A New Method of Approach for Percutaneous Thoracic Vertebroplasty in Vertebral Compression Fracture -Case report- (흉추 압박골절환자를 위한 경피적 척추성형술의 새로운 접근법 -증례 보고-)

  • Vertebral compression fractures commonly afflict the elderly. Some patients suffer from severe mechanical pain in spite of treatments with strong analgesics and bracing. Vertebroplasty, which was originally used for vertebral hemangioma, is effective for patients who do not respond to these more conservative treatments. However, the procedure has some risk. Leaks of bone cement into perineural tissues can be a serious complication. In contrast to the lumbar vertebrae, the outer margin of the pedicle of the thoracic vertebrae is almost in line with the outer margin of the body. This, combined with the thinner pedicle of the thoracic vertebrae, makes proper needle placement difficult. The posterolateral approach is preferred to the transpedicular approach in order to avoid the danger of destroying the inner cortex of the pedicle. But there can be a problems with the standard posterolateral approach. The rib can be broken, the pleura can be punctured. A new and safer approach is possible. Before penetrating the bone, the needle is positioned at the upper margin of the transverse process, 5 mm away from the pedicle. To achieve this positioning, the needle must puncture the skin 1~1.5 cm laterally and 3~5 mm cranially to the target point on the bone. This approach was used for 10 patients and we achieved good results with no serious complication.

  • Bilateral clavicle fractures caused by a strong external force acting at the same time is a rare case. While unilateral clavicle fracture with conservative treatment shows satisfactory clinical outcome, bilateral clavicle fractures with it has drawbacks such as severe pain, difficulty in rehabilitation and breathing. We experienced a rare case of bilateral clavicle fractures with multiple rib fractures, hemo-thorax and transverse process fracture of thoracic vertebra treated with surgical intervention promoted functional recovery and comfortable breathing. So, we report this case with article review.

Clinical Evaluation of Chest Trauma - Analysis for 97 cases - (흉부외상의 임상적 고찰)

  • A clinical analysis was performed on 97 cases of chest injuries experienced at Department of Thoracic and cardiovascular Surgery, Armed Forces Capital Hospital during 2 years period from 1986 to 1988. Of 97 patients of chest trauma, 39 cases were result from penetrating injuries whereas 58 cases were from non-penetrating injuries, and there were 77 cases of hemothorax and / or pneumothorax, 47 of rib fracture, 8 of foreign body, 6 scapular and clavicle fracture, 5 of diaphragmatic injuries, 4 of paraplegia. The majority of chest injuries were encounted in the age group between 21 and 30 years-old, mean age was 25.9 years-old and all cases were male except one. Gun-shot wound was the most common cause in the penetrating injuries and the majority of non-penetrating chest injury patients were traffic accident and fist or kick accounted for next. The principles of therapy for chest trauma were rapid expansion of the lung by closed thoracostomy[45 cases] and thoracentesis only[3 cases] but thoracotomy done at 27 cases because of massive bleeding or intrapleural hematoma, foreign body, cardiac injury, diaphragmatic injury and bronchial rupture. The over-all mortality was 2.07 percent[2 cases among all], a case was from penetrating injuries and another was from non-penetrating injuries.

Penetrating Chest Injuries Caused by the Sideview Mirror of the Patient's Car - Report of 2 cases - (자동차의 사이드미러가 흉강에 박힌 채로 내원한 흉부관통상 - 2례 보고 -)

  • Penetrating chest trauma caused by the components of one's own car is rare in motor vehicle accidents. We experienced two cases of penetrating chest injury caused by the sideview mirror of the patient's vehicle. One was a 25-year-old man. The sideview mirror penetrated the left chest, went through the diaphragm, and ruptured the spleen. He was in shock upon arrival at the emergency room. An emergency thoracotomy and laparotomy were done. The ruptured spleen was resected, the lung and the diaphragm were debrided and repaired, and the chest wall was reconstructed. The other patient was a 57-year-old male, who was transported to our emergency room with the sideview mirror of his truck stuck into his right chest wall as the result of an accident. He also had a right Bennet's fracture and an open fracture of the right tibia. Air had been sucked into the right pleural cavity through the wound. Multiple rib fractures and lung lacerations had also occurred. Removal of the sideview mirror, repair of the lacerated lung, and reconstruction of chest wall were done immediately. Both patients recovered without complication and were discharged.

Free Rectus Abdominis Muscle Flap for Treatment of Open Fractures of the Tibia (개방성 경골 골절의 치료에서 유리 복직근 피판술의 유용성)

  • Purpose: Management of soft-tissue defect after open tibial fractures includes immediate and repeated debridement, skeletal stabilization, and early soft-tissue coverage with muscle flaps. The purpose of this study was to evaluate the outcome of the free rectus abdominis muscle flap (RA flap) for treatment of open fractures of the tibia and to discuss its advantages compared with the latissimus dorsi muscle flap (LD flap) in poly trauma patients. Materials and Methods: We performed a retrospective review of 5 patients who had a severe (Gustilo IIIb or IIIc) open fracture of the tibia treated with RA flap from May 2003 to March 2006. All were men, and the mean age was 46.6 years (range, $28{\sim}68$). Three patients had combined injuries such as pelvic bone fractures, multiple rib fractures with hemothorax, and contralateral tibial fracture. All patients received RA flap within 7 days after trauma except two with established chronic osteomyelitis. Results: All flaps survived, and there was no marginal flap necrosis. During the follow-up period, there was no evidence of persistent or recurrent osteomyelitis. The size of RA flap ranged from $8{\sim}20\;cm$ in length and $6{\sim}10\;cm$ in width. The average time required for RA flap elevation was 32 minutes, which is shorter than LD flap. Flap elevation could be done in supine position which is essential in poly trauma patients. Conclusion: Although a wide variety of options are available, RA flap is regarded as an optimal method for coverage of soft-tissue defect of the open tibial fracture in poly trauma patients. LD flap is reserved for large sized soft-tissue defect which cannot be covered by RA flap.

Tibial bone fractures occurring after medioproximal tibial bone grafts for oral and maxillofacial reconstruction

  • Objectives: Oral and maxillofacial defects often require bone grafts to restore missing tissues. Well-recognized donor sites include the anterior and posterior iliac crest, rib, and intercalvarial diploic bone. The proximal tibia has also been explored as an alternative donor site. The use of the tibia for bone graft has many benefits, such as procedural ease, adequate volume of cancellous and cortical bone, and minimal complications. Although patients rarely complain of pain, swelling, discomfort, or dysfunction, such as gait disturbance, both patients and surgeons should pay close attention to such after effects due to the possibility of tibial fracture. The purpose of this study is to analyze tibial fractures that occurring after osteotomy for a medioproximal tibial graft. Materials and Methods: An analysis was intended for patients who underwent medioproximal tibial graft between March 2004 and December 2011 in Inha University Hospital. A total of 105 subjects, 30 females and 75 males, ranged in age from 17 to 78 years. We investigated the age, weight, circumstance, and graft timing in relation to tibial fracture. Results: Tibial fractures occurred in four of 105 patients. There were no significant differences in graft region, shape, or scale between the fractured and non-fractured patients. Conclusion: Patients who undergo tibial grafts must be careful of excessive external force after the operation.

Seismic Performance Evaluation of Beam-Column Connection for Panel Zone Strength (패널존의 강도비에 따른 기둥-보 접합부의 내진성능 평가)

  • The study proposes the method to cancel the scallop to avoid fracture of the circumstance of the scallop at H shape column-to-beam connection and reinforce at beam flange two faces with the cover plates and rib. A total of four specimens were tested to enhance seismic performance of building structure by reducing the frequency of stress concentration and preventing the brittle fracture of scallop. For this purpose, four full-scale test specimens were made and loaded with quasi-static reversed cyclic loading. The main analytical parameters are panel-zone-strength ratio, yield strengths, initial stiffness, total plastic rotation, contribution of each element to total plastic rotation and energy dissipation capability. For the specimens tested under repeated loading, the experimental result was satisfied with seismic performance requirement as the Special Moment Frames (SMF). The analysis results show that all of the test specimens were found to have good performance to 4% story drift and satisfied the criteria for the plastic roation capacity of SMFs that is 0.03 rad. according to the 1997 AISC seismic provision.

Experimental Study on the Flexural Behavior of Inverted T-Shaped Steel·Concrete Composite Deck for Bridges (역T형강·콘크리트 합성바닥판의 휨거동에 관한 실험적 연구)

  • This study is to suggest the details of new concept of bridge deck. Experimental studies on the behavior of a inverted T-shaped steelconcrete composite deck were carried out. The part of inverted T-shaped steel is embedded in concrete. Reinforced concrete deck specimen and composite deck specimens were fabricated and static bending fracture tests were conducted. The ultimate strength and fracture strength of specimens were evaluated. The effects of shear hole crossing bars of composite deck were also analyzed. From the results of experiments, composite deck with shear hole crossing bar increased shear strength, and showed typical tensile failure. Ultimate strength and fracture strength of composite deck with shear hole crossing bar are higher than those of reinforced concrete deck. The displacement of composite deck is higher than that of reinforced concrete deck.

Penetrating chest trauma from a "less lethal" bean bag in the United States: a case report

  • This case report presents the case of a 49-year-old man who presented to our level I trauma center after sustaining injuries in an altercation with local law enforcement in which he was shot with a less lethal bean bag and tased. In a primary survey, a penetrating left supraclavicular wound was noted in addition to a taser dart lodged in his flank. No other traumatic findings were noted in a secondary survey. Given hemodynamic stability, completion imaging was obtained, revealing a foreign body in the left lung, a left open clavicle fracture, a C5 tubercle fracture, a possible grade I left vertebral injury, and a left first rib fracture. Soft tissue gas was seen around the left subclavian and axillary arteries, although no definitive arterial injury was identified. The bean bag projectile was embedded in the parenchyma of the left lung on cross-sectional imaging. The patient underwent thoracotomy for removal of the projectile and hemostasis. A thoracotomy was chosen as the operative approach due to concerns about significant bleeding upon foreign body removal. A chest tube was placed and subsequently removed on postoperative day 5. The patient was discharged on postoperative day 7. At a 2-week outpatient follow-up visit, the patient was doing well. This case report is the first to describe this outcome for a drag-stabilized bean bag. Although law enforcement officers utilize bean bag projectiles as a "less lethal" means of crowd control and protection, these ballistics pose significant risk and can result in serious injury.

An Experimental Study on the Seismic Behavior of Box Type Concrete-Filled Steel Piers (박스형 강합성 교각의 내진 성능 평가를 위한 실험적 연구)

  • The steel piers and the concrete-filled steel piers, in spite of reasonable strength, high ductility, small section, and fast construction, have not been considered as the alternatives to the RC piers even in the highly populated urban area where aseismic safety, limited space and fast construction are indispensably required. In this paper, a steel pier and 4 box type concrete-filled steel piers were tested with the quasi-static cyclic loading to estimate the ductility and the strength. Additional devices such as base rib, turn-buckle, and anchor bolted added at the to increase the ductility with minimum additional cost. The result showed that the concrete filled-in steel piers had higher energy absorbtion and strength than steel piers had, but also showed that slight overlooking in the design and fabrication could lead to the abrupt fracture just after small local buckling at the bottom.
