Antibiotic resistance in Neisseria gonorrhoeae: broad-spectrum drug target identification using subtractive genomics

  • Neisseria gonorrhoeae is a Gram-negative aerobic diplococcus bacterium that primarily causes sexually transmitted infections through direct human sexual contact. It is a major public health threat due to its impact on reproductive health, the widespread presence of antimicrobial resistance, and the lack of a vaccine. In this study, we used a bioinformatics approach and performed subtractive genomic methods to identify potential drug targets against the core proteome of N. gonorrhoeae (12 strains). In total, 12,300 protein sequences were retrieved, and paralogous proteins were removed using CD-HIT. The remaining sequences were analyzed for non-homology against the human proteome and gut microbiota, and screened for broad-spectrum analysis, druggability, and anti-target analysis. The proteins were also characterized for unique interactions between the host and pathogen through metabolic pathway analysis. Based on the subtractive genomic approach and subcellular localization, we identified one cytoplasmic protein, 2Fe-2S iron-sulfur cluster binding domain-containing protein (NGFG RS03485), as a potential drug target. This protein could be further exploited for drug development to create new medications and therapeutic agents for the treatment of N. gonorrhoeae infections.

  • Approximately, 15-20% of all cancers worldwide are caused by infectious agents. Understanding the role of infectious agents on cancer development might be useful for developing new approaches to its prevention. Mycoplasma genitalium is a clinically important sexually transmitted pathogen that has been associated with several human diseases. There have been a few studies suggestive of probable roles of Mycoplasma genitalium in cancer development, including prostate and ovarian cancers and lymphomas, but the role of this microorganism like other Mycoplasma species in neoplasia is still conjectural. Considering the prevalence of Mycoplasma genitalium infections and also the emergence of resistant strains, Mycoplasma genitalium needs more attention in the infectious agent cancer-causing research area.

청소년 성매개 감염병 교육을 위한 스마트폰 어플리케이션 개발과정 (Developing a Customized Sexually Transmitted Infections (STIs) Smartphone Application for Adolescents: An Application of the Instructional System Design Model)

  • 청소년을 위한 성매개 감염병 교육의 필요성이 증가되고 있지만 대부분의 청소년들은 교과과정의 부족한 시간으로 인하여 제대로 된 교육을 받지 못하고 있다. 이 시기의 성매개 감염병 노출은 증상이 없거나 경미하여 청소년들이 성매개질환에 대한 인지를 못하는 경우가 많아 장기적인 생식기계의 문제를 야기할 수 있다. 스마트폰의 보급과 더불어 청소년들이 정보의 대부분을 스마트폰을 통하여 얻고 있어 스마트폰을 활용한 교육이 청소년에서 효과적이다. 청소년의 성 매개 감염병 지식을 향상시키기 위한 교육의 방법으로 딕 앤 케리의 체계적인 교수 설계 모델을 적용하여 성 매개 감염병 교육 스마트폰 어플리케이션 개발을 하였다. 본 연구는 이 스마트폰 어플리케이션 개발 전략 및 개발 과정을 기술하였다. 이 스마폰 어플리케이션은 6명의 전문가들과 함께 개발되었으며 교육 내용은 청소년들이 이해하기 쉬운 용어를 사용하였다. 학습능력 향상을 위한 주요 전략으로는 만화로 된 교육내용, 비밀 문의와 질의응답 기능, 버튼 색 변화 및 반복 학습기능 수록 등이다. 본 연구에서 개발된 스마트폰 어플리케이션은 학교, 청소년 관련 기관 및 병원에서 청소년들의 성매개 감염병 지식, 성매개 감염병 예방 및 성매개 감염병 대처 능력을 향상 시키는데 활용 될 수 있다.

AIDS의 학교 보건교육 도입에 관한 연구 (A Study for the Development of a School-based Health Education of AIDS)

  • AIDS and the spectrum of Human Immunodeficiency Virus(HIV) infections present a monumental challenge to the health of the Korean public. In response to this special challenge, I think public education and voluntary behavior changes are the most effective measures to fight the spread of the disease. Adolescents represent a critical risk group for prevention and intervention programming. Research indicates sexually active adolescents, homosexual contact, illicit drug use are an gradually increasing. These characteristically adolescent risk-taking behaviors suggest the need for schools and communities to mobilize intervention strategies. Schools are highly efficient ways to reach a majority of young people in Korea with HIV prevention programs. These programs include substantial attention to sexual and drug use behaviors with the long term objective of a multidimensional school health program. Information resulting from risk behavior surveillance activities and guidance on school health curricula is particularly useful. What is needed for adolescents is a revamping of education to give students the critical thinking and analytic skills that allow them to apply knowledge, make decisions, and think independently. The best HIV preventive education provides young people with opportunities to learn and practice just those skills. In the early stages of HIV education were focused solely on information. Providing information is easy but unfortunately, behavior change is not that simple to activate. Information must be combined with values exploration and skilly building, including responsible decision making, negotiation, refusal, and critical thinking skills. The same knowledge, attitudes and skills needed for effective HIV prevention also prevent or reduce other risks, including other sexually transmitted diseases, unwanted pregnancies, and alcohol or other drug use. The role of other youth serving organizations in HIV prevention is also important: parental and youth involvement is needed; it's important to presidential and governament leadership is essential to prevention education; promote integrated adolescent programs, to enhance health and education sector collaboration; and of course, we need to expand research on adolescent health and engage the media in health promotion. Among these changes, a school-based systematic health education of AIDS is certainly one of the essentials.

  • Along with a development of medical technology, a variety of tests, such as laboratory tests, x-ray and endoscopies are being used in health screening tests. As the tests determine the quality of health screening, test items of major health screening program in Korea. Most, of the health screening programmes focused upon detection of risk factors and diagnosis of life-style related diseases(diabetes, hypertension, cardiovascular diseases, hypercholesterolemia, overweight, drinking, smoking, cerebrovascular diseases, osteoporosis), cancers(stomach, cervix, lung, breast, liver, colon, prostate, ovary, pancreas, thyroid, esophagus), infections diseases(hepatitis, tuberculosis, sexually-transmitted diseases, parasites), chronic obstructive respiratory diseases, chronic renal diseases(bacteriuria, hematuria, proteinuria), anemia, glaucoma, hearing loss, Alzheimer disease, stress and earlypsychiatric diseases. The health screening tests were basic physical examination, basic laboratory tests( CBC, urinalysis, liver function tests, lipid tests, glucose, HbA1c, uric acid, electrolytes, serological tests(HBsAg, HBs-Ab, HCV-Ab, HIV-Ab, VDRL) EKG, x-ray(chest PA, CT) endoscopy(gastroscopy, colonoscopy), sonography (abdomen, thyroid, pelvis, breast), cytology(cervix), bone density, tumor markers(NMP22, alpha-FP, CEA, CA-19-9, CA125, PSA and eye tests. Advanced technologies, like CT, PET, MRI, MRI/Angio, molecular testing were widly used in hospital based health screening programmes. In summary, a variety of tests were untilized in health screening in Korea. Those tests were utilized by stages or according to sex and age in most of health screening programmes, however a few programs used tests excessvely disregarding health screening subjects.

Knowledge, Attitude and Practice of Malaysian Medical and Pharmacy Students Towards Human Papillomavirus Vaccination

  • Human Papillomavirus (HPV) infection is one of the most common sexually transmitted infections and oncogenic HPV is the main cause of cervical cancer. However, HPV vaccination is already available as the primary preventive method against cervical cancer. The objective of this study was to determine the level of knowledge, attitude and practice of HPV vaccination among Universiti Kebangsaan Malaysia (UKM) and Universiti Malaya (UM) students. This study was conducted from March until August 2009. Pre-tested and validated questionnaires were filled by the third year UKM (n=156) and UM (n=149) students from medical, dentistry and pharmacy faculties. The results showed that the overall level of knowledge on HPV infection, cervical cancer and its prevention among respondents was high and the majority of them had positive attitude towards HPV vaccination. Medical students had the highest level of knowledge (p<0.05). Very few students (3.6%) had already taken the vaccine with no significant difference between the two Universities (p=0.399). In conclusion, the knowledge and attitude of the respondents were high and positive, respectively. Only few students took HPV vaccination. Thus, more awareness campaigns and HPV vaccination services should be provided at universities' campuses with the price of the HPV vaccine reduced for the students.

Oral Syphilis Responds to Topical Antibiotic Therapy but Still Needs Definitive Systemic Treatment

  • Syphilis, one of the most common sexually transmitted diseases, is caused by the microorganism Treponema pallidum. Syphilis consists of several clinical stages that may include signs in the oral and perioral regions. Syphilis is treated effectively with systemic antimicrobial therapy using antibiotics such as penicillin. This article describes a case where topical antibacterial therapy with doxycycline was effective in treating oral papular lesions associated with primary syphilis in a 24-year-old male. He was immediately referred to a dermatologist, and antibiotic therapy was administered in response to positive diagnostic test results for syphilis. Although oral syphilitic lesions may be resolved by dental professionals using topical treatments, syphilitic infections should be managed in consultation with medical specialists using systemic antibiotic therapy.

Knowledge regarding factors that influence fertility in Thai reproductive-age population living in urban area: A cross-sectional study

  • Objective: To survey knowledge about the factors that influence fertility in a reproductive-age population living in an urban area. Methods: A cross-sectional study was conducted using a questionnaire-based survey among both males and females aged 18-45 years living in the Bangkok metropolitan area. Results: The mean age of the participants was $26.8{\pm}7.2years$ (male, 41.9%; female, 58.1%). Of the participants, 53.1% had an undergraduate degree and 57.1% were single. Only one-fifth of the participants correctly identified the age when fecundity declines in male and female, the definition of infertility, and the period during the menstrual cycle with the highest chance of pregnancy. Approximately three-fourths of the participants correctly identified that cigarette smoking, alcohol consumption, and sexually transmitted infections affect fertility. Conclusion: A considerable knowledge gap about the factors that influence fertility was identified in reproductive-age individuals in an urban area of Thailand. This issue should be urgently addressed by promoting fertility awareness through education, discussions about social perceptions regarding fertility, and reliable sources of knowledge.

In Vitro Effect of Methanolic Extract of Argemone mexicana against Trichomonas vaginalis

  • Infections caused by Trichomonas vaginalis in humans are one of the main public health problems caused by sexually transmitted diseases. Objective of this study was to evaluate potential biological activity of the medicinal plant Argemone mexicana (Mexican poppy) on T. vaginalis. Methanolic extracts of the stems and leaves of A. mexicana, and different fractions were prepared with solvents of different polarities. The extracts and functional groups were detected containing sterols, triterpenes, quinones, flavonoids and, alkaloids. Extracts from both the stems and leaves of A. mexicana inhibited the growth of T. vaginalis with half-maximal inhibitory concentration value of 70.6 and 67.2 ㎍/ml, respectively. In the active fractions, the most abundant compounds were berberine and jatrorrhizine, with presumed antiparasitic activity.

Mutation Detection of E6 and LCR Genes from HPV 16 Associated with Carcinogenesis

  • Human papillomavirus (HPV) is responsible for one of the most frequent sexually transmitted infections. The first phylogenetic analysis was based on a LCR region fragment. Nowadays, 4 variants are known: African (Af-1, Af-2), Asian-American (AA) and European (E). However the existence of sub-lineages of the European variant havs been proposed, specific mutations in the E6 and LCR sequences being possibly related to persistent viral infections. The aim of this study was a phylogenetic study of HPV16 sequences of endocervical samples from C${\acute{o}}$rdoba, in order to detect the circulating lineages and analyze the presence of mutations that could be correlated with malignant disease. The phylogenetic analysis determined that 86% of the samples belonged to the E variant, 7% to AF-1 and the remaining 7% to AF-2. The most frequent mutation in LCR sequences was G7521A, in 80% of the analyzed samples; it affects the binding site of a transcription factor that could contribute to carcinogenesis. In the E6 sequences, the most common mutation was T350G (L83V), detected in 67% of the samples, associated with increased risk of persistent infection. The high detection rate of the European lineage correlated with patterns of human migration. This study emphasizes the importance of recognizing circulating lineages, as well as the detection of mutations associated with high-grade neoplastic lesions that could be correlated to the development of carcinogenic lesions.
