Clopidogrel Response Variability in Unruptured Intracranial Aneurysm Patients Treated with Stent-Assisted Endovascular Coil Embolization : Is Follow-Up Clopidogrel Response Test Necessary?

  • Objective : The purpose of this study was to analyze the variability of clopidogrel responses according to duration of a clopidogrel drug regimen after stent-assisted coil embolization (SAC), and to determine the correlation between the variability of clopidogrel responses and thromboembolic or hemorrhagic complications. Methods : A total of 47 patients who underwent SAC procedures to treat unruptured intracranial aneurysms were enrolled in the study. Preoperatively, patients received more than seven days of aspirin (100 mg) and clopidogrel (75 mg), daily. P2Y12 reaction unit (PRU) was checked with the VerifyNow test one day before the procedure (pre-PRU) and one month after the procedure (post-PRU). PRU variability was calculated as the difference between the initial response and the follow-up response. Patients were sorted into two groups based on their response to treatment : responsive and hypo-responsive. Results : PRU variability was significantly greater in the hypo-responsive group when compared to the responsive group (p=0.019). Pre-PRU and serum platelets counts were significantly correlated with PRU variation (p=0.005 and p=0.004, respectively). Although thromboembolic complication had no significant correlated factors, hemorrhagic complication was correlated with pre-PRU (p=0.033). Conclusion : In conclusion, variability of clopidogrel responses during clopidogrel medication was correlated to serum platelet counts and the initial clopidogrel response. Thromboembolic and hemorrhagic complications did not show correlation with the variability of clopidogrel response, or the clopidogrel response after one month of medication; however, hemorrhagic complication was associated with initial clopidogrel response. Therefore, it is recommended to test patients for an initial clopidogrel response only, as further tests would be insignificant.

  • With the aid of finite element method, this paper deals with the problem of structural response variability of transmission tower subjected to the spatial variability of material properties, Young's modulus herein. The spatial variability of material property are modeled as two-dimensional stochastic field which has an isotropic auto-correlation function. Response variability has been computed based on two numerical techniques, such as the Neumann expansion method in conjunction with the Monte Carlo simulation method. The results by these numerical methods are compared with those by the deterministic approach.

  • The structures have intrinsic uncertainties in analysis/design parameters contrary to the assumptions of perfect constant over the structural domain. The material and geometrical parameters are the exemplary ones. The influences of uncertainties in Young's modulus, which are the representative random design variables, on the structural response have been the center of focus in the realm of stochastic analysis. In this study, a formulation to obtain the response variability due to the randomness in the Poisson's ratio is given. In that the previous researches in the literature deal with the response variability due mainly to the uncertainty in the elastic modulus, with the results of this research, it can be asserted to obtain the response variability taking into consideration of uncertainties in all the material constants becomes possible.

  • The objective of this study is to evaluate the liquefaction cyclic shear stress ratio considering the soil uncertainty. In this study, the probabilistic ground response analysis and the cyclic shear stress ratio analysis for the liquefaction potential evaluation are performed considering the soil variability. The statistical properties of input ground parameters were analyzed to investigate the parameters affecting the seismic response analysis. The Probabilistic analysis was carried out by Monte Carlo Simulation method. The ground response analysis was performed considering the soil variability and the probability distribution characteristics of the ground acceleration. The probability distribution of the peak ground acceleration by seismic characteristics was presented. The differences of liquefaction shear stress ratio results according to soil variability were compared and analyzed. The maximum acceleration of the ground by the deterministic method was analyzed to be overestimation of the ground amplification phenomenon. Also, the shear stress ratio was overestimated.

  • Dual response surface optimization (DRSO) attempts to optimize mean and variability of a process response variable using a response surface methodology. In general, mean and variability of the response variable are often in conflict. In such a case, the process engineer need to understand the tradeoffs between the mean and variability in order to obtain a satisfactory solution. Recently, a Posterior preference articulation approach to DRSO (P-DRSO) has been proposed. P-DRSO generates a number of non-dominated solutions and allows the process engineer to select the most preferred solution. By observing the non-dominated solutions, the DM can explore and better understand the trade-offs between the mean and variability. However, the non-dominated solutions generated by the existing P-DRSO is often incomprehensive and unevenly distributed which limits the practicability of the method. In this regard, we propose a modified P-DRSO using multiple objective genetic algorithms. The proposed method has an advantage in that it generates comprehensive and evenly distributed non-dominated solutions.

  • Great inter-variability in drug response and adverse drug reactions is related to inter-variability of drug bioavailability, drug interaction and patient's disease and physyological state that cause change in absorption, distribution, metabolism and excretion of drugs. However, these alone do not sufficiently predict and explain inter-variability in drug response. In recent studies, it is reported that inter-variability in drug response and adverse drug reactions may largely resulted from genetically determined differences in drug absoption, distribution, metabolism and drug target proteins. Especially, the major human drug-metabolizing enzymes such as CYP450, N-acetyl tranferase, thiopurine S-methyl transferase, glutathione S-transferase are identified as the major gene variants that cause inter-individual variability in drug's response and adverse drug reactions. These variations may have most significant implications for those drugs that have narrow therapeutic index and serious adverse drug reactions. Therefore, the genetic variation such as polymorphisms in drug metabolizing enzymes can affect the response of individuals to drugs that are used in the treatment of depression, psychosis, cancer, cardiovascular disorders, ulcer and gastrointestinal disorders, pain and epilepsy, among others. This review describes the pharmacogenomics of the drug metabolizing enzymes associated with the drug response and its clinical applications.

EFFECTS OF MECHANICAL PROPERTY VARIABILITY IN LEAD RUBBER BEARINGS ON THE RESPONSE OF SEISMIC ISOLATION SYSTEM FOR DIFFERENT GROUND MOTIONS

  • The effects of variability of the mechanical properties of lead rubber bearings on the response of a seismic isolation system are investigated. Material variability in manufacturing, aging, and operation temperature is assumed, and two variation models of an isolation system are considered. To evaluate the effect of ground motion characteristics on the response, 27 earthquake record sets with different peak A/V ratios were selected, and three components of ground motions were used for a seismic response analysis. The response in an isolation system and a superstructure increases significantly for ground motions with low A/V ratios. The variation in the mechanical properties of isolators results in a significant influence on the shear strains of the isolators and the acceleration response of the superstructure. The variation provisions in the ASCE-4 are reasonable, but more strict variation limits should be given to isolation systems subjected to ground motions having low A/V ratios. For application of seismic isolation systems to safety-related nuclear structures, the variation in the material and mechanical properties of the isolation system should be properly controlled during the manufacturing and aging processes. In addition, special consideration should be given to minimize the accidental torsion caused by the dissimilarity in the stiffness variations of the isolators.

요가 프로그램이 전문직 종사자의 스트레스에 미치는 효과 (The Effect of a Yoga Program on the Stress Levels of Professionals)

  • Purpose: The aim of this study was to examine the effects of a yoga program on perceived stress, stress response and heart rate variability in professionals. Methods: The research design was a quasi-experimental intervention study. The participants were 47 adults with professional jobs consisting of an experimental group with 23 adults and a control group with 24 adults. The duration of the yoga program was eight weeks from July 4 to August 30, 2012. Results: There were significant decreases in the stress response score after the yoga program. No significant changes in perceived stress and heart rate variability were observed between the experimental group and control group. However, there were significant decreases in perceived stress, stress response scale, and heart rate variability for some participants whose stress levels were high. Conclusion: An eight-week-long yoga program could be considered as an intervention to decrease the stress response of professionals even though it might not reduce their perceived stress and heart rate variability. To further analyze the effects of a yoga program in professionals, changes in intervention strength (such as longer periods and frequency) and the development of physiological measures, which would show the co-relationship between perceived stress and physiological response, are necessary in future studies.

Probabilistic seismic assessment of structures considering soil uncertainties

For this, response sensitivity of two 4- and 12-story RC shear walls to the soil properties uncertainty by considering soil structure interaction (SSI) effects is investigated. Beam on Nonlinear Winkler Foundation (BNWF) model is used for shallow foundation modeling and the uncertainty of soil properties is expanded to the foundation stiffness and strength parameters variability. Monte Carlo (MC) simulation technique is employed for probabilistic evaluations. By investigating the probabilistic evaluation results it's observed that as the soil and foundation become stiffer, the soil uncertainty is found to be less important in influencing the response variability. On the other hand, the soil uncertainty becomes more important as the foundation-structure system is expected to experience nonlinear behavior to more sever degree. Since full probabilistic analysis methods like MC commonly are very time consuming, the feasibility of simple approximate methods' application including First Order Second Moment (FOSM) method and ASCE41 proposed approach for the soil uncertainty considerations is investigated. By comparing the results of the approximate methods with the results obtained from MC, it's observed that the results of both FOSM and ASCE41 methods are in good agreement with the results of MC simulation technique and they show acceptable accuracy in predicting the response variability.

An Alternative Optimization Procedure for Parameter Design

  • Taguchi has used the signal-to-noise ratio (SN) to achieve the appropriate set of operating conditions where variability around target is low in the Taguchi parameter design. Taguchi has dealt with having constraints on both the mean and variability of a characteristic (the dual response problem) by combining information on both mean and variability into an SN. Many Statisticians criticize the Taguchi techniques of analysis, particularly those based on the SN. In this paper we propose a substantially simpler optimization procedure for parameter design to solve the dual response problems without resorting to SN.
