Population-related factors affecting the regional distribution of medical institutions in Korea (지역별 요양기관의 분포에 영향을 미치는 인구관련 요인)

  • Few public health researchers have paid research attention to the location of medical institutions in Korea. Previous studies were published in geography journals, and relied on limited data in terms of geographic regions and the type of medical institutions. This study utilized nationwide data covering 8 types of medical institutions. We obtained data from Health Insurance Review and Assessment Service and National Population and Housing Census. The correlation coefficients of resident, daytime, university-graduate population, and the population of different age groups (fewer than 15, 15~64, 65 or more) were compared to understand their relative association with the location of medical institutions. Medical clinic, dental clinic, oriental medical clinic, and pharmacy, all of which are almost completely operated by private sector, showed strong positive correlation with population. Hospital-level medical institutions, which are operated by both public and private sector, had moderate positive correlation. Daytime population and university-graduate population, rather than resident population, were more correlated with the location of medical clinics. The correlation coefficients of the population of 15~64 age group and the location of medical institutions were greater than that of other age groups. The results showed that daytime and university-graduate population are more important than resident population to explain the location of medicalrelated facilities. The results also suggests that the population of age groups (especially, 15~64) might be one of important influence factors in the location of medical institutions.

A Study on Industrial Safety Accidents Treated at A Primary Care Clinic (의원방문 근로자들의 업무상 사고.부상 실태에 관한 연구)

  • This study was surveyed to assess the status of safety accidents occurred in work-places and prepare the fundamental data and prevent the safety accidents. The authors reviewed and analysed the charts of accident cases treated at a primary care clinic in A city from January 1991 to December 2006. The data were classified according to the USA Standards Institute and International Labour Organization method. We analyzed the data using SPSS program. The results were as follows : 1. The total cases of accidents were 455 for 8 years. 2. Accidents were mostly common in the workers who are in thirties and forties age(84.4%). 3. As season variation, spring and summer were common than others, but there was no statistical significance on season, month and weekday. 4. The most frequent injured part of the body were hand and finger, which was 36.0% among total cases. 5. According to the accidents type, cases of caught in, under or between were most frequently observed as 53.9% of the total cases. 6. The most common source of injuries was power machine(50.5%). 7. According to the unsafe acts, cases of carelessness and unsafe information were most frequently observed as 71.2% of the total cases. 8. Admission rate(5.5%) and official report rate(2.2%) were very low rate. As above results, the authors recommend to prepare the systemic control programs on environmental and human factors of safety accidents such as improving the working conditions, working facilities, working methods and safety education, and control of working time for working day.

Effect of Prevention to Dental Caries by Incremental Oral Health Care Program among Students of Elementary School in some parts of Busan Title of Manuscripts (부산시 일부지역의 초등학교 계속구강건강관리사업의 치아우식 예방 효과)

  • This study was conducted to prepare the fundamental data on establishing an improvement plan for oral health promotion program, and to assess the effects of prevention to dental caries by incremental oral health care program among students of elementary school. The number of total subjects were 239 students in the care group who were controled incremental oral health care program and 195 students in the control group who were not controled incremental oral health care program. The sealant index was significantly more higher in care group than control group, especially the higher the grade. The sealant rate was significantly higher in care group than control group for all of grade. The DT index was significantly lower in care group than control group for male and female, the caries incidence rate of permanent tooth by gender was lower in male than female. The FT rate was more higher in care group than control group. The dental caries could be prevented very effectively when having continuously treated the incremental oral health care program at school dental clinic among elementary-school students. The author consider that this study will be prepare the fundamental data and contribute to oral health promotion according to a change in oral health behavior among elementary-school students. Therefore, the author recommend to expand applying the incremental oral health care program among students of elementary school.

Physical Therapists' Consciousness Level for Independent Establishment of Physical Therapy Clinic (물리치료실 자립개설에 대한 물리치료사의 의식도)

  • This study is conducted to investigate physical therapists' consciousness for independent establishment of their own physical therapy clinics, examining aspects of physical therapy rooms in medical institutions. Data were collected from 210 physical therapists working in Daegu and Kyungbuk regions from August 1, 1999 to August 31. 1999, The results were as follows; 1. As for general characteristics of the respondents. 56.3% was 'famele'; 60.4% 'twenties of age'; 56.1% 'working experience for blow 5 years'. 2. 75.3% of the respondents thinks that independent establishment of their own physical therapy clinics must be enacted without fail 50.1 % thinks that independent establishment of physical therapy clinic will be enacted within 5 to 10 years. 3. The important solutions in driving forward independent establishment of the clinic are accumulation of practice experience and introduction of new technology(31.3%), accumulation of practice experience and raising of academic career(19.0%), and accumulation of practice experience and legal action (14.0%). 10.9% believes that independent establishment will be achieved someday if they wait for it.

Factors Associated with Failure in The Continuity of Smoking Cessation Among 6 Month's Smoking Cessation Succeses in the Smoking Cessation Clinic of Public Health Center (보건소 금연클리닉 6개월 금연성공자의 금연지속 실패 요인)

  • This study was performed to investigate the factors related failure in continuity of smoking cessation among persons who were initially successful in quitting smoking for at least 6 months in smoking cessation clinic of public health center. Data were collected with the telephone questionnaire survey and the registered cards from 347 of 6 months quitters from 2006 to 2008 year. Data were analyzed by life table method and Cox-proportional hazard model. In Cox-proportional hazard model, Eup Myeon of residence(HR 2.50, 95% CI 1.69-3.68), without chronic diseases(HR 1.92, 95% CI 1.21-3.04), without another smoker in household(HR 1.93, 95% CI 1.21-3.09) and usage of supplement agent(HR 2.17, 95% CI 1.01-4.68) were independently associated with the failure in continuing to stay smoke-free. The cumulative rate of failure in the continuity of smoking cessation was 28.6% at 6 month and 36.1% at 24 month. For operating a clinic program for smoking cessation, Public health center should makes strategies that a person is continuing smoking cessation for over 6 months after the first 6 momth's smoking cessation.

A Study on Factors Affecting the Oral Health Promotion Behavior of middle school students based on subjective oral health (중학생들의 주관적 구강건강 상태에 따른 구강건강증진행위 영향요인 분석)

  • This study was carried out in order to be used as basic data in developing oral health program for adolescents hereafter by surveying which influence it has upon oral health knowledge and recognition depending on students' subjective oral health status targeting middle school students. First of all, examining about students' subjective oral health status, the 2nd graders accounted for 36.2% depending on school year, thereby having been indicated to be high in response as saying of being healthy in own teeth. It was indicated that the higher level of brushing teeth after lunch at school leads to the higher response as saying of being healthy in own teeth(p<0.05). The group with response as saying of healthy in own teeth was indicated to be higher in regularly visiting once or twice a year as the period of visiting dental clinic, and was indicated to be higher(p<0.001) in a visit for oral check-up even as for the aim of visiting dental clinic than other groups. Accordingly, the aim is to offer an opportunity of motivation that students will have interest in oral health, by carrying out continuously oral health education, and is also to increase knowledge and awareness level on oral health by allowing them to have positive attitude toward oral health.

Effects of Postpartum Exercise on Mental Health (산후운동이 산욕부의 정신건강에 미치는 영향)

  • Purpose: This study was to examine the effect of postpartum exercise on mental health. Mental health comprises a 9 symptom dimension of Somatization, Obsessive-Compulsive, Interpersonal Sensitivity, Depression, Anxiety, Hostility, Phobic anxiety, Paranoid Ideation, and Psychoticism. Method: A non-equivalent pre-test, post-test control group study was conducted. Fifty-two puerperal women who were admitted to a postpartum ward of a mother-baby clinic in Pusan were recruited; 26 women were assigned to the experimental group and 26 women to the control group. Postpartum exercise was applied to the experimental group from postpartum day 3 for a period of 8 weeks while no exercise program was applied to the control group. Mental health with SCL-90-R was measured before and after exercise. Data was analyzed using mean, $x^2$-test, t-test, and ANOVA by SPSS 10.0. Result: The mental health status in the exercise group did not show a statistically significant decrease when compared to that of the control group. Conclusion: There is no effect of postpartum exercise on women's mental health during the first 8 weeks of the postpartum period. Further studies are needed for randomized clinical trials with a larger sample size.

The impact of health belief model in the middle and high school students on oral health behaviors (중·고등학생의 구강건강신념이 구강건강관리행동에 미치는 영향: 건강신념모형을 적용하여)

  • Objectives: The aim of the study is to investigate the health belief model affecting the oral health behavior in middle and high school students. Methods: The subjects were 296 middle and high school students in Seoul, Gyeonggi and Incheon from February 15 to March 21, 2014. The students filled out the self-reported questionnaires after receiving informed consents. The instrument was adopted and revised from those of Kim & Hwang, and Choi & Joo. The questionnaire consisted of 4 questions of general characteristics, 9 questions of oral health status including subjective oral health status, frequency of tooth brushing, duration of tooth brushing, method of tooth brushing, use of oral health devices, dental clinic visit, scaling services, snack intake, and smoking. The oral health belief consisted of 25 questions including susceptibility, seriousness, barriers, benefit, and self-efficacy using Likert 5 scale. The reliability of Cronbach's alpha in the study was 0.725. Data were analyzed using SPSS ver 18.0 for frequency analysis, t-test, ANOVA, ${\chi}^2$-test, and Pearson's correlation coefficient, simple regression, and binary logistic regression. Results: Oral health beliefs of middle and high school students affected the oral health behaviors. Susceptibility, barriers and self-efficacy also influenced on the oral health behaviors. In order to provide the best oral health education, susceptibility and self-efficacy are the primary factors to increase motivation because the motivation endows the students with correction of oral health behaviors that improve the knowledge, attitudes, and decrease barriers in oral hygiene. Conclusions: It is important to correct oral health behaviors in the middle and high school students by providing the continuing and systematic oral health education.

The study on the Oral Health Knowledge and Behavior of Industrial Workers at Ulsan Province (울산지역 사업장 근로자의 구강보건지식과 행태에 관한 연구)

  • A survey using a questionnaire was conducted among industrial employees in Ulsan Total surveyed persons was 244. The purpose of this study was to analysis the relation and behavior of oral health promotion. Also, This study was to provide educational data of oral health. The obtained were analysed by SPSS program 12.0. The findings were as below: 1. Dental visit for prevention Y/N(%) was higher 50 years than 20 years(p < .001). Oral hygiene devices Y/N(%) for oral hygiene showed University graduates higher than Middle school graduates respectively(p < .05). 2. The knowledge of oral health was higher High school graduates than Middle school graduates respectively(p < .05). The rate of brushing teeth 3~5 times per day was higher females than males(p < .01) and better College graduates than Middle school graduates respectively(p < .001). 3. Oral health conditions of respondents were better 20years and 30years than 50years respectively(p < .001). Self-evaluation of Oral health sensitive was better College graduates than Middle school graduates respectively(p < .05). 4. Correlation between of Tooth brushing Frequency per Day by Oral health knowledge, dental clinic Visit, Extraction number had an effect on the significant dental clinic visit for prevention and the knowledge of Oral health had even a positive effect relationship(r = .233, p = .001). The knowledge of Oral health and the frequency of tooth brushing had even a positive effect relationship(r = .161, p = .05). The frequency of tooth brushing and the number of extraction of teeth had a negative effect relationship(r = -.145, p = .05).

A Study on the Status of Seeking Intervention among the Workers with Health Problems Identified by the Workers' Periodic Health Examination (특수건강진단에서 발견된 고혈압 및 간질환 유소견자의 건강관리 실태에 관한 조사)

  • Authors studied the workers' knowledge about the health problems detected through the previous workers' periodic health examination, content of follow-up management ana actions taken for their health problem detected on previous health examination. From June to September 1992, workers' periodic health examination was peformed on workers employed in 10 companies located in 2 middle-sized Korean cities. A questionnaire survey was done far 150 workers who reported to have $D_2$ result of either hypertension or liver disorder at the previous workers' periodic health examination done in 1991. The results are as follows; 1. Of 160 workers who had $D_2$ result of either hypertension or liver disorder in previous examination one year before, only 85 workers(51.3%, 43 workers with hypertension, 38 workers with live disorder) responded that they have such disorders. The other 65 workers responded to questionnaire were all those with C results. Respondents' knowledge about their diagnoses was relatively precise (95.2% in hypertension group, 94.6% in liver disorder group) but knowledge about classification of diseases was poor. 2. The main efforts to solve the health problem nab self management (20 spells, 55.3%), visiting clinic or hospital(6 spells, 12.8%), use of herb medicine (2 spells, 4.3%) and use of drug store(2 spells, 4.3%) in hypertension group. In liver disorder group, 30 spells (71.4%) relied on self management,6 spells (14.3%) on hospital or clinic and 9 spells (21.4%) had no effort to improve the health problem. Content of self management was low salt diet, quit smoking, regular exercise and quit alcohol drinking in order. Avoidance of salt in diet was high in hypertension group and quitting alcohol drinking was high in liver disorder group. In those with self management, 80.7% of hypertension group and 83.3% of liver disorder group continued previous effort. Those, however, who utilized clinic or hospital, only 16.7% and 50.0% were still visiting hospital or clinic. 3. Fifty seven percent of hypertension group and 64.3% of liver disorder group was presently smoking,8.5% and 11.9% reduced smoking and 21.3% and 14.3% stopped smoking. Forty nine percent of hypertension group and 28.6% of liver disorder group was presently drinking. Reduced alcohol intake was reported in 29.8% and 40.5%, 12.8ole and 23.8% stopped alcohol drinking. Sixty six percent of hypertension group and 73.8% of liver disorder group did no regular exercise, but 12.8% and 11.9% of each group increased their physical exercise far last one year. Forty three percent of hypertension group and 38.l% of liver disorder group was overweight (defined by bodymass index greater or equal than 25). Reduced body Weight was reported in 17.2% and 16.7% of each group. Reduced dietary salt intake was high in hypertension group (51.5%). The study results suggest that follow-up management after workers' periodic health examination is not satisfactory. In order to improve this situation, adequate information on the result of the workers' periodic health examination should be distributed to each worker group with health education and counselling.
