절박 유산의 한약 치료 효과에 대한 무작위 대조 임상시험 연구 고찰 (A Review of Randomized Controlled Trials on the Treatment Effects of Herbal Medicine for Threatened Abortion)

  • Objectives: This review is aimed to figure out the effectiveness of herbal medicine for treating threatened abortion by analyzing randomized controlled trials. Methods: We searched relevant studies using seven databases including 4 domestic databases and 3 foreign databases. Data retrieval was conducted on May 18, 2022. and the papers published from January 1, 2018 to May 18, 2022 were included. The risk of bias was assessed by using Cochrane's risk of bias tool. Results: 182 studies were searched in domestic and foreign databases, and 9 studies were finally selected. Among 9 studies, the treatment group was treated with only oral herbal medicine in 4 studies and with combined treatment of herbal medicine and western medicine in 5 studies. On the other hand, the control group was treated with only western medicine. In all 9 studies, evaluation indexes in the treatment group showed more effectiveness than those of the control group, evaluation indexes were different for each study though. Conclusions: Herbal medicine was more effective for treating threatened abortion whether using it alone or combining with western medicine compared to using western medicine alone. Additional studies which include large number are needed to confirm long-term effectiveness and safety of herbal medicine treatment.

자궁근종의 한약치료에 대한 최신 임상 연구 동향 - CNKI 검색을 중심으로 (Research Trends of Herbal Medicine Treatment for Uterine Leiomyoma - Research using CNKI)

  • Objectives: This study was conducted to find out research trends for uterine leiomyoma and to recognize the efficacy and safety of herbal medicine treatment for uterine leiomyoma and to help in the clinical research of herbal medicine treatment for uterine leiomyoma in the future. Methods: Based on eight domestic and foreign databases, We analyzed the Randomized Controlled Trials using herbal medicine for uterine leiomyoma. Data retrieval was carried out on September 10, 2019 and the latest five-year papers were included. The treatment group was treated with herbal medicine and the control group was treated with western medicine. Results: 473 studies were searched in domestic and foreign databases, and 12 studies were finally selected. Although the indicators varied, all studies used total efficiency rate as indicators. In all 12 studies, the treatment group treated with herbal medicine was more effective than the control group treated with Western medicine.

소음인(少陰人) 곽향정기산(藿香正氣散)의 위해물질에 대한 안전성 연구 (Safety on Hazardous Substances of Soeumin Kwakhyangjeonggi-san)

  • 1. Objective: To compare the contents and transfer rate of hazardous substances in crude, washing solution, crude after washing, decoction and remnant after boiling. 2. Methods: The heavy metal contents of each step were measured by inductively coupled plasma mass spectrometer (ICP-MS) and mercury analyzer (MA-2). In order to analyze pesticides in each sample we used simultaneous multi-residue analysis of pesticides by GC/ECD, which was followed by GC/MSD analysis to confirm the identity of the detected pesticide in each sample. In addition, the contents of sulfur dioxide (SO2) were performed by Monier-Williams distillation method. 3. Results: 1) Contents (mg/kg) of heavy metals in decoction of all herbal medicine prescriptions were not detected. 2) Transfer rates (%) of heavy metals from crude to remnant were As (83.3%), Cd (100.0%), Pb (182.6%) and Hg (100.0%). 3) Contents (mg/kg) of residual pesticides were not detected. 4) Transfer rate (%) of sulfur dioxide (SO2) from crude to remnant was 44.2%. 4. Conclusion: Our results showed that boiled herbal medicine prescriptions which we take is safe from the hazardous substances.

한약제제(액제)의 안전성 평가 (Safety Assessment of Liquid-Type Herbal Preparations)

  • The safety of liquid-type herbal preparations (47 samples) and beverages containing herbal medicines (18 samples), which are distributed in Gyeonggi province was studied by analyzing heavy metals, benzo(a)pyrene, preservatives and sugar content and to explain the differences between medicine and food based on the labeling standards. The herbal preparation is regarded as medicine and can be differentiated from the beverages containing herbal medicine, which is food, based on the fact that the herbal preparation should be labeled with usage, dosage, efficiency and effect. The mean concentrations of Pb, As, Cd, Hg were found to be 0.014 mg/kg, 0.113 mg/kg, 0.004 mg/kg and 0.003 mg/kg in herbal preparation and 0.009 mg/kg, 0.122 mg/kg, 0.003 mg/kg and 0.002 mg/kg in beverages containing herbal medicines, respectively. In 10 cases of herbal preparations and 6 cases of beverages containing Rehmanniae Radix Preparata (Sukjihwang), benzo(a)pyrene was not found. According to preservative test on herbal medicine, dehydroacetic acid was found less than the indicated content in 6 cases out of 37 cases, which labeled with preservatives, and benzoic acid, dehydroacetic acid and methylparaben were contained with 164.0~198.0 mg/kg, 149.8~272.5 mg/kg and 88.4 mg/kg, respectively, in 3 cases out of 10 cases, which was not labeled with preservatives. Among 16 cases of beverages containing herbal medicines, 3 cases of preservative-labeled beverages had the preservatives, and the rest of all, which was not labeled with preservatives, did not show that they contain any preservative, therefore, it was suitable for the labeling standards. As a result of sugar content test, preservative-labeled products were not appreciably different from the others.

복합한약물(가칭; 태음비만탕)을 이용한 비만치료약물 개발을 위한 연구 (Research and Developement of Herbal Medicine(Taeumbiman-tang) on Obesity)

  • A Korean traditional herbal formular, Taeumbiman-tang(TBT), was based on Taeumjowi-tang, currently the most widely used herbal formula compound, and which is known to be safe and have a positive effect on adult obesity. TBT was given to obese children for thirty days, and was found to be clinically safe and effective. The subjects were children who had been admitted into the hospital to be treated for obesity, that had more than 20% relative body weight. Originally there were 19 subjects, but 5 dropped out of the experiment. There were 5 girls and 9 boys, whose average ages were $11.00\;{\pm}\;2.60$ years, average weight was $54.01\;{\pm}\;18.59\;kg$. As a pretest, all of the volunteers were examined for height, weight, blood pressure, pulse rate, EKG, and chest radiography. The subjects underwent interview, body measurement tests, blood tests, and safety tests under 8 hour fasting state. After 15 days of TBT intake, the safety tests and the interview were conducted. The interview, body measurement tests, blood tests, and safety tests were conducted again after the 30 day trial was completed. The short term effects of TBT on obese children is weight loss. It did not cause any significant changes in the subject's livers, hearts, and kidneys, and clinically dangerous side effects or withdrawal symptoms were not observed.

수종의 약침제제 견정혈 자입이 활력징후 변화에 미치는 영향;황기 약침, 산삼 약침, 생지황 약침을 중심으로 (The Effects of Distilled Astragali Radix Herbal Acupuncture, Wild Ginseng Herbal Acupuncture and Rehmannia Glutinosa Herbal Acupuncture on Vital Sign;A Randomized, Placebo-controlled, Double-blind Clinical Trial)

  • Objectives: We investigated the safty of distilled Astragali Radix Herbal Acupuncture, Wild Ginseng Herbal Acupuncture and Rehmannia glutinosa Herbal Acupuncture on vital sign -BP, pulse, resperation-in adult man. Methods : We investigated on 106 healthy volunteers consisted of each 30 subjects in experiment (distilled Astragali Radix Herbal Acupuncture, Wild Ginseng Herbal Acupuncture and Rehmannia glutinosa Herbal Acupuncture) group and 30 subjects in control(Normal Saline) group. Study form was a randomized, placebo-controlled, double-blind clinical trial. 30 subjects in each experiment group were injected distilled Astragali Radix Herbal Acupuncture, Wild Ginseng Herbal Acupuncture, Rehmannia glutinosa Herbal Acupuncture at $GB_{21}$(Kyonjong) and 30 subjects in control group were injected Normal Saline at $GB_{21}$ (Kyonjong). except of 2 subjects(in control group) who can't be measured and 10 subjects(6 in experiment group and 4 in control group) who move or make unforceable error during measuring. Finally each 25. subject, subject, subject in experiment group and 24 subject in control group are studied. We measured resperation by PolyG-I and BP & pulse by electric manometer on 5 times : before injection per 5 minutes during and after injection per 10 minutes during 35 minutes. The SPSS 13.0 for windows was used to analyze the data and the Student t test(between two groups) were used to verify the result. Results : 1. After distilled Astragali Radix Herbal Acupuncture injection, Wild Ginseng Herbal Acupuncture and Rehmannia glutinosa Herbal Acupuncture , systolic BP is not significantly change in all experiment time. 2. After distilled Astragali Radix Herbal Acupuncture injection, Wild Ginseng Herbal Acupuncture and Rehmannia glutinosa Herbal Acupuncture , diastolic BP is not significantly change in all experiment time. 3. After distilled Astragali Radix Herbal Acupuncture injection, Wild Ginseng Herbal Acupuncture and Rehmannia glutinosa Herbal Acupuncture , pulse is not significantly change in all experiment time. 4. After distilled Astragali Radix Herbal Acupuncture injection, Wild Ginseng Herbal Acupuncture and Rehmannia glutinosa Herbal Acupuncture , resperation is not significantly change in all experiment time. Conclusions : The results suggest that distilled Astragali Radix Herbal Acupuncture, Wild Ginseng Herbal Acupuncture and Rehmannia glutinosa Herbal Acupuncture in healthy adult man don't influence vital sign within normal range. This result is derived from that distilled Astragali Radix Herbal Acupuncture, Wild Ginseng Herbal Acupuncture and Rehmannia glutinosa Herbal Acupuncture in healthy adult man are safety.

소아 기능성 복통의 한약치료에 대한 임상연구 동향 - 중의학 논문을 중심으로 - (Review of Clinical Researches for Herbal Medicine Treatment on Functional Abdominal Pain in Children.)

  • Objectives The purpose of this study is to evaluate the efficacy and safety of herbal medication for the treatment of functional abdominal pain in children by analyzing randomized controlled trials conducted in China. Methods Literatures from China National Knowledge Infrastructure published up to 23 November, 2020 were searched. Then they were collected and analyzed by their publication year, demographic information, treatment methods used, duration of illnesses, duration of treatments, follow-up periods, outcome measurement and adverse events. Results A total of 27 studies were selected for the analysis. In all studies, the total efficacy of the herbal medicine treatment group was reported to be higher than that of the control group. The most frequently used medical herb was Root of Glycyrrhiza uralensis Fischer (甘草). The most frequently used classes of herbal medications are Qi regulating medicine (理氣藥), Qi tonics (補氣藥), Aromatic wet medicine (芳香化濕藥), and Digestant (消食藥). The adverse events rate for the herbal medicine treatment group were significantly lower than those in the Western medicine treatment group. Conclusions This study showed that herbal medicine treatment can be effective and safe option for treating pediatric functional abdominal pain. However, additional well-designed clinical studies are needed to solidify the findings.

임신오조의 한약 치료 효과에 대한 무작위 대조군 임상 연구 고찰 (A Review of Randomized Controlled Trials on the Treatment Effects of Herbal Medicine for Hyperemesis Gravidarum)

  • Objectives: This study aimed to anaylze randomized controlled trials on the effectiveness of herbal medicine for hyperemesis gravidarum. Methods: We searched the randomized controlled trials that intervented oral herbal medicine treatment on hyperemesis gravidarum retrieved using seven domestic and foreign databases. Literature search was conducted on April 19, 2021. Results: 185 studies were searched, and 7 were finally selected based on inclusion and exclusion criteria. All subjects were treated with western symptomatic treatment and in treatment group, additional oral herbal medicine was administered. Although the evaluation index was different for each study, all the indexes in the treatment group were significantly improved compared to the control group. Conclusions: Herbal medicine treatment for hyperemesis gravidarum showed significant effectiveness compared to western symptomatic treatment. In the future, additional studies are needed to evaluate the safety of herbal medicine treatment.

한약제제 백출(白朮)과 한약재 백출에 대한 HPLC 분석 비교연구 (A Comparative Study on the Herbal Prescription and the Herbal Substance of Atractylodis Rhizoma Alba by HPLC analysis)

  • Objectives : To verify the equivalence between Atractylodis Rhizoma Alba herbal prescription(HP-ARA) and Atractylodis Rhizoma Alba herbal sub stance(HS-ARA). Methods : Safety tests by microbial regulation and heavy metal analysis (total heavy metal, Pb, As) and a stability test by long term shelf test for HP-ARA according to notification of the Ministry of Food and Drug Safety were carried out. Then, multi component profile of HP-ARA and HS -ARA were analyzed by HPLC. Results : The safety and stability of HP-ARA confirmed by several tests. Correlation coefficient of equivalence of HP-ARA and ARA-HS showed 0.992. Conclusion : Based on this result of equivalence between HP-ARA and HS-ARA, HP-ARA can substitute HS-ARA used to make herbal medicines (herbal decoction, pills and powder).

ICR 마우스를 이용하여 사물탕(四物湯)의 급성 독성에 관한 연구 (Acute Toxicity Study on Samul-tang(Siwu-tang) in Mice)

  • Objectives: Samul-tang(Siwu-tang) has been traditionally prescribed a medicine as a restorative. Methods: In this study, we investigated the acute toxicity about water-extracted Samul-tang(Siwu-tang). Twenty-five mice completed 14 days of oral Samul-tang(Siwu-tang) at the respective doses of 0(control group), 2560, 3200, 4000 and 5000mg/kg. Results: We observed survival rates, general toxicity, change of body weight, and autopsy. Conclusions: Compared with the control group, we could not find any toxic alteration in all treated groups (2560, 3200, 4000 and 5000mg/kg). $LD_{50}$ of Samul-tang(Siwu-tang) was over 5000mg/kg and it is very safe to ICR mice.
