Surgical management of a failed internal root resorption treatment: a histological and clinical

  • This article presents the successful surgical management of a failed mineral trioxide aggregate (MTA) orthograde obturation of a tooth with a history of impact trauma and perforated internal root resorption. A symptomatic maxillary lateral incisor with a history of perforation due to internal root resorption and nonsurgical repair using MTA was referred. Unintentional overfill of the defect with MTA had occurred 4 yr before the initial visit. The excess MTA had since disappeared, and a radiolucent lesion adjacent to the perforation site was evident radiographically. Surgical endodontic retreatment was performed using calcium enriched mixture (CEM) cement as a repair material. Histological examination of the lesion revealed granulation tissue with chronic inflammation, and small fragments of MTA encapsulated within fibroconnective tissue. At the one and two year follow up exams, all signs and symptoms of disease had resolved and the tooth was functional. Complete radiographic healing of the lesion was observed two years after the initial visit. This case report illustrates how the selection of an appropriate approach to treatment of a perforation can affect the long term prognosis of a tooth. In addition, extrusion of MTA into a periradicular lesion should be avoided.

Study on the Retreatment Techniques for NOAA Sea Surface Temperature Imagery (NOAA 수온영상 재처리 기법에 관한 연구)

  • We described for the production of cloud-free satellite sea surface temperature(SST) data around Northeast Asian using NOAA AVHRR(Advanced Very High Resolution Radiometer) SST data during 1990-2005. As a result of Markov model, it was found that the value of Markov coefficient in the strong current region such as Kuroshio region showed smaller than that in the weak current. The variations of average SST and regional difference of seasonal day-to-day SST in spring and fall were larger than those in summer and winter. In particular, the distribution of the regional difference appeared large in the vicinity of continental in spring and fall. The difference of seasonal day-to-day SST was also small in Kuroshio region and southern part of East Sea due to the heat advection by warm currents.

  • When restoring their anterior dentition, patients become more demanding on esthetics compared to posterior region during treatment planning phase. Digital Smile Design (DSD) procedure is performed in presentation software and digital photographs. This can widen diagnostic visualization and aid in transferring information between clinician, patient, and technician. This case presented is that of patient with dissatisfaction of his anterior old restoration. Retreatment procedures were carried out in two different manners: (1) using DSD protocol for diagnosis, smile simulation, communication and fabricating interim and definitive prosthesis by totally digitized workflow. (2) Using diagnostic wax-up for smile design and fabricating restorations by conventional workflow. Comparing two methods, DSD was easier to communicate between the dental team than the diagnostic wax-up method. But the final result obtained failed to meet total esthetic factors. Therefore, to obtain predictable esthetic results, more advanced design tool would be needed, including consideration of various esthetic factors besides successful communications.

  • Monogenic diseases are resulted from modifications in a single gene of human cells. Because their treatment with pharmacological medicine have a temporary effect, continuous nursing care and retreatment are required. Gene therapy, gene targeting and induced pluripotent stem cell (iPSC) are considered permanent treatment methods of them. In gene therapy, however, retroviral vectors that have potential toxicity caused by random insertion of harmful virus are used as vehicles for transferring genetic materials. On the other hand, gene targeting could replace and remove the modified gene though homologous recombination (HR) induced by site-specific endonucleases. This short review provides a brief overview on the recently tailored endonucleses with high selectivity for HR.

Complementary Management of Residual Intracranial Aneurysms after Endovascular or Surgical Treatment

  • Objective: The purpose of this paper is to report our experiences in managing seventeen cases of residual intracranial aneurysms following surgical or endovascular treatment and discuss the incidence of residual aneurysms, the indications and technique of retreatment of residual aneurysms. Methods: During a period of 42 months, we treated 391 aneurysms in 339 patients with microsurgical clipping or GDC embolization as a primary treatment. In 39 of them, follow-up angiography revealed residual aneurysms and seventeen of whom were retreated. There were eleven cases in ACoA, three cases in distal ICA, one, in each of MCA, ACA and basilar artery. We reviewed retrospectively the clinical notes, operation records and cerebral angiograms of seventeen patients who had been treated for residual aneurysms. Results: Complementary treatment was performed in 8 cases by means of surgery and in 9 cases by means of GDC embolization. There were eleven females and six males with an age variation between 29 and 78 years. The mean duration of angiographic follow-up was 17.3 months. Of the seventeen cases that were treated for residual aneurysms, fourteen achieved complete occlusion. Of 17 retreated patients, fifteen patients had good recovery according to the Glasgow Outcome Scale. Conclusion: When occlusion after endovascular or surgical treatment is incomplete, a new multidisciplinary approach should be carried out. Given our experiences, we recommend coil embolization of the choice in cases that the residual aneurysmal neck had been narrowed by previous clipping. On the other hand, if the residual aneurysm has enough space to clip but not enough to coil, we recommend the microsurgical clipping.

Are Blood Blister-Like Aneurysms a Specific Type of Dissection? A Comparative Study of Blood Blister-Like Aneurysms and Ruptured Mizutani Type 4 Vertebral Artery Dissections

  • Objective : Blood blister-like aneurysms (BBAs) resemble arterial dissections. The purpose of this study was to investigate the relationship between these two disease entities and highlight commonalities and distinct features. Methods : Among 871 consecutive patients with aneurysmal subarachnoid hemorrhage, 11 BBAs of internal carotid artery and seven vertebral artery dissections (VADs) with a short segmental eccentric dilatation (Mizutani type 4), which is morphologically similar to a BBA, were selected. The following clinical factors were studied in each group : age, gender, risk factors, Hunt and Hess grade (HHG), Fisher grade (FG), vasospasms, hydrocephalus, perioperative rebleeding rate, and treatment outcome. Results : The mean age was 47.9 years in the BBAs group and 46.4 years in the type 4 VADs group. All the BBA patients were female, whereas there was a slight male predominance in the type 4 VAD group (male : female ratio of 4 : 3). In the BBA and type 4 VAD groups that underwent less aggressive treatment to save the parent artery, 29% (n=2/7) and 66.6% (n=2/3), respectively, eventually required retreatment. Perioperative rebleeding occurred in 72.7% (n=8) and 28.6% (n=2) of patients in the BBA and type 4 VAD groups, respectively. There was no statistical difference in the other clinical factors in both groups, except for the male dominancy in the type 4 VAD group (p=0.011). Conclusion : BBAs and ruptured type 4 VADs have a similar morphological appearance but there is a distinct clinical feature in gender and perioperative rebleeding rates. Complete isolation of an aneurysm from the parent artery might be the most important discipline for the treatment of these diseases.

Degradation Characteristics of Algae Coagulated with Poly Aluminum Chloride by Thermophilic Oxic Process (고온·호기법을 이용한 Poly Aluminum Chloride에 의해 응집된 조류의 분해특성)

  • The biodegradation of algae coagulated with poly aluminum chloride(PAC) was investigated by using the thermophilic oxic process. The compositions of coagulated algae were 83.5% of water content, 24.6% of ash, 32% of organic carbon with in total solid, respectively. In present study, food waste oil was used for the increment of calorie of mixtures in order to accelate the microbial activity. As a result, the maximum temperature of mixtures was higher than $50^{\circ}C$ when the mixing ratio of food oil was over 10%. However the temperature indicated the lower than $50^{\circ}C$ when conditions of no mixing with waste food oil, and 5% of mixing ratio. Therefore, the optimum condition was 10% of the mixing ration at $217l{\cdot}m^{-3}{\cdot}min^{-1}$ of air supply rate. The conversion efficiency of carbon was highest as 92% at the optimum condition. And then water was evaluated from imxture without accumulation at 10% of mixing ratio. The thermophilic oxic process well conducted that is good process for the treatment of waste algae without effluents however it has to consider the retreatment of accumulated aluminum in the reactor.

Clinical and Radiogical Outcomes of Endovascular Detachable Coil Embolization in Paraclinoid Aneurysms : A 10-Year Experience

  • Objective : Direct surgical clipping of paraclinoid aneurysms poses technical challenges to even very experienced neurosurgeons, making endovascular treatment an alternative treatment modality in many centers. We have therefore retrospectively evaluated the safety and efficacy of endovascular detachable coil embolization of paraclinoid aneurysms. Methods : From June 1997 to June 2007, 65 patients underwent endovascular detachable coiling for 67 paraclinoid aneurysms (of which 9 were ruptured and 58 were unruptured) in our institute. Their medical records, radiological images and readings, and operation records were reviewed retrospectively. Results : After the initial embolization procedure, complete occlusion was achieved in 29 (43.3%) of the aneurysms treated by endovascular detachable coiling. Six aneurysms required retreatment, with two each requiring one, two, or three additional endovascular procedures. Fifty-five (82.1%) aneurysms were measured by three-dimensional time of flight (TOF) magnetic resonance images (MRI) or transfemoral cerebral angiography (TFCA) at a mean follow-up of 29.7 months (range from 4 to 94 months), with 39 aneurysms (70.9%) showing complete occlusion. Thromboembolic events (3.8%) were the most frequent complication. Rupture did not occur during or after any of the procedures. According to the Glasgow Outcome Scale (GOS), 98.4% of the patients treated by coil embolization had a score of 4 or 5. Conclusion : Our results indicate that endovascular detachable coiling is a safe and effective treatment modality in paraclinoid aneurysms.

THE COLOR STABILITY OF AESTHETIC RESTORATIVE MATERIALS RESULTING FROM ACCELERATED AGING

  • STATEMENT OF PROBLEM: The discoloration of anterior teeth restoration is one of the material problems demanding retreatment. OBJECTIVES: To evaluate the color stability and affecting factors on esthetic restorative materials when subjected to accelerated aging. MATERIAL AND METHODS: This study was conducted using porcelain disks (IPS Empress 2-glazed, IPS Empress 2-polished), direct restorative resin disks (SYNERGY Duo) and indirect restorative resin disks (Sinfony, TESCERA ATL). Accelerated aging was done by precipitating the specimens in 38.C distilled water and irradiating with xenon light, and the total irradiation was 397.98 KJ/mm. Color and microhardness change of the specimens were measured before accelerated aging and after 100 hours, 200 hours and 300 hours of accelerated aging, and Surface of the specimens were examined with SEM before and after 300 hours of accelerated aging. RESULTS: 1. After 300 hours' accelerated aging, a ${\Delta}E$ value was 3.3 or lower in IPS Empress 2-glazed, IPS Empress 2-polished and Sinfony. 2. After 300 hours' accelerated aging, gloss was lost and surface changes including microcracks were observed in TESCERA ATL and SYNERGY Duo, and color changes of them ranged between 3.58 and 6.40 ${\Delta}E$ units. 3. During 300 hours' accelerated aging, the microhardness of surface was increased by 3.21 - 19.64% in all kinds of composites resin. CONCLUSION: After 300 hours' accelerated aging, SEM images IPS Empress 2-glazed, IPS Empress 2-polished and Sinfony showed little morphological change and their color changes were considered to be clinically acceptable. And there was significant correlation between microhardness changes and color changes of composites (P < .05).

A Prognostic Assessment of First Permanent Molars Showing Molar-Incisor Hypomineralization Based on Restorative Materials and Defect Class (MIH에 이환 된 제1대구치에서 치질결손 정도와 수복재료에 따른 예후 평가)

  • The objectives of this study were to classify the first permanent molars showing molar-incisor hypomineralization (MIH) on the basis of defect size and to assess differences in the prognoses of restorations performed with different materials. The first permanent molars with MIH and posteruptive breakdown were categorized into MIH class I, II, and III. After performing restorations, retreatment frequencies were examined according to the defect class and initial restorative material used. Sealants, composite resins, and stainless steel crowns showed the highest survival rates in cases of MIH class I, II, and III defects, respectively.
