Purpose : The risk of severe tuberculous disease such as meningitis or miliary tuberculosis increases as younger is the child at the time of infection. Therefore, the early diagnosis and prompt treatment is mandatory for infants with tuberculosis. This study was undertaken to describe the epidemiology, clinical and radiographic manifestations, and response to therapy in infants with tuberculous disease. Methods : Medical records of 29 infants with tuberculosis diagnosed at the Seoul National University Children's Hospital from July, 1985, to April, 1997, were reviewed, retrospectively. A case of tuberculosis was confirmed if M. tuberculosis was isolated from any body site or if there was histologic proof of tuberculosis. Otherwise, the diagnoses were individualized considering history of contact with contagious adult case, clinical manifestations, chest X-ray findings, result of a Mantoux test reaction with 5 tuberculin unit of PPD, and the response to therapy. Results : The mean age at diagnosis was $7.00{\pm}2.65$ months (range, 3 to 12 months). Twelve cases had isolated pulmonary diseases, and the rest had pulmonary disease and meningitis, 5 cases; pulmonary disease and cervical lymphadenitis, 3; isolated meningitis, 3; and miliary tuberculosis, 6. Source case was identified in 19 cases, 7 of which were detected with retrograde manner. Twenty seven of 29 were symptomatic at their initial visit. The presenting symptoms were mainly respiratory or neurologic, and respiratory difficulty was accompanied in 7 cases. Physical examination revealed wheezing in 7 cases and decreased breath sounds in 9. Hepatomegaly or hepatosplenomegaly were frequent. Chest radiographs showed lung parenchymal disease with hilar lymphadenopathy in 18 cases, and focal or generalized emphysematous change in 7 cases. Conclusion : Most of the infants with tuberculosis are symptomatic at diagnosis, and many of infants with intrathoracic tuberculosis presented with symptoms of bronchial obstruction. When tuberculosis is suspected in an infant, the adult source case should be vigorously investigated to aid in diagnosis and for the prevention of further transmission of tuberculous disease. Almost half of infant tuberculosis are preventable if prophylaxis were given when adult cases were diagnosed.

Background : In intubated patients, cultures of endotracheal aspirates (EA) are apt to contamination throughout the endotracheal tube. Therefore, the identification of etiologic agents via conventional EA cultures is not always reliable. In order to differentiate a pulmonary infection from a non-infectious disease, and to identify the true etiologic agent of acute pulmonary infection, blinded protected specimen brushing (PSB) was used, and its efficacy evaluated. Methods : In 51 intubated patients, with suspected pneumonia, blind PSB were performed, and the results compared with blood and EA cultures. A protected specimen brush was introduced through the endotracheal tube, and settled at the affected large bronchus. A specimen brush was introduced to the expected region using the blind method. The tip of the brush was introduced with an aseptic technique after vigorously mixed for 1 minute in $1cm^3$ of Ringer's lactate solution. The specimens were submitted for quantitative culture within 15 minutes, with a culture being regarded as positive if the colony forming units were above $10^3/ml$. Results : Of the 51 patients, 15 (29.4%) had community-acquired pneumonia (CAP), 27 (52.9%) hospital-acquired pneumonia (HAP) and 9 (17.6%) non-infectious diseases. The sensitivity and specificity of the quantitative PSB culture for the diagnosis of pneumonia were 52.4 and 88.9%, respectively. The sensitivity and specificity of EA were 78.6 and 77.8%, respectively. The blind PSB was superior to the EA for the identification of true etiologic agents. Of 53 episodes of 27 HAP patients, MRSA (Methicillin-resistant staphylococcus aureus) (41.5%) was the most common causative agent followed by Pseudomonas aeruginosa (15.1%), Klebsiella sp. (7.5%) and Acinetobacter sp. (7.5%). Conclusions : As a simple, non-invasive diagnostic modality, the blind PSB is a useful method for the differentiation of a pulmonary infection from non-infectious diseases and to identify the etiologic agents in intubated patients. A blind PSB can be performed without bronchoscopy, so is safer, more convenient and cost-effectiveness for patients where bronchoscopy can not be performed.
Purpose: This study aimed to investigate the recent age distribution of Mycoplasma pneumoniae in patients with respiratory infections and the differences in diagnostic usefulness according to the methods used in these patients. Methods: We retrospectively reviewed the medical records of patients aged ≤18 years with respiratory infectious diseases who underwent polymerase chain reaction (PCR) or a specific immunoglobulin M (IgM) test between July 2016 and February 2019. The diagnosis of M. pneumoniae infection was confirmed by a positive result in the PCR or IgM test. Results: Of the 2,721 patients tested for M. pneumoniae, 2,197 underwent IgM, and 1,144 underwent PCR, with positivity rates of 17% and 20%, respectively. Among the 620 patients tested for both IgM and PCR tests simultaneously, 35% had M. pneumoniae infection, with 14% under 1 year old and 13% under 1-2 years old. The positive rate increased with age in both tests. Higher positive rates were observed in the IgM test before 3 years of age and in the PCR test after 3 years of age. The agreement rate between the two tests was 77.9% (Cohen's kappa 0.402). Conclusions: As age increased, the rates of M. pneumoniae infection also increased. In patients under 2 years of age, 4¬-14% of infections were confirmed depending on the method used. The moderate agreement between the PCR and IgM tests suggests that the simultaneous use of PCR and the IgM test for the early diagnosis should be approached with caution.
This study was conducted to investigate the infection of livestock diseases using 500 blood samples from wild boars captured in six cities and one county in northern Gyeonggi province, South Korea. We examined 239 cases of classical swine fever virus (CSFV), and each of 500 cases of foot and mouth disease virus (FMDV), porcine reproductive and respiratory syndrome virus (PRRSV), porcine circovirus type 2 (PCV2), Mycoplasma hyopneumoniae (MH), Actinobacillus pleuropneumoniae (APP), Pasteurella multocida type A (PMA), Hemophilus parasuis (HP), Salmonella (Sal.) spp. infections. Antibodies were detected against CSFV (23.4%), PRRSV (4.0%), PCV2 (60.4%), MH (3.0%), APP (69.2%), PMA (52.8%), HP (11.8%), and Sal. spp. infections (37.2%). No antibodies were detected against FMDV. As a result of antigenic analysis of 68 positive cases (13.6%) out of 500 PRRS antigen tests, 61 North American cases, 6 European cases, 1 North American-European complex case. PCV2 has 158 positive cases (31.6%) out of 500 antigen tests, and the results indicate that a considerable number of individuals are infected. To our knowledge, this is the first seroprevalence report of MH, APP, PMA, HP, and Sal. spp. infections in wild boars in South Korea.

Actinobacillus pleuropneumoniae is an important pig pathogen, which is responsible for swine pleuropneumonia, a highly contagious respiratory infection. To develop subunit vaccines for A. pleuropneumoniae infection, the Apx toxin genes, apxI and apxII, which are thought to be important for protective immunity, were expressed in Saccharomyces cerevisiae, and the induction of immune responses in mice was examined. The apxI and apxII genes were placed under the control of a yeast hybrid ADH2-GPD promoter (AG), consisting of alcohol dehydrogenase II (ADH2) and the GPD promoter. Western blot analysis confirmed that both toxins were successfully expressed in the yeast. The ApxIA and ApxIIA-specific IgG antibody response assays showed dose dependent increases in the antigen-specific IgG antibody titers. The challenge test revealed that ninety percent of the mice immunized with ApxIIA or a mixture of ApxIA and ApxIIA, and sixty percent of mice immunized with ApxIA survived, while none of those in the control groups survived longer than 36 h. These results suggest that vaccination of the yeast expressing the ApxI and ApxII antigens is effective for the induction of protective immune responses against A. pleuropneumoniae infections in mice.

Streptococcus anginosus is a member of Streptococcus milleri group, and is found in the oral mucosa, respiratory tract, and gastrointestinal tract as normal flora. It can develop into a disease in patients with deteriorating clinical condition or with clinical risk factors. A previously healthy 15-year-old boy was admitted due to fever, abdominal discomfort and vomiting which lasted for 7 days. He had a history of dental procedure 1 day before the development of fever. He was diagnosed with acute acalculous cholecystitis based on the clinical, laboratory, and imaging finding, and S. anginosus was isolated from the blood culture. The patient was successfully treated with antibiotic therapy.
Swine influenza is an acute, infectious respiratory disease caused by type A influenza viruses in pigs. In the previous studies, serological surveys have indicated the presence of H3N2 swine influenza virus (SIV) since 1995 in Korea. And the percentage of the antibody-positive rate was 39.12% in the survey determining the prevalence of H1N1 SIV antibodies in 2002. The purpose of this study was therefore to investigate the sero-prevalence of SIV regard to the age of the pig and the season between June 2004 and May 2005. In this study, a total of 932 sera were used. These sera were randomly selected from blood samples, which were submitted to Department of Veterinary Pathology, Kangwon National University and Department of Veterinary Virology, Seoul National University from June 2004 to May 2005. These sera have been tested by ELISA test kit (IDEXX Lab, USA) for the SIV H3N2, H1N1 respectively. SAS version 9.1 was used for the statistical analysis based on the age of the pig and the season. The overall sero-prevalence of the antibody against H3N2 SIV was 20.82% (194/932). The overall sero-prevalence of the antibody against H1N1 SIV was 37.23% (347/932). The overall dual sero-prevalence of the antibody against H3N2 and H1N1 SIV was 10.62% (99/932). H3N2 has significant difference in statistically regarding the age of the pig and the season (p<0.0001). H1N1 has significant difference in statistically regarding the age of the pig (p<0.0001) but has not significant difference in statistically regarding the season (p=0.5882).

Exposure to bioaerosols causes various adverse health effects including infectious and respiratory diseases, and hypersensitivity. Controlling exposure to bioaerosols is important for disease control and prevention. In this study, we evaluated the efficacies of various functional filters coated with antimicrobial chemicals in deactivating representative microorganisms on filters or as bioaerosols. Tested functional filters were coated with different chemicals that included (i) Ginkgo and sumac, (ii) Ag-apatite and guanidine phosphate, (iii) $SiO_2$, ZnO, and $Al_2O_3$, and (iv) zeolite. To evaluate the filters, we used a model ventilation system (1) to evaluate the removal efficiency of bacteria (Escherichia coli and Legionella pneumophila), bacterial spores (Bacillus subtilis spore), and viruses (MS2 bacteriophage) on various functional filters, and (2) to characterize the removal efficiency of these bioaerosols. All experiments were performed at a constant temperature of $25^{\circ}C$ and humidity of 50%. Most bacteria (excluding B. subtilis) rapidly decreased on the functional filter. Therefore, we confirmed that functional filters have antimicrobial effects. Additionally, we evaluated the removal efficiency of various bioaerosols by these filters. We used a six-jet collision nebulizer to generate microbial aerosols and introduced it into the environmental chamber. We then measured the removal efficiency of functional filters with and without a medium-efficiency filter. Most bioaerosol concentrations did not significantly decrease by the functional filter only but decreased by a combination of functional and medium-efficiency filter. In conclusion, functional filters could facilitate biological removal of various bioaerosols, but physical removal of these by functional was minimal. Proper use of chemical-coated filter materials could reduce exposure to these agents.
Severe acute respiratory syndrome (SARS) is an emerging infectious disease associated with a novel coronavirus (CoV) that was identified and molecularly characterized in 2003. Previous studies on various coronaviruses indicate that protein-protein interactions amongst various coronavirus proteins are critical for viral assembly and morphogenesis. It is necessary to elucidate the molecular mechanism of SARS-CoV replication and rationalize the anti-SARS therapeutic intervention. In this study, we employed an in vitro GST pull-down assay to investigate the interaction between the membrane (M) and the nucleocapsid (N) proteins. Our results show that the interaction between the M and N proteins does take place in vitro. Moreover, we provide an evidence that 12 amino acids domain (194-205) in the M protein is responsible for binding to N protein. Our work will help shed light on the molecular mechanism of the virus assembly and provide valuable information pertaining to rationalization of future anti-viral strategies.
Diphtheria was the world-spread, respiratory infectious disease in the past. But after developing the diphtheria toxoid, diphtheria vaccine, composed of diphtheria toxoid, is generalized, so prevalence of diphtheria is sharply dropped. But diphtheria is come back in NIS(New Independent States) in 1990s. It is believed that recurrence of diphtheria is mainly due to shortage of protective diphtheria antibody titer in adults. And it is related to 70% prevalence in adults, not in children. Considering this changing environment, this study was designed to research the diphtheria antibody titer of whole population and existence of titer difference by sex. Also it was studied whether ELISA has fitness as determination method of diphtheria antibody titer compared with neutralization method on microcell culture. This study was done to 277 samples of three hospitals in Seoul area, and sera was tested by neutralization method on microcell culture and ELISA method. The results of this study were as follows 1) Of this studied population, 31.05% had an antibody titer below the protective level (<0.01 IU/ml), 40.79% had a titer of a relative degree of protection (0.01-0.09 IU/ml) and 28.16% had a reliable degree of protection ($\geq$0.1 IU/ml). Therefore, 68.95% had a basic protective antibody titer level. 2) 20-40 age group showed the lowest diphtheria antibody titer among the studied population. 3) GMT of diphtheria antibody titer was the highest in the children. But, after child period, GMT was fallen. Standard deviation value was the lowest in older group. 4) Protective level of diphtheria antibody titer of male was slightly higher than female (70.94% vs 66.66%). But this difference was not statistically significant. 5) Diphtheria antibody titer by ELISA method was wholly higher than by neutralization method on microcell culture. Compared the two results by ELISA and neutralization, the regression coefficient was 0.38. And in titer, which was obtained by ELISA method, false-positive results was abundant.
