The Relationship between Both Upper Extremity Function and Activities of Daily Living in Stroke Patients (뇌졸중 환자의 양측 상지기능과 일상생활 수행능력의 연관성)

  • The purpose this study was to investigate the relationship between Upper extremity's function and Activities of Daily Living(ADL) in stroke patients. The participants were 112 stroke patients who underwent occupational therapy. Data were analyzed using descriptive statistics, Pearson's correlation coefficient, and multiple linear regression analysis. The results are as foolows. MFT of both unaffected upper limbs and affected upper limbs were significantly correlated with total MBI score. The all area of MFT on the affected upper limbs were significantly correlated with sub-item of MBI. And finger manipulation area of MFT on the unaffected upper limbs were significantly correlated with sub-item of MBI. Significant factors influencing MBI, both unaffected upper limbs and affected upper limbs total score. Significant factors influencing sub-items of MBI, the function of affected upper limbs by MFT were MBI all sub items exculsive bowel, bladder control and the function of unaffected upper limbs by MFT were personal hygiene, bathing, feeding, toilet, bowel & bladder control, chair/bed transfer of MBI sub items. Above results show that ADL is highly correlated with affected upper limbs and unaffected upper limbs function in the stroke patients.

Sexual Function of Breast Cancer Survivors and Healthy Women : A Comparative Study (유방암 생존자와 정상여성의 성기능)

  • Purpose: The purpose of this study was to analyse the level of sexual function among breast cancer survivors as compared with that among healthy women. Method: A total of 208 women participated in this study: 103 breast cancer survivors attending self-help group and 105 community-residing healthy women in G city. A self-reported questionnaire with the Female Sexual Function Index (FSFI) was used to obtain data. Data were analyzed using the SPSS Win 14.0 $K^+$ for descriptive statistics, t-test, and two-way ANOVA. Results: The sexual function score of breast cancer survivors was significantly lower than that of healthy women, respectively $14.9{\pm}9.9$ and $20.6{\pm}11.1$ (p<0.001). The sexual function of recurrent breast cancer survivors was significantly lower than that of women without recurrence. However, there were no significant differences in the sexual function score according to the cancer stage, period since diagnosis, and types of cancer treatment modality. Conclusion: The findings indicate the importance of sex-related counseling and educational programs to improve the quality of life of breast cancer survivors.

Relationship Between Trunk Control and Respiratory Function in Stroke Patients (뇌졸중 환자의 체간조절과 호흡기능의 관계)

  • Purpose: This study aimed to investigate the relationship between trunk control and pulmonary function and respiratory muscle strength in stroke patients. Methods: This study included 30 patients who had been clinically diagnosed with strokes, and trunk control abilities were measured using the trunk impairment scale (TIS). The subjects were classified into a group with high trunk control ability (TIS score ${\geq}20$) and a group with low trunk control ability (TIS score < 20). The patients' forced vital capacity (FVC), forced expiratory volume in one second (FEV1), peak expiratory flow (PEF), maximal inspiratory pressure (MIP), and maximal expiratory pressure (MEP) were measured. To compare the pulmonary function and respiratory muscle strength between the two groups, the measurement data were analyzed using an independent T-test, and the relationship between TIS and respiratory function was analyzed using a Pearson correlation. Results: The high trunk control ability group had significantly higher pulmonary function and respiratory muscle strength than the low trunk control ability group. Significant positive correlations were found between trunk control and FVC, FEV1, PEF, MIP, and MEP. Conclusions: This study demonstrated that trunk control affects pulmonary function and respiratory muscle strength in stroke patients.

Using computer software as an instructional tool for enhancing relational understanding of function concept : three case studies (관계적 이해를 위한 수업 도구로서의 소프트웨어 활용에 관한 사례 연구 - 고등학교 1학년 함수 단원을 중심으로 -)

  • Although ‘to understand mathematics’ is an important educational purpose, most student do not have a relational understanding of the basic concept of mathematics but have a instrumental understanding. This paper will investigate the possibility of using computers for enhancing relational understanding. In the ‘Qualitative case study’, three students who are in the first grade at E-High school took part in 7 activities during four weeks, and were later interviewed and engaged in informal discussion and were observed. This is the result of this study. 1. The three students were passive participants in mathematics problem solving situation at school. Therefore, student B just applied formulas which she had memorized, and student C would forgot the formulas occasionally. These common students needed to participate actively in doing mathematics. 2. The activities utilized two software healing with connection between graphs and function, giving the students the opportunity to plan, practice, and test by themselves. As a result, they understood the mathematical formulas and rules more deeply through their own trial and error, and then they gained thinking abilities necessary for doing mathematics. In addition, the activities boosted their confidence. 3. The understanding type of students was slightly different. Student A who received a high score, understood the most relationally, but student B who received a very high score, understood instrumentally and so couldn't app1y her knowledge to solving problems related to function concept. Student C who received a middle score lacked knowledge of mathematics but thought more creatively. The result is that students need an opportunity to think rotationally regardless of score. Therefore, this study concludes that using computer software will provide a positive effect for relational understanding in loaming function concept.

Normative Data of the Upper Extremity Performance Test for the Elderly (TEMPA) for Korean Older Adults and Characteristics of Hand Function and Strength (한국 노인의 TEMPA 표준치, 손기능과 근력에 관한 연구)

  • Objective : This study aimed to identify the characteristics of hand function according to age and sex in older adults. Methods : This study included 103 healthy adults aged ${\geq}60$ years. The Korean version of TEMPA was used to assess hand function and a Jamar dynamometer and pinch gauge were used to assess hand and pinch strength in all participants. Results : The results of hand function assessment showed that speed of task execution (mean = 121.32 s, SD = 18.07 s in subjects aged 60-69; mean = 144.97 s, SD = 28.43 s in subjects aged 70-79; and mean = 160.93 s, SD = 38.33 s in subjects aged ${\geq}80$, p < .001) and fine movement (mean score = -.14, SD = .40 in subjects aged 60-69; mean score = -.63, SD = 1.07 in subjects aged 70-79; and mean score = -.57, SD = .65 in subjects aged ${\geq}80$, p = .01) decreased significantly with age. The male group showed better speed of task execution (mean = 133.54, SD = 22.83 in males vs. mean = 150.55, SD = 39.89 in females, p < .01) and fine movement (mean score = -.16, SD = .37 in males vs. mean score = -.46, SD = .58 in females, p < .01) than the female group. Hand strength also decreased significantly with age (p < .05, to p < .001). Conclusion : Occupational therapists should be aware of the decline in hand function (especially speed of task execution and fine movement) and strength in older adults, as well as the need to provide interventions to treat this decline.

A case study of competing risk analysis in the presence of missing data

  • Observational data with missing or incomplete data are common in biomedical research. Multiple imputation is an effective approach to handle missing data with the ability to decrease bias while increasing statistical power and efficiency. In recent years propensity score (PS) matching has been increasingly used in observational studies to estimate treatment effect as it can reduce confounding due to measured baseline covariates. In this paper, we describe in detail approaches to competing risk analysis in the setting of incomplete observational data when using PS matching. First, we used multiple imputation to impute several missing variables simultaneously, then conducted propensity-score matching to match statin-exposed patients with those unexposed. Afterwards, we assessed the effect of statin exposure on the risk of heart failure-related hospitalizations or emergency visits by estimating both relative and absolute effects. Collectively, we provided a general methodological framework to assess treatment effect in incomplete observational data. In addition, we presented a practical approach to produce overall cumulative incidence function (CIF) based on estimates from multiple imputed and PS-matched samples.

A Study on the Discriminant Variables of Face Skin Colors for the Korean Females (한국 여성의 얼굴 피부색 판별을 위한 색채 변수에 관한 연구)

  • The color of apparel products have a close relationship with the face skin colors of consumers. In order to extract the favorable colors which flatter to consumer's face skin colors, this study was carried our to classify the face skin colors of Korean females. The criteria that select new subjects who have the classified face skin colors have to be decided. With color spectrometer, JX-777, face skin colors of subjects were measured and classified into three clusters that had similar hue, value and chroma with Munsell Color System. Sample size was 324 Korean females and other new 10 college girls. Data were analyzed by K-means cluster analysis, ANOVA, Duncan multiple range test, Stepwise discriminant analysis using SPSS Win. 12. Findings were as follows: 1. 324 subjects who have YR colors were clustered into 3 face skin color groups. 2. Discriminant variables of face skin colors were 5 variables : b value of cheek, V value of forehead, L value of cheek, C value of forehead and H value of cheek by the standardized canonical discriminant function coefficient 1. 3. Hit ratio of type 1 was $96.8\%$, of type 2 was $94.9\%$, of type 3 was $100.0\%$ and mean of hit ratio was $96.9\%$ by canonical discriminant function of 5 variables. 4. With the unstandardized canonical discriminant function coefficient and constant, canonical discriminant function equation 1 and 2 were calculated. And cutting score and range of score of the classified types were computed. The criteria that select the new subjects were decided.

A study of Sexual Function, Sleep Quality and Depression in Male Patients with Liver Cirrhosis (남성 간경변 환자의 성기능, 수면의 질, 우울에 관한 연구)

  • This study is a descriptive study to investigate the relationship between sexual function, sleep quality, and depression in male cirrhosis patients. Data were collected from a structured questionnaire from August 16, 2017 to January 30, 2018. Data were collected by t-test, ANOVA, Scheffe test, Pearson correlation. AS a result, the lower the sexual function of male cirrhosis patients, the lower the quality of sleep (r=-.232, p =.005) and the higher the depression score (r=-.381, p<.001) The lower the score, the higher the score of depression (r=.561, p<.001). Sexual function, quality of sleep and depression were found to be statistically significant. The results of this study can be used as basic data for the development of nursing intervention for male cirrhosis patients.

Ligament Injuries of Knee in the Recreational Skiers (스키에 의한 슬관절 인대 손상)

  • Purpose: This study is to analyse the injury patterns of knee ligament and the factors influencing ligament injuries of knee, and to evaluate the changes of knee function and activity after ski injury. Materials and Methods: Thirty cases of ligament injuries of knee were studied with a questionaire, stress radiographs, magnetic resolution imaging, and physical examination. Mean age was 28.6 years old and mean follow up was 29.6 months. Clinical evaluation of Lysholm knee scoring scale was used for knee function and Tegner activity score was used for activity change after ski injury. Results: The anterior cruciate ligament (ACL) injury accompanied with medial collateral ligament(MCL) injury was most in 11 cases and isolated MCL injury was the next in 9 cases. The common types of injury mechanism were Phantom foot phenomenon (13 cases, 43$\%$) and valgus external rotation injury (12 cases, 40$\%$), which constitute 83$\%$ of all case. At the last follow up, the mean Lysholm score was 93.4 and mean Tegner activity score was 4.2. The reduced Tegner activity score after injury was 1.9. Among several injury groups, the evaluation of knee function and activity was best in the isolated MCL injury group and worst in the ACL injury accompanied with MCL injury group. The factors to influence ski injury were participation to ski class, release of binding, and skiing long time more than 2 hours. Conclusions: Lysholm score at last follow up revealed good grading, but sports activity after ski injury was reduced when compared with pre-injury state. It seems to need a active, systemic sports rehabilitation program after sports injury.

The Effects of Swallowing with Oropharyngeal Sensory Stimulation in Nasogastric Tube Insertion in Stroke Patients (구강인두 감각자극이 비위관 삽입 뇌졸중환자의 연하기능에 미치는 효과)

  • Purpose: This study was to identify the effects of oropharyngeal sensory stimulation on a functional oral intake scale, ore-pharyngeal swallowing functions, and aspiration pneumonia symptoms with nasogastric tube insertion in stroke patients. Method: This study was a nonequivalent control group non-synchronized design. The subjects or the study were 32 patients who were hospitalized in Kosin Gaspel hospital. The experimental subjects were stimulated for $10{\sim}20$ minutes, 1 time per day. The intervention was implemented for 2 weeks. Result: Participants in the experimental group significantly received a higher score in ore-pharyngeal swallowing function than those in the control group. However the participants in the experimental group only got a high score in the function oralintake scale which doesn't imply a statistical significance. In addition, they didn't geta remarkably higher score in aspiration pneumonia symptoms than those in the control group. Conclusion: This study demonstrates that oropharyngeal sensory stimulation is effective in promoting recovery oro-pharyngeal swallowing function of nasogastric tube insertions in stroke patients.
