Midline Granulcma(Malignant midline reticulosis) of upper respiratory tract was known as one of the fatal diseases that involve the nasal cavity, paranasal sinuses, pharynx and larynx, with progressive localized necrotizing inflammation and granuloma formation. The disease was first described in 1897 by McBride and after Stewart many clinical and histological studies have been done in 1933. But its etiology is still unclear and therapeutic methods are still under development. The authors carried out a clinical study of 15 cases which were diagnosed and treated as midline granuloma, including 7 cases which received combination radiation and steroid therapy from January 1964 to December 1980. The results are as follows: 1) Age and Sex distribution: 6 cases fell into the ages from 30 to 39 years: 13 cases (87%) were male and 2 cases (13%) were female. 2) Primary lesion sites were nasal cavity and paranasal sinuses area (8 cases: 53%), palatine tonsil (3 cases: 20%) and pharynx (2 cases: 13%) in order. 3) Common symptoms were nasal stuffiness and sore throat (6 cases: 40%), headache (5 cases: 33%), nasal discharge and facial edema (3 cases: 20%) in order. 4) Microorganisms were identified in 4 cases: staphylococcus in 2 cases, pseudomonas in 1 case and streptococcus in 1 case. 5) All 7 cases who received combination radiation and steroid therapy revealed complete regression and no recurrence. 6) Among the 5 cases, who received steroid and antibiotic therapy, 2 cases died and other 3 cases improved temporarily and then developed aggrevation of symptoms.
Objectives: This study aims to identify the influential factors on the pulmonary function of targeted general residents in the areas surrounding Gwangyang Steel Mill. Methods: An PFT (Pulmonary Function Test) was conducted from May 2007 to November 2007 on 974 target residents (438 male, 536 female), including an exposed group (674 people) who resided within a radius of 5 km from Gwangyang Steel Mill in Gwangyang-si, Jeollanam-do and a control group (300 people) who resided outside a radius of 15 km. A survey related to personal characteristics, life habits, respiratory diseases and allergic symptoms, medical histories and living environments of the residents was also conducted to identify influential factors on pulmonary function. Results: As %$FEV_1$ and %FVC of the exposed group are 99.17% and 96.98%, respectively, and those of the control group are 105.47% and 101.91%, respectively, with the PFT values of the exposed group being lower than those of the control group (p<0.001), it turns out that the pollution in the industrial complex is likely to trigger a reduction in the pulmonary function of local residents. The odds ratio analysis result for asthma diagnosis history indicates that the odds ratios tend to be higher among surveyed residents who are living near above-average traffic volumes and are using beds, where it is statistically meaningful that especially the odds ratios are higher if the residences are closer to roads (p<0.01, CI=1.12-4.52). The factors that affect the FEV1 are identified as smoking, passive smoking, asthma diagnosis history, nasal congestion symptoms and allergic eye disease ($R^2$=0.154, p<0.001). The factors that affect the FVC are identified as smoking, passive smoking, asthma diagnosis history and allergic coryza ($R^2$=0.158, p<0.001). In addition, the analysis result for the relevance of air pollution levels to pulmonary function ($FEV_1$, FVC) shows that FEV1 and FVC tend to decrease as the concentration of $O_3$ increases. Conclusions: We believe that this study may provide preliminary data for the development of preventive measures for health effects on the residents and environmental health control measures for environmental pollution in the industrial complex area.

Background and Objectives: Sleep disturbances and excessive daytime sleepiness (EDS) are the major symptoms of obstructive sleep apnea (OSA). This study aimed to investigate clinical implications of insomnia and EDS in patients with OSA using the Pittsburgh Sleep Quality Index (PSQI) and Epworth Sleepiness Scale (ESS). Materials and Method: We evaluated 131 subjects with suspected OSA who were undergoing polysomnography (PSG) and performing the PSQI and ESS surveys. OSA was diagnosed when the apnea-hypopnea index was five or more. EDS was defined when ESS score was 11 points or higher. Detailed history and questionnaire were used to categorize insomnia. We compared clinical variables and PSG results in subgroups with or without insomnia and EDS. Results: There were no significant differences of PSQI and ESS score between controls and OSA. OSA with insomnia had significantly increased total score (p<0.001) and decreased total sleep time (p=0.001) and sleep efficiency (p=0.001) on the PSQI compared to those without insomnia. OSA with EDS showed significantly increased PSQI score (p=0.022) and decreased total sleep time (p=0.018) on PSG compared to those without EDS. Neither PSQI nor ESS score had a correlation with respiratory variables such as AHI and oxygen saturation. Total sleep time had a significant effect on both insomnia and EDS in patients with OSA. Conclusion: Decreased total sleep time had important effects on subjective symptoms of OSA and comorbid insomnia. Therefore, restoration of decreased sleep time is important in the management of OSA.
Experimental studies on the water content of muscles and the histological changes of muscles, digestive tract and kidney, influenced by forced oral administration of excessive tap water, were performed by using 10 Korean bulls. Experimental animals were divided into two groups, five heads for test group and five for control group. All of the bulls used in this experiment were weighing between 250 kg and 300 kg and 3 and 4 years of age. In each test animal, a total of 140 litters of tap water was passed in the forestomach by means of catheterization within 12 to 18 hours. And each of them was slaughtered immediately after the animals showing symptoms of respiratory distress. In control group, the animals were allowed to drink tap water normally. From test and control animals after slaughter, each 10 gm of M. biceps femoris, M. satorius, M. adductor, M. gluteous supercialis, M. iliocostalis lumborum, and M. transversus costarum were taken from definite parts in order to measure water content. In the histopathological studies tissues of rumen, reticulum, omasum, abomasum, duodenum, colon and kidney were taken as wall as the above mentioned muscles. The results obtained were as follows; 1. Increase of water content in the muscles of test group were 4.6% in M. satorius, 4.24% in M. transversus costarum, 4.14% in M. gluteous supercialis, 4.02% in M. adductor, 3.88% in M. biceps femoris, and 2.46% in M. iliocostalis lumborum respectively. The highest increase was found in M. satorius and the lowest in M. iliocostalis lumborum. 2. In test animal, average increasing value of water content in muscles was 3.9% and shown highly significant (p<0.01). 3. On the microscopical findings of carcass, marked watery edema was observed all of the subcutaneous tissues and intermuscular connective tissues. 4. Microscopically, the epimysium, perimysium and endomysium were widened, and the muscle fibers were lacerated. The cells of stratum lucidum in the epithelium of rumen and reticulum were shown marked vacuolization. In the kidney, dilatation of Bowman's spaces and proximal tubles was observed.
Influenza A viruses are circulating in a variety of hosts, including humans, pigs, and poultry. Swine influenza virus is a zoonotic pathogen that can be readily transmitted to humans. The influenza viruses of the 2009 H1N1 pandemic were derived from swine influenza viruses, and it has been suggested that the 1957 H2N2 pandemic and the 1968 H3N2 pandemic both originated in pigs. Pigs are regarded as a mixing vessel in the creation of novel influenza viruses since they are readily infected with human and avian influenza viruses. We isolated three novel H1N2 influenza viruses from pigs showing respiratory symptoms on a Korean farm in 2019. These viruses were reassortants, containing PA and NP genes from those of the 2009 H1N1 influenza virus in addition to PB2, PB1, HA, NA, M, and NS genes from those of triple-reassortant swine H3N2 and classical swine H1N2 influenza viruses circulating in Korean pigs. Mice infected with the isolated H1N2 influenza virus lost up to 17% body weight and exhibited interstitial pneumonia involving infiltration of many inflammatory cells. Results suggest that close surveillance to detect emerging influenza viruses in pigs is necessary for the health of both pigs and humans.
Objectives: To present the case of a 76-year-old man diagnosed with idiopathic interstitial pneumonias (IIPs) treated exclusively with traditional Korean medicine (TKM). Methods: The patient was treated with the revised Sasammaekmundong-tang (rSMT), acupuncture, and pharmacopuncture from September 2013 to May 2014 in our outpatient clinic. We prescribed rSMT three times a day for 4 months. Acupuncture and pharmacopuncture were performed twice a week for 9 months. To evaluate therapeutic efficacy, we recorded the patient's chief complaints at each visit, and pulmonary function test was performed at intervals of three months. Chest radiography and chest computed tomography were performed before and after treatment, respectively. Results: During the course of the treatment, dyspnea, cough, sputum, and overall pulmonary function improved. Conclusions: We suggest that traditional Korean medicine (TKM) for IIPs patients might be effective in the control of the main symptoms of IIPs and in the recovery of respiratory function.
The work of hairdressers includes washing, coloring, bleaching, permanent waving, conditioning, and cutting hair. Hairdressers are subjected to a number of physical and toxicological hazards. The toxicological hazards are those resulting from exposure to a wide range of chemicals that are usually classified active processes. In this study, twenty beauty shops were selected to assess the exposure to indoor air pollutants such as VOCs and particulate matter $(PM_{10})$ during one month from September 1 to September 30, 2003. Indoor air quality of beauty shops might be worse by vehicle emissions because the beauty shops were generally located near roadways. Personal exposures to VOCs and $PM_{10}$ were related to indoor concentrations of beauty shops. According to the questionnaire, hairdressers complained of sore throat, eye irritation, and nervousness as physical symptoms. The measured mean concentrations of respiratory particulates were $30.5ng/m^3$ in indoor, $30.5ng/m^3$ in outdoor and $44.0ng/m^3$ on personal levels. The personal concentration was found higher than indoor and outdoor concentrations. The heavy metals mean concentrations were shown as indoor (Na>Zn>Cr), outdoor (Cr>Zn>Pb), and personal (Na>Cr>Zn) levels. Conclusively, customers as well as workers in the beauty shops might be highly exposed to air pollutants from indoor and outdoor sources. Therefore, proper management should be taken to improve the indoor air quality in beauty shops.
Air pollutants were measured to get useful data in preventing and controlling the pollution at industrial and semi-industrial, commercial, cross-road and residential areas by season in Busan from September 1973 to June 1974. Teated were dustfalls(ton/$km^2$/month)by Deposit Gauge method, sulfuric anhydride(mg $SO_3/day/100cm^2 PbO_2$) by Lead Peroxide candle method. The relations between pollution and metherologlcal factors and source of pollution were discussed, The findings are as follows: 1. The mean value of dustfalls was 24.8 tons rangi~ng from 12.5 tons to 44.5 tons. The highest amount of dustfalls of 29.0 tons was measured in crossroad areas while the 1oeest of 22.7 tons in commercial areas, and Winter the highest of 27.7 tons while in Spring the lowest of 21.2 tons. 2. The mean value of water soluble substances was 31.7 per cent, and seasonal variation of pH was shown as 4.20 in Autumn, 3.85 in Summer and 2.76 in Winter. 3. The mean value of sulfuric anhydride was 1.54mg ranging from 0.197mg to 4.162 mg. The highest concentration of sulfuric anhydride of 2.03mg was detected in cross-road areas while thelowest of 1.23mg in residential areas, and Summer the highest of 2.18mg while in Spring the lowest of 1.09mg(0. 92mg in Nov. 1973) 4. Dustfalls are more with the decreased of relative humidity and precipitation while concentrat:on of sulfuric anhydrides are more with the increased of relative humidity. 5. There is a probability of occuring chronic symptoms(respiratory and others) as the dustfalls with 24.8 tons and sulfuric anhydride with 1.54mg in Busan. According to this, it is the time to discuss monitoring system and systematic preventing methods.
We need to understand the outcomes into adulthood for survivors born either extremely low birthweight (ELBW; <1,000 g) or extremely preterm (EP; <28 weeks' gestational age), particularly their blood pressure and cardiovascular metabolic status,respiratory function, growth, psychological and mental health performance, and functional outcomes. Blood pressure is higher in late adolescence and early adulthood in ELBW/EP survivors compared with controls. In some studies, expreterm survivors have higher insulin and blood lipid concentrations than controls, which may also increase their risk for later cardiovascular disease. ELBW/EP survivors have more expiratory airflow obstruction than do controls. Those who had bronchopulmonary dysplasia (BPD) in the newborn period have even worse lung function than those who did not have BPD. As a group, they are unlikely to achieve their full lung growth potential, which means that more of them are likely to develop chronic obstructive airway disease in later life. Although they are smaller than term born controls, their weight gradually rises and ultimately reaches a mean z-score close to zero in late adolescence, and they ultimately attain a height z-score close to their mid-parental height z-score. On average, ex-preterm survivors have intelligence quotient (IQ) scores and performance on tests of academic achievement approximately 2/3 SD lower than do controls, and they also perform less well on tests of attention and executive function. They have similar high rates of anxiety and depression symptoms in late adolescence as do controls. They are, however, over-represented in population registries for rarer disorders such as schizophrenia and Autism Spectrum Disorder. In cohort studies, ex-preterm survivors mostly report good quality of life and participation in daily activities, and they report good levels of self-esteem. In population studies, they require higher levels of economic assistance, such as disability pensions, they do not achieve education levels as high as controls, fewer are married, and their rates of reproduction are lower, at least in early adulthood. Survivors born ELBW/EP will present more and more to health carers in adulthood, as they survive in larger numbers.
Nitric oxide(NO) is a potent and selective pulmonary artery vasodilator. Erythropoietin(EPO) is produced in the kidney in response to reduced oxygen availability. In this study, blood samples were collected for determination of NO and EPO concentrations in 18 normal newborns and 16 newborns with respiratory distress syndrome(RDS). Serum was measured by Ding's method for NO concentration and by enzyme-linked immunosorbant assay for EPO concentration. Nitric ion concentration in serum was 14.9$\pm$3.2 $\mu$M in normal control group and 12.8$\pm$3.3 $\mu$M in RDS group. EPO concentration in serum was 16.2$\pm$3.4 mU/ml in normal control group and 21.2$\pm$5.4 mU/ml in RDS group. These results show the decrease of NO and increase of EPO in RDS newborn patients in comparison with normal newborns. Such imbalances may contribute to the development of several clinical symptoms.
