ON INTUITIONISTIC FUZZY R-SUBGROUPS OF NEAR-RINGS

  • The notion of normal intuitionistic fuzzy R-subgroups in near-rings is introduced, and related properties are investigated. Characterization of an intuitionistic fuzzy R-subgroup is given. Using a collection of right R-subgroups, an intuitionistic fuzzy right R-subgroup is established. Using a chain of right R-subgroups, an intuitionistic fuzzy right R-subgroup is also established.

  • In this paper, we begin with some basic concepts of substructures of near-rings, and then using some right substructures of near-rings, we may define the right semidirect sum of near-rings. Next, we investigate that every near-ring can be decomposed with right semidirect sum of right ideal by right R-subgroup, and then give some examples.

SOME PROPERTIES OF (m, n)-POTENT CONDITIONS

  • In this paper, we will consider the notions of (m, n)-potent conditions in near-rings, in particular, a near-ring R with left bipotent or right bipotent condition. We will derive some properties of near-rings with (1, n) and (n, 1)-potent conditions where n is a positive integer, and then some properties of near-rings with (m, n)-potent conditions. Also, we may discuss the behavior of R-subgroups in (1, n)-potent or (n, 1)-potent near-rings..

Right Ventricular Mass Quantification Using Cardiac CT and a Semiautomatic Three-Dimensional Hybrid Segmentation Approach: A Pilot Study

  • Objective: To evaluate the technical applicability of a semiautomatic three-dimensional (3D) hybrid CT segmentation method for the quantification of right ventricular mass in patients with cardiovascular disease. Materials and Methods: Cardiac CT (270 cardiac phases) was used to quantify right ventricular mass using a semiautomatic 3D hybrid segmentation method in 195 patients with cardiovascular disease. Data from 270 cardiac phases were divided into subgroups based on the extent of the segmentation error (no error; ≤ 10% error; > 10% error [technical failure]), defined as discontinuous areas in the right ventricular myocardium. The reproducibility of the right ventricular mass quantification was assessed. In patients with no error or < 10% error, the right ventricular mass was compared and correlated between paired end-systolic and end-diastolic data. The error rate and right ventricular mass were compared based on right ventricular hypertrophy groups. Results: The quantification of right ventricular mass was technically applicable in 96.3% (260/270) of CT data, with no error in 54.4% (147/270) and ≤ 10% error in 41.9% (113/270) of cases. Technical failure was observed in 3.7% (10/270) of cases. The reproducibility of the quantification was high (intraclass correlation coefficient = 0.999, p < 0.001). The indexed mass was significantly greater at end-systole than at end-diastole (45.9 ± 22.1 g/m2 vs. 39.7 ± 20.2 g/m2, p < 0.001), and paired values were highly correlated (r = 0.96, p < 0.001). Fewer errors were observed in severe right ventricular hypertrophy and at the end-systolic phase. The indexed right ventricular mass was significantly higher in severe right ventricular hypertrophy (p < 0.02), except in the comparison of the end-diastolic data between no hypertrophy and mild hypertrophy groups (p > 0.1). Conclusion: CT quantification of right ventricular mass using a semiautomatic 3D hybrid segmentation is technically applicable with high reproducibility in most patients with cardiovascular disease.

치아교모에 영향을 미치는 요인에 관한 연구 (A STUDY ON THE FACTORS AFFECTING TO TOOTH WEAR)

  • This study was performed to investigate the factors related to tooth wear. For this study, 78 patients with temporomandibular disorders and 76 dental students without any signs and symptoms of temporomandibular disorders were selected as the patients group and as the normal group, respectively. Preferred chewing side, Angle's classification, lateral guidance pattern, head and shoulder posture were observed clinically. Electromyographic activity of anterior temporalis and masseter muscle were recorded with $BioEMG^{(R)}$ and occlusal status were recorded with $T-Scan^{(R)}$. Wear facet area of each tooth was measured from working model of upper arch corresponding to the occlusal status from T-Scan. Wear facet area were measured with planimeter in $mm^2$. Total area were divided into incisal, canine, posterior tooth area. Anterior wearfacet area was incisor area plus canine area, and unilateral area was anterior area plus posterior area. The data collected were analyzed by SAS statistical program and the results of this study were as follows: 1. There was no significant difference between the two groups in total werafacet area, and male subjects showed tendency to have larger area in the normal group but female subjects showed tendency vice versa. 2. There was no significant difference related to preferred chewing side and Angle's classification, however some difference was observed by lateral guidance pattern. Anterior wear facet area in subjects of canine guidance was the largest in the three subgroups. 3. Subjects with head tilting to right side had larger posterior and total area, and subjects with higher shoulder in right side had larger canine and anterior area than any other subgrous. 4. Electromyographic activity of masseter muscle was more correlated with wear facet area than anterior temporalis muscle, and tooth contact number and force were significantly correlated with wear facet area, but the most important factor affecting tooth attrition was age.

Clinical Implication of Aortic Wall Biopsy in Aortic Valve Disease with Bicuspid Valve Pathology

  • Background: Although unique aortic pathology related to bicuspid aortic valve (BAV) has been previously reported, clinical implications of BAV to aortopathy risk have yet to be investigated. We looked for potential differences in matrix protein expressions in the aortic wall in BAV patients. Methods: Aorta specimens were obtained from 31 patients: BAV group (n=27), tricuspid aortic valve (TAV) group (n=4). The BAV group was categorized into three subgroups: left coronary sinus-right coronary sinus (R+L group; n=13, 42%), right coronary sinus-non-coronary sinus (R+N group; n=8, 26%), and anteroposterior (AP group; n=6, 19%). We analyzed the expression of endothelial nitric oxide synthase (eNOS), matrix metalloproteinase (MMP)-9, and tissue inhibitor of matrix metalloproteinase (TIMP)-2. Results: Based on the mean value of the control group, BAV group showed decreased expression of eNOS in 72.7% of patients, increased MMP-9 in 82.3%, and decreased TIMP in 79.2%. There was a higher tendency for aortopathy in the BAV group: eNOS $(BAV:TAV)=53%{\pm}7%:57%{\pm}11%$, MMP-9 $(BAV:TAV)=48%{\pm}10%:38%{\pm}1%$. The AP group showed lower expression of eNOS than the fusion (R+L, R+N) group did; $48%{\pm}5%$ vs. $55%{\pm}7%$ (p=0.081). Conclusion: Not all patients with BAV had expression of aortopathy; however, for patients who had a suspicious form of bicuspid valve, aortic wall biopsy could be valuable to signify the presence of aortopathy.

선천성심장병 환아에서의 Respiratory syncytial virus 감염례 관찰 (Respiratory syncytial virus infection cases in congenital heart disease patients)

  • 목 적 : RSV는 영아기에 세기관지염이나 폐렴을 일으키는 중요한 요인으로 알려져 있다. 건강한 영아에서는 RSV 감염으로 위중한 병을 일으키는 경우는 드물지만 선천성심장병과 관련된 심부전이나 청색증, 폐동맥고혈압의 상태에 있거나 조산 미숙 등과 관련된 만성폐질환 상태에 있는 영아의 경우 위중한 병을 일으킬 수 있다. 본 연구에서는 선천성심장병 소아와 비선천성심장병 소아에서 RSV 감염례를 조사하여 RSV 감염이 선천성심장병 환아에 미치는 영향에 대하여 조사하고자 하였다. 방 법 : 2003년 7월부터 2006년 6월까지 세종병원에 세기관지염이나 폐렴으로 입원한 343례의 환아의 비강 분비물에서 RSV 항원검사를 시행하여 RSV 감염에 의한 하기도염의 연령별 계절별 발생 양상을 조사하였다. RSV 감염된 하기도염 환아들에서 선천성심장병 환아의 여러 경우와 비심장병 환아의 입원 기간, 중환자 치료, 인공환기 치료 등 임상 경과를 비교하였다. 결 과 : RSV에 의한 하기도염은 어린 영아기에 많이 발생하고 있으며 온대지방에서는 겨울철에 유행하는 양상을 보이고 있다. RSV에 의한 하기도염 환아 중 선천성심장병 환아에서 비심장병 환아에 비하여 중증 감염이 많았고 장기입원을 보였다(P <0.01). 비교적 간단한 단순단락 병변인 ASD, PDA, VSD 등의 선천성심장병에서 심장수술로 교정되고 충분히 회복된 환자에서는 RSV에 의한 하기도염에서 비교적 짧은 입원기간과 가벼운 경과를 보여 비심장병 환아와 큰 차이가 없었다. 선천성심장병에 대한 완전교정 수술이나 단계적 고식수술 후에도 감염, 폐동맥고혈압, 판막역류, 혈관협착 등의 원인으로 심폐계에 상당한 문제가 남아있는 경우 RSV 감염시 비심장병 환아에 비하여 장기 입원을 보이고(P <0.01) 중증 경과를 보였다. 교정수술 받지 않은 상태의 선천성심장병을 가진 환아에서는 대부분 6개월 이내의 어린 나이에 RSV에 의한 하기도염이 발생하고 있으며 비심장병 환아에 비하여 장기 입원을 보이고(P <0.01) 중증 경과를 보였다. 선천성심장병 심장수술 후 1개월 이내의 회복기에 병원 감염을 포함하여 상당수의 RSV 감염이 발생하고 있음을 확인하였으며 이들은 심장수술 직후의 면역기능 저하와 관련하여 중증 감염으로 발전할 가능성이 있을 것으로 생각된다. 결 론 : 미교정 상태의 선천성심장병 환아와 교정 후에도 계속 심폐계에 문제가 남아있는 선천성심장병 환아에서 RSV 감염에 의해 중증 경과와 장기 입원이 초래될 수 있다. 선천성심장병 환아에서 이러한 RSV 감염의 피해를 줄이기 위해서는 RSV 감염으로 심각한 위험이 초래 될 수 있는 고위험 세부군의 파악과 이들에 대한 예방 항체 치료 등 적절한 예방 노력이 필요하다.
