Pylorus-Preserving Gastrectomy for Gastric Cancer

  • Pylorus-preserving gastrectomy (PPG) is a function-preserving surgery for the treatment of early gastric cancer (EGC), aiming to decrease the complication rate and improve postoperative quality of life. According to the Japanese gastric cancer treatment guidelines, PPG can be performed for cT1N0M0 gastric cancer located in the middle-third of the stomach, at least 4.0 cm away from the pylorus. Although the length of the antral cuff gradually increased, from 1.5 cm during the initial use of the procedure to 3.0 cm currently, its optimal length still remains unclear. Standard procedures for the preservation of pyloric function, infra-pyloric vessels, and hepatic branch of the vagus nerve, make PPG technically more difficult and raise concerns about incomplete lymph node dissection. The short- and long-term oncological and survival outcomes of PPG were comparable to those for distal gastrectomy, but with several advantages such as a lower incidence of dumping syndrome, bile reflux, and gallstone formation, and improved nutritional status. Gastric stasis, a typical complication of PPG, can be effectively treated by balloon dilatation and stent insertion. Robot-assisted pylorus-preserving gastrectomy is feasible for EGC in the middle-third of the stomach in terms of the short-term clinical outcome. However, any benefits over laparoscopy-assisted PPG (LAPPG) from the patient's perspective have not yet been proven. An ongoing Korean multicenter randomized controlled trial (KLASS-04), which compares LAPPG and laparoscopy-assisted distal gastrectomy for EGC in the middle-third of the stomach, may provide more clear evidence about the advantages and oncologic safety of PPG.

  • Preoperative chemo- and radiotherapeutic strategies followed by surgery are currently a standard approach for treating locally advanced gastric and esophagogastric junction cancer in Western countries. However, in a large number of cases, the tumor is extremely resistant to these treatments and the patients are exposed to unnecessary toxicity and delayed surgical therapy. The current clinical trials evaluating the combination of preoperative systemic therapies with modern targeted and immunotherapeutic agents represent a unique opportunity for identifying predictive biomarkers of response to select patients that would benefit the most from these treatments. However, it is of utmost importance that these potential biomarkers are corroborated by extensive preclinical and translational research. The aim of this review article is to present the most promising biomarkers of response to classic chemotherapeutic, anti-HER2, antiangiogenic, and immunotherapeutic agents that can be potentially useful for personalized preoperative systemic therapies in gastric cancer patients.

A Study on the Application and Grouting Effects of Akali-Silicasol Grout Material from Field Test (현장시험을 통한 알칼리성 실리카졸 주입재의 적용성 및 주입효과에 대한 연구)

  • This research have investigated the application and the injection effect of the alkaline silicasol grouting materia (which is) recently developed. To compare the engineering characteristics of the alkaline silicasol which is formed with 3D-gel and the sodium silicate, it have studied the strength and environmental effects through the laboratory test, and field application was investigated with a review of the injection effect and waterproof through the field test. As a result of the laboratory test, unconfined compression strength of alkaline silicasol was 5 times more than sodium silicate. As a result of the leaching test, a total amount of the eluviated elements was very small, it considered the environment-friendly material. As a result of the field test, considerable reduction of the coefficient of permeability at the hydraulic layer was confirmed, and it could secure that the coefficient of permeability showed a lower than standard value of $10^{-5}cm/sec$.

Primary Study to Develop the Instrument of Pattern Identification for Common Cold (감모변증도구 개발에 관한 예비 연구)

  • Common cold occurs frequently and occupies an important position in medical treatment however obvious treatment is not suggested. There has been no agreement of pattern identification for common cold in oriental medicine. The purpose of this study is to develop a standard instrument of pattern identification for common cold which will be applied to clinical research. The items and structure of the instrument were based on review of published literature. The advisor committee on this study was organized by 9 oriental respiratory internal medicine professors of 11 oriental medical colleges nationwide. The experts attended 3 consultation meetings and discussed developing the instrument, and we also took professional advices by e-mail. The results were as follows; First, we divided the pattern identification of common cold into nine pattern: Wind-cold type, Wind-heat type, Contain summerheat type, Contain dampness type, Qi deficiency type, Blood deficiency type, Yang deficiency type, Yin deficiency type, Influenza. Second, we got the mean weights to each symptom of nine pattern identification which had been scored on a 5-point scale - ranging from 0 to 4 by the 9 experts. Third, we made out the Korean instrument of the pattern identification for common cold. It was composed self reporting questionnaire and researcher reporting questionnaire. Though this study is not proved about validity, reliability, the instrument of pattern identification for common cold is meaningful and expected to be applied to the subsequent research. And also, we hope to improve the instrument and make up for this study through various research and discussion.

A Study of the Roles of the Teacher-Librarians as Perceived by Supervisors, Principals, Teachers and Teacher-Librarians in Korea (사서교사의 역할에 대한 인식연구)

  • The school library plays very important and central role for the fullfillment of educational objective in the process of implementing the school curriculum. And the media specialist known as teacher-librarian is. primarily responsible for the successful operation and effective management of the media center. It is true that effectiveness of the school library depends greatly on the cooperative coordination among teacher-librarian, teacher and principal in addition to the strong support and effective supervision of school district authority, specifically, supervisor of school library program. The successful program therefore is influenced by their perception of the teacher-librarian's role. The purpose of this study was to investigate the teacher-librarian's. educational role perceived by teacher-librarian, teacher, principal and school district supervisor. A standard task model describing both the management and educational functions and expectations of teacher-librarian was proposed after studying the relevant current standards of four countries. Serveral hypotheses were made regarding role perception and a survey instrument was designed based on this proposed task model and the results of the literature review to validate the hypotheses. The questionnaires were mailed out and the returned questionaires were analyzed by SPSS computer routine. The major findings are summarized as follows: 1) There was difference of the role perception among teacher-librarian, teacher, principal and district supervisor at the significance level of p<0.001 for both management and educational functions of teacher­librarian. The mean score of the teacher-librarian was highest among four groups and the score for the educational function tasks were lower than that of management function tasks. 2) The positive correlation exists at the significance level of p<0.001 between the role perception and the attitude toward changing social trends such as acceptance of new technology, need for better communication and information. 3) The positive correlation exists at the significance level of p<0.001 between the role perception and the view on the curriculum and teaching method.

An Evaluation of Biotope Based on Its Valuation Criteria in Terms of Conservation of Species and Habitat (평가지표에 의한 도시 비오톱의 가치평가 -생물종과 서식처 보전을 중심으로-)

  • The purpose of this study is to construct the evaluation criteria based on the relative importance and various combination of each evaluation index and then to evaluate urban biotope in terms of conservation of species and habitat. After the standard of evaluation had been developed through the literature review, the relative importance of the evaluation index through expert survey was found out, and the evaluation model of biotope value has been established. The results of this study are as follows; 1) We have constructed the nineteen evaluation criteria through literature reviews, and have provided relative importance to each evaluation index of the diversity of typical species in biotope, afforestation rate, stratification, Hemeroby grads, and the diversity of landscape patterns based on the results of expert survey. and then we have constructed the evaluation system using the relative importance criteria. 2) To evluate urban biotope using the evaluation system constructed, we selected 14 sites. The final evaluation results of each site is as follows; The III grade areas are Seohan and Hwasung high-rise apartment at Beummul-Dong, Korea Men Power /Agency in Sungs 대 industrial zone, the adjacent area of St. Tongil, and Hyomyung elementary school. The II grade areas are the farm land behind the third Anshim Jugong apartment, and Ehyun park. The I grade areas are swampy land in front of Kangchon and Anshim construction site, water space with semi-natural style in front of Gangchon Woobang apartment, forest zone near page Temple at mt. Palgong, the edge area of Jisan and Gosan, and the tendril yard against Anshim middle school. The only grade IV area is the commercial zone near Taegu Department 3) It is highly useful to evaluate urban biotope using the evaluation criteria constructed for this research, the evaluation criteria we developed may be difficult to guarantee the ability for reproduction and recovery, the appearance-number of the rare and endangered species are difficult to estimate urban biotopes. In the future, it is highly useful to evaluate biotope in terms of conservation of species and habitat through its valuation criteria developed in this research and it is necessary to develop the evaluation criteria applicable to estimation of biotope inventory and reproduction of biotope.

Do Obese Children Exhibit Distinguishable Behaviours from Normal Weight Children?-Based on Literature Review (비만어린이와 정상체중 어린이의 행동 특성에 관한 문헌적 고찰)

  • Since obesity has been a crucial issue regarding children's health in Korea, numerous studies have been conducted. However, it is difficult to locate studies that can help researchers and/or medical professionals who want to study behavioral differences between obese and normal weight (non-obese) children. The collected literature about obese childrens behaviors were accumulated by performing 'key word' searches from several web engines: 'obesity' 'children obesity' 'behavior' 'habit' 'eating behavior or habit' 'exercise' 'physical activity' and 'daily behavior'. The information was extracted and the behaviors were reviewed utilizing 30 papers that revealed any differences from normal weight children with at least p .05 level of significance. According to those studies, the obese children tended to skip meals more often as a method of losing weight. They also have an increased appetite, eat larger portions and rarely leave food on their plates. The obese children prefer to eat high fat, high calorie foods and eat faster than their normal weight counterparts. Also, the children usually eat alone and eat at irregular meal times. Beside the behaviors related to the standard three meals a day, obese children have high preferences for snacking. The children reported that with pocket money they usually eat certain types of snacks without parent approval. The children stated that they exercise more on a regular basis. However, when asked about their participation they were not likely to engage actively. Meanwhile, they tend to spend free time doing sedentary activities such as watching TV or playing on the computer. Since these studies used self -reported questionnaires with a small sample, size there may be difficulty in forming a basis about obese children's behaviors. Nevertheless, these findings are in accordance with other studies conducted in local as well as western countries. In conclusion, obese children have different daily behaviors or routines from normal weight children. It is recommended that altering these behavioral patterns to match those of normal weight children should be encouraged.

A Study on the Deduction of performance Point of Nonseismically Designed Reinforced Concrete Apartment (비내진 설계된 철근콘크리트 아파트의 성능점 도출에 관한 연구)

  • It has been commonly assumed that during the 21st century, the korean peninsula may suffer huge earthquake damage to people, society, and economic system. The recent report of "Seoul Earthquake Response model development" conducted by the city of Seoul indicated that a magnitude 6.3 earthquake possibly hit Seoul, the capital of Korea. However, due to the insufficient amount of study on seismic performance of structures reflecting the various types of element peculiar to Korea application of the currently available earthquake damage evaluation methods has limitations. In order to conduct various studies on seismic hazards that are suitable for the actual conditions in Korea, therefore, fundamental studies first have to be properly conducted. The purpose of this study is to serve as the basis of establishing a reliable earthquake damage estimation system, and to provide essential data for the seismic damage evaluation of nonseismically reinforced concrete apartment structures. In this study, a standard type of nonseismically reinforced concrete apartments has been determined based on an extensive survey and careful review of such structures in Korea, and their performance level on seismic loading has been estimated.

A Study on Carbon Reduction Effect in On-site Improvement Type Residential Regeneration (현지개량형 주거지 재생의 탄소저감 효과 분석)

  • This study aims to figure out the effectiveness of low-carbon oriented residential regeneration by on-site improvement method. For this, we examined some technical planning elements for carbon reduction and selected standard criteria of computation on energy use-based amount of carbon emission through previous literature review and case studies. Next, based on this, we compared and analyzed three types of carbon emission in the designated renewal area in Cheongju City. The three types of carbon emission were the existing development, the redevelopment of apartment by overall removal, and the development of low-carbon oriented residential regeneration by on-site improvement method. Finally, we examined proper way to reduce carbon emission in low-carbon oriented residential regeneration.

Urban Mine Resources and Metals Recycling Industries in Japan (일본(日本)의 도시광산(都市鑛山)(사용후제품) 자원(資源)과 금속(金屬) 재자원화산업(再資源化産業))

  • In order to review the recycling status of urban mine resources in Japan, metal consumption, metal recycling rate and metal recycling industry such as iron scrap, end of life vehiclcs(ELV), waste home appliances and spent IT equipments were surveyed. Japan took rank of top class in the world on the metal consumption and urban mine stock reserve. Metal recycling industries in Japan have been developed through excellent technologies for mineral processing and non-ferrous smelting. On the other hand, the technologies for recycling of rare metals are being developed now. Recycling rate of EL V, waste home appliances and personal computer are higher than the guidelines of the legislative standard.
