A Quantitative Method for the Assessment of Myocardial Function using the Polar Analysis of Tc-99m-MIBI Myocardial SPECT (Tc-99m-MIBI 심근 SPECT 극성지도 분석에 의한 심근 기능의 정량적 평가)

  • As the Tc-99m-MIBI myocardial SPECT demonstrated wide application in the diagnosis of myocardial function, the quantitative and severity-dependent information is currently re quired. In this study, we proposed a computerized method for scoring the fixed defects in terms of extent-weighted severity and for identifying the reversibility in ischemic regions. At the first stage of this method, the transverse slices were reconstructed with 0.4 Nyquist freq. and order 5 Butterworth filter. From the oblique/sagittal slices, maximal count per pixel circumferential profiles were extracted for each sector, and then stress/redist. polar maps were normalized and plotted. For reversibility, the stress polar map was subtracted from the de-layed image and positive-valued pixels were categorized into three grades. The extent-weight-ed severity scores were calculated using the assigned grades and their number of pixels. This procedure was done automatically and the reversibility and severity scores were produced for each of the coronary territories (LAD, RCA, LCX) or any combination of these. Clinical ap-plication has shown that the changes In reversibility scores after PTCA were correlated linearly with the pre PTCA scores(r>0.8) in postinfarct cases as well as in angina, and severity scores of persistent defects in stress/rest SPECT study matched to the regional ejection fraction and visual analysis of regional wall motion of gated blood pool scan(r>0.6). We conclude that the computerized severity scoring method for the analysis of myocardial SPECT could be useful in the assessment of the myocardial ischemia and fixed defect.

Studying the Comparative Analysis of Highway Traffic Accident Severity Using the Random Forest Method. (Random Forest를 활용한 고속도로 교통사고 심각도 비교분석에 관한 연구)

  • Purpose: The trend of highway traffic accidents shows a repeating pattern of increase and decrease, with the fatality rate being highest on highways among all road types. Therefore, there is a need to establish improvement measures that reflect the situation within the country. Method: We conducted accident severity analysis using Random Forest on data from accidents occurring on 10 specific routes with high accident rates among national highways from 2019 to 2021. Factors influencing accident severity were identified. Result: The analysis, conducted using the SHAP package to determine the top 10 variable importance, revealed that among highway traffic accidents, the variables with a significant impact on accident severity are the age of the perpetrator being between 20 and less than 39 years, the time period being daytime (06:00-18:00), occurrence on weekends (Sat-Sun), seasons being summer and winter, violation of traffic regulations (failure to comply with safe driving), road type being a tunnel, geometric structure having a high number of lanes and a high speed limit. We identified a total of 10 independent variables that showed a positive correlation with highway traffic accident severity. Conclusion: As accidents on highways occur due to the complex interaction of various factors, predicting accidents poses significant challenges. However, utilizing the results obtained from this study, there is a need for in-depth analysis of the factors influencing the severity of highway traffic accidents. Efforts should be made to establish efficient and rational response measures based on the findings of this research.

Tail dependence of Bivariate Copulas for Drought Severity and Duration

  • Drought is a natural hazard with different properties that are usually dependent to each other. Therefore, a multivariate model is often used for drought frequency analysis. The Copula based bivariate drought severity and duration frequency analysis is applied in the current study in order to show the effect of tail behavior of drought severity and duration on the selection of a copula function for drought bivariate frequency analysis. Four copula functions, namely Clayton, Gumbel, Frank and Gaussian, were fitted to drought data of four stations in Iran and Canada in different climate regions. The drought data are calculated based on standardized precipitation index time series. The performance of different copula functions is evaluated by estimating drought bivariate return periods in two cases, [$D{\geq}d$ and $S{\geq}s$] and [$D{\geq}d$ or $S{\geq}s$]. The bivariate return period analysis indicates the behavior of the tail of the copula functions on the selection of the best bivariate model for drought analysis.

  • Rheumatoid arthritis, unlike other chronic diseases, causes the patients to experience uncertainty in their daily lives and thus to feel threat on their emotional comfort because of inconsistent and unpredictable symptoms such as pain. Therefore, a theoretical framework is needed for explanation of uncertainty in patients having rheumatoid arthritis. A hypothetical model was constructed on the basis of Mishel's Uncertainty Theory and other literature review. The model included 9 theoretical concepts and 19 paths. Subjects of the study constituted 330 partients who visited outpatient clinics of two university hospitals and one general hospital in Seoul. Self report questionnaires were used to measure the variables affecting uncertainty. Reliability coefficients of these instruments were found Cronbach's Alpha=$.70{\sim}.94$. In data analysis, SAS program and PC-LISREL 8.03 computer program were utilized for descriptive statistics and covariance structure analysis. The results of covariance structure analysis for model fitness were as follows : 1) Hypothetical model showed a good fit to the empirical data : Chi-square($X^2$)=41.81 (df=11, P=.000), Goodness of Fit Index=.974, Root Mean Square Residual=.049, Normed Fit Index=.928, Non Normed Fit Index=.814. 2) For the validity and the parcimony of model, a modified model was constructed by appending 2 paths and deleting 5 paths according to the criteria of statistical significance and meaningfulness. 3) The results of hypothesis testing were as follows : (1) Educational level, event familiarity and severity of illness had a direct effect on uncertainty : Event congruency had both direct and indirect effect on uncertainty : Credible authority and symptom consistency had a nonsignificant direct effect on uncertainty, (2) Illness duration, symptom consistency, and event congruency had a direct effect on severity of illness ; Credible authority had a both direct and indirect effect on severity of illness ; Event congruency had the greatest effect on severity of illness, and event familiarity had a nonsignificant direct effect on severity of illness.

  • This study focused on estimating influential factors of traffic accidents and analyzing traffic accident severity of elderly and non elderly using traffic accident data. In order to reclassify elderly and non elderly traffic accident by a statistical method from entire traffic accident data, multiple discriminant analysis was applied. Also ordered logit model was applied for analyzing traffic accident severities using traffic accident severities as an independent variable and transportation facilities, road conditions and human characteristics as dependent variables. As results of the comparison between elderly and non elderly traffic accident, the traffic accident severity was affected by the age, types of traffic accidents, human characteristics and road conditions as well. Also, transportation facilities and road conditions affected to more elderly traffic accident than non elderly. Therefore, traffic accident severity would be decreased with the improvement of transportation facilities and road conditions for the elderly.

Factors Related to Workload of Intensive Care Unit Nurses: Systematic Review and Meta-Analysis (중환자실 간호사의 업무부담 관련 요인: 체계적 문헌고찰 및 메타분석)

  • Purpose: This study aimed to identify factors related to the workload of intensive care unit nurses through a systematic literature review and meta-analysis to provide basic data to explore the direction of development of nursing staffing standards. Methods: This study involved quantitative studies about nurses working in intensive care units related to nursing workload published in English or Korean since 2000. Search terms included 'intensive care unit', 'nursing workload', and their variations. Databases such as RISS, DBpia, MEDLINE(PubMed), CINAHL, PsycINFO, and Web of Science were utilized. Quality assessment was conducted using the Joanna Briggs Institute's Critical Appraisal Checklist for Analytical Cross-Sectional Studies. JAMOVI software facilitated the analysis of effect sizes, employing a meta-analysis approach for 7 studies with correlational or regression data. Results: From 16 studies on the workload of intensive care unit nurses, a total of 20 patient and nurse-related factors were identified. Patient-related factors included severity of illness, length of stay, and age. Meta-analysis was conducted for three patient-related factors: age, severity of illness measured by SAPS 3, and length of stay. Only severity of illness measured by SAPS 3 was significantly associated with nurse workload (Zr=0.16, p<.001, 95% CI=0.09-0.24). Conclusion: In previous studies, the characteristics of intensive care units and patients varied across studies, and a variety of scales for measuring workload and severity of illness were also used. Sustained research reflecting domestic intensive care unit work environments and assessing the workload of intensive care unit nurses should be imperative.

Study on the Correlation between Digital Infrared Thermal Imaging-induced Severity of Cold Hypersensitivity of Hands and Feet and Heart Rate Variability (Digital Infrared Thermal Imaging에 따른 수족냉증 중증도와 Heart Rate Variability의 상관관계 연구)

  • Objectives: Cold Hypersensitivity of Hands and Feet (CHHF) has been diagnosed objectively by Digital Infrared Thermal Imaging (DITI) and has been known to be associated with autonomic nervous system (ANS), which can be assessed by Heart Rate Variability (HRV). This study evaluated the correlation between severity of CHHF and HRV variables.Methods: We studied 155 non-menopausal women with CHHF who visited Kyung Hee University Hospital at Gangdong from 01 October 2013 to 30 April 2016. We measured DITI and HRV of each patient. We used DITI to calculate the severity of CHHF with thermal difference between upper arm (L4, 俠白穴) and palm (P8, 勞宮穴) of both hands and anterior thigh (ST32, 伏兎穴) and dorsum of foot (LI3, 太衝穴) of both feet. The correlation between severity of CHHF and HRV variables were analyzed.Results: In time domain analysis, there was significantly positive correlation between the severity of CHHF and both SDNN and RMSSD. In frequency domain analysis, there was significantly positive correlation between the severity of CHHF and TP, HF and HF Norm while there was significantly negative correlation between the severity of CHHF and LF Norm as well as between the severity of cold hypersensitivity of both hands and LF/HF ratio.Conclusions: The more severe CHHF is, the more increased the function of parasympathetic nerve system (PNS) and relatively decreased the function of sympathetic nerve system (SNS) is. Also, it has known that cold hypersensitivity could be caused by deficiency syndrome and qi deficiency has the same ANS tendency as CHHF. Therefore, in practical fields, this result can be helpful in planning treatment and deciding prognosis in respect of deficiency syndrome.

Factors Influencing Crash Severity by the Types of Bus Transportation Services Using Ordered Probit Models (순서형 프로빗 모형을 이용한 버스 운송사업 유형 별 사고심각도 영향요인 분석)

  • Buses, one of the representative public transportation modes, are divided into a vareity of service types according to the purpose of operation, operating distance, and management agencies. Although bus-involved crashes may cause large amount of damage due to the higher number of passengers boarded on a bus, prior research has little focused on crash severity according to bus service types. This study aims to investigate factors influencing crash severity in bus-involved crashes and to present policy implications to reduce crash severity by bus service type. To do this, bus-involved crash data from the Traffic Accident Analysis System (TAAS) during five-year period are used. Ordered probit models for three types of bus service, i.e., city bus, suburban and express buses, and charter buses, are estimated to analyze the factors of accident severity. The results show that there are significant differences of factors affecting crash severity among the types of bus services while speed and road surface influence all the types of buses. In case of local buses, time of day, roadway alignment, and installation of a traffic signal are found to be statistically significant factors. Seat belt and road class have significant effects on injury severity of the intercity and express buses. Chartered buses have time of day, driving experience, seatbelt, traffic signal, and day of week as the significant factors. The results of this study are expected to contribute to the reduction of the crash severity by each bus service type.

Features Extraction and Mechanism Analysis of Partial Discharge Development under Protrusion Defect

  • In order to study the development of partial discharge (PD) under typical protrusion defects in gas-insulated switchgear, we applied step voltages on the defect and obtained the ${\varphi}-u$ and ${\varphi}-n$ spectrograms of ultra-high frequency (UHF) PD signals in various PD stages. Furthermore, we extracted seven kinds of features to characterize the degree of deterioration of insulation and analyzed their values, variation trends, and change rates. These characteristics were inconsistent with the development of PD. Hence, the differences of these features could describe the severity of PD. In addition, these characteristics could provide integrated characteristics regarding PD development and improve the reliability of PD severity assessment because these characteristics were extracted from different angles. To explain the variation laws of these seven kinds of parameters, we analyzed the relevant physical mechanism by considering the microphysical process of PD formation and development as well as the distortion effect generated by the space charges on the initial field. The relevant physical mechanism effectively allocated PD severity among these features for assessment, and the effectiveness and reliability of using these features to assess PD severity were proved by testing a large number of PD samples.

A Development of Models for Analyzing Traffic Accident Injury Severity for Signalized Intersections (신호교차로 안전성 향상을 위한 사고심각도 모형개발)

  • As the interest in traffic safety has been increasing recently, social movement is being made to reduce the number of traffic accidents and the view on improving the mobility of the existing roads is being converted into on establishing traffic safety as a priority. The increase of traffic accidents related to an intersection in a state that traffic accidents are decreasing overall may suggests the necessity to investigate the specific causes. In addition, we have to consider them when establishing the measures against traffic accidents in a intersection by investigating and analyzing the influences and factors that may affect traffic accidents. To induce the accident severity model, we collected the factors that affect accidents and then applied the Poisson Regression Model among nonlinear regression analysis by verifying the distribution of variables. As a result of the analysis, it turned out that the volume of traffic on main roads, the right turn ratio on sub-roads, the number of ways out on sub-roads, the number of exclusive roads for a left turn, the signals for a right turn on main roads, and an intersect angle were the factors that affect the accident severity.
