Changes of Renal Function and Treatment after CABG in Patients with Elevated Serum Creatinine (크레아티닌치가 높은 환자에서 관상동맥우회술 후 신장기능의 변화와 처치)

  • Background: Preoperative elevated serum creatinine values are associated with increased risk for both morbidity and mortality in patients undergoing on-pump coronary artery bypass surgery (CABG). We investigated the postoperative changes of renal function and proper management in the patients. Material and Method: Among 74 consecutive patients who underwent isolated on-pump CABG, 17 patients with increased serum creatinine level $(creatinine\;\geqq\;1.5\;mg/dL)$ within preoperative one week wereincluded in the study. Seven patients showed pre­operative serum creatinine level of 2.0 mg/dL or higher, and 3 of them had been undergoing hemodialysis. Preoperative hemodialysis was performed in the 3 patients due to end-stage renal failure (ESRD) the day before the operation. We started peritoneal dialysis immediately after the cardiopulmonary bypass in patients with ESRD or postoperative acute renal failure if it was necessary to remove intravascular volume and lower serum creatinine level. Result In most of the patients with CABG, postoperative serum creatinine level increased and recovered to the preoperative level at the discharge. In 2 of the 4 patients with serum creatinine level of 2.0 mg/dL or higher and 3 patients with ESRD, intravascular volume, serum creatinine level and serum electrolyte were controlled with peritoneal dialysis. Conclusion: Postoperative serum creatinine level increased transiently in most of CABG patients, and intravascular volume and serum creatinine level were controlled by peritoneal dialysis only in the patients with acute renal failure postoperatively and those depending on hemodialysis.

  • Creatinine is a general marker as a screening test for renal disease. This study was conducted to provide basic data about pediatric concentration for serum and urine creatinine. The data from the 2011 Korean National Health and Nutrition Examination Survey was used. Analysis was done on 488 Children and Adolescents (boys 278, girls 210) aged 10 to 19, the Jaffe reaction method was used to gather the data. The highest serum creatinine levels were found in boys aged 19 to 20 years, the mean being 0.97 mg/dL (min 0.81 mg/dL, max 1.14 mg/dL). The levels showed increase over those in the 10 year old group. The highest urine creatinine levels were found aged 19 to 20 years, and the mean was 222.68 mg/dL (min 133 mg/dL, max 324 mg/dL). In the case of girls, the highest serum creatinine levels were found with those aged between 18 and 19, the mean being 0.71 mg/dL (min 0.49 mg/dL, max 0.84 mg/dL). The levels showed increase over the 11 to 18 year old group. The highest urine creatinine levels were found aged 14 to 15 years, and the mean was 218.44 mg/dL (min 131 mg/dL, max 321 mg/dL). The mean difference in serum creatinine in all age groups was statistically significant except for those aged 10 to 14, that of urine creatinine in all age groups was not significant statistically except for those aged 12 to 13, 17 to 18. Therefore, it is suggested that reference values for children and adolescents should be divided into different groups according to gender, further studies are needed using complementing data of the pediatrics.

A Correlation Analysis of Serum Creatinine Based eGFR and Serum Cystatin C Based eGFR with Thyroid Dysfunction Patients (갑상선기능이상 환자에서 크레아티닌과 시스타틴을 이용한 사구체여과율 검사결과의 상관성 연구)

  • Serum creatinine-based eGFR and serum cystatin C-based eGFR are the most popular methods for measuring renal function. Thyroid hormone is known to affect serum creatinine-based eGFR and serum cystatin C-based eGFR; however, the clinical significance of thyroid dysfunctional patients of renal function evaluation has not been fully elucidated to date. This study examined the effect of thyroid hormone on serum creatinine-based eGFR and serum cystatin C-based eGFR. Moreover, we also evaluated the correlation analysis between serum creatinine-based eGFR and serum cystatin C-based eGFR in patients with thyroid dysfunction. A total of 442 patients with hypothyroidism and 284 patients with hyperthyroidism were investigated. A correlation analysis between thyroid hormone and serum creatinine- (and cystatin C-) based eGFR was performed. A correlation analysis between thyroid hormone and serum cystatin-C based eGFR indicated that serum cystatin-C based eGFR is more of an independent biomarker than serum creatinine-based eGFR in thyroid dysfunction patients. Therefore, serum cystatin C-based eGFR more accurately reflects renal function than serum creatinine-based eGFR in thyroid dysfunction patients.

  • The effect of exercise on serum cholesterol and triglyceride concentration and on urinary creatinine and 3-methylhistidine excretion was studied in eight healthy women, aged 21 to 25yr. Subjects were tested before and after a 3-day experimental period, during which 1000 times rope-jumping a day were performed. Serum cholesterol and triglyceride levels decreased significantly after exercise. Urinary excretion of creatinine and 3-methylhistidine slightly increased after exorcise.

The Clinical Usefulness of Cystatin C in Evaluating Renal Function in Children with Various Renal Diseases (다양한 신질환을 가진 소아에서 Cystatin C 검사의 임상적 유용성)

  • Purpose : GFR(glomerular filtration rate) is a fundamental parameter in detecting renal impairment and predicts the progression of renal disease. Because serum creatinine has several disadvantages, serum cystatin C has been recently proposed as a new endogenous marker for GFR. We compared serum cystatin C with creatinine and creatinine clearance to investigate the clinical usefulness of cystatin C. Methods : We retrospectively analyzed 46 patients(60 case numbers) who had various renal diseases and classified them into 3 groups according to creatinine clearance(Group 1 : CrCl <40 mL/min/$1.73m^2$, Group 2 : CrCl 40-60 mL/min/$1.73m^2$, Group 3 CrCl >60 mL/min/$1.73m^2$). We measured serum creatinine, cystatin C and creatinine clearance and also analyzed the correlations among them. Results : Serum cystatin C and creatinine showed a similar correlation to creatinine clearance (r=0.685, r=0.640, respectively) and showed similar diagnostic accuracy in detecting decreased GFR(AUC, cystatin C 0.829 vs. creatinine 0.826, P=0.848). Serum cystatin C showed a greater sensitivity for detecting a decreased GFR than creatinine in Group 2 and 3(Group 1 : 100% vs. 100%, Group 2 : 70% vs. 35%, Group 3 : 46% vs. 15%). Conclusions : Serum cystatin C could be a useful endogenous marker for GFR and would be superior to serum creatinine in early detection of renal impairment in pediatric patients with renal diseases.

Comparison of various methods of glomerular filtration rate measurements in children (소아 환아에서 다양한 사구체 여과율 측정법의 비교)

  • Purpose : Glomerular filtration rate (GFR) is a fundamental parameter in assessing renal function and predicting the progression of chronic renal disease. Because the use of serum creatinine has several disadvantages, many studies have investigated the use of cystatin C for estimating GFR. We compared creatinine clearance and GFR with formulas using serum creatinine and cystatin C. Methods : We retrospectively analyzed 211 patients with various renal diseases and classified them into two groups according to creatinine clearance (Group 1: CrCl >$90mL/min/1.73m^2$, Group 2: CrCl <$90mL/min/1.73m^2$). We measured serum creatinine, cystatin C, and creatinine clearance. We calculated GFR using the Schwartz, Counahan, Filler and Lepage, Bokencamp et al, and Grubb et al formulas. Results : GFR determined by the Schwartz formula had the highest correlation to creatinine clearance (r=0.415, P=0.00). GFR determined by various formulas using cystatin C had lower correlation to creatinine clearance (r=0.187, r=0.187, r=0.291). The Schwartz and Counahan formulas showed greater diagnostic accuracy in detecting decreased GFR than cystatin C in group 2 (areas under the curve: Schwartz, 0.596; Counahan, 0.572; Filler, 0.512; Bokencamp, 0.508; and Grubb, 0.514). Conclusion : GFR determined by the Schwartz and Counahan formulas using serum creatinine showed higher correlation coefficient than that determined by formulas using cystatin C. The formulas using cystatin C were not superior to those using serum creatinine in detecting decreased GFR. Cystatin C measurement was not satisfactory for assessing GFR in patients whose renal function was not severely decreased.

  • Renal dysfunction was experimentally induced with reductions in renal mass in dogs and then the remaining kidney function was estimated by blood urea nitrogen and serum creatinine concentrations. During experimental period, blood urea nitrogen and serum creatinine concentrations showed no variance in dogs with 50% reductions in renal mass, but these values were remarkably increased in dogs with 75% reductions in renal mass. It was considered that 75% reductions in renal mass was applicable as experimental models for renal dysfunction.

Chromium and Nickel Concentrations in Urine and Serum of None Exposed Group and Workers in Electroplating Plants (정상인과 도금업 근로자의 요 및 혈청중 크롬 및 니켈 농도)

  • The exposure levels of chromium and nickel for chromeplating workers were evaluated. Chromium and nickel concentrations in urine and serum from 82 exposed workers and 69 controls, who were not exposed occupationally to metals, were analyzed by flameless atomic absorption spectrophotometry. The results were as follows: 1. Chromium concentrations in urine of exposed group and control were $3.49{\pm}1.83g/g$ of creatinine, $5.59{\pm}2.83g/g$ of creatinine, and in serum were $0.69{\pm}0.30g/l$, $2.31{\pm}1.16g/l$ respectively. There were significant difference of concentrations for chromium in urine and serum by group respectively. 2. Nickel concentrations in urine of exposed group and control were $0.92{\pm}0.23g/g$ of creatinine, $2.20{\pm}1.93g/g$ of creatinine, and serum concentrations were $0.52{\pm}0.34g/l$, $1.41{\pm}0.74g/l$ respectively. There were significant difference of concentrations for nickel in serum by groups statistically. 3. Chromium and nickel concentrations in urine and serum of exposed groups were not significant by workplaces(grinding, electroplants, packaging).

Risk Factors of Treatment Failure in Diabetic Foot Ulcer Patients

  • Background Some diabetic feet heal without complication, but others undergo amputation due to progressive wounds. This study investigates the risk factors for amputation of diabetic feet. Methods A total of 55 patients who visited our institution from 2008 to 2012 were included in the study. The patients with abnormal fasting blood sugar levels, lower leg vascularity, and poor nutrition were excluded from the study group, and the wound states were unified. The patients were categorized into a treatment success group (n=47) and a treatment failure group (n=8), and their hemoglobin A1C (HgA1C), C-reactive protein (CRP), white blood cell count (WBC), and serum creatinine levels were analyzed. Results The initial CRP, WBC, and serum creatinine levels in the treatment failure group were significantly higher than that of the treatment success group, and the initial HgA1C level was significantly higher in the treatment success group. The CRP and WBC levels of both groups changed significantly as time passed, but their serum creatinine levels did not. Conclusions The initial CRP, WBC, and serum creatinine levels were considered to be risk factors for amputation. Among them, the serum creatinine level was found to be the most important predictive risk factor. Because serum creatinine represents the renal function, thorough care is needed for the feet of diabetic patients with renal impairment.

Effects of CPs (Combined Preparations) of Radix Salvia Miltiorrhiza, Radix Notoginseng, and Borneolum on Hyperlipidemia (CPs(단삼, 삼칠, 빙편 복합제)가 고지혈증에 미치는 영향)

  • Objectives: In order to determine the effect of CPs, experimental studies were performed in hyperlipidemia humans. The contents of serum total cholesterol, TG (triglyceride), LDL cholesterol, HDL cholesterol, AST/ALT and BUN/creatinine were measured. Methods: We made a comparative study of total cholesterol, TG (triglyceride), LDL cholesterol, HDL cholesterol, AST/ALT and BUN/creatinine during one month off and on for twenty patients with hyperlipidemia who visited Gunpo Oriental Medical Center of Wonkwang University from December 2002 to July 2003. Results: The following result were obtained: 1. The serum TG and total cholesterol of patients who took CPs for one month showed significant (p<0.01) decrease. 2. The serum HDL-cholesterol and LDL-cholesterol of patients who took CPs for one month showed significantly increase in HDL, and decrease in LDL (p<0.05). 3. The serum AST/ALT and BUN/creatinine of patients who took CPs for one month showed no significant effects. Conclusions: According to the results mentioned above, the effects of CPs on serum triglyceride and total cholesterol of hyperlipidemia patients were significantly confirmed for decreasing effects.
