A Case of Large Cell Neuroendocrine Carcinoma of the Maxillary Sinus (상악동에 발생한 대세포 신경내분비 암종 1례)

  • Large cell neuroendocrine carcinoma is a rare epithelial neuroendocrine malignancy and is preferentially located in gastrointestinal tract and pancreas. Cases of large cell neuroendocrine carcinoma have been reported in many other locations, including the thymus, gallbladder, prostate, larynx, salivary glands, nasopharynx, tonsil and mastoid. However, primary sinonasal large cell neuroendocrine carcinoma never have been reported in Korea. We experienced a case of primary large cell neuroendocrine carcinoma arising from left maxillary sinus recently. A 82-year-old male patient presented with nasal obstruction and epistaxis. The biopsy revealed large cell neuroendocrine carcinoma with poor differentiation. After a general evaluation, the patient was staged as cT3N0M0. The patient was treated by combined radiotherapy and chemotherapy. We report this rare case with literature review.

Result of Surgical Repair of Intracardiac Defects Associated with Corrected Transposition - 33 cases - (교정형 대혈관 전위증에 동반된 심장기형의 수술 성적에 대한 보고)

  • From November 1978 through June 1989, 33 patients aged 3 months to 27 years [mean 9.7 years] underwent repair of intracardiac defects associated with corrected transposition. Five patients had had previous palliative surgery. Operation were performed in 31 for ventricular septal defect, 22 for pulmonary outflow tract obstruction, 16 for atrial septal defect, and 5 for anatomical tricuspid valve regurgitation. Pulmonary outflow tract obstruction was relieved by pulmonary valvotomy in 9, Rastelli procedure in 5, modified Fontan procedure in 3, and by REV procedure in 5 patients recently. Early mortality was 21.2%[7/33] and no late mortality during follow up period. Two had residual pulmonary outflow tract obstruction and one residual VSD. In eight patients, transient arrhythmia was found but soon returned to sinus rhythm. Five patients developed complete heart block and 2 were given permanent pacemaker insertion. There were 8 RBBB, 1 LBBB and one second degree atrioventricular block patients, but all showed no clinical significance. This report suggests that surgical repair of intracardiac defects associated with corrected transposition can be achieved with acceptable low risk. Though the mortality is still high, we can improved the result by advancing surgical technique, knowledge of the special conduction system, and by improving postoperative care.

Systematic Review on the Effects of Herbal Medicine Compared to Amoxicillin in Patients with Upper Respiratory Tract Infection : Focusing on PubMED (상기도 감염 환자의 amoxicillin 대비 한약의 효과에 대한 체계적 문헌고찰 : PubMED를 중심으로)

  • Objective : This study aims to examine the effectiveness and safety of herbal medicines in upper respiratory tract infection by conducting systematic review against amoxicillin. Methods : Relevant randomized controlled trials (RCTs) published by December 2020 were searched in PubMED database. Study selection was conducted according to the PRISMA guideline. The Risk of Bias of the RCTs were evaluated based on the Cochrane criteria. Study selection and bias assessment were conducted independently by two researchers, and the third party was involved if there is a disagreement. Results : Three RCTs were included. Meta-analysis was not performed due to different specific disease and outcome measurements among the included studies. Herbal medicine showed statistically significant improvement in symptoms and endoscopic findings compared to amoxicillin in acute rhinosinusitis. In addition, herbal medicine showed statistically significant improvement in symptoms and a smaller dose of additional antibiotics in acute tonsillitis. However, herbal medicine showed no significant effects compared to amoxicillin in chronic sinusitis patients after functional endoscopic sinus surgery. In terms of adverse events, no significant difference was observed between the herbal medicine group and the amoxicillin group in all studies. Conclusion : Herbal medicine might be more effective than amoxicillin in the treatment of upper respiratory tract infection. Nevertheless, it is hard to clear that conclusion due to the fact that the collected studies were conducted on different specific diseases and herbal medicine, and that the included studies had flaws in the research methodology.

Early and late Complications after Arterial Switch Operation for Transposition of the Great Srteries -7 Year Experience- (대혈관 전위증에 동맥치환술 후의 합豆증)

  • We reviewed our entire experience of 44 consecutive patients undergoing the arterial switch operation [ASO] for transposition of the great arteries [TGA] since March 1985.There were 28 patients with simple TGA[group I] and 16 with associated ventricular septal defect[VSD] [Group II] There were five hospital deaths[11.4%, 5/44], two related to single right coronary artery anatomy. There have been no late deaths. For group I hospital mortality was 14.3%[4/28], and for group II this was 6.25%[1/16]. Mean follow-up was 3.3 years[range 1 month to 8 years] and was completed for all patients. Actuarial survival at 7 years for hospital survivors was 85 $\pm$ 3.2 % in group I and 94 $\pm$ 3.5% in groupII. One patient has mild asymptomatic left ventricular outflow tract obstruction, and five patients [12.8 %,5/ 39] have right ventricular outflow tract gradients[RVOTO] exceeding 25 mmHg; only one patient has required reoperation for RVOTO. Mild neoaortic regurgitation is present in one patient. All survivors are currently in NYHA class I without medicalion, and all are in sinus rhythm. The ASO is associated with low operative risk and excellent medium-term outcome in most subsets of patients undergoing this operation. With more experience, improved results can be expected also in those patients currently at higher risk.

Observations of lymphoid polyps on the gastrointestinal tract (위장벽(胃腸壁)에 발생(發生)한 Lymphoid Polyps의 병리학적관찰(病理學的觀察))

  • The present observations were focussed mainly on the macroscopic and histopathological observation to polyps encountered on the stomach of 4-month-old pig, small intestine of 3-month-old pig and cecum of 9-month-old dog. The results were summarized as follows; 1. The polyps were gray or pink hemispherical shapes attached by the broad base to the gastrointestinal walls and were 3 to 8 mm in diameter and diffuse appearance. The surfaces of polyps were flat or indented and were ulcerated in some large polyps. 2. In histopathological findings, the polyps were nodules composed of lymphoid follicles, which were characterized by the differentiation and proliferation of the lymphoid cells in the lamina propria or submucosa of the gastrointestinal tract. The lymphoid nodules have the thin septa and thick capsules of the connective tissues, also comprised more numerous and larger sinus-like-blood vessels and more diffuse lymphoid cells in the peripheral zones than those in the central zones of nodules. In addition, the external surface of the polyps bulged toward the intestinal lumen were covered with the abnormal epithelium, or the degenerative and necrotic lymphocyte aggregates. 3. These polyps were considered as lymphoid polyps caused by the hyperplasia of the lymhoid follicles.

A Clinical Study of Branchial Apparatus Anomalies (새성기형 50예의 임상적 고찰)

  • Branchial apparatus anomaly is rarely encountered congenital neck disease, it presents a palpable non-tender mass or fistulous opening existed at any site from external auditory canal or mandible angle to lower part of neck We have reviewed the records of 50 patients operated upon for branchial cleft anomaly, at Department of Surgery, Inje University Hospital, between 1981 and 1990, and the following results were obtained. I) In the classificiation of branchial cleft anomaly, first branchial fistula was 1 case, second branchial cyst 32 cases, second branchial sinus 11 cases, second branchial fistula 5 cases and third branchial fistula 1 case. 2) There were 20 men and 30 women in this series and male to female ratio was 2:3. 3) The age at first clinical presentation was 1st decade 15 cases, 2nd decade 10 cases, 3rd decade 17 cases, 4th decade 5 cases and 5th decade 3 cases. The peak age incidence was 3rd decade in overall, but the cyst was most common in 2nd decade, and majority of sinus or fistula was seen below 10 years old age. 4) The prevalent side of this anomaly was right side in 19 cases, left side in 29 cases and bilateral 2 cases, and so left side was more common than right. 5) The clinical presentation was characterized by the lesion along anterior border of sternocleido muscle, non-tender palpable mass were 28 cases, drainage sinus 18 cases, recurrent abscess and drainage 5 cases and intermittent ear discharge 1 case. 6) The mean size of cyst was about 4cm that containing turbid white-yellowish fluid but discharge from sinus or fistula was clear mucoid. 7) The culture of cyst fluid was no bacteria, but 2 cases showed staphyloccoci suggesting secondary infection. 8) The surgical procedure were complete excision of cyst 32 cases, sinus excision 11 cases, fistula excision 6 cases and I&D 1 case. And the recurrent 1 case was that fistula tract could not be identified due to severe scar from previous several operations.

Thoracic (Bronchopulmonary) Actinomycosis : A Case Report (폐 Actinomycosis 치험 1례)

  • Actinomycosis is a chronic suppurative granulomatous disease due to Actinomyces israelii characterized by multiple abscess and sinus tract formation with dense fibrous scarring. This disease, especially thoracic infection, is very rare in Korea so we are not famiIliar with to make diagnosis and treatment. Otherwise the unspecificity of the clinical symptoms and the lack of adequate examination recedure (as anaerobic fungus culture) are the causes of misdiagnosis. Thoracic actinomycosis is very similar to chronic infectious disease of the lung and chest or thoracic neoplasm. Recently we experienced a case of thoracic actinomycosis (bronchopulmonary) which had been confused with chronic lung abscess and pathologically confirmed as broncho-pulmonary actinomycosis. The purpose of this report is to review our experience more thorouly to enhance consideration of Artinomycosis.

Isolated Ventricular Inversion and Anatomically Corrected Malposition of the Great Arteries Associated with Right Juxtaposition of Left Atrial Appendage: A case of Successful surgical repair

  • A seven month old female infant with isolated ventricular inversion and anatomically corrected malposition of the great arteries in situs solitus, associated with ventricular septal defect, patent ductus arteriosus, right-sided juxtaposition of left atrial appendage, is reported. The patient showed usual atrial arrangement with somewhat superoinferior relation, a discordant atrioventricular connection, and a concordant ventriculoarterial connection with aorta in the right-sided position. A normal sized left atrium was connected to the left superiorly positioned morphologic right ventricle through a tricuspid valve, which crossed the left ventricular outflow tract anteriorly. Well developed bilateral[subaortic and sub-pulmonary]conus was documented at operative field. successful surgical repair was done by performing the Senning procedure and by closing the ventricular sepal defect with a patch through the right ventriculotomy. The infant’s postoperative course was uneventful with normal sinus rhythm. Postoperative cardiac catheterization revealed no hemodynamic obstruction or residual shunt.

A STUDY ON THE ISOLATION OF BLACK-PIGMENTED BACTEROIDES IN THE NECROTIC PULP (괴사치수에서의 Black-pigmented Bacteroides 균주분리에 관한 연구)

  • The purpose of this study was to compare Black-pigmented Bacteroides isolated from necrotic pulp with the presence or absence of eight symptoms associated with pulpal necrosis and to identify the proportion of each Black-pigmented Bacteroides species. The canal contents of fourteen traumatically and cariously necrotized teeth were sampled with a special technique and cultured aerobically and anaerobically for growth in blood agar plate and for Black-pigmented Bacteroides on selective blood agar plate. Each Black-pigmented Bacteroides species were identified by Gram's stain, hemolysis reaction, colony color and morphology and biochemical tests. The results were as follows; 1. 60.9 percent of the bacteria isolated were anaerobic and 39.1 percent were aerobic. 2. Four Black-pigmented Bacteroides species were isolated; B. loescheii (74.1%), B. denticola (11.1%), B. intermedius (7.4%) and B. gingivalis (7.4%). 3. Black-pigmented Bacteroides was found to be significantly related to sinus tract formation and exudate.

A Surgical case of Tetralogy of Fallot with Single Coronary Artery (단일관상동맥의 활로4징증 치험 1례)

  • In the surgical repair of tetralogy of Fallot, the anomalous origin of the coronary artery has significant surgical implication. The coronary anomalies of concern are origin of the anterior descending coronary artery from the right coronary artery and certain variations of single coronary branching. We experienced a case of tetralogy of Falloff with single coronary artery from the left coronary sinus, from which the right coronary artery originated crossing the right ventricular outflow tract. This ty e of coronary anomaly in tetralogy of Fallot is known to be extremly rare.
