Experimental Study for Construction of Mouth Care in Chemotherapy Patients (화학요법을 받는 암환자의 구강간호전략을 위한 연구)

  • Stomatitis is a common toxicity associated with the administration of certain cancer chemotherapeutic agents used in the treatment of malignant tumors. It represents one of the most distressing side effects of cancer chemotherapy and can interfere with the patient's ability to eat, be the cause of much pain and discomfort, and require the use of potent analgesics. The situation also creates favorable conditions for local infection which may lead to septicemia. Several authors have identified the need to establish protocols for the control and treatment of the oral discomfort associated with oral mucositis as a result of chemotherapy. Thus this study attempted to development of oral care protocol for chemotherapy patients. The effects of the mouth care using sterile normal saline, nystatine solution on oral stomatitis were investigated in 30 patients on chemotherapy. The subsect were divided into three groups : control group : not gargling experimental group A : normal saline gargling (4 times a day) experimental group B : nystatine solution gargling(4 times a day) The Oral Assessment Guide (OAG) was used to assess oral status three times(once in the prechemotherapy period, on 5th, 10th day of post chemotherapy) Oral culture was used to assess oral infection on 5th day of postchemotherapy. Data was analyzed on SAS program which used repeated ANOVA, t-test, X/sup²test. The results are as follows : 1. The incidence of stomatitis was higher in the control group and experimental group A than in experimental group B.(X/sup²=0.002 P=0.001). The grade of stomatitis(mean of total score) for patients in the experimental group B were significantly lower than in the experimental group A F=1.96 P=0.0024). In incidence of tongue change, control group, experimental group B were significantly higher than experimental group B(F=6.84 P=0.0039). In control group and experimental group A. oral infection due to pathogenic bacteria were identified. In conclusion, mouth care with nystatine solution four times a day could reduce the incidence of stomatitis and secondary oral infection due to stomatitis. Thus active mouth care protocol which used to nystatine solution gargling need to prevention of stomatitis in chemotherapy patients.

  • Objectives: The pathogenic Vibrios genus denotes halophilic bacteria that are distributed in aquatic environments, including both sea and freshwater. Vibrio cholerae, Vibrio vulnificus, and Vibrio parahaemolyticus are the most important species since they can be potent human pathogens and leading causes of septicemia, wound infections, and seafood borne gastroenteritis. The recent emergence of a potential pandemic clone, V. cholera serotype O1 and the cholera outbreak in South Korea in 2016 indicates the importance of consistent surveillance of pathogenic Vibrio genus within coastal areas. Methods: The present study was undertaken to determine where and how vibrios live in the aquatic environment adjacent to coastal areas of South Korea. For this survey, a total of 838 samples were obtained at 35 different sites in South Korean coastal areas during the period from January 2016 to December 2016. Pathogenic vibrios was determined using the real-time PCR method, and its clones were isolated using three selective plating media. We also monitored changes in seawater and atmospheric temperature, salinity, turbidity, and hydrogen ion concentration at the collection points. Results: The total isolation rates of V. vulnificus, V. cholera (non-pathogenic, non-O1, non-O139 serogroups), and V. parahaemolyticus from seawater specimens in 2016 were 14.2, 13.48, and 67.06%, respectively. Conclusions: The isolation rates of pathogenic vibrios genus showed a positive correlation with temperature of seawater and atmosphere but were negatively correlated with salinity and turbidity.

Characterization of Vibrio vulnificus Isolated from Domestic Coastal Area (국내 연안에서 분리한 Vibrio vulnificus의 특성)

  • Five strains of Vibrio vulnificus, which cause serious septicemia in human worldwidely, was isolated from marine environments of Korea costal area from May to July of 2002. The isolated strains were identified by API 20E kit and partial 16S rDNA sequence analysis. 16S rDNA sequence of the isolates showed $99\%$ similarity with V. vulnificus ATCC 27562. The proteins of V. vulnificus isolates were examined by analyzing patterns of the cell lysates and outer membrane proteins (OMP). The OMP separated from cell lysates showed the common protein band. Therefore OMP profiles might be useful for the identification of V. vulnificus sp.

Yersinia enterocolitica isolation from three adult enteritis patients (성인 장염 환자에서의 Yersinia enterocolitica 분리 3예)

  • Yersinia enterocolitica has been known to be an important enteric pathogen especially in Scandinavian countries and Canada. In Korea, the authors reported the first case of Y. pseudotuberculosis septicemia in 1979. In 1980, three isolates of Y enterocolitica were obtained from 3 adult patients with enteritis, besides the already reported one in a 5-month-old child, during March to June 1980. Difficulty in the isolation was experienced; ie., the organism was isolated only from the SS primary isolation plate in one case and in the other two cases only from the SS plates inoculated with overnight culture of selenite broth. The isolates showed typical cultural and biochemical characteristics except for the nonmotility even at room temperature. Two isolates were indole negative possibly belonging to Wauter's biotype 3 and the other one was indole positive belonging to biotype 2. One patient was tested for the serum agglutinin titer on the 8th hospital day and it was found to be 1:128. All of the isolates were susceptible to chloramphenicol, colistin, gentamicin, kanamycin, tetracyclne, and tobramycin by the Kirby-Bauer disc diffusion method. All of the infections were controled by ampicillin, amoxicillin, amikacin, or gentamicin treatment. It is considered urgent to broaden our knowledge on yersiniosis in Korea not only by isolating, serotyping and biotyping of the organism, but also by surveying serum agglutinin titer of enteritis patients and normal individuals.

Isolation and Characterization of Morganella morganii from Asian Water Monitor Varanus salvator (아시아 물왕도마뱀에서 분리된 모가넬라 모가니의 분리동정)

  • An Asian water monitor Varanus salvator with physical wound due to bite which was subsequently infected with bacterium resulting to hemorrhage and pus in the skin blisters, abdominal distention and septicemia. Morganella morganii was isolated and identified from the blood and kidney of the reptile, and confirmed by PCR and biochemical tests. The sensitivity of isolated strains to different groups of antibiotics was also evaluated using the disc diffusion method. Pathogenicity test using M. morganii (SNUFPC-MM01) (1.6 ${\times}$ $10^{11}$CFU/mouse) to suckling and adult mice resulted to the death of all mice. This paper describes the first isolation of M. morganii from Asian water monitor in Korea.

Studies on the Appearance of the Dwarfishness Silkworm caused by Peroral and Hypodermic Inoculation of the Flacherie Virus, Bombyx mori (연화병의 병원체를 경구 또는 피하접종했을 때에 출현하는 왜소잠에 관한 조사연구)

  • Flacherie of all other silkworm diseases greatly affects cocoon crop as it is far-reaching and wide spreading. Fleacherie which kills silkworms caused by bacteria can be classified as bacterial digestive organ disease, and "Sotto" disease. Bacterial digestive organ disease is caused by the bacteria living in the silkworms alementary canal and a majority of flacherie belongs to this disease. Septicemia is caused by bacterias breeding in silkworms body fluid but its attach is comparatively limited during the larva period. "Sotto" disease is caused by eating mulberry leaves infected with bacteria which produce toxin and silkworms are intoxicated and killed by the toxin. The cause of flacherie is mainly due to a poor environment. The unclean and unsanitary silkworm rearing beds help bacterias breeding and bacteria enter silkworms body through mouth organ or skin. The present study is to investigate various causes of flacherie by means of pulverization of silkworm. Filtrated fluid is extracted by centrifuge and hypodermic of peroral inoculation-is given to young and medium silkworms of spring and autumn. The gained results of the experiment are summarized as follows: 1. Silkworms infected with flacherie were pulverized and their filtrated fluid was extracted by centrifuge and inspected under microscope to find polyhedron from the fluid. 2. The experimenting group of peroral inoculation. a) From the third day of peroral inoculation silkworms appetite generally decreased and ate less compared with the control group. b) After 7 days of the inoculation silkworms suffered from empty head, loose bowels and fainting. c) Some of the silkworms still ate but as were shown in Fig. 3 and 4 some dwarfish silkworms were found. d) There was no remarkable difference between 1st and 2nd instar inoculation groups. e) There was a tendency that the number of diseased silkworms was decerased as the increase of instars. 2. The experimental group of hypodermic inoculation a) Both of 3rd and 4th instar inoculation groups showed no remarkable singularity and the number of diseased silkworms decreased. b) The rate of diseased silkworms was comparatively low because the body fluid was acidy or toxin was hard soluble. Hypodermic inoculation could not give much harm to the silkworms compared to peroral inoculation.

Development of an environment-friendly moving aquatic animal rendering equipment and evaluation of fertilizer value for recycling of fish waste (친환경 이동식 수산생물 폐사체 처리장치 개발 및 재활용을 위한 비료 가치 평가)

  • Although aquaculture production rates grown over the years, aquatic animal diseases occur every year which causes substantial economic losses. When an aquatic animal is infected with an aquatic animal pathogen it is either incinerated or buried according to the aquatic life disease control act. Although these methods prevent the spread of disease, it is not environment friendly. Here, we developed an aquatic animal rendering equipment for disposal of fish waste which is environment-friendly and efficient. Also, fertilizer components of fish waste were evaluated value for recycling. The mobile rendering equipment was designed for field operation and/or high temperature and pressure system, oil and water separator, and shredding drying apparatus. During the experiment (July-2016 to November-2016), a total of 53,824 kg fish waste was collected, and 29,216 kg compost of rendering by-product was made. Also, compost made from viral (Viral hemorrhagic septicemia virus) infected fish did not reflect any detectable pathogen. The concentration of nitrogen, phosphorus, and organic matter in the fish waste compost were 2.17%, 26.98%, and 92.44%, respectively. The results suggest that fish waste used in this study was decomposed efficiently as per the official standard for fertilizer product. This equipment can be useful for efficient inactivation of the aquatic animal pathogenic agents and recycling of the fish waste in an environment-friendly manner.

Chest Injuries due to Blunt Chest Trauma (둔좌상에 의한 흉부손상의 임상적 관찰)

  • Chest injuries due to blunt trauma often result in severe derangements that lead to death. And we have to diagnose and treat the patients who have blunt chest trauma immediately and appropriately. A clinical analysis was made on 324 cases of chest injury due to blunt trauma experienced at department of Thoracic and Cardiovascular Surgery, College of Medicine, Kyung Hee University during 8-year period from 1972 to 1979. Of 324 patients of blunt chest injuries, there were 189 cases of rib fracture, 121 of hemothorax or/and pneumothorax, 108 of soft tissue injury of the chest wall only, 41 of lung contusion, 24 of flail chest, 13 of scapular fracture, 7 of diaphragmatic rupture and others. The majority of blunt chest injury patients were traffic accident victims and falls accounted for the next largest group of accidents. Chest injuries were frequently encountered in the age group between 3rd decade and 4th decade [60%] and 238 patients were male comparing to 86 of female [Male: Female = 3:1 ]. In the patients who have the more number of fractured ribs, the more incidence of intrathoracic injury and intraabdominal organ damage were found. The principal associated injuries were head injury on 58 cases, long bone fractures on 37, skull fractures on 12, pelvic fractures on 10, renal injuries on 6 and intraabdominal organ injuries on 5 patients. The principle of early treatment of chest injury due to blunt trauma were rapid reexpansion of the lung by closed thoracotomy which was indicated on 96 cases, but open thoractomy was necessary on 14 cases because massive bleeding, intrapleural hematoma and/or fibrothorax, or diaphragmatic laceration-On 15 cases who were young and have multiple rib fracture with severe dislocation delayed elective open reduction of the fractured ribs with wire was done on the purpose of preserving normal active life. The over all mortality was 2.8% [9 of 324 cases] due to head injury on 3 cases, massive bleeding on 2,wet lung syndrome, acute renal failure on 1 and septicemia on 1 patient.

Identification of the Vibrio vulnificus fexA Gene and Evaluation of its Influence on Virulence

  • Vibrio vulnificus is the causative agent of foodborne diseases such as gastroenteritis and life-threatening septicemia. Microbial pathogenicity is a complex phenomenon in which expression of numerous virulence factors is frequently controlled by a common regulatory system. In the present study, a mutant exhibiting decreased cytotoxic activity toward intestinal epithelial cells was screened from a library of V. vulnificus mutants constructed by a random transposon mutagenesis. By a transposon-tagging method, an open reading frame, fexA, a homologue of Escherichia coli areA, was identified and cloned. The nucleotide and deduced amino acid sequences of the fexA were analyzed, and the amino acid sequence of FexA from V. vulnificus was $84\%\;to\;97\%$ similar to those of AreA, an aerobic respiration control global regulator, from other Enterobacteriaceae. Functions of the FexA were assessed by the construction of an isogenic mutant, whose fexA gene was inactivated by allelic exchanges, and by evaluating its phenotype changes in vitro and in mice. The disruption of fexA resulted in a significant alteration in growth rate under aerobic as well as anaerobic conditions. When compared to the wild-type, the fexA mutant exhibited a substantial decrease in motility and cytotoxicity toward intestinal epithelial cell lines in vitro. Furthermore, the intraperitoneal $LD_{50}$ of the fexA mutant was approximately $10^{1}-10^{2}$ times higher than that of parental wild-type. Therefore, it appears that FexA is a novel global regulator controlling numerous genes and contributing to the pathogenesis as well as growth of V. vulnificus.

luxS and smcR Quorum-Sensing System of Vibrio vulnificus as an Important Factor for In Vivo Survival

  • Vibrio vulnificus is an opportunistic pathogen that causes a septicemia and expresses numerous virulence factors, in which luxS and smcR are genes encoding for components responsible for quorum-sensing regulation. In the present study, null mutants were constructed with lesions in each or both of these two genes from the V. vulnificus Vv$\Delta$Z strain, which is a lacZ$^{-}$ and chloramphenicol/streptomycin-resistant derivative of the wild-type ATCC29307 strain, and their phenotypes related to virulence were compared with those of the parental cells. $LD_{50}$ and histopathological findings of luxS-, smcR-, or luxS- smcR- deficient mutant were not different from those of the parent strain, a lacZ-deficient streptomycin-resistant strain in mice. However, time of death in mice was delayed, and numbers of bacteria survived in bloodstream after intraperitoneal injection in mice were decreased by mutation, especially luxS and smcR double mutant (VvSR$\Delta$ZSR). These phenomena were supported by increased serum sensitivity and delayed bacterial proliferation in both murine blood and iron-restricted medium. These results suggest that the luxS and luxR homologous genes in V. vulnificus could playa role in bacterial survival in host by enhancing proliferation and adjusting to changed environment.
