A Study about Self-care for Hypertensives in Rural area (일부 농촌지역 고혈압 환자의 자가간호 수행요인 연구)

  • Essential hypertension is a typical chronic disease requiring adequate and continuous management. Many studies supported that self-care was the essential factor to promote the wellbeing of hypertensives and self-efficacy increased healthy, behavior. The comprehensive self-regulation program was conducted as a nursing intervention on the promotion self-care performance for hypertensives in company. And this self-regulation program was recommended to apply for hypertensives in rural area. The purpose of this study is to identify the general characteristics affecting self-care and interrelationship among the factors including self-care, self-efficacy, HLOC, perceived benefits, barriers and family support. 40 subjects were interviewed from Dec. 1996 to Jan. 1997 and the data was analyzed by the SPSS PC+ program with t-test and multiple correlation to determine the variables affecting the self-care behavior. The results were as follows: 1) Level of self-care was significant difference according to sex(t= -2.27, p=.0l9). religion (t=1.57, p=.055) and smoking habit (t=4.42, P=.000). Perceived self-efficacy was more significantly high among the non-smoking group (t=3.25, P=.000) and female group (t=-2.534, p=.0l3). 2) There were significant positive correlation among the variables: self-care and self-efficacy (r=.5460, p=.000), external-LOC and self-care(r=.2548, p=.056), external-LOC and self-efficacy(r =. 2901, p=.035), self-efficacy and perceived benefits (r=3307, p=.019). And there were significant negative correlation between self-care and barriers (r=-.5438, p=. 000), self-efficacy and barriers (r= - .4153, p=.004). From the above results, it can be concluded that the self-care is more required in male hypertensives and self-efficacy is one of the important factors to increase healthy behavior in cluding self-care. Thus self-regulation program can be recommended in the management of the hypertensives in community settings.

The effect of a diabetic group teaching program (당뇨병 환자 집단교육의 효과 및 교육효과 및 지속에 관한 연구)

  • This study evaluated the effect of diabetic group teaching programs in one university hospital in Seoul to predict when re-education would be needed. This study examined the patients’ knowledge at four points (before, directly after, three months and six months after the teaching program) and self-care performance related to diabetes twice (three months and six months after the teaching program). The subjects of the study were 24 admitted diabetic patients who participated in four-day teaching programs. Data were gathered from January to October, 1992 by means of an instrument developed from two diabetic knowledge tests which were equivalent in item differentiation and item discrimination coefficients. Collected data were analyzed by paired t-test, Pearson correlation, t and F tests. The results of study were as follows. 1. The analysis of the effect of the diabetic group teaching program and the duration of the effect of teaching. 1) The first hypothesis, that the diabetes knowledge score directly after the teaching program would be higher than before, was rejected (t=-1.40 ; p=.172). 2) The second hypothesis, that the diabetes knowledge score directly after teaching would be higher than three months later, was rejected(t= -4.27 ; p=.000). 3) The third hypothesis, that the diabetes knowledge score three months after teaching would be higher than six months later, was supported(t=2.43 : p=.020). 2. The relation of knowledge and self-care performance 1) The forth hypothesis, that the level of self-care performance related to diabetes three months later would be higher than six months later, was rejected( t=1.49 ; p=.146). 2) The fifth hypothesis, that the higher the diabetes knowledge, the higher the level of self-care performance, was rejected(r=.2086 ; p=.118). 3. The relation of diabetes knowledge and self-care performance according to demographic variables and structural variables of diabetes. 1) Diabetic knowledge scores varied according to the educational levels of the clients directly after the teaching. Three months after the teaching program higher educational levels and higher economic status were related to higher diabetic knowledge scores and men had higher knowledge scores than women. 2) Self-care performance scores of men were higher than those of women at three months and six months after the teaching program. 3) Before the diabetes teaching, the diabetic knowledge scores of subjects who had a diabetic patient in the family were higher than those who did not have patient in their family. Six months after the teaching, the diabetic knowledge scores of subjects who read the distributed books about diabetes were higher than those who did not read them. 4) No significance differences were found be-ween self-care performance and structural variables of dialetes. The results of this study indicated that the levels of diabetes knowledge and self-care performance incense of three months after the teaching program but decrease of six months. Reeducation would be needed between three and sir months. The investigator thinks that a study of the content and teaching methodology is needed to increase the education effect. The subjects want to hear patient histories of diabetic management. Group discussion would be helped after the teaching sessions.

Job Satisfaction, Self-Esteem, and Nursing Task Performance among Registered Nurses and Nurse Assistants in Long-Term Care Hospitals (요양병원 간호사와 간호조무사의 직무만족과 자존감 및 간호업무성과에 관한 연구)

  • Purpose: This study was conducted to compare job satisfaction, self-esteem, and nursing task performance between registered nurses (RN) and nurse assistants (NA) in long-term care hospitals. Method: The sample consisted of 203 nursing personnel (RN 99 and AN 104), who were working in one of 5 long-term care hospitals in 5 different areas. Data were collected by structured questionnaires from March 15 to April 30, 2010, and analyzed using the descriptive statistics, independent t-test, One-way ANOVA, and Pearson correlation coefficients with SPSS/Win 17.0 program. Result: There was no significant difference between nurses and nurse assistants in job satisfaction and self-esteem. But nurses had higher levels of nursing task performance than nurse assistants. Conclusions: These findings demonstrate the necessity of developing programs that will help to improve job satisfaction and self-esteem in nurses and nursing task performance in nurse assistants. Strategies need to be developed to maximize the value of the existing workforce without impacting on the quality of care delivered.

Effects of Self Efficacy Promoting Programs on Self Efficacy, Self Care Behavior and Psychosocial Adaptation in Patients with a Colostomy (자기효능 증진 프로그램이 결장루 형성술 환자의 자기효능, 자가 간호 행위 및 심리사회적 적응에 미치는 효과)

  • Purpose: This study was conducted to investigate the effects of self-efficacy promoting program on self-efficacy, self-care behavior and psychosocial adaptation in patients with a colostomy. Method: A non-equivalent control group pre test post test design was used. The self efficacy promoting programs was composed of a CD image program based on varicaious experience, education and telephone coaching program based on verbal persuasion, and Stoma care practice, and Self care performance based on performance accomplishment. This study was carried out from July 2, 2005 to April 20, 2006 and 21 patients with a colostomy at one of 2 hospitals participated. Descriptive statistics, ${\chi}^2-test$, and Mann-whitney U test were used to analyze the data. Results: There were significantly different for specific self efficacy, self care and psychosocial adaptation between the experimental group and control group. Conclusion: The self efficacy promoting program for patients with stomas was effective in improving degree of specific self efficacy, self care and psychosocial adaptation.

  • This study analyzes the relationship between teachers' efficacy and difficulties on job performance depended on self-esteem. To achieve this goal, 282 child care teachers who inhabit J city responded to the questionnaire. The collected data were analyzed by Pearson correlation and multiple regression techniques using the SPSS statistics program. First, the self-esteem was closely associated with child care teachers' efficacy and difficulties on job performance. Second, the self-esteem was the strongest predictor for general teaching efficacy of child care teacher and Infant's protection and guidance also notable variables. This significant result shows the importance of self-esteem. Therefore, it is desirable to encourage child care teachers to experience and raise self-esteem.

A Study on the Relationship between Self Effcacy and Self Care Behavior in Patients with Pulmonary Tuberculosis (폐결핵 환자가 지각한 자기효능과 자가간호행위와의 관계에 관한 연구)

  • This study is a descriptive survey research to investicate the relation between the performance of self-care behavior and degree of self-efficacy by evaluating them respectively. The subjects were consisted of seventy nine tuberculosis patients registered in two different public health care centers in the city of Kwangju from April 1st to 30th June, 1995. The results were as follows : 1) Demorgraphically, the proportion of males was$(59.5\%)$, the highest proportion of age group was twenties with $(31.6\%)$, the higest proportion of the patients graduated high school. the married was much more than the single$(51.9\%)$. The highest proportions in each variables were as followes : middle class in economic status with $70.9%$, the jobless in job status with $26.6\%$. the duration less than 6 months in ailing duration with $63.3\%$. The patients of $60.8\%$ and $74.7\%$ showed no family history and received no health education respectively. 2) The average drgree of self-care behavior was 3.44 points (1-5 points). 3) The average dregee of recognized self efficacy was 78.18 points(10-100points). 4) The recognized self efficacy showed close relationship with the performance of self-care behavior (r=0.6378. p<0.001) 5) The points of performance of self-care nursing behavior showed statistical significance in the variables of martial status. duration of treatment and economic status. (F=3.142. p<0.05), (F=3.701. p<0.05). (F=6.213. p<0.01) 6) The point of self efficacy showed statistical significance in the variables of sex. degree of education and marital status. (F=5.438. p<0.05), (F=4.28. P<0.01), (F=2.899, p<0.05) These results suggest as follows: 1) It is necessary to develop a program to improve the ability of self health care for the family member as well as the patient. 2) It is necessary to do further study on much more variables that influence on the performance of self-care behavior by expanding the number of subjects and duration of study. and to evaluate the self efficacy with time interval.

Influence of Professional Self-concept, Moral Sensitivity on Elderly Care Performance of Geriatric Hospital Nurses (요양병원 간호사의 전문직 자아개념과 도덕적 민감성이 노인간호실천에 미치는 영향)

  • This study was conducted to investigate the relationship among professional self-concept, moral sensitivity and elderly care performance nurses in geriatric hospitals, as well as to clarify factors that affect nurses' elderly care performance. Participants consisted of 153 nurses working in geriatric hospital in G Province were evaluated. Date were collected from September 15 to October 15, 2018 and analyzed using the SPSS/WIN 23.0 program. The mean of professional self-concept was $2.87{\pm}0.35$ out of 4, that of moral sensitivity was $4.70{\pm}0.47$ out of 7 and that of elderly care performance $3.51{\pm}0.41$ out of 4. Elderly care performance was siginificantly positively correlated with professional self-concept (r=.48, p<.001) and moral sensitivity (r=.31, p<.001). Factors influencing elderly care performance were professional self-concept (${\beta}=.32$, p=.001), moral sensitivity (${\beta}=.18$, p=.021), educational experience of geriatric nursing (${\beta}=.17$, p=.029), which explained 31.6% of elderly care performance. The results of this study suggest that supporting systems should be arranged to provide various learning opportunities as a way to increase the professional self-concept of nurses with short career as well as intervention strategies may be necessary to make education of geriatric nursing obligatory.

The Relations among Self-care, Symptom Experiences, and Disease Status of the Patient with Liver Cirrhosis (간경변증 환자의 자가간호수행과 증상경험 및 질병상태와의 관계)

  • Purpose: The purpose of this study was to examine the degree of self-care, symptom experience and disease state among the patients with liver cirrhosis and to identify its correlation. Methods: The subjects were 220 patients with liver cirrhosis who were treated in D-university hospital in Busan. The instruments used for study were the self-care inventory, the symptom experience inventory and modified childpugh classification scale. Results: The average points of the patient's self-care performance, symptom experience, and disease status were 3.67, 29.10 and 6.12 respectively. Their self care was negatively correlated to symptom experience, and symptom experience was positively correlated to disease state. Conclusion: This research findings suggested that active self-care by patients with liver cirrhosis might contribute to reduce various symptoms they can undergo and to prevent aggravation of disease state. Therefore, patients with liver cirrhosis have to carry out self care actively in daily life and discreet nursing intervention should be offered to facilitate their self-care.

Self Care Behavior according to the Risk of Vascular Complications in Elderly Women with Hypertension (고혈압 여성노인의 혈관합병증 위험정도에 따른 자가간호 행위)

  • Purpose: The purpose of this study was to examine self care behaviors according to the risk levels of vascular complications in elderly women with hypertension. Methods: The subjects of this study were 162 women living in the community who had been diagnosed with hypertension in clinics. The data were analysed by the SPSS 10.0 program using descriptive statistics, t-test, and ANOVA with Scheff$\dot{e}$ post-hoc test. Results: The average self care behavior score of the elderly women was 2.79. Total self care behavior was significantly different depending on the risk levels of vascular complications such as systolic blood pressure, diastolic blood pressure, ankle-brachial index, and the framingham point score. Among sub-scores of self care behaviors, exercise management was the poorest performance compared with other self care behaviors. Conclusion: This study proved the differences in self care behaviors according to the risk levels of vascular complications. To decrease the prevalence of vascular complication, it is necessary to develop programs specifically to enhance self care behaviors of elderly women with hypertension.

Effects of Individual Education for Cancer Patients and Their Family about Chemotherapy on the Self­-Care Knowledge, Self­-Care Performance, Family Support, and Anxiety (항암화학요법에 관한 가족참여 개별교육이 암환자의 자가간호 지식, 자가간호 수행, 가족지지 및 불안에 미치는 효과)

  • Purpose: The study was to examine the effects of the individual education about chemotherapy for cancer patients as well as their family who are receiving the first chemotherapy on self-care knowledge, self-care performance, family support, and anxiety level. Methods: A non-equivalent control group was used in this interventional study. A total of 50 patients with cancer aged above 19 in the K university hospital, Seoul, Korea, participated in the study (Experimental group: n=25, Control group: n=25). The education about chemotherapy was offered one week after hospital discharge, which was followed by telephone counseling Data were analyzed by the SPSS/WIN 20.0 program using percentage, frequency, mean, standard deviation, $x^2$ test, and independent t-test. Results: The experimental group showed a significantly better self-care knowledge (t=4.094, p<.001), self-care performance (t=3.532, p=001), family support (t=2.170, p=.035), and anxiety control (t=4.780, p<.001) than the control group. Conclusion: The findings suggest that the individual education should be considered for cancer patients and their family in the clinical fields.
