Ramsay Hunt Syndrome 환자군과 Bell's Palsy 환자군에 대한 한방치료 비교연구 (The Comparative Study on Oriental Medicine Treatments of Ramsay Hunt Syndrome and Bell's Palsy)

  • Objective: The purpose of this study is to compare the improvement of Ramsay Hunt Syndrome and Bell's palsy after Oriental Medicine treatment Methods : Twelve patients with Ramsay Hunt Syndrome and twenty patients with Bell's Palsy who visited the Iksan Oriental Medical Hospital of Wonkwang University were evaluated. Evaluation was carried out by facial palsy grading system of Yanagihara. Results : 1. In the analysis of causes. the most common cause was 'none' with 14 cases(43.8%). stress with 8 cases(25.0%). fatigue with 4 cases(12.5%), cold and URI with 3 cases(9.0%) each. 2. In the analysis of prodrome, there was retroauricular pain in 16 cases(50.0%). headache and 'none' in 8 cases(25.0%) each. 3. There was no significant difference between RHS and Bell's palsy in evaluation score. Conclusions : RHS and Bell's palsy showed no statistical significant difference between severity of before treatment and that of after treatment.

구안와사(口眼喎斜)에 대한 체침(體鍼)과 동씨침(董氏鍼)의 효과(效果) 비교(比較) (The Effect of Acupuncture and Dong's Acupuncture about Bell' palsy)

  • Objective: The aim of this research was to investigate the effect of acupuncture and the effect of Dong's acupuncture on Bell's palsy. Methods: We measured the facial palsy changes of the patients who were admitted for Bell's palsy in the Oriental Medical hospital of Dongeui medical center from 07-01-2003 to 09-30-2003. Bell's palsy patients were divided into two groups. One group(A group) was treated by acupuncture. The other group(B group) was treated by Dong's acupuncture. The effects of these treatment was evaluated by Yanagihara's unweighted grading system. Results : B group was marked more higher than A group in treatment outcome. we discovered that it is significant differences between two groups after 2 week. Conclusion : These results provided that A group is more effective than B group on. For clearly comparing the Effect of Acupuncture and Dong's Acupuncture on Bell's palsy, it is need more sample's number and more treatment's duration.

Bell's palsy 환자에서 두면부 경혈의 적외선 체열촬영의 활용 (Study on the Application of DITI in Bell's Palsy Patients with Acupoints)

  • Objective: The aims of this study are to find out proper methods to assess Bell's palsy with DITI and to validate the correlation between DITI of acupoints and H-B scale. Methods: 36 Bell's palsy patients and 10 health people were measured. We checked thermal differences between abnormal and normal acupoints sites measured by using DITI and H-B scale. Results: There was no significant thermal difference between abnormal and normal sites. The H-B scale was significantly correlated with thermal differences of ST6 and TE17. Conclusion: DITI was useful for evaluating the severity of Bell's palsy.

사상방을 활용한 임신 중 구안괘사(口眼喎斜) 치험 4례 (Four Cases of Bell's Palsy Patients during Pregnancy Treated with Sasang Constitutional Medicine)

  • Purpose: This study was aimed to report the effect of oriental treatment on Bell's palsy during pregnancy with Sasang Constitutional method. Methods: 4 patients who had Bell's palsy during pregnancy were treated with Sasang constitutional prescriptions. After treatments we evaluated the effects by House-Brackmann facial nerve grading system. Results: As a result, the clinical symptoms of Bell's palsy during pregnancy were improved. Conclusions: This case studies show that Sasang constitutional method can be effectively used on Bell's palsy during pregnancy. Especially the diagnostic results by using Diagnosis System of Oriental Medicine (DSOM) revealed that some of patients were in a very weak condition. We could apply these results to choose prescription and get good therapeutic effects.

안면신경마비 후유증에 대한 임상적 고찰 (The Clinical Observation of Facial Palsy Sequela)

  • Objectives : In order to obtain the clinical type of facial palsy sequelae and try to make the treatment protocols for each, I observed patients who visited Gunpo-Wonkwang oriental medicine center with Bell’s palsy sequelae that were treated over three months. Methods : I make the value standard of muscle paralysis, contraction, synkinesis and acquired the results as follows. Results and Conclusions : 1. The distribution of age and sex was as follows : females of 41-50 years were the most common demographic, females of 51-60 years and males of 31-40 years were the second, males of 51-60 years were the third, females of 21-30 years and males of over 60 years were the fourth, and males of 41-50 years were the fifth. 2. The distributions of period of disease were as follows : 3-6 months was the most, 12-18 months was the second, 6-12 months and over 24 months was the third, and 18-24 months was the fourth. 3. The sequelae distributions of disease were as follows. In the group of 3-6 months, 12 persons (80%) showed palsy and atrophy, 10 persons (66.6%) showed synkinetics. In the group of over 6 months, all patients showed muscle palsy, muscle atrophy and synkinetics. All groups showed lower sensitivity of muscles, but the group of 18-24 months and the group of over 24 months showed more. Tinnitus was shown by the groups of 12-18 months and 3-6 months. Facial muscle pain was shown by the group of3-6 months only, Crocodile's tear was shown by the groups of 18-24 months and over 24 months. 4. The total palsy rates of sequela patients and palsy rates by muscle for disease period were as follows. The total palsy rate was 27.94%; the palsy rates for the group of 6-12 months and the group of over 24 months was lower than the total palsy rate. The rates of the groups of 3-6, 12-18, 18-24 months were higher than the total palsy rate. The palsy rate of zygomatic minor, levator labii superior muscle was higher than the total palsy rate for all groups. 5. Synkinetics manifestation rates by disease period were as follows. Total synkinetics manifestation rate was 73.81 %; the manifestation rate of the group of 6-12 months was lower than total synkinetics manifestation rate. For the groups of 12-18, 18-24, and over 24 months it was more than the total synkinetics manifestation rate. The group of over 24 months, total synkinetics induced by orbicularis oculi muscle and orbicularis oris muscle. 6. Facial muscle atrophy rates by disease period were as follows. Total atrophy rate was 5.26%; in the groups of 6-12, 18-24, over 24 months, the atrophy rates were higher than the total atrophy rate. The groups of 3-6 and 12-18 months showed lower than the total atrophy rates, while the atrophy of the levator palpebrae superioris muscle and levator palpebrae inferioris muscle was higher than in other groups.

Utilization of Averaging Process of Blink Reflex to Improve Diagnosis of Facial Nerve Palsy

  • The conventional blink reflex has limited clinical application since it displays a wide range of change in the responses. Thus, we studied the blink reflex using the averaging process which is engaged in evoked potential studies in order to measure various responses such as latency, amplitude, and duration, which have been difficult to measure due to their wide ranges of changes. Among the Bell's palsy patient group, 13 patients who had the symptoms of incomplete palsy were examined to assess the results of the blink reflex through the averaging process. The subjects were 54 people in a normal group (108 eyes) and 18 patients (36 eyes) with Bell's palsy. For the study method, the conventional blink reflex and the blink reflex using the averaging process were measured for the people in the normal group, while in the Bell's palsy group, only the blink reflex using the averaging process was analyzed. In the case of the normal group, the blink reflex using the averaging process could measure all of the latency, amplitude, and duration. It was also observed that the latency, amplitude, and duration of R1, ipsilateral R2, and contralateral R2 significantly differed on the affected side of the Bell's palsy patients, compared to the unaffected side. The blink reflex using the averaging process should be more effective than the conventional method since the former can evaluate the latency, amplitude, and duration for Bell's palsy, while the latter can only assess latency.

양측성 안면신경마비 치험 1례에 관한 고찰 (Clinical Observation on 1 Case of Patient with Bilateral Facial Palsy)

  • Facial palsy is commonly encountered disease in the clinic but bilateral facial palsy is known as rare disease. Type of facial nerve paralysis include unilateral, recurrent ipsilateral, recurrent alternating and bilateral simultaneous palsies. Among the types, the reported incidence of bilateral simultaneous palsy is 0.3~2% of facial paralysis patients. We experienced I case of patient with bilateral simultaneous facial palsy that was concluded as bilateral bell's palsy. Objective : The purpose of this paper is to report the patient with bilateral facial palsy, who improved by oriental medical treatment. Another purpose is to review the current literature and to differential diagnosis of bilateral facial paralysis. Methods and Results : The patient was treated by acupuncture, herb medication and self-massage on facial muscle for 14 weeks. House-brackmann grading score was improved into I/I (Rt/Lt) from IV/IV. Conclusion : Through reviewing some literatures and reports, It is concluded that bilateral facial palsy was related to many other disorders and more ominous than unilateral facial palsy. therefore, its work-up should include a complete neurologic assesment and thorough evaluation. also, we consider that bilateral Bell's palsy can improve by oriental medical treatments.

컴퓨터 적외선(赤外線) 전신체열촬영(全身體熱撮影)을 통해 살펴본 Bell' palsy에 대한 한방적 치료의 임상적(臨床的) 고찰(考察) (Clinical evaluation of Acupuncture and Herb-Medication on Bell's palsy by DITI)

  • Object : This study is designed to evaluate effects of acupuncture and herb-medication by the data of DITI(Digital Infrared Thermographic Imaging) examination and the changes of clinical symptoms after the therapy of acupuncture and herb-medication in the patients with Bell's palsy. Contents : The conservative therapy with acupuncture and herb-medication was performed during 1-8weeks. The acupuncture points of S4, S6, G14, S2, BL2, SI18, TE23, LI4 and S36 was used. In the pre- and post therapy, DITI examinations were performed in patients who had Bell's palsy and were treated by acupuncture simultaneously, and then tried to correlate the results of clinical symptoms with the difference of thermographic findings at pre-and post acupuncture. Setting : The standard routine themographic examinations were performed with thermography (DITI) in the 16 patients with Bell's palsy at pre- and post acupuncture. patients : Thermographic imaging of 16cases was analyzed. They had diagnosed Bell's palsy. They were treated by acupuncture and moxibustion therapy in Wonkwang Oriental Hospital from Jauary, 1999 to February, 2000. Results : The results of treatment showes that 56.25% of patients achieved excellent recovery and 31.25% achieved good recovery. After compairing the DITI results before and after treatment, we found 43.75% of patients achieved excellent recovery and 43.75% achieved good recovery. Conclusion :Acupuncture showed good results over 87.5% in clinical evaluation and 87.5% in DITI. Thermographic examination showes terapeutic effect of acupuncture treatment.

TCD를 이용한 말초성(末梢性) 구안괘사환자에 대(對)한 임상적(臨床的) 고찰(考察) (The Clinical Study on Bell's Palsy Patients with TCD Measurement)

  • Objective : This study was carried to make out the connection between cerebral artery blood flow velocity and ischemic theory that presumed the cause of Bell's palsy. Method : We measured cerebral artery blood flow velocity each external carotid artery, internal carotid artery, common carotid artery, siphon, superficial temporal artery by TCD to 20 patients who diagnosed as facial nerve palsy from march 2001 to July 2001 and all objectives devided two groups as palsy side. A group is right side facial nerve palsy and B group is left facial nerve palsy. Results : 1. There is no effective change of blood flow in external carotid artery either A, B group. 2. There is no effective change of blood flow in internal carotid artery either A, B group. 3. There is no effective change of blood flow in common carotid artery either A, B group. 4. There is no effective change of blood flow in siphon artery either A, B group. 5. There is no effective change of blood flow in superficial temporal artery either A, B group.
