Purpose: To identify the importance and performance levels of health management duties, work-related and general attributes of health managers in medical institutions and analyze their impact on the performance of managing health-care related tasks. Methods: This research identified the performance levels of 150 health managers who have been executing industrial health-related duties for more than six months as nurses in medical institutions with more than 30 hospital beds. The variables which affect their performances were then analyzed. Results: The average importance of health care duties was 8.1 out of 10 and 5.5 for performance levels. Multi-regression analysis on the variables affecting performance levels of health managers in medical institutions showed that health managers exhibit higher numbers under the following conditions: over 300 full-time employees, more than 1 year but less than 3 years of experience, positions above section chief level, affiliation to the safety and health department, and high perception of duty importance. Conclusion: Improved cognizance of health manager importance should occur initially; then, health-care center setup, assigning of exclusive occupational health managers, and organizational efforts to improve the working environment in tandem with the provision of educational training programs to improve work quality are necessary.
Purpose: Gastroesophageal reflux disease (GERD) occurs in pediatric patients when reflux of gastric contents presents with troublesome symptoms. The present study compared the effects of omeprazole and ranitidine for the treatment of symptomatic GERD in infants of 2-12 months. Methods: This study was a clinical randomized double-blind trial and parallel-group comparison of omeprazole and ranitidine performed at Children Training Hospital in Tabriz, Iran. Patients received a standard treatment for 2 weeks. After 2 weeks, the patients with persistent symptoms were enrolled in this randomized study. Results: We enrolled 76 patients in the present study and excluded 16 patients. Thirty patients each were included in group A (ranitidine) and in group B (omeprazole). GERD symptom score for groups A and B was $47.17{\pm}5.62$ and $51.93{\pm}5.42$, respectively, with a P value of 0.54, before the treatment and $2.47{\pm}0.58$ and $2.43{\pm}1.15$, respectively, after the treatment (P=0.98). No statistically significant differences were found between ranitidine and omeprazole in their efficacy for the treatment of GERD. Conclusion: The safety and efficacy of ranitidine and omeprazole have been demonstrated in infants. Both groups of infants showed a statistically significant decrease in the score of clinical variables after the treatment.
Purpose: The purpose of this study was to identify nurses' duties and tasks for job analysis of the nurses in the operating room. Data were collected by 422 OR nurses from 57 hospitals. Results: Job of OR nurses included 13 duties and 105 tasks. The 13 duties were 'management of operative patient' 'assistive work for operation' 'supportive work for operation' 'infection control in OR' 'management of equipments and instruments for operation' 'administrating medicine for operation' 'supplies management for operation' 'safety management for operation' 'environmental management for operating room' 'administrative work for operation' 'education for operating room personnel' 'development of operation procedure books' and 'elf development of OR personnel' Conclusion: This study identified duties and tasks performed by OR nurses. Based on these results, the importance, frequency, and difficulty in this study will provide research evidence for developing training programs for OR nurses.
Purpose : The purpose of this study was to investigate the effects of virtual reality(VR) program on standing balance in stroke with sensory deficit. Methods : Subjects were assigned randomly to either VR group (n=28) or the control group (n=31) when study began. Both groups were conducted electricity therapy, exercise therapy 5 times per week during six weeks, and a group who have VR group 3 times per week during six weeks. Both groups assessed for standing balance before and after virtual reality program. Results : In standing balance, the virtual reality program which have sensory deficit in stroke group, open eyes from the center area of the body and center line significantly reduced the training period(p<.05). The virtual reality program to instantly move the weight through maximum voluntary safety limits to sensory deficit of virtual reality program(p<.05). Conclusion : Stroke patients with sensory deficit affects the recovery of the standing balance. In addition, virtual reality program was stroke patients with sensory deficit affects the recovery of standing balance.
Objectives: The purpose of this review is to evaluate the efficacy of moxibustion for stress urinary incontinence (SUI) in women. Methods: For relevant randomized controlled trials (RCTs), we searched the following databases from their inception to September 1, 2020: The Cochrane Library, PubMed, EMBASE, Chinese National Knowledge Infrastructure Database (CNKI), Koreanstudies Information Service System (KISS), Research Information Sharing Service (RISS), and National Digital Science Library (NDSL). The key search terms were 'stress urinary incontinence' and 'moxibustion'. Data extraction and assessment of risk of bias were conducted by two authors independently. Results: A total of 11 RCTs were finally included in this systematic review. In all studies, moxibustion treatment was applied as an adjuvant therapy to the conventional treatment, and the most common conventional treatment was pelvic floor muscle training (PFMT). The treatment group (conventional treatment plus moxibustion) showed statistically more significant effect than the control group (conventional treatment only) in various evaluation indicators including urinary incontinence frequency, 1 hour urine pad test, quality of life, the clinical efficacy rate, and pelvic muscle strength. Conclusions: In this study, we investigated the efficacy of moxibustion as an adjuvant therapy for female SUI patients. Further studies are needed to supplement the safety of moxibustion and the evaluation of moxibustion dose.
Purpose: The purpose of this study was to determine the factors related to airway failure during endotracheal intubation among patients with and without predicted airway difficulty. Methods: Medical records were examined retrospectively. 329 patients who were admitted to the general ward and underwent endotracheal intubation were included. The incidence of airway failure in the two groups was investigated. Results: The group predicted to have airway difficulty consisted of 79 patients (24.0%) and the group without airway difficulty, 250 (76.0%). The number of cases of airway failure was 50 (15.2%). The factors that were associated with airway failure in the group with predicted airway difficulty were the jaw relaxation score, Cormack-Lehane score, and the device of the first endotracheal intubation attempt. The factors that were associated with the airway failure in the group predicted not to have airway difficulty were the induction agent, jaw relaxation score, Cormack-Lehane score, level of training of the personnel with the first endotracheal intubation success, and the device of the first endotracheal intubation attempt. Conclusion: The prediction of airway difficulty during endotracheal intubation was not effective; however, it was meaningful from the perspective of patient safety.
Purpose: The purpose of this study was to evaluate the effects of mechanical ventilation simulation on the clinical judgment and self-confidence of nursing students. Methods: This study was a quasi-experimental study. From one university, 118 undergraduate nursing students participated in this study. Sixty students were in the intervention group, and fifty-eight students were in the control group. A simulation scenario utilizing a high-fidelity human simulator focusing on nursing care for patients with a mechanical ventilator was developed for this study. Data were collected with a self-report survey method before the intervention, right after intervention, and two weeks later. Results: Students in the intervention group showed significantly higher increases in clinical judgment and self-confidence than those in the control group at the immediate posttest. Moreover, 2 weeks later, the increase in clinical judgment and self-confidence from the pretest among the intervention group was significantly larger than those in the control group. Conclusion: Utilizing simulation education focusing on patients with a mechanical ventilator may contribute to training more competent nurses in the area of critical care nursing. It may also serve to provide a better critical care environment for the safety and health of patients.
Background: Self-checked monitoring home exercises are recommended for preventing falls among people with Parkinson's disease. However, as these home exercises are performed autonomously by patients without professional management, their accuracy and efficiency can be compromised. Objective: To investigate the effects of providing regular training sessions to patients and caregivers and of patient self-monitoring of exercise performance following the implementation of a self-checked monitoring exercise program for people with Parkinson's disease. Design: Randomized Pretest-Posttest Control Group Design. Methods: We provided regular self-checked monitoring home exercise and general home exercise programs to 30 participants for 12 weeks. Once a month at the first, fifth, and ninth-week sessions, a rehabilitation team attended the Parkinson's group education. In addition to the subject in the experimental group perform the home exercises program to provide feedback regarding the home exercises program and to carry out a self-monitoring checklist performance for 12 weeks. Results: The 10 m walk test, functional reach test, and sit to stand test and the modified Barthel index significantly improved in the self-checked monitoring home exercise group. Conclusion: These results suggest that self-checked home exercise programs, which facilitate safety and consistent performance of exercises at home, are beneficial for people with Parkinson's disease.
This article deals with the application of reliability analysis for determining the safety of simply supported beam under the uniformly distributed load. The uncertainties of the existing methods were taken into account and hence reliability analysis has been adopted. To accomplish this aim, Generalized Regression Neural Network (GRNN), Extreme Learning Machine (ELM) and Gaussian Process Regression (GPR) models are developed. Reliability analysis is the probabilistic style to determine the possibility of failure free operation of a structure. The application of probabilistic mathematics into the quantitative aspects of a structure and improve the qualitative aspects of a structure. In order to construct the GRNN, ELM and GPR models, the dataset contains Modulus of Elasticity (E), Load intensity (w) and performance function (${\delta}$) in which E and w are inputs and ${\delta}$ is the output. The achievement of the developed models was weighed by various statistical parameters; one among the most primitive parameter is Coefficient of Determination ($R^2$) which has 0.998 for training and 0.989 for testing. The GRNN outperforms the other ELM and GPR models. Other different statistical computations have been carried out, which speaks out the errors and prediction performance in order to justify the capability of the developed models.
