  • Objectives: The purpose of this study was to examine teachers' safety education activities to determine the significant educational diagnosis variables and to identify their needs of safety education in early-child care centers based on the PRECEDE model. Methods: A total of 304 teachers in early-child care centers participated in this study selected by a multi-stage stratified sampling method considering 11 regions in Seoul, Korea. Self-report type questionnaires were posted to all teachers in 220 early-child care centers by ground mailing service and the 304 teachers completed the questionnaires. The participants' responses were anonymously coded into and analyzed in SPSS program. Results: 'Scratch or bite' was the most frequent accident type(78.3%) and the frequent accident places were 'classroom(88.8%)' and 'playground(67.8%)'. The most frequently conducted safety education activities were 'reminding children their safe behaviors at the beginning and the end of daily class' and the next was 'saving a special time for safety education.' For educational diagnosis factors, related to safety education activities, teachers' safety education activity was more frequent when teachers' safety knowledge was high(p<.001), when teachers had good application skills of their knowledge to their teaching activities(p<.001), when they had strong needs on safety training opportunities(p<.05), and their interests on safety education(p<.001). For enabling factors, class preparation by safety education guide-book review(p<.001), by development of educational materials(p<.001), and by search for the related reference (p<.001), and by participation to safety education training programs for teachers(p<.01) were the significant enabling factors on teachers' safety class activities. For the reinforcing factors, the center-wide support of safety education brochures to children (p<.001), the concerns of centers utilizing safety education specialists(p<.001), and the concerns about safety information collection out of centers(p<.001) were significant factors related with teachers' safety education activities. Conclusions: The significant educational and institutional factors on teachers' safety education activities were teachers' concerns on safety education, their interests on safety knowledge, and the strong concerns on child safety education from the centers.

  • To explore the relationship between economic status and food and nutrient intake patterns, the 2001 National Health and Nutrition Survey result was analyzed. Dietary intake data of 6,978 Korean adults of 20 years and older who participated in the 2001 National Health and Nutrition Survey were used along with their demographic data. Economic status of the subjects was classified into the following 4 groups based on the self-reported average monthly income of household with reference to the minimum monthly living expenses (MLE) in 2001 : low < $100\%$ MLE $\leq$ middle < $200\%$ MLE $\leq$ high < $300\%$ MLE $\leq$ higher, Individuals in the higher income class had significantly higher mean intake for most of the nutrients including energy, protein, carbohydrate, fat, calcium, thiamin, riboflavin, niacin and vitamin C, and a higher percentage of energy intake from fat. In addition, they consumed more animal foods including meats, eggs, fish/shellfish, milk/dairy products and fats. On the other hand, the mean intakes of individuals in the lower economic class for calcium, vitamin A, and riboflavin were lower than $75\%$ of RDAs. And, there was a predominant difference in contribution of fat to total energy intake among the groups of different economic status. These results showed that household income is an important factor influencing the food and nutrient intake patterns of the Korean adult population. Although individuals at different age classes may respond differently to a change in economic status, developing and implementing nutrition policy and intervention programs for those nutritionally vulnerable groups should consider the economic status as an important factor to customize and differentiate the content of the Program. • In ordo to investigate the professional job perception and job satisfaction of interns and residents and its related factors, the author conducted survey using self administered questionnaire for 2 months(August and September, 1996). The study subjects were 562 interns and residents who worked at one university hospital(tertiary hospital) and two general hospitals(secondary hospital) in Taegu, Korea. Among them, 297 (52.8%) responded completely. The results were as follows ; Respondents thought that people component was most important, science and status components were next among professional value scales which meant the desirable attitudes required to conduct professional works. But, the score of professional value scales was generally high and not affected by other variables in multiple linear regression analysis. They seemed to have very normative and ideal perceptions about professional values. The score of professionalism scales, which were attitudinal and behavioral traits about professional job, of high grade residents was significantly higher than that of interns and lower grade residents. The score of reference to professional organization and autonomy factor were increasing significantly as the grade increased. Working conditions and perception for socioeconomic status of doctors influenced the score of professionalism scale significantly. It seemed that professional socialization was made during the training periods of interns and residents. Most of respondents answered that current socioeconomic status of doctors were middle and high strata but they responded that the socioeconomic status of doctors would fall in the future. They seemed to have a pessimistic thought about doctor's status. Generally the respondents thought that they were satisfied with doctor job and fit to the job, but 51.9% answered that if possible they would get other jobs. It seemed to reflect their critical thinking on doctors' status. Perceptions about socioeconomic status of doctors, professional value and professionalism influenced job satisfaction significantly. The interns and residents had high sense of calling to doctors and thought that doctors were socially important job. Generally they were satisfied with their job. Interns and residents had normative thoughts about the trait which compentent doctors must have. During the training period, they seemed to have attitude and perception as a professional and to make professional socializations.

  • Background: Guideline for practice is useful because it can be the standard for assessment and way to improve quality. We need to take account of expert opinion and consensus as well as scientific evidences to develope practice guideline because it should be practical. Delphi method has been developed to gather opinions from experts fairly. This study was designed to develop practice guideline and apply it to practices as a method to improve quality in primary medical care. Methods: Hypertension(in adults over 18 years old) was selected as a target problem. Self questionnaires about management of hypertensive patients were developed by a researcher with advice of 11 experts. The questionnaires were designed to response as a 5 pont scale. The results of previous questionary were given to respondents in second and third questionnaires. If needed, the questionnaires were changed on the previous responses. The items with medium greater than or equal to 4 point in 1st and 2nd responses were accepted in guideline. The items with medium lesser than or equal to 3 point were questioned again about whether they can be excluded or not. The criteria for assessment was made with reference to guideline and applied to 85 hypertensive patients of 9 family practitioners. At 3 months after practice guideline had been given to family practitioners, performance of same practitioners was assessed with 36 new hypertensive patients. Results: 23 professors in family medicine, 22 family practitioners and 6 cardiologists, responded among 50, 50, 15 respectively. Practice guideline with 33 items was developed as a result of 3 times questionaires. The difference of responses between professors in family medicine, family practitioners and cardiologists was not significant. Performance of practice was improved in diagnosis, history taking about 6 fields, laboratory examination and decision making about time of pharmaceutical prescription. It was not improved in physical examination, life style modification, method of pharmaceutical prescription, choice of initial antihypertensives and history taking about duration of disease and diet habit. It was decreased in history taking about psychosocial factors. The assessment scores were low in history taking, physical examination and life style modification before and after use of practice guideline. Conclusion: Practice guideline for hypertension could be developed by Delphi method. Performance of practice improved partially after use of guideline.

  • Background: Prospective cohort studies to determine cofactors with oncogenic HPV-infections for cervical cancer are very rare from developing countries and such data are limited to the few screening trials. Large screening trials provide such data as a by product. Some of the cases are prevented by screening and do not surface as invasive cancers at all. Also, pre-invasive lesions are detected almost entirely by screening. Screening causes selection bias if attendance in or effectiveness of screening is correlated with the risk factors. The aim of this study was to quantify the influence of screening on risk factors for cervical cancer. Materials and Methods: Our material stems from a rural cohort of 80,000 women subjected to a randomised screening trial. The effect of screening on the incidence of cervix cancer was estimated with reference to socio-demographic and reproductive risk factors of cervical cancer. We compared these risks with the incidence of cancer in the randomised control population by the same determinants of risk. Results: The results in the screening arm compared to the control arm showed that the women of low SES and young age were benefitting more than those of high SES and old age. The relative risk by age (30-39 vs 50-59) was 0.33 in the control arm and 0.24 in the screening arm. The relative risk by education (not educated vs educated) was 2.8 in the control arm and 1.8 in the screening arm. The previously married women did not benefit (incidence 113 and 115 per 100,000 women years in control vs screening arms) whereas the effect was substantial in those married (86 vs 54). Conclusions: The results in controls were consistent with the general evidence, but results in attenders and nonattenders of the screening arm showed that screening itself and self-selection in attendance and effectiveness can influence the effect estimates of risk factors. The effect of cervical cancer screening programmes on the estimates of incidence of cervical cancer causes bias in the studies on etiology and, therefore, they should be interpreted with caution.

  • Objectives: Sleep quality is directly related to the health, quality of life and working capacity and is affected by age, gender, exercise, life habits. Gender dissimilarities in sleep quality are acknowledged. However, the gender difference in the quality of sleep in shift workers was not well known. Our aim was to study the less known gender differences in the quality of sleep of shift workers. Methods: 1,008 study subjects aged between 21 and 57 years among these, 637(525 male and 112 female workers) were daytime workers and 371(253 males and 118 female workers) were shift workers. Sleep quality was measured by self-report through the Pittsburg Sleep Quality Index(PSQI) and fatigue was measured by using the Chalder Fatigue Scale(CFS). All data was analyzed using SPSS version 19.0 program. The study was approved by the Institutional Review Board of the Occupational Safety and Health Research Institute. Results: Overall, sleep quality was found to be worse in shift workers than in daytime workers and in female than in male. Fatigue in shift female workers was significantly higher than in daytime female workers and shift male workers. In Spearman correlation analysis results, fatigue were significantly associated with gender, drink habit, regular exercise, working hours and sleep quality. With group male workers as the reference, the odds ratio(OR) for having a CFS was 2.115(${\beta}$ value=0.749, p<0.01), and subjective sleep quality, sleep latency, daytime dysfunction and PSQI were 1.541(${\beta}$ value=0.432, p<0.05), 2.297(${\beta}$ value=0.831, p<0.001), 1.798(${\beta}$ value=0.587, p<0.01) and 2.224(${\beta}$ value=0.799, p<0.01), respectively. Conclusions: Shift work played an important role in lowering the quality of sleep, and the effect was more pronounced in female workers than in male workers. Sleep quality was related to fatigue, especially sleep quality, sleep duration, sleep disturbances and daytime dysfunction were significantly related. Therefore, to improve fatigue and sleep quality of shift worker, it is necessary to improve healthy habits such as regular exercise, smoking cessation, and limited drinking. In particular, it is urgent to develop and implement a regular exercise program to reduce fatigue and improve the quality of sleep in workplace.

  • This research aims to integrate study results through a meta-analysis of previous studies on the variables related with social support for female marriage immigrants. Based on the results, this study established the foundation of an integrated social support system for female marriage immigrants which considers both the functional perspective and structural perspective of social support, and suggested plans for an efficient support system. All social support was positively associated with female marriage immigrants marriage life, child-rearing behavior and attitude, reduction of stress and depression and psychological stability. In relation to the effect size of each variable related with all social support, the marriage variable had the biggest relationship with it, followed by the parenting variable, the psychological variable and the stress variable in that order. With reference to the relationship with related variables according to the sub-domain of social support, from the structural perspective, spousale support showed a high relationship with the marriage variable and the stress variable. It was also especially, very highly related with the marriage variable. In addition, a married woman's family support and expert support had an intermediate relationship with the marriage variable, and the husband's family support and friend support had a low relationship with it. From the functional perspective, material support had a very high relationship with the marriage variable, whereas it had a low relationship with the stress variable and the psychological variable. Emotional support was also highly related with marriage variable, but showed an intermediate or low relationship with the psychological variable and the stress variable. On the other hand, informational support displayed an intermediate relationship with the stress variable and the psychological variable, and a high relationship with the marriage variable. Lastly, evaluative support had a high relationship with the marriage variable and the psychological variable. Based on these results, this study proposed plans for an integrated social support system as follows. First, education ought to be provided for the spouses, and support for marital adjustment should be offered. Second, there plans should be made for continuous bonding with the family of origin, and support should be given to address changes in awareness of the relationship with the husband's parents. Third, it is required to revitalize the local community and self-help groups, and provide the female marriage immigrants with opportunities to participate in social activities.
