  • Background: A job-specific Worker's Health Surveillance (WHS) for hospital physicians is a preventive occupational health strategy aiming at early detection of their diminished work-related health in order to improve or maintain physician's health and quality of care. This study addresses what steps should be taken to determine the content of a job-specific WHS for hospital physicians and outlines that content. Methods: Based on four questions, decision trees were developed for physical and psychological job demands and for biological, chemical, and physical exposures to decide whether or not to include work-related health effects related to occupational exposures or aspects of health reflecting insufficient job requirements. Information was gathered locally through self-reporting and systematic observations at the workplace and from evidence in international publications. Results: Information from the decision trees on the prevalence and impact of the health- or work-functioning effect led to inclusion of occupational exposures (e.g., biological agents, emotionally demanding situations), job requirements (e.g., sufficient vision, judging ability), or health effects (e.g., depressive symptoms, neck complaints). Additionally, following the Dutch guideline for occupational physicians and based on specific job demands, screening for cardiovascular diseases, work ability, drug use, and alcohol consumption was included. Targeted interventions were selected when a health or work functioning problem existed and were chosen based on evidence for effectiveness. Conclusion: The process of developing a job-specific WHS for hospital physicians was described and the content presented, which might serve as an example for other jobs. Before implementation, it must first be tested for feasibility and acceptability.

  • Background: Korea's health screening program has been faced the need for changes as the population and diseases structure are changing. In addition to Korea, many countries operate state-led health check-up programs to improve the health level of the people, and the operating methods of the program appear in various forms according to each country's health insurance system. This study examines other state-led health screening programs and proposes a direction for the development of Korea's health screening program. Methods: The study was conducted using the literature review method, and the "country" was set as a unit for the case analysis. The operating method of the health screening programs and the financial resources were compared according to the health insurance system. Five countries were selected as Korea, the United States, the United Kingdom, Japan, and Taiwan. Results: The analyzed countries mainly operate the health screening program as a management method for chronic diseases, but there were differences in the operating method, financing, and targeted subjects and examination items. In most countries, a risk assessment was performed prior to the examination (screening), and the subjects who needed the examination were first selected, and a follow-up management service was provided in accordance with the risk each individual exposed. Conclusion: Rather than applying the same screening method to populations with different risk levels, a health screening program will be constructed in consideration of the individual's health level and exposure risk, and the healthcare delivery system will be reorganized so that screening and treatment services can be linked.

  • Frequency responses of ambulatory electrocardiogram systems were measured. Phase shift was assessed by our improved Wagner's method. The characteristics of the systems were found much worse than that of ordinary ECG equipment both in gain and phase responses. The decay of -3dB in amplitude observed in 0.2Hz and the lead of 45 dog in phase was observed in 0.6Hz. In order to investigate which of these 2 responses play major role in generating false S -T deviation, mathe- matical filters were Composed in a computer and actual ECGs were fed. The false S-T deviation were found to be caused mostly by enormous lead in phase, and then, the compensation circuit to diminish the lead in phase was inserted in the commercial systemes. The compensated systems showed no false deviation in S-T segment.

The data were collected through the participant observation and direct interview for about 18 months in P city. The participants were 7 mothers, from 32 to 37 years old. They have children of ages 3 to 10 and 1 or 2 children in total. The data were analyzed through the Spradley's Ethnographic method and the results were summarized as follows : The viewpoints which influence maternal health promoting behaviors include “having childlike appearance”. “having childlike character”, “living without illness”, and “eating well”. The maternal health promoting behaviors for children are classified as “adapting to nature”, “supporting of ability”, “sharing with the family”, “training”, and “praying”. The adapting behaviors include “recognizing child's innate character”, “controlling maternal desires”, “preparing natural food”, which includes breast feeding. The supporting behaviors include “recognizing childlikeness”, “empowering”, and “restricting certain actions to preventing accidents”. In order to promote child's health, it is essential to promote mother's health first of all. • The School Health Act was revised in 2007 and the contents of school health education was officially organized in the school curriculum in elementary, middle and high schools, since the contents of school health education disappeared in 1963. For the successful school health education, sufficient time for health education should be provided by opening health education course as an essential subject in every grade. The large scale schools need to have more school health teachers for performing efficient health education and in all schools there needs to be a reorganization of the teacher's complex works. For quality improvements of the health education, the change of principal's attitude and budget plan for health education are essential. Additionally, various training programs for school health teachers and developing effective educational materials should be provided.
