Fecal Respiratory Viruses in Acute Viral Respiratory Infection and Nasopharyngeal Diarrheal Viruses in Acute Viral Gastroenteritis: Clinical Impact of Ectopic Viruses Is Questionable

  • Our aim was to determine the detection rate of respiratory viruses (RVs) in feces of patients with acute viral respiratory infection (AVRI) and the detection rate of diarrheal viruses (DVs) in nasopharyngeal samples from patients with acute viral gastroenteritis. The relationships between the presence of fecal RVs or nasopharyngeal DVs and their impacts on the clinical severity were also investigated. A total of 144 fecal specimens were collected from AVRI patients and 95 nasopharyngeal specimens were collected from acute viral gastroenteritis patients. Clinical characteristics and laboratory profiles were compared between subgroups on the basis of the presence or absence of virus in the specimens. The detection rate of RVs in feces was 17.4% (25/144), whereas the detection rate for viruses identical to the respiratory pathogen was 10.4% (identical group, 15/144). Within the identical group, adenovirus (86.7%, 13/15) was most commonly found. Patients in the identical group showed statistically higher values for C-reactive protein, mean age, increased frequency of vomiting, and decreased frequency of chest film involvement and cough (p < 0.05). The detection rate of nasopharyngeal DVs among acute viral gastroenteritis patients was 19.0% (18/95), and in the identical group it was 15.8% (15/95). Norovirus group II and enteric adenovirus were the major pathogens detected in the identical group. There were no significant differences in clinical characteristics and laboratory profiles between the subgroups. In conclusion, the major pathogens of fecal RV and nasopharyngeal DV were adenovirus and norovirus group II, respectively. However, their relationship with the clinical symptoms or disease severity is unclear.

대구지역 단일병원에서 입원 환아의 호흡기 바이러스 역학 및 임상 양상: 2010-2012년 (Epidemiology and Clinical Features of Respiratory Viruses in Pediatric Inpatients in a Single Medical Center in Daegu from 2010 to 2012)

  • Background: This study was performed to investigate the epidemiologic and clinical features of acute respiratory viral infection in hospitalized children. Methods: From 2010 to 2012, we tested nasopharyngeal swab specimen in 1,584 hospitalized children with multiple real-time polymerase chain reactions to identify 10 kinds of respiratory viruses (including influenza virus A, B (FluA, FluB), respiratory syncytial virus (RSV), human metapneumovirus (MPV), adenovirus (AdV), human coronavirus (CoronaV), human enterovirus (HEV), human bocavirus (HBoV), parainfluenza virus (PIV), and human rhinovirus (Rhinovirus)). We analyzed the positive rate, annual and seasonal variations, and clinical features (respiratory tract and non-respiratory tract) according to the retrospective review of medical records. Results: Respiratory viruses were detected from 678 (42.8%) of 1,584 patients. The most common detected virus was RSV (35.0%), and then AdV (19.0%), HEV (18.1%). The critical period of the respiratory viral infection was during the first 12 months of a child's life. PIV increased by 8.4%, 12.1%, and 21.1% annually. Bronchiolitis was most frequently caused by RSV, and croup was frequently caused by PIV. The most common cause of meningitis was HEV. Hepatitis-associated respiratory virus was developed 111 in 678 cases. Conclusion: Although this study was confined to a single medical center for three years, we identified the epidemiology and clinical feature of respiratory viruses in Daegu from 2010 to 2012. Future surveillance will be necessary for annual and seasonal variations.

  • Avian influenza viruses playa crucial role i,n the creation of human pandemic viruses. In this study, we have demonstrated that both human and avian influenza receptors exist in cells in the respiratory and intestinal tracts of chickens. We have also determined that primarily cultured chicken lung cells can support the replication of both avian and human influenza viruses.

Blood test results from simultaneous infection of other respiratory viruses in COVID-19 patients

  • Since 2019, severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) has spread rapidly, infecting millions of people worldwide. On March 11, 2020, the World Health Organization declared coronavirus disease (COVID-19) a pandemic owing to the worldwide spread of SARS-CoV-2, which created an unprecedented burden on the global healthcare system. In this context, there are increasing concerns regarding co-infections with other respiratory viruses, such as the influenza virus. In this study, clinical data of patients infected with SARS-CoV-2 and other respiratory viruses were compared with patients infected with SARS-CoV-2 alone. The hematology and blood biochemistry results of 178 patients infected with SARS-CoV-2 , who were tested on admission, were retrospectively reviewed. In patients with SARS-CoV-2 and adenovirus co-infection, C-reactive protein levels were elevated on admission, whereas lactate dehydrogenase (LDH), prothrombin time, international normalized ratio, activated partial thromboplastin clotting time, and bilirubin values were all within the normal range. Moreover, patients with SARS-CoV-2 and human bocavirus co-infection had low LDH and high bilirubin levels on admission. These findings reveal the clinical features of respiratory virus and SARS-CoV-2 co-infections and support the development of appropriate approaches for treating patients with SARS-CoV-2 and other respiratory virus co-infections.

Detection of Respiratory Viral Pathogens and Mycoplasma spp from Calves with Summer Pneumonia in Korea

  • Respiratory pathogens of calves including bovine parainfluenza type 3 virus (BPI3V), bovine respiratory syncytial virus (BRSV), infectious bovine rhinotracheitis virus (IBRV) and Mycoplasma spp is well-known for winter pathogens. However, there are no studies about summer pneumonia pathogens of calves in Korea. The aim of this study was to detect respiratory pathogens from calves with summer pneumonia. Eighty calves from 5 regions were chosen and their nasal swabs were used to detect respiratory pathogens with real-time PCR. Mycoplasma spp was major primary respiratory pathogens in calves with summer pneumonia. Although, the detection rates of respiratory viruses were very low, serological assays showed that respiratory viruses exist widely in farms.

소아 크룹 환자들의 원인 바이러스에 따른 임상 양상과 입원 경과 비교 (Clinical presentation of croup in children according to causative viruses)

  • Purpose: We evaluated the clinical features of croup in children according to viral etiology. Methods: This study enrolled pediatric patients with croup, who showed positive results on respiratory virus reverse transcriptase polymerase chain reaction performed between January 2012 and December 2017. We retrospectively reviewed the medical records. Results: A total of 179 patients (119 boys and 60 girls) were enrolled with the mean age of $18.9{\pm}14.7$ months. The viruses commonly identified were parainfluenza, respiratory syncytial virus, rhinovirus, and influenza. Among these 4 viruses, patients with rhinovirus infection showed significantly shorter fever and admission durations. Patients with parainfluenza infection showed significantly lower incidences of epinephrine nebulization and patients with influenza infections showed significantly higher incidences of steroid treatment. Conclusion: Clinical manifestations of croup differ according to causative viruses. Further studies should be conducted to evaluate the severity and prognosis of croup according to viral etiology.

서울의 2008-2009년 가을, 겨울철에 유행한 소아의 하기도 호흡기 바이러스 병원체의 규명 (Identification of Viral Pathogens for Lower Respiratory Tract Infection in Children at Seoul During Autumn and Winter Seasons of the Year of 2008-2009)

  • 목 적 : 서울 지역의 급성 호흡기 감염의 원인이 되는 바이러스를 분석하고 임상적 특징과 비교, 분석하였다. 방 법: 2008년 9월부터 2009년 3월까지 3개 대학병원에 입원한 5세 미만의 소아의 급성하기도감염환자에서 418개의 비인두흡입검체를 채취하여, 14종 바이러스를 multiplex PCR을 통한 진단하였다. 바이러스 양성환자의 임상정보를 병원별, 시기별로 분석하였다. 결 과: 418명이 분석되었고, 연령은 평균 16.4개월, 성별비는 1.36대 1이었다. 환자들 중 56.2%에서 바이러스 양성을 보였고, RSV (35%)가 가장 많이 검출된 바이러스였으며, 다음은 hRV (22%)였으며, HBoV, ADV, hMPV, PIV1, IFV, hCoV, PIV2, PIV3가 검출되었다. 양성검체 중 21.9%에서 동시감염이 있었다. 결 론:바이러스의 지역별, 시기별 지속적인 감시가 필요하며, 급성호흡기질환을 보이는 소아의 진료에, 유행하는 바이러스의 특징을 고려해야 할 것이다.

소아 보카바이러스 호흡기 감염증의 역학적 특징 (Epidemiologic Characteristics of Human Bocavirus-Associated Respiratory Infection in Children)

  • 목 적 : 본 연구는 소아에서 보카바이러스와 연관된 하기도 감염증의 역학적인 특성에 대해 알아보고자 하였다. 방 법: 2000년 3월부터 2005년 9월까지 서울대학교 병원과 분당 서울대학교병원에 하기도 감염증으로 입원한 658명의 소아에게서 비인두 흡입 검체를 채취하였다. 다중 역전사 중합효소 연쇄 반응을 이용하여 11종의 바이러스를 조사하였고, 의무기록을 후향적으로 분석하였다. 결 과:전체 호흡기바이러스는 658명의 환아 중 325명(49.4%)에서 검출되었다. 보카바이러스는 62명(9.4%)에서 검출되었으며, 3개월 이상 5세 미만에서 많았다. 보카바이러스가 중복감염이 확인된 것은 37.1%로, 전체 바이러스 양성 환자들의 중복감염률(12.3%)보다 의미 있게 높은 것으로 확인되었다(P =0.000). 또한 늦은 봄부터 이른 여름인 5월부터 7월에 많이 검출되었다. 결 론 : 보카바이러스는 국내에서 소아의 하기도 감염증에서 유병률이 높았고, 특히 늦은 봄부터 초여름에 유행하였다.

국내 영유아에서의 Chlamydia trachomatis 호흡기 감염 (Chlamydia trachomatis respiratory infection in Korean young infants)

  • 목 적 : C. trachomatis는 성인의 성 매개성 질환중의 하나로 감염된 산모로부터 수직 전파에 의해 영유아에서 호흡기 감염과 결막염을 일으킨다. 영유아에서 호흡기 감염의 원인으로 C. trachomatis외에도 호흡기 바이러스가 흔한 원인이 되고 있어 최근 영유아에서의 C. trachomatis에 의한 호흡기 감염의 임상양상과 빈도를 알아보고 호흡기 바이러스와의 중복감염에 대해 알아보고자 하였다. 방 법 : 2002년 1월에서 2007년 7월까지 단국대학교병원에 호흡기 감염의 증상으로 입원한 6개월 이하의 환아를 대상으로 하였고 이들에게서 입원 2일 이내 인후 면봉법 혹은 비인두 흡인으로 채취한 객담검체에서 PCR에 의한 Chlamydia PCR 검사를 시행하였다. 또한 Chlamydia 양성인 환아의 $-70^{\circ}C$ 냉동고에 보관되어 있던 비인두 흡인 검체에서 호흡기 바이러스에 대한 multipex PCR 검사를 시행하였다. 의무기록을 후향적으로 검토 분석하였다. 결 과 : 5년 6개월의 연구기간동안 C. trachomatis 항원에 대한 PCR 검사를 시행한 환아 690명으로 그중에서 36명(5.2%)에서 양성을 보였다. Chlamydia 양성인 36명의 환아를 보면 남아가 28명이었고 이들의 평균 나이는 45.4일(12-183일) 이었으며 36명중 30명이 자연분만이었고 제왕절개가 6명 있었다. 36명중 26명에서 호흡기 바이러스를 검사를 시행하였으며 이중 12명에서 호흡기 바이러스가 검출되었다. 검출된 호흡기 바이러스의 종류를 보면 RSV 6명, RV 2명, 그 외 IFV A, PIV 3, AdV, CoV OC43, HMPV가 각각 한명에서 검출되었다. Chlamydia PCR 양성인 환아의 임상양상을 보면 기침, 수포음, 흉부견축, 빈호흡을 보였고 7%에서 발열 증상이 있었다. 호흡기 바이러스와 중복감염이 있는 12명의 환아와 비교한 결과 발열과 수포음이 의미있는 차이를 보였으며 혈액 검사상 호흡기 바이러스 중복감염이 있는 환아에서 CRP의 증가로 의미있는 차이를 보였다. 결 론 : C. trachomatis에 의한 호흡기 감염은 자연분만이 아닌 환아에서도, 또한 생후 6개월인 환아에서도 감염을 보였으며 46%에서 호흡기 바이러스와 중복감염을 보였다. 호흡기 바이러스와 중복감염이 있는 경우 RSV가 가장 많은 빈도를 보였다. C. trachomatis에 의한 호흡기 감염에서 발열을 동반하거나 CRP가 높은 경우 호흡기 바이러스와의 중복감염을 의심할 수 있다. 생후 6개월 이하의 환아에서 호흡기 감염의 원인으로 C. trachomatis의 감염을 고려해야 하며 호흡기 바이러스와의 중복감염에 대한 지속적인 연구가 필요하다고 생각된다.

Serological characterization of bovine viral diarrhea virus isolates

  • Bovine viral diarrhea viruses (BVDVs) were isolated from cattle with respiratory and diarrhea signs as well as persistently infected cattle. These isolates were analysed serologically to characterize serogroups and to compare serological relationship with reference viruses of type I and II. Most isolates from calf diarrheal cases and persistently infected individuals showed a significant difference in cross-neutralization test with the viruses isolated from nasal discharges showing severe respiratory signs. Serologically most of the commercial vaccine strains could be classified into classical BVDV (type I) such as NADL strain. This serological difference among BVDV isolates suggested the need for new vaccines to protect cattle from both respiratory and enteric BVDV infections in field. The immunogenicity of BVDVs which showed a good propagation capability in MDBK cells and high rates of neutralizing activity (isolate : KD26-1, PHG, B5 and 95002) against all viruses used in this study, was confirmed in guinea pig when treated as single or combined groups.
