취약계층 아동의 구강건강증진 실천프로그램 적용 및 평가 (Oral Health Promotion Program Application and Effect for Underprivileged Lower Grade Students)

  • Objectives: The purpose of this study was to apply an effective oral health promotion program and effect for underprivileged lower grade students. This study attempted to resolve some oral health inequality issues by providing underprivileged children with an oral health action program designed to promote positive oral health maintenance habits. Methods: This study was carried from May to August of 2009 with 77(control group 27 subjects, experimental group 36 subjects) elementary school students selected among first to third graders attending regional welfare center's after school program in Seoul. To the control group, conventional education methods were applied. To the experimental group, newly designed program was conducted by three project experts(1 expert, 1 dentist, 1 dental hygienist). It consisted of 5 sessions and progressed with various activities. It was focused on achieving target knowledge and changing oral health behavior. In order to evaluate the designed educational program, learners' achievement was evaluated with implementing the questionnaire and one-on-one interviews. And also the oral health improvement was evaluated based on change of Patient Hygiene Performance(PHP) index. Results: Based on the results of learners' achievement, the experimental group showed the significant increment of the oral health knowledge by the relapsed time, and it maintained after finished the program(p<0.001). The patient hygiene performance index showed not significant changed(p>0.05), but 1 month later the control group showed an increment of the PHP score, but the experimental group showed a decrement of it. Conclusions: Oral health promotion program by project approach could give a long-lasting educational effect to the children and encouraged proper oral hygiene behavioral changes.

청소년의 건강행태와 구강질환 경험의 관련성 (Correlation between health behaviors and experiences of oral diseases in adolescents)

  • Objectives: The purpose of the study was to investigate the correlation between health behaviors and experiences of oral diseases in Korean adolescents. Methods: This study used the results of the seventh, eighth, and ninth adolescent health behavior online survey which was completed by a self-administered questionnaire targeting 222,264 middle school and high school students in September, 2011, June, 2012, and from June to July, 2013. The questionnaire consisted of four questions of socio-economic characteristics of the subjects, three questions of health behaviors, and four questions of eating habits. Data were analyzed by SPSS 18.0 program. Results: Oral health knowledge and behaviors were closely correlated to smoking, alcohol drinking, and toothbrushing after meal. Liking for fruit, soda, snack, and vegetable had a significant influence on oral diseases. Smoking and alcohol drinking had a significant relation to oral disease prevalence rate. Conclusions: In Korea, the prevalence rate of oral diseases is still high in the adolescents. Therefore, continuous dental health education program is very important to decrease the oral disease morbidity. The incentive program for the adolescents will improve the concern for the oral health.

중·고생의 치아우식증과 구강건강행태와의 관련성 연구: 제9차(2013년) 청소년 건강행태 온라인 조사 (Relationship between dental caries and oral health behavior in middle and high school students: The Ninth(2013) Korean Youth Risk Behavior Web-based Survey)

  • Objectives: The purpose of the study is to investigate the relationship between dental caries and oral health behaviors in middle and high school students by web-based survey of the ninth(2013) Korean youth risk behavior. Methods: The subjects were 75,149 students from 400 middle schools and 400 high schools. Finally, the survey participation rate was 96.4%(72,435 students from 799 schools) by complex sample design. The questionnaire consisted of general characteristics of the subjects, oral health behaviors, and oral health status. General characteristics included gender, types of schools, economic condition, and residential types. Oral health behaviors included subjective oral health condition, frequency of tooth brushing, tooth brushing after meal within recent 7 days, sealant within 12 months, fluoride application within 12 months, scaling within 12 months, and oral health education experience within 12 months. Oral health condition included dental caries incidence within 12 months. Results: Oral health behavior influenced on dental caries in the middle and high school students in Korea. This study showed the relationship between the sociodemographic characteristics, tooth brushing frequency, sealant experience, dental caries and halitosis experience, and scaling within 12 months. Scaling is the best prevention method for dental caries and halitosis in the students. Conclusions: The study showed the necessity for practical oral health education and the effect of fluoride application for dental caries prevention.

한국노인의 전신건강상태와 주관적 구강건강 관련성 (Association between Systemic Health Conditions and Self-Reported Oral Health in Korean Elderly)

  • Systemic health conditions increase with advancing age, and may be linked to poor self-reported oral health. The purpose of this study was to evaluate the association between systemic health conditions and poor self-reported oral health among Korean elderly. The study used a nationally representative sample of Koreans (2012 Korea National Health and Nutrition Examination Survey) aged 65~98 years (n=1,595). Systemic health conditions in this population were assessed by the presence of one or more of the following conditions: obesity, hypertension, diabetes, and hypercholesterolemia. The relative risk of poor self-reported oral health according to the occurrence of systemic health conditions was estimated by multivariate logistic regression after controlling for several potential confounders (i.e., socio-demographic factors, oral health behaviors, health behaviors, and psychological factors). After adjustment for these confounders, the relative risk of having poor self-reported oral health was greater among the elderly with one or more systemic health conditions than in those without a systemic health condition. The odds ratio of having poor self-reported oral health according to the occurrence of systemic health conditions was 1.51 (95% confidence interval, 1.08~2.12). Among the Korean elderly, perception of poor oral health was associated with the presence of one or more systemic health conditions. Future studies are needed to examine the detailed causal relation between systemic health conditions and poor oral health longitudinally.

대안학교 고등학생의 구강건강증진행위에 관한 연구 (An Oral Health Promotion Behavior Model for Alternative High School Students)

  • 이 연구는 횡단면 조사연구로 Pender (1996)의 건강증진모형에 관한 선행 문헌고찰을 통해 청소년의 구강건강증진행위를 설명, 예측하는 가설적 모형을 구성하고, 모형의 적합성과 제시된 가설을 PLS 구조방정식 모형 분석을 통하여 검증하여 다음과 같은 결론을 얻을 수 있었다. 대안학교 고등학생의 구강건강증진행위에 미치는 영향에서 지각된 유익성, 통제의 소재, 자기 효능감, 자아 존중감이 높을수록 구강건강증진행위가 높아지는 것으로 나타났다. 간접효과를 검정한 결과에서는 주관적 구강건강상태가 좋을수록, 과거 구강건강관련행위를 잘할수록 구강건강증진행위가 높아지는 것으로 나타났다. Pender의 건강증진모형을 이용하여 구성한 대안학교 청소년의 구강건강증진 예측모형은 구강건강증진행위를 설명하고 예측하는 데 유용한 것으로 판단되었다. 따라서 향후에는 구강건강증진 예측모형에 입각한 구강건강증진방안이 개발되어야 할 것이다.

학교교육과정 보건영역에서의 구강보건교육과 구강보건인지도 (Oral health education in the health segment of the 7th National curriculum and Oral health awareness)

  • The purpose of this study was to examine the health segment of the 7th national physical education curriculum in an effort to grasp the state of oral health education in the school curriculum. And it's also meant to investigate the relationship between oral health education and the oral health awareness of some high schoolers to check into any possible problems with oral health education as part of health education. The subjects in this study were 240 students from two different girls' high schools in Seoul, on whom a survey was conducted to find out their oral health knowledge, behavior and awareness. And the health category of the current P.E. curriculum announced by the Ministry of Education was analyzed. The findings of the study were as follows: Oral health education provided by the high schools as part of health education in P.E. class wasn't linked to P.E. theories and practice. The weight of oral health education was extremely small in the health segment, and there was no sequence, continuity and integration among the content systems of oral health education for different grades. When some high schoolers were investigated, they weren't properly oral health conscious, though it was very important for students to receive oral health education to prevent and manage dental diseases. And they didn't consequently take care of their own oral health. Future research efforts should be channeled from diverse angles into developing oral health education curriculums to bolster school oral health education to improve national oral health.

일부 고등학생들의 구강건강인식도와 구강관리습관 (A oral health care and oral care habits in high school students)

  • 본 연구는 일부 강원지역 고등학생을 439명을 대상으로 구강건강인식도와 구강관리습관을 알아보기 위하여 실시하였다. 학생들이 하루 평균 양치질을 하는 횟수에 대해 살펴본 결과 하루 평균 양치질을 세 번 이상 하는 학생이 54.4%로 가장 많았으며, 학생들이 식사나 간식 후 30분 ~ 1시간 이내에 양치질을 하는 학생이 47.6%로 가장 많았다. 학생들이 이를 닦는 주된 이유는 구강 청결을 위함이 45.1%로 가장 많았으며, 충치 예방을 위해 칫솔질을 올바르게 하려 노력하는 학생이 54.2%로 가장 많았다. 구강보건에 관한 지식은 학년별로는 1학년 학생이 구강보건지식이 가장 높았고, 한달 용돈별로는 한달 용돈이 적은 학생일수록 높게 조사되었다. 올바른 구강관리와 구강보건 교육이 강화되어야 할 것이고 올바른 구강건강인식을 가지고 구강관리습관이 정착될 수 있도록 고등학생들의 구강보건교육이 더욱 더 필요할 것이다.

초등학교 보건교사의 구강보건교육 진단 (Diagnostic Analysis on Oral Health Education of Primary School's Health Teacher)

  • 본 연구는 초등학교 구강건강을 담당하고 있는 보건교사를 대상으로 PRECEDE model을 이용하여 구강보건교육 실태와 요구하는 내용을 파악함으로써 초등학교 구강건강증진을 향상하고자 한다. 광주광역시 초등학교 보건교사 전수 114명을 대상으로 설문조사하였다. 1. 행동적 진단에서는 구강보건교육 비중은 10% 이하(58.8%)로 주로 재량활동시간(86.0%)에 이루어지며, 보건교사가 필요할 때 실시(53.5%)하였다. 지난 1년간 시행한 구강보건교육은 78.1%였으며, 학년별로 3학년이 77.2%로 가장 높았으며, 5학년 55.3%, 1학년 50.9%였다. 2. 교육적 진단의 준비 인자에서는 보건교육의 8분야 중 구강보건교육은 4,5순위였다. 다음해에도 계속 수행하고자 하는 사업에서 구강보건교육은 21.9%였으며, 세부주제로 잇솔질, 치아우식증, 치아외상과 응급처치가 80.0%정도 필요하다로 나타났다. 3. 교육적 진단의 실현 인자에서는 매년 구강보건교육 연수 경험율 13.2%, 만족도는 33.3% 였다. 구강보건교육 정보 수집 경로는 교사용 지도서(72.8%), 인터넷(52.6%)였으며, 구강보건교육 자료 수집 경로는 보건교육학회 및 관련단체(56.0%), 스스로 제작(41.0%)순이었다. 4. 교육적 진단의 강화성 인자에서는 정규 교육과정 중 구강보건관련 내용이 부족(46.7%), 교육에 필요한 자료가 부족(30.6%), 교육을 실시 기회부족(21.4%)이 장애 요소였다. 구강보건교육을 활성화시키는데 관련된 요인 중 전문가 초청(43.0%), 선배나 동료 협조 (35.1%)였다. 이상과 같은 결론에서, 구강보건교육 교재, 자료 및 방법이 개발되어야 하며, 초등학교 보건교사의 구강보건교육을 체계화 시키기 위한 제도적 장치가 필요할 것으로 사료된다.

중고등학생의 구강병으로 인한 학업손실실태와 연관요인 (School loss due to oral disease and the related factors for a middle schools and high schools in Busan, Gyeongnam province)

  • Objectives : This study was to prepare basic data about middle and high school students' school loss due to oral diseasea and to investigate the relevant factors. Methods : The survey is conducted for 575 middle school students in Busan and Gyeongnam. School loss was investigated based on the experiences of absences and leaving school early, which had happened from oral diseasea. The independent variables were oral health behaviors and sociopeconomic factors such as sex, age of their parents, educational level of their parents, family income. Factors related with school loss was analyzed by the multiple logistic regression method. Results : The experience ratio of leaving school early to the dental clinic or having difficulty in studying was higher in the case of high school student than in middle school student case. The parameters related with absence or leaving school early for oral disease were the education level, the distinction of sex, fear about medical examination and the standard of living. The reasons of absence or leaving early for visiting the dental clinic were related with education level, the distinction of sex, fear about medical examination, distrust of oral care and the satisfaction of oral health. The parameters having effect on school performance were education level, sex, fear about medical examination, distrust of oral care, cost burden, interests in oral health by the parents and school record. Conclusions : The oral health promotion should be developed to decrease school loss for students.
