Childhood Hypersomnia and Sleep Apnea Syndrome (소아수면과다증과 수면무호흡)

  • Natural sleep pattern and its physiology in childhood are much different from those in adulthood. Several aspects of clinical evaluation for sleepiness in childhood are more difficult than in adulthood. These difficulties are due to several factors. First, excessive sleepiness in childhood do not always develop functional impairments. Second, objective test such as MSLT may not be reliable since it is hard to be certain that the child understand instructions. Third, sleepiness in children is often obscured by irritability. paradoxical hyperactivity, or behavioral disturbances. Anseguently, careful clinical evaluation is needed for the sleepy children. Usual causes of sleepiness in children are the disorders that induce insufficient sleep such as sleep apnea syndrome, schedule disorder, underlying medical and psychiatric disorder, and so forth. After excluding such factors, we can diagnose the hypersomnic disorders such as narcolepsy, Kleine-Levin syndrome, and idiopathic central nervous system hypersomnia. Among the variety of those causes of sleepiness, I reviewed the clinical difference of narcolepsy and obstructive sleep apnea syndrome in childhood compared with in adulthood. Recognition of the childhood narcolepsy is difficult because even severely sleepy children often do not develop pathognomic cataplexy and associated REM phenomena until much later. Since childhood narcolepsy give srise to many psychological, academical problem. Practicers should be concerned about these aspects. Childhood obstructive sleep apnea syndrome is different from adult obstructive sleep apnea syndrome too. Several aspects such as pathophysiology. clinical feature, diagnostic criteria, complication, management, and prognosis differ from those in the adult syndrome. An important feature of childhood obstructive sleep apnea syndrome is the variety of severe complications such as behavioral disorders, cognitive impairment, cardiovascular symptoms, developmental delay, and ever death. Fortunately, surgical interventions like adenotosillectomy or UPPP are more effective for Childhood OSA than adult form. CPAP is a "safe, effective, and well-tolerated" treatment modality too. So if early detection and proper management of childhood OSA were done, the severe complication would be prevented or ever cured.

EFFECTS OF EARLY FUNCTIONAL LOADING ON BONE FORMATION AROUND TITANIUM PLASMA SPRAYED IMZ IMPLANTS IN DOGS (성견에서 Titanium Plasma Sprayed IMZ 임플란트에 가해진 조기 기능하중의 골유착 및 골형성에 미치는 영향)

  • The purpose of this study was to observe the effects of early functional on bone formation around titanium plsama sprayed IMZ implants. 15 IMZ implants were inserted on the mandibular premolar areaa in 5 mondgrel dogs. The inplants were divided into 4 groups according to it’s loading schedule : Temporary abutments were connected for functional loading 6weeks, 9weeks, and 12weeks after implants installation in group I, II, and III, respectively. Abutments were not connected in the control group. After polyfluorochrome sequential labeling, all animals were sacrificed and bone sections includig implants and the 1st. molar were removed and processed for histologic evaluation. From light and fluorescence microscopic evaluation as well as histomorphometric and EPMA analysis, following results were obtained : 1. Light microscopically, the osteoid tissues were observed much in the compact bone of unloaded group, and cancellous bone formation observed more in early loaded group. 2. Calcification and remodeling of bone took place faster in loaded group, especially in 6 weeks group. 3. Moderate to severe marginal bone resorption was observed in 6 weeks group, mainly on buccal crest. 4. There was no difference in the percentages of bone-to-metal contact among the experimental groups. 5. In quantitative analysis using EPMA, there was no difference in the content of calcium and phosphorus among the experimental groups, regardless of distance.

  • Special Protection Scheme(SPS) that operates scenarios about faults beyond the normally protective action is wide system protection technology for the purpose of wide areas protection. Therefore, the SPS focuses on the improvement of the power supply capability by protecting the system rather than protecting the system equipments. Since the SPS requires emergency operation, the operation schedule is set up in advance by analyzing various scenarios. Since the SPS's action scheme uses generator tripping and is a classical method it is presently the most powerful one. However, as the setting of SPS is set to the most severe disturbance, the scheme tends to trip more generators than required to prevent fault propagation. It is highly likely that tripping generator units to prevent fault propagation would result in difficulty of system management and possibility of load shedding. Accordingly, it is desirable that generators are connected to the system within the range that ensures system stability and intelligent SPS is currently under development to solve the problem being stated. In this paper, as a part of developing the intelligent SPS, application of the fast-valving and braking resistor scheme to the generators is being proposed and analysed to reduce the number of tripped generators.

Factors Affecting of Long Term Care Hospital Patient's Intention of Transfer to a Nursing Home (요양병원 입원환자의 요양시설 이동의사에 영향을 미치는 요인)

  • Purpose: To examine factors affecting long-term care hospital patients' intention of transfer to a nursing home. Method: A questionnaire survey was conducted in Aug. 2007 that included 655 patients from 49 long-term care hospitals. The survey aimed to assess the patients' health status, family status, cost and intention of transfer to a nursing home. Institutional characteristics were analyzed from the nationwide database of Health Insurance Review & Assessment Service. The affecting factors were examined by employing chi-square test and logistic regression using SAS 8.2. Result: Of the subjects, 32.4% had intention of transfer to a nursing home. The intention of transfer to a nursing home was affected by moderate or severe pain, living together with the primary carer, high cost uncovered by insurance, and recognition of nursing home. Conclusion; For appropriate service utilization. a higher level of care is needed to satisfy patients at nursing homes and a balanced fee schedule is needed between long term care hospitals and nursing homes. It is desirable to encourage transfer to a nursing home at which nurses support patients and their families by giving information, coordination, and to make efforts to establish a reference system.

  • The antigenic potential of M3S tumor necrosis factor-$\alpha$(M3S TNF), which is a mutated form of TNF(TNF mutein) designed to reduce adverse effects of wild type human TNF, was investigated in the present study. The antigenicity of M3S TNF was examined by conducting active systemic anaphylaxis (ASA) test in guinea pigs, heterologous(mouse-rat) passive cutaneous anaphylaxis(PCA) test and passive hemagglutination(PHA) test. The experimental animals were divided into low, medium, high and the highest dose groups and the groups with or without immunoadjuvant, sensitized according to the appropriate schedule and challenged. In ASA test, when challenged with 120 $\mu\textrm{g}$ /animal, moderate to severe positive anaphylactic responses were observed in groups sensitized with 12 $\mu\textrm{g}$ /animal, 120 $\mu\textrm{g}$ /animal and 120 $\mu\textrm{g}$ /animal+Freund's complete adjuvant. In PCA test, positive responses were observed in the group sensitized with the highest dose emulsified with an alum(12 $\mu\textrm{g}$ /animal+alum). In PHA test, positive responses were observed in the group sensitized with 3 $\mu\textrm{g}$ /animal emulsified with an alum. All the other groups in each experiment showed negative responses. Based on these results, M3S TNF is considered to have some antigenic potential.

  • The abnormal grain coarsening in wire rolling induces detrimental defects, such as jagged size tolorance, severe bending after heat treatments and drawing troubles, in the following secondary processes. Neishi et al observed that there is a band type region where grain coarsenting occurs in the plastic strain vs. deformation temperature plot. Based on the finding, we have investigate whether grain sizes and ferrite volume fractions are correlation to deformation strain with three kinds of wire rod diameters as for the different average deformation conditions. The samples were chosen from the No.2 Wire Rod Mill of POSCO where 3-roll type of finishing mill stand are used. It was found in the present work that the grain size and ferrite volume fraction of the rolled and cooled microstructure were changed with rolling reduction and rolling temperature. Abnormally grown grains at various observed points were also found. To have homogeneously fine grains of microstructure from the No. 2 Wire Rod Mill, it will be easier to control finish rolling temperature at around 750$^{\circ}C$ rather than to find another rolling schedule.

  • A 2-year-old, female, American cocker spaniel dog presented for a 1-year history of severe ascites, exercise intolerance, tachypnea. At that time, she was in an emergency state. First, the dog was stabilized with oxygen therapy. A diagnosis of cardiac problem was made from history, auscultation, radiograph, ECG, and echocardiography. Jugular pulsation was palpated and a harsh, systolic murmur of tricuspid regurgitation was prominent at the right cardiac apex. Tricuspid valve dysplasia (TVD) was confirmed with echocardiography, accompanying enormous myocardial hypertrophy. The clinical signs had been improved for 8 months with careful therapy and periodic abdominocentesis, and ascites was well controlled. The situation, however, became worse quickly in a week because the client did not follow our management schedule. Finally, she died due to dyspnea and shock. After the spontaneous death, necropsy and histopathological examination were performed and when we opened the thorax, a significantly large heart was observed. On histopathological findings, grossly myocardium appeared pale initially, then progressed to yellow and white. Microscopically, there was an extensive hemorrhage along with loss of myocardial striations. Interstitial fibrosis and various degenerative alterations in myocytes were also present.

Chemical Control of bacterial Canker of Kiwifruit (참다래 궤양병의 약제 방제)

  • Chemical control of bacterial canker of kiwifruit caused by Pseudomonas syringae pv. actinidiae was attempted by spraying of streptomycin sulfate ·oxytetracycline WP streptomycin WP streptomycin ·copper hydroxide WP kasugamycin SL kasugamycin·copper oxychloride WP and copper hydroxide WP. The control efficacies of the bactericides were variable depending upon the spraying schedule,. Application of streptomycin WP and streptomycin sulfate·oxytetracycline WP from middle April to early May was found to be the most effective in controlling the bacterial canker. For copper hydroxide WP the spraying from middle January to early February showed the highest control efficacy. Kasugamycin SL was the most effective in controlling the disease by spraying from middle April to early May but it was still relatibvely effective during other spray periods. Foliar application of copper hydroxide WP and copper-antibiotic formulaions after middle April caused severe phytotoxicity. Kasgamycil SL streptomycin WP streptomycin·copper hydroxide WP and copper hydroxide WP were potential bactericides which could substitute streptomycin sulfate·oxytetracycline WP. Selective applications of the bactericides according to their optimum spray time can enhance the control efficacies against bacterial canker of kiwifruit and retard the emergency of resistant strains of P. syringae pv. actinidiae to the bactericides. The optimum spray number of streptomycin sulfate·oxytetracycline WP was 3 times with 15-day-intervals or 4 times with 10-day-intervals. The result suggested that the potential bactericides to bacterial canker of kiwifruit should be also used according to their optimum spray schedules in order to get their highest control efficacies.

  • Prader-Willi syndrome (PWS) is a multisystemic complex disorder characterized by hyperphagia and impaired satiety which lead to severe and early obesity. In infancy, hypotonia and poor suck are main problems, and a child goes through Failure-to-thrive. During childhood, clinical manifestations change to food seeking as well as excessive weight gain, short stature, developmental delay, cognitive disability and behavioral problems. Also, growth hormone insufficiency is frequent. Most patients receive the recombinant growth hormone (rGH) therapy that provides improvement in growth, body composition, and physical attributes. The clinical care guideline for rGH therapy in PWS had been noticed in 2013. The rGH therapy helps in body fat, lean body mass, height SDS and head circumference. Also, the rGH therapy helps motor function, psychomotor development and cognition and behavioral issues.In Samsung medical center, there are clinical care guidelines for rGH therapy in PWS and an useful application for the patients. 'Living with PWS', the name of an moblie application for PWS patients, was introduced in the lecture. The application revised to version 2. It was made more convenient to users than in version 1. It helps caregivers to schedule the rGH therapy and to monitor height and weight.
