파킨슨병 환자의 골밀도 영향요인 (Factors Affecting Bone Mineral Density of the Femur in Parkinson's Disease Patients)

  • Purpose: The purpose of this study is to explore the level of bone mineral densities of the femur neck and to identify factors affecting bone mineral density of the femur in Parkinson's disease (PD) patients. Methods: Participants were 121 PD patients visiting the outpatient clinic of D University Hospital in B City. Bone mineral density was measured at the femur neck by using a dual-energy x-ray absorptiometry. The serum vitamin D level, the amounts of milk intake, caffeine intake, cigaret smoking, and alcohol consumption, and the number of steps taken daily were measured. Past and present disease history, the medication history, and duration of the disease were also collected. The level of disability was obtained by neurologists using the Hoehn and Yahr stage and the Unified Parkinson's Disease Rating Scale. Results: Among PD patients, 57% had osteopenia and 15.7% had osteoporosis. The bone mineral density of the femur was associated with weight, serum vitamin D level, age, and working status. Conclusion: The identification of weight and serum vitamin D level as important predictive factors emphasizes the importance of balanced nutrition for PD patients.

고유수용성신경근촉진법을 이용한 체간 운동과 트레드밀을 결합한 훈련이 파킨슨병 환자의 균형과 보행 능력에 미치는 영향 (Effects of the Trunk Exercise Using PNF Combined with Treadmill on Balance and Walking Ability in Individuals with Parkinson's Disease)

  • Purpose: This study was to explore the effects of trunk exercise using PNF combined with treadmill training on balance and walking ability in patients with Parkinson's disease. Methods: This study included 16 patients with Parkinson's disease. Participants were randomly assigned to 2 groups: an experimental group (n=8) and a control group (n=8). All participants underwent treadmill training for 30 minutes. In addition, the experimental group (trunk exercise using PNF) and control group (conventional training) participated in a 30-minute exercise program. Both groups performed the training 5 times per week for 4 weeks. Disease severity (determined using the unified Parkinson's disease rating scale motor subscale, UPDRS-3), balance (determined using the Berg balance scale, BBS), walking speed (determined using the 10-meter walking test, 10MWT), and walking endurance (determined using the 6-minute walking test, 6MWT) were measured at baseline and after 4 weeks. Results: Pre- to post-intervention improvement was noted on all outcome measures for both groups (p<0.05). Post-intervention, there was a significant improvement in the experimental group as compared to the control group for the following measured outcomes (p<0.05): UPDRS-3 (p=0.03; 95% CI, -5.52 to -0.24), BBS (p=0.04; 95% CI, 0.59 to 6.45), 10MWT (p=0.01; 95% CI, -2.19 to -0.42), and 6MWT (p=0.04; 95% CI, 1.81 to 96.72) Conclusion: The results of this study revealed that trunk exercise using PNF plus treadmill training improves balance and walking ability as compared to conventional training plus treadmill training in patients with Parkinson's disease.

알츠하이머형 치매 및 경도인지장애 환자에서 인지기능 및 행동심리증상과 내측두엽 위축의 연관성 (Association between Cognitive function, Behavioral and Psychological Symptoms of Dementia and Temporal Lobe Atrophy in Patients with Alzheimer's Disease and Mild Cognitive Impairment)

  • 연구목적 본 연구의 목적은 알츠하이머형 치매 및 경도인지장애 환자에서 내측두엽 위축 정도의 차이에 따른 중증도, 신경인지기능 및 행동심리증상을 비교하는 것이다. 방 법 본 연구는 단면 연구로서, 알츠하이머형 치매 및 경도인지장애로 진단받은 114명을 대상으로 하였다. 내측두엽 위축은 표준화된 시각 기반 척도(Scheltens scales)에 의해 평가되었으며, 대상군은 두 그룹으로 분류되었다. 중증도는 임상치매평가척도(Clinical dementia rating, CDR)와 전반적 퇴화 척도(Global deterioration scale, GDS)로 평가되었으며 신경인지기능은 한국어판 간이 블레스드 검사(The Korean version of the short blessed test, SBT-K), 한국형 CERAD (The Korean version of the consortium to establish a registry for Alzheimer's disease, CERAD-K), 행동심리증상은 한국형 신경정신행동검사(The Korean version of the Neuropsychiatric Inventory, K-NPI)로 평가되었다. 내측두엽 위축의 정도에 따른 중증도와 신경인지기능 및 행동심리증상의 차이를 분석하기 위해 독립표본 t-test를 시행하였다. 결 과 내측두엽 위축의 중증도가 높은 군은 낮은 군에 비하여 CDR, SBT-K, 그리고 CERAD-K의 하부항목 중 네가지 항목에서 유의하게 낮은 점수를 보였다(p<0.05). 결 론 내측두엽 위축과 중증도 및 신경인지기능 사이에는 유의한 양의 상관관계가 나타났으나 행동심리증상과는 유의한 상관관계가 나타나지 않았다. 내측두엽 위축의 평가는 인지장애의 진단과 치료에 잠재적 이점을 제공할 것으로 생각된다. 행동심리증상과 뇌구조의 연관성에 대해서는 추가적인 연구가 필요할 것으로 보인다.

파킨슨병 환자에게 Lee Silverman Voice Treatment-BIG(LSVT-BIG) 프로그램의 적용이 운동기능과 삶의 질에 미치는 효과 (Application of Lee Silverman Voice Treatment-BIG(LSVT-BIG) Intervention to Improve Motor Functions and Quality of Life in People With Parkinson Disease)

  • 목적: 파킨슨병 환자를 위해 개발된 표준화된 운동프로그램과 작업기반의 활동으로 구성된 Lee Silverman Voice Treatment (LSVT) BIG 치료 프로그램을 지역사회에 거주하는 파킨슨병 환자에게 제공한 후 파킨슨병 환자의 운동기능과 삶의 질에 어떠한 영향을 미치는 지를 알아보고자 하였다. 연구방법: 본 연구는 단일군 전후실험설계(one-group pretest and post test design)를 사용하였다. 치료 전 후에는 통합형 파킨슨병 평가척도(Unified Parkinson's Disease Rating Scale-III: UPDRS-III), Time up and go(TUG), 파킨슨 질병 검사지(Parkinson's Disease Questionnaire-39: PDQ-39)을 사용하여 운동기능 및 삶의 질의 변화를 측정하였다. LSVT-BIG치료는 프로토콜에 의거하여 매일 1회 주4일 60분간 실시하여 대상자별로 총 16회기를 실시하였다. 결과분석은 그래프를 통한 시각적 분석과 치료 전 후 측정값의 변화를 비교하였다. 결과: 치료 전 후로 UPDRS, TUG에서 모든 대상자들의 수행능력이 향상되었다. 또한, PDQ-39의 감소로 치료 후 모든 대상자들의 삶의 질 또한 긍정적으로 향상되었다. 결론: 지역사회 내에서 거주하는 파킨슨병 환자에게 LSVT-BIG 치료의 적용은 파킨슨환자의 운동기능 증진 및 삶의 질 증진에 긍정적인 영향을 주었다.

레보도파제제의 위장관계 부작용에 대한 침치료 효과 연구-특발성 파킨슨병 환자를 대상으로- (Clinical Study of Acupuncture Theraphy of Gastrointestinal Side Effect of Levodopa: Focused on Idiopathic Pakinson's Disease Patients)

  • Objective : This study was aimed at investigating the therapeutic effects of acupuncture on gastrointestinal side effect of Levodopa on idiopathic Pakinson's disease patients. Methods : The subjects of this study were 42 patients with idiopathic Parkinson's disease. We divided them into two groups; acupuncture treatment group, no treatment group. We treated the former group with acupuncture therapy focusing on gastrointestinal side effect of Levodopa while administering Levodopa as well. And the latter group was also dosed up with Levodopa without acupuncture therapy. To see the effect of acupuncture treatment clearly, we used gastrointestinal syndrome rating scale (GSRS) and visual analog scale (VAS) and compared the GSRS grade and VAS score of two groups statistically, after 1, 2, 3, 4 weeks since they have been under the treatment. Results : This study suggests that the group who has been treated with acupuncture on gastrointestinal side effect of Levodopa on idiopathic pakinson's disease patients showed higher GSRS grade and VAS score than the one that has not. But, We could't find statistical significance between the two groups on improvement of GSRS grade and VAS score. Conclusions : These results proved that acupucture theraphy might be available for re lieving symptoms related with gastrointestinal side effect of Levodopa than the one that has not. But further studies are necessary.

한방치료로 호전된 진전 치험 1례 (A Case of Tremor Patient Treated by Traditional Korean Medicine)

  • ■ Objectives The purpose of this clinical study is to evaluate the effect of Traditional Korean Medicine(TKM) on a patient with tremor. ■ Methods A patient with tremor of one side hand diagnosed with Parkinson's disease was treated with herbal medication, acupuncture, electro-acupuncture(EA), and herbal medical injection. Then we evaluated the improvement by Unified Parkinson's Disease Rating Scale(UPDRS), Parkinson's disease Quality of Life(PDQL) and Visual Analog Scale(VAS). ■ Results Decrease of UPDRS, VAS score and improvement of PDQL score were observed after the TKM treatment. ■ Conclusion This study proved the effect of TKM treatment on tremor due to Parkinson's disease.

Two-Step Incision for Periarterial Sympathectomy of the Hand

  • Background Surgical scars on the palmar surface of the hand may lead to functional and also aesthetic and psychological consequences. The objective of this study was to introduce a new incision technique for periarterial sympathectomy of the hand and to compare the results of the new two-step incision technique with those of a Koman incision by using an objective questionnaire. Methods A total of 40 patients (17 men and 23 women) with intractable Raynaud's disease or syndrome underwent surgery in our hospital, conducted by a single surgeon, between January 2008 and January 2013. Patients who had undergone extended sympathectomy or vessel graft were excluded. Clinical evaluation of postoperative scars was performed in both groups one year after surgery using the patient and observer scar assessment scale (POSAS) and the Wake Forest University rating scale. Results The total patient score was 8.59 (range, 6-15) in the two-step incision group and 9.62 (range, 7-18) in the Koman incision group. A significant difference was found between the groups in the total PS score (P-value=0.034) but not in the total observer score. Our analysis found no significant difference in preoperative and postoperative Wake Forest University rating scale scores between the two-step and Koman incision groups. The time required for recovery prior to returning to work after surgery was shorter in the two-step incision group, with a mean of 29.48 days in the two-step incision group and 34.15 days in the Koman incision group (P=0.03). Conclusions Compared to the Koman incision, the new two-step incision technique provides better aesthetic results, similar symptom improvement, and a reduction in the recovery time required before returning to work. Furthermore, this incision allows the surgeon to access a wide surgical field and a sufficient exposure of anatomical structures.

파킨슨병의 한약 치료에 대한 최신 임상연구 동향 고찰 - 2010년부터 2014년까지 중국 논문을 중심으로 - (Review of Current Clinical Studies for Herbal Medicine of Parkinson’s Disease in Traditional Chinese Medicine)

  • The objective of this study was to review the current clinical studies about the effect of herbal medicine for Parkinson's disease in China over the last 5 years and then to suggest the foundation for treatment and further studies. We had searched for studies in China National Knowledge Infrastructure(CNKI, http://www.cnki.net) and PubMed from January 2010 to December 2014. Key words were the various combinations of '帕金森', '湯', '丸', '散', '中醫藥', 'Parkinson's disease', and 'herbal medicine'. Total 53 clinical studies were selected and analyzed. The most frequently used diagnostic criteria of Parkinson's disease was the Unified Parkinson's Disease Rating Scale(UPDRS). The most frequently used medical herb was Paeoniae Radix alba(白芍藥) more 30 times and the highest amount was Astragali radix(黃芪) 100g per day. The most frequent syndrome differentiation was liver kidney yin deficiency(肝腎陰 虛). We found out there are many clinical studies of herbal medicine for Parkinson's disease in China. These studies would be able to provide the basis of clinical research on Parkinson's disease, and also applied to the treatment of Parkinson's disease in Korea.

Charcot-Marie-Tooth Disease 환자에 대한 한방치료 1례 (Clinical Study on the Case of Charcot-Marie-Tooth Disease Treated with Korean Medicine: A Case Report)

  • Objectives : The purpose of this study is to report the effect of Korean Medicine treatment on a patient with Charcot-Marie-Tooth disease. Methods : A 60-year-old woman who was diagnosed as Charcot-Marie-Tooth disease was admitted with both lower and upper limbs weakness, difficulty in walking, palm pain and neck pain. The patient was treated with acupuncture, electroacupuncture, herbal medicine, cupping therapy and physical treatment from 2th September 2013 to 14th November 2013. Improvement of the patient's symptoms was evaluated by numeric rating scale(NRS), visual analog scale(VAS), SF-36 bodily pain and patient global assessment(PGA). Results : After treatment, pain Intensity evaluated by VAS was significantly decreased(from 7.2 to 2). Increased SF-36 bodily pain score(from 10 to 67.5) showed that patient's quality of life has been improved. Conclusions : These results suggest that Korean medicine treatment may be effective in reducing the symptoms of Charcot-Marie-Tooth disease.

5-HT 수용체 길항제를 이용한 파킨슨 환자의 정신 증상의 치료 (Psychosis in Parkinson's Disease-Treatment with 5-HT Receptor Antagonist)

  • Current treatment strategies for levodopa-induced psychosis in advanced Parkinson's disease have had limited success. Reduction or discontinuation of levodopa and coadministration with dopamine-blocking neuroleptics may attenuate the psychotic symptoms, but these strategies are associated with worsening of parkinsonian symptoms. Administration of 5-HT3 receptor antagonist ; ondansetron, a newer strategy to attenuate psychosis of Parkinson'disease without motor deterioration was introduced. A 41-year-old young-onset male, who was diagnosed as Parkinson's disease 7 years ago, was treated with levodopa therapy, and had levodopa-induced psychosis(delusion, hallucination, paranoid, insomnia). After trial of ondansetron, he showed improvement in the Brief Psychiatric Rating Scale(from 21 points to 9 points) in spite of increasing the dosage of levodopa. With ondansetron, we could increase the dosage of levodopa without psychotic complications(esp, hallucination), and he showed improvement in the motor fluctuation.
