Traditional method of pulpectomy for a necrotic primary anterior tooth was done on lingual side. But it could not recover the discoloration of crown effectively. For the purpose of treating the discoloration of crown after lingual pulpectomy, additional methods of crown restoration were needed like : celluloid crown, open-faced crown, rasin-faced crown. Neverthless, these kinds of complete coverage methods had some disadvantages such as possibility of tooth fracture by increased tooth preparation. In order to overcome the shortcomings of lingual pulpectomy, labial treatment could be considered as an alternative. It is a method that treats necrotic pulp through the labial access opening. After finishing the pulp treatment, discolored labial tooth structure was removed extending from access opening. Discoloration of deep area could be masked effectively using opaque sealant. Cavity on labial side was restored with composite resin. This labial approach method has several advantages. First, it gives a direct vision for effective pulp treatment which is also very useful for children with poor behavior. Second, most of lingual tooth structure could be saved and occlusal contact of lingual surface remains undisrupted. Only nonfunctional discolored labial surface may removed. Third, complete removal of discolored part of a labial tooth and immediate resin restoration could be done effectively after pulp treatment. Moreover, it also could be used for pulp treatment having serious dental caries on labial surface with sound lingual tooth structure. This report presents cases with discolored upper anteior primary tooth, approaching labial side with successful restoration.

Background Infantile hemangioma (IH) is a common vascular tumor in pediatric patients, and is commonly treated with propranolol. We describe our experiences with dosage, response to treatment, and side effects in 23 IH patients treated with propranolol. Methods For this nonrandomized comparative cohort study, the authors enrolled 23 patients treated with propranolol. Photographs were taken before propranolol administration and at 3, 6, 9, and 12 months after treatment. Treatment responses were objectively analyzed with a computer program. Results There were three male and 20 female patients. Common tumor locations were the head and neck (13 cases, 56.5%), trunk (four cases, 17.4%), extremities (three cases, 13.0%), and combined locations (three cases, 13.0%). The response to propranolol was significantly lower in patients with two or more lesions than in patients with a single lesion in terms of both color fading (P<0.001) and size reduction (P<0.001). In male patients ($42.2{\pm}3.9$), the change in a-values, indicating coloration, was higher than in female patients ($19.8{\pm}13.8$) (P<0.001). In patients who started treatment before 6 months after birth, the size reduction was greater than in their counterparts (62.3%; range, 3.0%-93.0% vs. 15.8%; range, 1.0%-79.0%; P<0.001). Conclusions Propranolol is an efficacious treatment with a good safety profile. In patients with a single lesion, the response to treatment was better in terms of color fading and size reduction. Furthermore, male patients responded better to propranolol treatment in terms of color fading than female patients, and starting treatment before 6 months after birth was more advantageous for size reduction.

Injuries of permanent teeth by trauma occur frequently in childhood and adolescence. Crown fractures are as frequent as 25-76% and especially maxillary incisors are prone to fractures. There have been numerous efforts to achieve both aesthetically and functionally satisfying restoration. When a mature tooth is fractured, porcelain crown or laminate veneer could be a choice of prosthodontic treatment. However, in a case of immature permanent incisor fracture, prosthodontic treatment is more complicated due to the immaturity of the tooth. Moreover, if endodontic treatment is accompanied with the prosthodontic treatment, the treatment period is prolonged. In the past, restoration using an orthodontic band, a ready-made crown, or glass ionomer cement did not exhibit esthetically satisfying result. As restorational materials have been improved, now more esthetic restoration is possible by reattaching fractured fragments or light-curing composite resin restoration. We reports cases of patients with fractured maxillary incisors and their successful treatment results through reattachment of fractured fragments and composite resin restoration.

BACKGROUND. Though implant dentistry is very successful and predictable in treatment of patients with destroyed dentition, there are some cases with limitations to implant therapy. In these cases, alternative treatment modality should be considered. CASE DESCRIPTION. A patient with destroyed dentition was rehabilitated with a lateral rotational path removable partial denture. According to the diagnosis, we determined to raise vertical dimension for esthetic and functional restoration. The final restoration was performed after four months of provisional period. CLINICAL IMPLICATION. The edentulous patients with compromised esthetics and functions can be successfully treated with a rotational path removable partial denture through adequate treatment planning and precise laboratory procedure.

The ecosystem of Jaemin stream, flowing into the center of Gongju-si, had been damaged by low water quality and lack of water quantity of the steam. However, after applying the SSB (Sustainable Structured wetland Biotop) system to the flood plain and the upstream of Jaemin stream, the efficiency of ecological water purification and ecological restoration are as follows. Through the constant maintenance and monitoring from year 2009 to year 2013 after restorative design and construction the average influent concentration of BOD5 was 4.2 mg/L, and the average effluent concentration was 1.8 mg/L, reaching ecological water purification rate of 57%. As for the T-N, the average influent concentration was 9.983 mg/L, and the average effluent concentration was 6.303 mg/L, showing the rate of 37%. For the T-P, the average influent concentration was 0.198 mg/L, and the average effluent concentration was 0.098 mg/L, being the rate of 51%. The vegetation of Jaemin stream monitored for 2 years after the restoration was composed of 51 species in 28 families which show high ratio of planted native species. As for the animals in the site, 5 species in 3 families of reptiles and amphibians, 34 species of 23 families of birds, and 3 species in 2 families of mammals were monitored, indicating that the bio-diversity of the site has improved, as well.

Purpose: Multiple symmetric lipomatosis (MSL) is a relatively rare disorder characterized by presence of multiple, symmetric, nonencapsulated fat masses in face, neck, shoulder and other areas. There has been only a few cases reported in Korea. The main purpose of this research is to examine the Korean patients to see what kinds of special characteristics occurred due to this disease and to decide the proper treatment.Methods: A total of 16 patients were evaluated retrospectively. 5 patients were treated at our hospital. The other patients were reviewed from literature. We analyzed the biological characters of patients, location of fat deposit, morphologic characters of patients, clinical evidence of neuropathy, associated metabolic disorders and treatment modality.Results: All cases were male patient. The mean age of onset was 47.43 years. All patients were moderate to heavy alcoholics. The most common location of fat deposition was posterior neck and abdomen. In neurologic exam of 9 patients, 5 patients showed muscle weakness, tremor, pain and autonomic nerve dysfunction. In metabolic studies of 9 patients, total cholesterol values were higher in 1 patient. A glucose tolerance test was abnormal in 1 patient. In treatment modality, 14 patients were treated with surgical resection, 1 patient was treated with liposuction and surgical excision, 1 patient was treated only with liposuction. Conclusion: To treat MSL patients successfully, we should concentrate not only on the removal of the fatty tissue but also on neurologic abnormities, metabolic disorders and associated diseases.
The purpose of this study is to effectively treat low back pain for the out patients who have been treated at the oriental hospital. Sorting the low back pain patients into four types based on scapular and ilium tilting hyper/hypo mobility characteristics, it has used several kinds of acupuncture stimuli on the 4 motor points of limb skeletal muscles, analyzing the acupuncture treatment on the 12 acupuncture points with which oriental hospitals has given to the low back pain patients and comparing with this effect. It has been analyzed how effectively the acupuncture stimuli has been contributed to the low back pain patients who suffered from skeletal muscles of spinal limb thorax abdominal and spinal skeletal system. The results are as follows. 1. There has been changes after treatment in both the control and experimental groups for low back pain patients, but the latter has been more increased in changes than the former. 2. There were much effects on the balanced restorations for spinal skeletal muscle in both the control group and experimental one, but on terms of restoration degree, the degree of the experimental group was higher than that of the control group. 3. Both the contrastive treatment and the experimental one have been contributed to the balanced restoration for all of the limb thorax abdominal muscles, but there were a few differences in terms of the way for restoration and the effects for improvement according to body types. 4. On both the control and the experimental group, spinal skeletal system has been restored with balance, but the experimental group has been higher effective on the balanced restoration than the one of the control group. Considering these results both the control and the experimental treatments have been contributed to the balanced restorations for all of low back pain patients who were suffering from skeletal muscles of spinal limb thorax abdominal and spinal skeletal system, but the contribution degree for the experimental group has been higher than that of the control group.
Purpose: The rate of fistulas occuring followed by resection of oral cavity, oropharyngeal, hypopharyngeal, and laryngeal cancer are reported to be 9 ~ 23% according to various documents. Neglected treatment of the fistula can result in a setback in proper treatment with restrictions in oral intake leading to delayed return to daily life. Furthurmore, in severe cases, it may injure important vessels and adjacent structures of the neck area. The author reviewed previously reported cases of treatment methods for fistulas recurring after diverse head and neck operations and with sharing the treatment experiments of our patients, we tried to present a treatment algorism for different fistula types. Methods: Our study was based on retrograde analysis of 64 patients who were clinically diagnosed with fistula after operation for cancer of the head and neck from 1997 to 2008 at Severance Hospital. Their primary sites of cancer were 8 oral cavity, 22 oropharynx, 25 hypopharynx, and 9 larynx. The patients were aged 45 to 75 years and the male to female ratio was 11 to 1. The patient's operation records and progress notes were evaluated for determination of degree of fistula and treatment methods. Results: Most fistulas were clinically suspected after postoperative 5 days and symptoms noted for detection of the fistula were erythema, purulent discharge, edema, tenderness, and fluctuation. The fistula was definitely diagnosed at postoperative 2 weeks with barium test and treatment method ranging from conservative management to operative procedure were applied to each patients. Total 21 patients were managed with conservative protocol. In 15 cases, direct repair of the fistula was done and more stable repair of the fistula was possible with using of TachoComb$^{(R)}$. Pharyngostoma was performed in 14 patients. Among them, 4 patients healed spontaneously, 5 patients were taken direct closure, 4 patients were taken pectoralis major musculocutaneous flap, and one patient was taken esophageal transfer. The other 14 patients were taken 11 pectoralis major musculocutaneous flaps and 3 free flaps without pharyngostoma formation. Conclusion: Fistula is a troublesome complication resulting after resection of head and neck cancer. Early detection and adequate treatment according to the period and condition of the fistula may prevent further complications and reduce the pain of the patient.

Immediate implantation is widely applied as it is recognized as a useful implant treatment in dental clinic due to its various advantages as well as being able to shorten the treatment period. Immediate implant treatment was performed on 5 patients who needed extraction of the mandibular first molar for various reasons, shortening the overall treatment period, and obtaining satisfactory results. Immediate implantation after extraction is a high-level treatment that requires proper initial fixation at the exact location of the extraction socket and the surrounding soft tissues, but is considered to be a clinically very useful prosthetic restoration method
This case report describes a technique in which endodontic treatment and permanent indirect restoration were completed in the same clinical appointment with the aid of a computer-aided design/computer-aided manufacturing (CAD/CAM) system. Two patients were diagnosed with irreversible pulpitis of the mandibular first molar. After access preparation, root canals were located, irrigation was performed until bleeding ceased, and the coronal tooth structure was prepared for indirect restoration. Then, utilizing an interim 3-mm build-up of the endodontic access cavity, a hemi-arch digital scan was performed with an intraoral scanner. Subsequent to digital scanning, restoration design was performed simultaneously with the endodontic procedure. The root canals were shaped using the Race system under irrigation with 2.5% sodium hypochlorite followed by root canal filling. The pulp chamber was subsequently filled with a 3-mm-thick composite resin restoration mimicking the interim build-up previously utilized to facilitate block milling in the CAD/CAM system. Clinical try-in of the permanent onlay restoration was followed by acid etching, application of a 5th generation adhesive, and cementation of the indirect restoration. Once the restoration was cemented, rubber dam isolation was removed, followed by occlusal adjustment and polishing. After 2 years of follow-up, the restorations were esthetically and functionally satisfactory, without complications.
