from (1) extraction socket due to dental caries (2) mandibular recipient site (3) patient younger than 40 years old. Autotransplantation of third molar to replace molars with advanced periodontal disease also showed considerably high success rate(84%). Conclusion: With appropriate case selection, autotransplantation of third molar with complete root formation remains a viable alternative for replacing a missing molar tooth.

  • 1. CT에 의한 매복치의 위치결정은 적절한 치료계획의 수립을 가능하게 하여 시술시간의 단축과 인접치, 인접구조물의 손상을 줄여 시술후의 부작용을 줄일 수 있었다. 2. 매복된 영구치의 견인시 CR를 활용한 결과 개창부위의 정확한 위치결정과 예후에 대한 예측이 가능하였다. 3. 평면방사선사진의 판독후 악골내 병소가 의심되어 CT를 활용한 결과 병소의 확진이 가능하였다. 4. 소아의 CT촬영은 평면 방사선 촬영에 비해 증가된 방사선 피폭량, 높은 수가 때문에 적절한 증례의 선택이 중요하다.
