Meningeal Solitary Fibrous Tumor

  • We report a rare case of a patient with meningeal solitary fibrous tumor. A 60-year-old woman presented with right leg monoparesis. Brain magnetic resonance imaging demonstrates a well enhancing huge mass, located in left parietal lobe. Cerebral angiography demonstrating increased vascularity in area of the tumor, which had feeder vessels extending from the internal carotid artery and external carotid artery. A presumptive diagnosis of meningioma or hemangiopericytoma was considered. At surgery, the consistency was firm and had destroyed the dura and skull. A gross total resection was performed. Immunohistochemically, tumor was strongly, and widely, positive for CD34 and vimentin. There was no staining for epithelial membrane antigen(EMA), S-100 protein, cytokeratin, and glial fibrillary acidic protein (GFAP). Differential diagnosis of intracranial solitary fibrous tumor includes fibroblastic meningioma, meningeal hemangiopericytoma, neurofibroma, and schwannoma.

Low Grade Myofibroblastic Sarcoma Occurred in the Scalp

  • Low-grade myofibroblastic sarcoma (LGMS) is a rare sarcoma with myofibroblastic differentiation. LGMS has a propensity for local recurrence and is associated with a low risk of metastatic spread. A 26-year-old man presented with a 12-month history of a slow growing palpable hard mass in the right parietal scalp. Enhanced CT scan of head showed a $3{\times}4cm$ sized well-defined and heterogeneously enhancing scalp mass. The patient underwent excision of the tumor. The histological and immunohistochemical features were consistent with a LGMS. We performed re-operation for remnant tumor removal after diagnosis. After 14 months of surgery, the patient was well-being state.

The Effect of Oxygen Administration on Cerebrum Lateralization in Verbal Task (언어 과제 수행 시 산소 공급이 대뇌 편측화에 미치는 영향)

  • The present study attempted to observe what changes the supply of highly concentrated(30%) oxygen cause to people's ability and cerebrum lateralization of verbal cognition, compared to air of normal oxygen concentration(21%). The experiment consisted of two runs, one for verbal cognition test with normal air(21% of oxygen) and for verbal cognition test with more oxygen in the air(30% of oxygen). Functional brain images were taken form 3T MRI using the single-shot EPI method. There were more activations observed at the occipital, parietal, temporal, and frontal lobes, but there were no changes in cerebrum lateralization with 30% oxygen administration. The result of task performance showed the accuracy increased at 30%'s concentration of oxygen rather than 21%'s. It is concluded that the positive effect on the verbal cognitive performance level by the highly concentrated oxygen administration was due to changeless increase of left and right cerebrum activation.

Progressively Enlarged Intracerebral Ependymal Cyst Presenting with Movement Disorder

  • Ependymal cysts are neuroepithelial cysts, typically found in the central white matter of the temporoparietal and frontal lobes. Clinical symptoms usually result from neurological deficits referable to these regions, from seizures, and chronic headaches associated with increased intracranial pressure. We describe here a case of ependymal cyst on the right fronto-parietal lobe, presenting with tremor and weakness. The cyst was resected surgically and presurgical neurological abnormalities were improved. An exploratory surgery with establishment of an adequate route of drainage and histological examination of the cyst wall are mandatory in the management of patients with a progressive and symptomatic intraparenchymal cyst.

Rapidly Enlarging Pediatric Cortical Ependymoma

  • We report a 10-year-old boy with supratentorial cortical ependymoma that rapidly grew in the course of 3 years. He suffered generalized seizures when he was 5 years old; MRI showed a small cortical lesion in the right postcentral gyrus. MRI performed 2 years later revealed no changes. For the next 3 years he was free of seizures. However, at the age of 10 he again suffered generalized seizures and MRI disclosed a large parietal tumor. It was resected totally and he remains free of neurological deficits. The histopathological diagnosis was ependymoma. Pediatric supratentorial cortical ependymomas are extremely rare. We recommend including cortical ependymoma as a differential diagnosis in pediatric patients with cortical mass lesions presenting with seizures and careful follow-up even in the absence of symptoms because these tumors may progress.

Burr Hole Drainage : Could Be Another Treatment Option for Cerebrospinal Fluid Leakage after Unidentified Dural Tear during Spinal Surgery?

  • Authors report a rare case of acute intracranial subdural and intraventricular hemorrhage that were caused by intracranial hypotension resulted from cerebrospinal fluid leakage through an unidentified dural tear site during spinal surgery. The initial brain computed tomography image showed acute hemorrhages combined with preexisting asymptomatic chronic subdural hemorrhage. One burr hole was made over the right parietal skull to drain intracranial hemorrhages and subsequent drainage of cerebrospinal fluid induced by closure of the durotomy site. Among various methods to treat cerebrospinal fluid leakage through unidentified dural injury site, primary repair and spinal subarachnoid drainage are well known treatment options. The brain imaging study to diagnose intracranial hemorrhage should be taken before selecting the treatment method, especially for spinal subarachnoid drainage. Similar mechanism to its spinal counterpart, cranial cerebrospinal fluid drainage has not been mentioned in previous article and could be another treatment option to seal off an unidentified dural tear in particular case of drainage of intracranial hemorrhage is needed.

Brain Activation Associated With Verbal Fluency Tasks : A fMRI study (단어 유창성 과제 수행에 동반된 뇌활성화 양상)

  • 정상 피험자에 있어서 수행 성적의 차이를 보이는 음소적 단어 생성과 의미적 단어생성에 관련한 대뇌 피질의 활동양상의 차이를 밝히기 위해 피험자에게 단어 생성의 과제를 주면서 기능적 자기공명 영상술(functional magnetic resonance imaging, fMRI)을 시행하였다. 모두 7명의 정상인에서 음소 단서에 의한 단어생성은 의미 범주 단어에 의한 단어생성보다 광범위한 피질 영역의 활성화를 보였는데, 특히 bilateral posteroinferior temporal cortices, left premotor cortex, right cerebellum, bilateral superior parietal lobules에서 더 높은 활성화를 보였다. 이에 반하여 의미적 단어생성은 주로 bilateral posterior cingulate gyri에서 더 높은 활성화를 보였다. 이런 결과는 음소적 생성은 가능한 자모를 조합하여 어휘를 생성한 후 작업기억에 일시 저장시켜 놓고 verbal rehearsal 과정을 사용하여 단어 여부를 판별하는 것을 시사하며, 의미적 생성은 mental imagery를 통하여 시각적 인출 단서를 찾은 후, 의미 기억으로부터 단어를 직접 인출하는 것으로 보인다.

Spontaneous Epidural Hematoma from Skull Base Metastasis of Hepatocellular Carcinoma

  • We report a case of an acute spontaneous epidural hematoma (EDH) due to skull base metastasis in a 46-year-old male patient with hepatocellular carcinoma (HCC). The patient presented with the acute onset of severe headache followed by unconsciousness, and computed tomography showed a large EDH in the right temporal and parietal lobes with midline shift. Emergency evacuation of the EDH was performed, and the hemorrhage was determined to be secondary to skull base metastasis of HCC.

  • An eipdural hematoma in an infant is a very rare entity. We report a case of an acute traumatic intracranial epidural hematoma that developed with a lucid interval in a 4-month-old infant after a fall down from a bed. The infant was admitted at the emergency room. The child had initially cried and may have had a decreased level of consciouseness due to brain injury, but then returned to normal level of consciousness for several hours prior to admission. However, the infant had vomited twice after taking milk and then was lethargic. The brain CT revealed a lentiform-shaped huge hematoma on the right parietal area with a midline shift of 8 mm. An osteoplastic craniotomy was performed, and the intracranial epidural hematoma was totally removed. Postoperatively, the infant recovered well and was dischaged.

Effects of Auditory Stimuli Using Pop Music on EEG (대중음악이 뇌파에 미치는 영향)

  • The purpose of this study is to analyze the effects of the auditory stimulation using pop music on EEG. The subjects in the study were eight healthy university students; five men and three women, age group of 22-28 years old. EEGs were measured for one minute at 19 channels, according to the international 10-20 system method, after both pop musics which the subject likes and not were provided with 100% and 50% volume of standard value, respectively, and the ratio of $\beta$/$\alpha$ was obtained. As a result, when the subjects heard their favorite music, the values of $\beta$/$\alpha$ ratio at the Right-Parietal.Temporal.Occipital lob(T6, O2) showed to be lower than stable state in the situation with 50% volume of standard value.
