Long-Term Changes in the Distal Aorta after Aortic Arch Replacement in Acute DeBakey Type I Aortic Dissection

  • Background: We analyzed the long-term results of ascending aortic replacement and arch aortic replacement in acute DeBakey type I aortic dissections to measure the differences in the distal aortic changes with extension of the aortic replacement. Methods: We reviewed 142 cases of acute DeBakey type I aortic dissections (1996-2015). Seventy percent of the cases were ascending aortic replacements, and 3 0% of the cases underwent total arch aortic replacement, which includes the aorta from the root to the beginning of the descending aorta with the 3 arch branches. Fourteen percent (20 cases) resulted in surgical mortality and 86% of cases that survived had a mean follow-up period of $6.6{\pm}4.6years$. Among these cases, 64% of the patients were followed up with computed tomography (CT) angiograms with the duration of the final CT check period of $4.9{\pm}2.9years$. Results: There were 15 cases of reoperation in 13 patients. Of these 15 cases, 13 cases were in the ascending aortic replacement group and 2 cases were in the total arch aortic replacement group. Late mortality occurred in 13 cases; 10 cases were in the ascending aortic replacement group and 3 cases were in the total arch aortic replacement group. Eight patients died of a distal aortic problem in the ascending aortic replacement group, and 1 patient died of distal aortic rupture in the total arch aortic replacement group. The follow-up CT angiogram showed that 69.8% of the ascending aortic replacement group and 35.7% of the total arch aortic replacement group developed distal aortic dilatation (p=0.0022). Conclusion: The total arch aortic replacement procedure developed fewer distal remnant aortic problems from dilatation than the ascending aortic replacement procedure in acute type I aortic dissections.

Short Term and Midterm Surgical Results for Infective Endocarditis - Does Wide Debridement and Reconstruction Affect the Post Operative Mortality and Morbidity? (90년대 중반 이후 시행한 감염성 심내막염의 중단기 수술 성적)

  • Background: We present here the early and midterm surgical results for infective endocarditis and we especially focus on the effect of aggressive reconstruction or root implantation after wide debridement. Material and Method: Between January 1995 and Jun 2006, we enrolled 79 adult infective endocarditis patients who underwent surgical treatment. There were 63 and 16 native and prosthetic valve endocarditis cases, respectively. They included 27 cases of culture negative endocarditis. With performing valve replacement or repair, 28 of the patient underwent a more aggressive surgical option, for example, aortic root replacement or reconstruction, or heart base reconstruction etc. Result: There were statistical relationships between the in-hospital mortality and staphylococcal infection, urgent-based operation and operation during the active phase of endocarditis. Wide debridement and aggressive reconstruction were not related to either the post operative mortality or the early morbidity. Culture negative endocarditis was not related to the postoperative mortality and morbidity. Conclusion: Physicians must pay attention to patients' medical treatment during the preoperative period of the infective endocarditis. If surgery is considered for treating infective endocarditis, it should be performed before the downhill course of the disease so that the surgical outcome is improved. Wide debridement and more aggressive reconstruction are also warranted.

Determination of Growth, Yield and Carbohydrate Content of Allium hookeri Grown under Shading Treatment (차광처리에 의한 삼채의 생육, 수량 및 당함량 변화)

  • Background: This study was aimed at evaluating the growth, yield, and carbohydrate content in the whole Allium hookeri plant with shading treatment in hot summer. Methods and Results: Different shading rate, including 0 (control), 35 or 55%, was employed from the June $21^{st}$ to August $31^{st}$. Daily average air and soil temperature, which were approximately $2.5^{\circ}C$ and $3.8^{\circ}C$ lower, respectively, were observed with both 35% and 55% treatments in July and August, with no significant difference in daily maximum air temperature. Dry weights were high, approximately 40% and 48% for the shoot and 20% and 12% for the root, with the 35% and 55% treatments, respectively, 8 weeks after shading. Division number was increased by 13% and 19.8% with the 35% and 55% treatments, respectively. The mortality rates of 150 plants were 9.1%, 4.0%, and 1.3% with the 0 (control), 35% and 55% treatments, respectively. At 4 weeks after shading, the highest and lowest sucrose levels in both shoot and root were observed with the 35% and 55% treatments, respectively. At 8 weeks after shading, there was no significant difference in the sucrose content in the shoot among the treatments. Conclusions: The highest plant growth rate and yield with the 55% treatment may be related with the decrease in both air and soil temperatures, resulting in reducted leaf respiration and thus compensate net photosynthesis.

Composite valve graft Replacement of the Aortic Root (Composite valve graft를 이용한 대동맥근부 치환술)

  • This study was undertaken to analyze the outcome of composite valve graftreplacement(CVGR) for the treatment of aneurysms of the ascending aorta involving the aortic root. Material and Method: Between April 1995 and June 2001, 56 patients had replacement of the ascending aorta and aortic root with a composite graft valve and were reviewed retrospectively. Aortic regurgitation was present in 50 patients(89%), Marfan's syndrome in 18 patients(32%), and bicuspid aortic valve in 7(12.5%). The indications for operation were annuloaortic ectasia(AAE) in 30 patients(53.6%), aortic dissection in 13(23.2%), aneurysms of the ascending aorta involving aortic root in 11(19.6%), and aortitis in 2(3.6%). Cardiogenic shock due to the aortic rupture was present in 2 patients. Nine patients(16%) had previous operations on the ascending aorta or open heart surgery. The operative techniques used for CVGR were the aortic button technique in 51 patients(91%), the modified Cabrol technique in 4, and the classic Bentall technique in 1. The concomitant procedures were aortic arch replacement in 24 patients(43%), coronary artery bypass graft in 8(14.3%), mitral valve repair in 2, redo mitral valve replacement in 1, and the others in 7 The mean time of circulatory arrest, total bypass, and aortic crossclamp were 21$\pm$14 minutes, 186$\pm$68 minutes, and 132$\pm$42 minutes, respectively. Result: Early mortality was 1.8%(1/56). The postoperative complications were left ventricular dysfunction in 16 patients(28.6%), reoperation for bleeding in 7(12.5%), pericardial effusion in 2, and the others in 7. Fifty-three patients out of 55 hospital survivors were followed up for a mean of 23.2 $\pm$ 18.7 months(1-75 months). There were two late deaths(3.8%) including one death due to the traumatic cerebral hemorrhage, and CVGR-related late mortality was 1.9%. The 1- and 6-year actuarial survival was 98.1$\pm$1.9% and 93.2$\pm$5.1%, respectively. Two patients required reoperation for complication of CYGR(3.8%) and two other patients required subsequent operations for dissection of the remaining thoracoabdominal aorta. The 1- and 6-year actuarial freedom from reoperation was 97.8$\pm$2.0% and 65.3$\pm$26.7%, respectively.

Recent Early Operative Outcomes of Comprehensive Aortic Root & Valve Reconstruction (CARVAR) Procedure (종합적 대동맥 근부 및 판막 재건술의 최근 초기 수술성적)

  • Background: A Comprehensive Aortic Root and Valve Reconstruction (CARVAR) procedure is comprised of aortic root wall reconstruction and corrections of the leaflets for treating various aortic valve diseases. We evaluated our recent early clinical experience with the CARVAR procedure. Material and Method: From October 2007 to September 2008, 114 cases (66 males) of CARVAR procedures were performed, The mean patient age was 53 years (range: 14~84) The patients were divided into 4 groups: 1) the AAR group: aortic regurgitation with aortic root wall deformity such as annulo-aortic ectasia or ascending aortic aneurysm (n=18), 2) the IAR group: isolated AR with leaflet abnormality (n=42), 3) the IAS group: isolated aortic stenosis (n=51) and 4) the PAVR group: previous aortic valve replacement (n=3). Sinotubular junction (STJ) reduction was done in all the patients, leaflet correction was done in 10 of the AAR group patients and in all the patients of the other groups, annulus reduction was done in 14 of the MR group patients and in 6 of the IAR group patients. Aortic dissection was excluded from this analysis. Result: There was no mortality or follow-up death. The diameter of the aortic sinus decreased from $54.6{\pm}8.4$ mm to $38.3{\pm}3.8$ mm in the AAR group, the mean AR grade decreased from 3.2 to 0.2 in the IAR group, the mean aortic valve pressure gradient decreased from $47.1{\pm}24.4$ mmHg to $15.1{\pm}11.7$ mmHg in the IAS group and the mean AR grade decreased to 0 in the PAVR group. Balloon type coronary perfusion cannula-related coronary ostial stenosis developed in 4 patients and this was treated with OPCAB in three patients and with PTCA in one patient. Two patients developed postoperative infectious endocarditis. All the patients were discharged and followed up in a stable condition. Conclusion: The CARVAR procedure showed excellent short term results, but a good further follow up result is required to apply this procedure to most kinds of aortic valve diseases.

Effect of a Bacterial Grass Culture on the Plant Growth and Disease Control in Tomato

  • This study aimed to investigate the plant growth-promoting and biocontrol potential of a grass culture with Paenibacillus ehimensis KWN8 on tomato. For this experiment, treatments of a chemical fertilizer (F), a bacterial grass culture (G), a 1/3 volume of G plus 2/3 F (GF), and F plus a synthetic fungicide (FSf) were applied to tomato leaves and roots. The result showed that the severity of Alternaria solani and Botrytis cinerea symptoms were significantly reduced after the application of the bacterial grass culture (G and GF) and FSf. In addition, root mortality in G and GF was lower compared to F. Tomato plants treated with G or GF had better vegetative growth and yield compared to F. Application of G affected the fungal and bacterial populations in the soil. In conclusion, treatment with a bacterial grass culture decreased disease severity and increased tomato growth parameters. However, there were no statistically significant correlations between disease occurrence and tomato yields. This experiment presents the possibility to manage diseases of tomato in an environmentally friendly manner and to also increase the yield of tomato by using a grass culture broth containing P. ehimensis KWN38.

A study on efficacy and safety of Bio-Gammamix by oral administratiion to cultured fish (양식어류에 있어서 생약제제(바이오감마믹스)의 경구투여에 의한 효능 및 안전성에 관한 연구)

  • A study on efficacy and safety of Bio-Gammamix was performed to use for the preventive drug against bacterial diseases to mirror carp(Cyprinus carpio), rainbow trout(Salmo gairdneri), eel(Anguilla japonica) and flounder(Paralichthys olivaceus). The main ingredients of Bio-Gammamix were ferrous fumarate and mixed extrated powders of licorice root, ox bile and garlic. The test drug was proved to improve feeding habit and feed efficiency and decrease cumulative mortality remarkably. After administration, no side effects and histopathological changes were observed. We can prevent outbreaks of bacterial diseases of fish by the dosage of 1kg/ton of fish body weight perorally once or two times in a day for 7 days and Bio-Gammamix is thought to be used effectively an widely against most bacterial fish pathogens.

Effects of Light Intensity on the Growth Characteristics and Net Photosynthesis of Piper kadzura Native to Korea for Indoor Plants (실내식물 개발을 위한 광조건이 자생 후추등의 생육과 광합성에 미치는 영향)

  • This study was carried out to investigate the effects of light intensities on the growth and net photosynthesis of Piper kauzura under different shading levels : 0%, 50%, 70% and 90% of sunlight. Mortality rate was lowest under a 70% shading level but 0% and 90% shading levels were about 46% and 53% each respectively. Plant height was shorter and leaf size was smaller and yellowish under a 0% shading level but increased when light intensity was decreased. However, under a 90% shading level, growth of Piper kauzura was inferior to other treatments. Top fresh weight was about 11.24g under a 50% shading level and about two times higher than that observed in about 6.6g under a 90% shading level. Root fresh weight was about 7.7g under a 0% shading level and was about two times higher than that showed in about 3.84g and 3.64g under 90% and 70% respectively. Total chlorophyll content and chlorophyll a/b rate were increased when light intensity was decreased. Net Photosynthesis achieved the highest under a 70% shading level and maximum photosynthetic photon flux density was 150 molㆍm/sup -2/ㆍs/sup -1/. Therefore, growth of Piper kauzura was good under 50∼70% shading, Meaning that it is an indoor plant that could be highly utilized.

Single Oral Dose Toxicity Test of Platycodin D, a Saponin from Platycodin Radix in Mice

  • The object of this study was to evaluate the single oral dose toxicity of platycodin D, a saponin from the root of Platycodon grandiflorum in male and female mice. Platycodin D was administered to female and male mice as an oral dose of 2000, 1000, 500, 250 and 125 mg/kg (body wt.). Animals were monitored for the mortality and changes in body weight, clinical signs and gross observation during 14 days after treatment, upon necropsy, organ weight and histopathology of 14 principle organs were examined. As the results, no platycodin D treatment related mortalities, clinical signs, changes on the body and organ weights, gross and histopathological observations against 14 principle organs were detected up to 2000 mg/kg in both female and male mice. Therefore, $LD_{50}$ (50% lethal dose) and approximate LD of playtcodin D after single oral treatment in female and male mice were considered over 2000 mg/kg - the limited dosages recommended by KFDA Guidelines [2009-116, 2009], respectively.

Biosafety and Toxicological Evaluation of Tissue-Cultured Echinacea purpurea Adventitious Roots

  • Echinacea purpurea (L.) Moench (purple cone flower) is an important medicinal plant; it can enhance immunity, relieve pain, and reduce inflammation, and also has hormonal, antiviral, and antioxidant effects. Adventitious root biomass of Echinacea purpurea was produced in commercial-scale bioreactors for use as a dietary supplement in the food industry and in traditional medicine. Biosafety and toxicological evaluations of tissue-cultured Echinacea purpurea adventitious roots (TCEPARs) were performed. Reverse mutation and chromosomal aberration tests showed no significant mutagenicity. Furthermore, repeated four-week oral dose tests performed in Sprague-Dawley rats did not show any notable changes in the general behavior of the rats, in the gross appearance of their internal organs, or in their mortality rate. There were no differences between the control group and the treatment group in parameters such as absolute body weight, hematology, blood chemistry, and absolute and relative organ weights. These findings indicate that TCEPARs are safe and nontoxic when consumed at an average dietary level and can be used as raw material for traditional medicine and the food industry.
