정상 성인의 혈청 Vitamin D 수준과 이에 영향을 주는 변인에 관한 연구 (A Study on Vitamin D Status and Factors Affecting It in Young Adults)

  • In this study, the serum level of 25-hydroxyvitamin D(25-(OH)D) was measured by high pressure liquid chromatography(HPLC), and factors affecting it were investigated in 72 young adults age ranging from 21 years to 39 years with normal bone density. The mean level of serum 25-(OH)D was 20.0$\pm$6.8ng/ml in males and 26.1$\pm$12.3ng/ml in females, which was significantly higher in females (p<0.01). The serum level of parathyroid hormone(PTH) showed a negative correlation with that of 25-(OH)D(p<0.05). Time spent outdoors in a day correlated positively with the serum level of 25-(OH)D(p<0.01). During the day, a specific time between 12:00 a.m. and 2:00 p.m. showed the most significant correlation with the level of 25-(OH)D(p<0.005). Among the nutrients studied, fat and vitamin D intake were positively correlated with the serum 25-(OH)D level. Stepwise multiple regression analysis showed that the serum level of 25-(OH)D could be fit by vitamin D intake(34.7% explained), serum PTH level (27.3% explained) and the time spent outdoors during the specific time(28.4% explained).1996)

비만 지표와 심혈관계질환 위험인자간의 상관성 및 복부비만지표의 유용성 (Correlation between Obesity Indices and Cardiovascular Risk Factors and Usefullness of Abdominal Obesity Indices)

  • It is a well known fact that obesity is an important cause of cardiovascular disease, emphasized by many studies. Recently, cardiovascular diseaase has been found to correlate not only to the extent of obesity, but also the fat distribution of the individual; especially, focusing on obesity of the abdomen. Unfortunately, the proposed indices for abdominal obesity are numerous, and the results vary according to the index chosen. Three-hundred and twelve bus drivers in November, 1995, were chosen as subjects of this study. The author chose to measure serum lipid levels, fasting blood sugar levels and blood pressure, that are thought to be important risk factors of cardiovascular diseases. Obesity indices were calculated using anthropometric measurements. We were able to evaluate the significance of obesity indices by examining correlations between these indices and the risk factors of cardiovascular disease. The results obtained were as follows: 1. The abdominal obesity indices and risk factors of cardiovascular disease, the levels of total cholesterol in the serum, fasting blood sugar levels, and diastolic blood pressure, increased significantly according to age. 2. There was a significant difference in the abdominal obesity indices according to drinking and smoking habits controlled for age. Among the risk factors of cardiovascular disease, triglyceride and diastolic pressures had significant differences according to the presence or absence of a drinking history controlled for age. 3. Although all obesity indices showed significant correlations, the weakest correlation was between BMI and abdominal diameter index and the strongest correlation was between sagittal diameter and sagittal diameter matched for height. 4. There was a negative correlation between HDL-cholesterol and obesity indices. The weakest correlation was between fasting blood sugar levels and both SD and SDH showed correlations with the risk factors. 5. There was a significant correlation between SD and total cholesterol in the serum and fasting blood sugars controlled for age, drinking, and BMI. 6. After categorizing the subjects into 2 separate age groups at the 40 year mark, in the less than 40 you old age group, controlled for drinking and BMI, the results of comparitive studies have shown correlations between total cholesterol serum levels and waist-hip ratio, conicity-index, and SD. There were correlations between fasting blood sugar levels and SD, ADI, and SDH. There were no correlations between obesity indices and both total cholesterol serum levels and fasting blood sugar levels in the greater than 40 year old age group. There were significant correlations between abdominal obesity indices and total serum cholesterol or fasting blood sugar levels in the less than 40 year old age group, but no correlations in the age group over 40. These correlated factors between abdominal obesity and cardiovascular disease are assumed to exist in Korea as well. Furthermore, in this study a high correlation was found between SD, SDH and the risk factors of cardiovascular disease. Even when controlled for age, drinking, smoking, and BMI, the correlations between risk factors of cardiovascular disease and these indices exist. Therefore, the obesity indices, SD and SDH may prove to be important prognostic indicators or risk factors of cardiovascular disease.

Evaluation of Dietary Risk Factors for Abnormal Serum Cholesterol in Korean Sedentary Male Adults

  • This study investigated whether dietary factors are more influential factor than other health behavior such as drinking, smoking and exercise on abnormal serum cholesterol level inspite of Korean dietary pattern differences compared to Europeans and Americans. A double case control study model has been used for the study design. One model consisted of high blood cholesterol cases and control. the other model consisted of low blood cholesterol cased and controls. 5.398 sedentary male workers who had taken medical examinations at a university hospital were used as the study subjects. Out of the study subjects, 36individuals with high blood cholesterol cases and 30 individuals with low blood cholesterol cases were selected. For the 66 individual control selection, the individual control selection, the individuals matching method was adopted. The food frequency method was used to collect the data for assessment of the dietary factors. A standardized questionnaire was used to investigate other health behavior. logistic regression analysis was employed to measure the relative importance between the factors considered. There were no statistically significant differences observed in nutrients consumption or other health behavior among the low, normal and high blood cholesterol groups, An overmatching effect had been suspected as the cause of those findings. However, the results of logistic regression analysis to identify the factors influencing high serum cholesterol showed that odd ratios of dietary factors such as tocopherol(3.0) and saturated fatty acid(1.6) were higher than I. I of smoking and 1.2 of drinking. Similar results were also observed incases of low serum cholesterol. The above findings imply that although the dietary pattern is quite different from that of Europeans and America, the dietary factor is still a significant factor for abnormal blood cholesterol in Koreans. Therefore, the dietary risk factor identified in high fat consumption populations are still relevant for the relatively healthy Korean as guideline for preventive health practices. 도시 저소득층 여자노인의 고지혈증과 관련된 우울정도와 식생활 요인에 관한 연구 (Depression and Dietary Factors Related to Hyperlipidemia in Urban Living Elderly Female from Low Income Group)

  • We conducted a case-control study to examine the relationship of depression and dietary related factors with the hyperlipidemia for urban living elderly women from low income group. The case group consisted of 45 elderly females with hyperlipidemia (serum cholesterol $\geq$ 240mg/dl or serum TG $\geq$ 250mg/dl and the control group of 95 age matched elderly women with serum cholesterol levels less than 240mg/dl and serum TG less than 250mg/dl. In a univariate analysis, vitamin C intake, the number of family members living with the subject, and their depression scores were significantly higher in the hyperlipidemic group than in the control group. In the logistic regression analysis, the vitamin C intake ($\geq$75% Korean RDA), the number of family members living with the subject ($\geq$ 1), depression scores ($\geq$7), BMI ($\geq$27), and subscapular skinfold thickness ($\geq$18mm) were associated with significantly higher (p<0.05) risks of hyperlipidemia in the elderly women. However after adjustment for other covariables, the depression scores (Odds Ratio 2.48 for depression score$\geq$7;95%CI:1.10-5.60) and subscapular skinfold thicknesses (Odds Ratio 5.69 for SBT$\geq$18mm, 95%CI:1.87-17.32) were the significant risk factors associated with hyperlipidemia in the elderly women.

일부 폐경전 성인직장여성의 겨울철 혈청 25-Hydroxyvitamin D상태에 관한 연구 (Serum 25-Hydroxyvitamin D Status in Wintertime in Premenopausal Working Women)

  • This study was performed to estimate serum 2S-hydroxyvitamin D (25-OHD) level in wintertime and to evaluate the relationship between serum 25-OHD level and associated factors in 50 premenopausal working women aged 30-49 y in Busan. The serum 25-OHD level was measured by radioimmunoassay. Data for physiological characteristics, lifestyle factors, physical activity and nutrient intake was assessed by questionnaire including information about outdoor activity time, daily activity diary and 24hr recall method. The mean vitamin D intake was $4.24{\mu}g$, which corresponded to 84.9% of the Korean RDA. The mean level of serum 25-OHD was 25.7 ng/mL. Vitamin D deficiency (25-OHD < 7nmol/L) and toxicity $(25-OHD{\geq}75\;nmol/L)$ were not found in the subjects. However, the prevalence of vitamin D insufficiency (25-OHD < 15 nmol/L) and hypovitaminosis D (25-OHD < 30 nmol/L) were 12% and 60% respectively. The serum 25-OHD level showed positive significant correlations with the duration of outdoor activity per weekdays (p < 0.05). Our findings suggest that hypovitaminosis D was common in the subjects in wintertime. So nutritional education for increasing outdoor activities is needed for premenopausal working women to increase vitamin D status in wintertime.

  • The study was undertaken to examine the levels of serum lipids and the correlations between serum lipid levels and other factors(nutrient intakes, anthrometric measurement, blood pressure, drinking, smoking). The participants in this study were healthy volunteers over the age of 40 residing in Whachon area, Kang-Wan Do. The level of serum TG was higher in males than in females, particularly in age of 40(209.8mg/dl$\pm$154.9, p<0.05). The HDL-c/total cholesterol ratio(HTR) in males was the lowest(0.23$\pm$0.07, p<0.05) in age of 40 and in females was the lowest(0.25$\pm$0.1) in age of 60 and over. LDL-c/HDL-c ratio(LHR) was the highest (males 3.64$\pm$6.29, females 3.28$\pm$3.03) in age of 40 in both sexes. In the age of 40-59, serum TG, total cholesterol and LDL-cholesterol and HDL-cholesterol tended to increase with BMI. Serum TG showed positive correlation with drinking (r=0.026, p<0.001) and BMI (r=0.2096, p<0.01). BMI was negatively correlated with HTR(r=-0.2242, p<0.01) and HDL-cholesterol (r=-0.2054, p<0.01).

Risk Factors of Treatment Failure in Diabetic Foot Ulcer Patients

  • Background Some diabetic feet heal without complication, but others undergo amputation due to progressive wounds. This study investigates the risk factors for amputation of diabetic feet. Methods A total of 55 patients who visited our institution from 2008 to 2012 were included in the study. The patients with abnormal fasting blood sugar levels, lower leg vascularity, and poor nutrition were excluded from the study group, and the wound states were unified. The patients were categorized into a treatment success group (n=47) and a treatment failure group (n=8), and their hemoglobin A1C (HgA1C), C-reactive protein (CRP), white blood cell count (WBC), and serum creatinine levels were analyzed. Results The initial CRP, WBC, and serum creatinine levels in the treatment failure group were significantly higher than that of the treatment success group, and the initial HgA1C level was significantly higher in the treatment success group. The CRP and WBC levels of both groups changed significantly as time passed, but their serum creatinine levels did not. Conclusions The initial CRP, WBC, and serum creatinine levels were considered to be risk factors for amputation. Among them, the serum creatinine level was found to be the most important predictive risk factor. Because serum creatinine represents the renal function, thorough care is needed for the feet of diabetic patients with renal impairment.

한국농촌 성인의 혈청 Carotenoids 농도와 이에 영향을 미치는 요인에 관한 연구 (Levels and Factors Affecting Serum Carotenoids Concentrations of Adults Living in Rural Area of Korea)

  • Recent researches suggest that carotenoids are important not only as provitamin A but also for prevention of chronic diseases. This study was conduction to determine levels and factors affecting serum levels of lutein + zeaxanthin, $\beta$-cryptoxanthin, and $\beta$-carotene in 93 adults living in rural area of Korea. Fasting blood samples were collected and serum carotenoid levels were measured by HPLC. Dietary intake was estimated by 24 hour recall method and frequency questionnare of major food groups. Mean serum concentration of lutein + zeaxanthin was 616.32 nmol/L, $\beta$-cryptoxanthin was 856.95nmol/L, and $\beta$-carotene was 242.90nmol/L. Serum $\beta$-carotene levels in study subjects were very low. Both $\beta$-cryptxanthin and $\beta$-carotene were negatively correlated with serum triglyceride and positively correlated with total-choesterol and LDL-cholesterol. Serum levels of female subjects were significantly higher than males in all carotenoids. For age groups, subjects in their 30's were shown to have the highest concentration of all carotenoids. Lutein + zeaxanthin were lowest in subjects in theri 40's while $\beta$-crytoxanthin and $\beta$-carotene levels were lowest in subjects in their 60's. The $\beta$-carotene levels in non-smokers were significantly higher than in drinkers. Lutein+zeaxanthin levels were significantly higher among subjects consuming more green and yellow vegetables by frequency questionnarie. In conclusion, serum carotenoids were affected by sex, age, serum lipids, smoking, and alcohol intake. Intake of vegetables and fruits could affect by sex, serum lipids, smoking, and alchol intake. Intake of vegetables and fruits could affect serum lutein+zeaxanthin level. This data indicated that compared to other studies, Korean adults in rural areas have high lutein+zeaxanthin concentratins and low $\beta$-carotene concentrations in serum. High lutein+zeaxanthin levels may be related to high consumption of vegetables in these subjects.

Relation of serum total antioxidant status with metabolic risk factors in Korean adults

  • The purpose of the present study was to examine the relation of total antioxidant status (TAS) to metabolic risk factors in Korean adults. Anthropometric measures, blood pressure, serum lipids and fasting glucose were determined in 406 men and women. TAS was measured by using commercially available Randox kit. Serum TAS was significantly positively correlated with body weight (p=0.004), body mass index (BMI) (p=0.033), waist circumference (p=0.017), total cholesterol (p=0.038) and triglyceride (TG) (p<0.001). The mean TAS of hypertriglyceridemic subjects (TG ${\geq}$150 mg/dl) was significantly higher than that of subjects whose TG was lower than 150 mg/dl (p=0.001). When central obesity, TG, high density lipoprotein cholesterol, fasting glucose and blood pressure were considered as metabolic risk factors, TAS was shown to be elevated with increased number of metabolic risk factors (p=0.004). The positive association between TAS and a number of metabolic risk factors suggests that increased TAS may not always indicate one's healthier condition. In order to help understand TAS as a marker of total antioxidant capacity in humans with various metabolic conditions, it is needed to clarify the factors affecting TAS in relation to changes in metabolic risk factors.

Relations of Serum Visfatin and Resistin Levels with Endometrial Cancer and Factors Associated with its Prognosis

  • Background: The aims of this study were compare the serum visfatin and resistin levels between endometrial cancer (EC) patients and controls and evaluate their power to predict prognosis. Materials and Methods: This prospective study was conducted between March 2013 to June 2014 on the Gynecologic Oncology Department of the University of Selcuk, Konya, Turkey. A total of 42 EC patients and 42 controls were included and assessed for differences in serum visfatin and resistin levels, along with prognostic factors. Results: Endometrial cancer patients had significantly higher visfatin levels than control s (p: 0.011), associated with deep myometrial invasion (p: 0.019). In contrast the serum level of resistin did not significantly differ between EC patients and controls (p: 0.362). However, high resistin level in EC patients was associated with increase lymph node metastasis (p: 0.009). On logistic regression analysis, we found that serum visfatin elevation was associated with risk of myometrial invasion (OR: 1,091; 95%CI: 1.021-1.166; p: 0.010) and serum resistin with risk of lymph node metastasis (OR: 1.018; 95%CI: 1.000-1.035; p: 0.046). For myometrial invasion prediction, a serum visfatin level greater than 26.8 ng/mL demonstrated a sensitivity and specificity of 66.6 % and 96.4%, respectively. For lymph node metastasis prediction, the best cut-off for serum resistin level was 599ng/mL. A serum resistin level greater than this demonstrated a sensitivity and specificity of 87.5% and 77.1%, respectively. Conclusions: Our data suggest that serum visfatin is elevated in patients with EC and serum visfatin and resistin levels could be used to predict the risk of advance stage lesions.
