청소년의 알레르기성 질환과 관련된 사회경제적 및 사회인구학적 요인: 제14차 한국청소년건강행태조사를 기반으로 (Socioeconomic and Sociodemographic Factors related to Allergic Diseases in Korean adolescents based on the 14th Korean Youth Risk Behavior Survey)

  • 본 많은 연구에서 알레르기성 질환과 사회경제적 및 사회인구학적 변수의 상관관계가 있음을 보여주었다. 그러나 이들 변수가 한국 청소년에게 미치는 영향에 대한 연구는 거의 이루어지지 않았다. 본 연구는 한국 청소년의 알레르기성 질환에 대한 사회경제적 및 사회인구학적 위험요인을 파악하고 예방 및 관리 하기 위한 정보 제공을 목적으로 수행되었다. 2018년 한국청소년건강행태조사를 기반으로 60,040명 청소년의 자료가 사용되었다. 사회경제적 및 사회인구학적 변수는 익명으로 관리되는 온라인 설문 조사를 통해 수행되었고, 알레르기성 질환은 천식, 알레르기성 비염, 아토피 피부염이 포함되었다. 자료의 분석을 위해 SPSS 21.0 프로그램을 이용하였고, 다변량 로지스틱 회귀분석을 통한 복합표본분석이 사용되었다. 낮은 경제적 수준, 어머니와 동거, 부모의 높은 교육수준, 높은 학업성적, 성별, 비만, 음주경험 및 흡연 경험 등이 천식, 알레르기성 비염, 아토피 피부염과 관련이 있었다. 낮은 사회경제적 수준과 건강에 해로운 행동은 알레르기성 질환의 위험요인 이었다. 그러므로 지역사회의 균형적 경제성장과 함께 청소년의 해로운 건강행태를 통제하는 것은 한국 청소년의 알레르기성 질환의 유병률 감소에 도움이 될 것이다.

우리나라 어린이의 생활안전행동과 구강보건행동의 관련성 (Relationship between General Safety Behaviors and Oral Health Behaviors among South Korean Children)

  • 본 연구는 2005년도에 실시된 제 3기 국민건강영양조사 자료를 이용하여 어린이의 생활안전행동과 구강보건행동의 관련성을 알아보고 사회인구학적 연관 요인을 평가하여 어린이의 생활안전행동와 구강보건행동을 효과적으로 증진하기 위한 기본 방향을 세우고자 수행되었다. 본 연구를 통하여 우리나라 어린이들의 생활안전행동과 구강보건행태에 대하여 다음과 같은 결론을 얻었다. 1. 대체로 어린이의 생활안전행동과 구강보건행동의 연관성이 발견되지 않았고 따라서 어린이의 생활안전행동과 구강보건행동의 변화를 추구함에 있어서 공통위험요인접근법 적용의 근거를 발견하지 못하였다. 2. 우리나라 3-11세 어린이는 56.3%가 하루 2회 이상 칫솔질을 실천하였고 74.7%가 예방적 치과진료 경험이 있는 것으로 나타났다. 자동차 뒷좌석 이용을 제외한 생활안전행동의 실천은 현저하게 부족하여 자동차 보호장구 이용율 10.4%, 자전거 안전모 착용율 4.3%, 및 인라인스케이트 안전모 착용율 29.0%에 그쳐(Table 2), 특히 생활안전교육의 강화의 필요성이 절실함을 보여주었다. 3. 미취학 어린이에 비하여 7세 이후 취학어린이에게서 생활안전행동과 구강보건행동의 실천률이 낮은 것으로 조사되어, 초등학생의 생활안전교육과 구강보건교육이 강조되어야 할 것으로 사료된다. 4. 3-6세 아동에서 거주지역 또는 가정의 경제수준 등 사회경제적 요인과 생활안전행동 및 구강보건행동의 연관성이 관찰되었으나, 취학아동에서는 대체로 유의한 연관성이 없었다(Table 3).

Socioeconomic Disparities in Osteoporosis Prevalence: Different Results in the Overall Korean Adult Population and Single-person Households

  • Objectives: The present study was conducted in order to examine the association between socioeconomic status (SES) and osteoporosis prevalence in Korea and to assess whether different associations are found in single-person households. Methods: A cross-sectional population-based study was conducted using the Korea National Health and Nutrition Examination Survey, from 2008 to 2011. The study subjects were people aged ${\geq}50$ years with osteoporosis as defined by bone mineral density. Multivariate logistic models were used to estimate prevalence odds ratios (pORs) and 95% confidence intervals (CIs). Gender differences in the likelihood of osteoporosis were analyzed based on household income, education level, and residential area. Results: There were 8221 osteoporosis patients aged ${\geq}50$ years, of whom 927 lived in single-person households. There was a gender-specific association between osteoporosis prevalence and all three SES factors that we analyzed: income, education, and residential area. After adjusting for age, SES, and health behaviors, including body mass index (BMI), low household income was only significantly associated with osteoporosis in men, whereas education level had an inverse relationship with osteoporosis only in women (p=0.01, p<0.001, respectively). However, after controlling for age and BMI, rural residency was only associated with osteoporosis in women living in single-person households (pOR, 1.59; 95% CI, 1.05 to 2.43). Conclusions: The Korean adult population showed a gender-specific relationship between SES and osteoporosis prevalence, with a different pattern found in single-person households.

지역자원시설세에 관한 연구 -구 공동시설세를 중심으로- (A Study on the Regional Resources Facilities Tax)

  • 주민의 안전한 생활을 보장할 수 있는 소방서비스를 충분히 제공하기 위하여 지방재정인 소방예산을 증가시킬 필요성이 있다. 그런데 소방예산을 증가시킬 수 있는 하나의 방법은 소방의 목적세인 지역자원시설세를 개편하는 것이다. 이 연구의 목적은 지역자원시설세와 소방예산, 소방사업비예산, 사회 경제적 요인, 소방수요 간의 관계를 살펴보는 것이다. 이를 위하여 본 연구는 과거의 연구와 이론적 분석을 통하여 5가지 가설들을 설정하였다. 이 가설을 검증하기 위한 통계적 방법은 회귀분석과 t검증이다. 가설의 검증 결과, 지역자원시설세는 소방예산과 소방사업비 예산에 유의적인 정의 관계를 보이고 있었으며, 사회 경제적인 요인도 지역자원시설세에 유의적인 정의 관계를 보였다. 또한 소방수요 변수 중에서 화재, 구조변수가 지역자원시설세에 유의적인 관계를 보이고, 구급, 잠재적 수요변수는 유의적이지 않았다.

건강의 사회적 결정요인과 필요 치과진료 미수진과의 관련성 (Relationship between Unmet Dental Needs and Social Determinants of Health)

  • 필요 치과진료 미수진과 관련된 요인을 파악하고자 지역사회건강조사 원자료를 분석하였다. 21만 4861명의 만 25세이상 인구집단을 대상으로 인구학적 요인, 사회경제적 요인, 사회적지지 및 사회적 환경요인, 사회물리적 환경요인과 필요 치과진료 미수진과의 연관성을 확인하였다. 연구결과, 연령이 낮을수록, 소득분위가 높을수록, 교육수준이 낮을수록, 친척, 친구와 자주 접촉하지 않을수록, 친목, 여가 활동에 참여하지 못할수록, 사회물리적 환경(안전수준, 자연환경, 생활환경, 대중교통, 의료서비스)에 불만족할수록 필요 치과진료 미수진율이 높게 나타났다. 필요 치과진료 미수진에 인구학적 요인, 사회경제적 요인 뿐 아니라 사회적지지 및 사회적 환경요인, 사회물리적 환경요인이 통계적으로 유의한 영향이 있음이 확인되었다. 필요 치과진료 수진율을 높일 수 있는 방안으로 건강의 사회적 결정요인에 정책적 요인을 포함하는 추후 연구가 필요하겠다.

아시아지역 국가들의 보건문제 우선과제에 대한 조사연구 (Key Public Health Issue Priorities in Asian Countries)

  • Asian Branches of International Life Science Institute (ILSI), i.e. China, India, Japan, Korea and South East Asian Region, identified five key public health issue priorities of each region and compared the results. In case of China, India and South East Asian countries (Indonesia, Malaysia, Philippines, Thailand etc. ASEAN countries), communicable diseases were the first priority issue, while elderly issue and food safety were prime issues for Japan and Korea, respectively. Malnutrition was the second priority issue for India and ASEAN countries, whereas non-communicable disease like cancer and degenerative diseases was for Korea and China, and obesity far Japan. Typical issues were smoking for China, nutrition education for China and Japan, biotechnology aiming GMO for India, and functional food causing health claim problem for Korea and Japan. Although the priority varied with the socioeconomic situation of each county, food and water safety recorded the highest priority of all the countries. The key public health issues of Korea were discussed in detail.

국내 주요 직업병 및 생활제품 위해 사건과 전문가의 역할 (Occupational and Environmental Safety Issues in South Korea and Their Implications for Health Experts)

  • The industrial development and socioeconomic structures of a society are inextricably linked to occupational accidents and diseases. Since the 1970s, a series of major occupational diseases have occurred in South Korea, such as mercury poisoning in Munsong-myeon, carbon disulfide poisoning at Wonjin Rayon, and leukemia at Samsung Electronics. These incidents have caused us to take a critical look at the level of worker safety and health management within companies and the government. These serve as symbolic and representational events. Doctors, lawyers, and labor activists concerned about employee safety and lives came together for years to struggle with the government and industry in order to determine what caused the diseases and to obtain compensation for victims. They finally achieved recognition for occupational diseases and recompense for losses. It is difficult to find similar cases to the baby powder asbestos talc cases and the radon bed case internationally, or there is no case where such a large number of consumers have been potentially harmed. It is also difficult to prove that the damage took place due to the victims' diverse geographic locations and ages, as well as the disease's long incubation period. Based on this premise, businesses and the government have been apathetic in acknowledging these diseases and compensating for damages, with few practical outcomes. Furthermore, unlike the large-scale occupational disease cases, only a few people including expert groups were organized and actively participated in the settlement of these issues, so it remains unfinished business for our society to address.

Priority survey between indicators and analytic hierarchy process analysis for green chemistry technology assessment

  • Objectives This study presents the indicators and proxy variables for the quantitative assessment of green chemistry technologies and evaluates the relative importance of each assessment element by consulting experts from the fields of ecology, chemistry, safety, and public health. Methods The results collected were subjected to an analytic hierarchy process to obtain the weights of the indicators and the proxy variables. Results These weights may prove useful in avoiding having to resort to qualitative means in absence of weights between indicators when integrating the results of quantitative assessment by indicator. Conclusions This study points to the limitations of current quantitative assessment techniques for green chemistry technologies and seeks to present the future direction for quantitative assessment of green chemistry technologies.

Workability and Life Satisfaction: Effects of Workers' Positive Perceptions on Their Return to Jobs

  • Background: The death rate of workers due to industrial accidents in South Korea (3.61 persons in 2017) is higher than the Organization for Economic Cooperation and Development average (2.43) and the fifth highest among Organization for Economic Cooperation and Development member countries. Although the pandemic of novel coronavirus (COVID-19) has changed, the socioeconomic aspects of Korean society, the number of Koreans suffering accidents and the number of deaths in 2020 have increased. It is necessary to take measures to prevent accidents and make comprehensive efforts to return to work. This study proposes research questions about the effect of workers' positive perception on whether to work after accidents and the impact of the experience of rehabilitation services on the return to work. Methods: This research performed a panel logistic regression analysis using data on workers' compensation insurance in Korea for two years (2018-2019). Results: This research finds that workers' positive perceptions of workability and life satisfaction contributed affirmatively to their re-employment. Several factors related to employment (e.g., work period, the number of job qualifications) also positively affect their return to work. However, the experience of rehabilitation services did not have a significant effect on re-employment. The variables of their health conditions (e.g., disability grade, feelings of health problems, age) negatively influenced their return to jobs. Conclusion: These results suggest the importance of workers' mental recovery and the need to innovate rehabilitation services for their employment. Positive thinking and self-rehabilitation could be critical for workers, parallel with social welfare policies.

근골격계질환 예방을 위한 인간공학적 작업시스템 평가킷의 표준화 (Standardization of the Ergonomics Kit for Evaluating Musculoskeletal Hazards in the Work System)

  • A standardized ergonomics kit is suggested to evaluate and reduce musculoskeletal hazards that yield large socioeconomic burdens in the work system. This kit comprises basic and detailed analysis tools with proper tool selection rules. The basic tool includes a checklist for various risk factors to screen potentially hazardous jobs among manual works. Selective detailed analysis tools can then be applied to the screened job lists for more quantitative and precise evaluation. The tool selection rules are devised for using the basic and detailed analysis tools in a mutually supplemental way. To validate the standardized ergonomics kit, it was applied to evaluate jobs related to the musculoskeletal hazards in a paper-making industry. Among 101 manual jobs investigated, 44 potential hazardous jobs were screened during basic investigation phase and finally 16 hazardous jobs were identified by the detailed analysis phase. The result provided fairly promising ideas of ergonomic interventions for the hazardous jobs.
