Effect of IL-1 Polymorphisms, CYP2C19 Genotype and Antibiotic Resistance on Helicobacter pylori Eradication Comparing Between 10-day Sequential Therapy and 14-day Standard Triple Therapy with Four-Times-Daily-Dosing of Amoxicillin in Thailand: a Prospective Randomized Study

  • Background: Studies of effects of IL-1 polymorphisms, CYP2C19 genotype together with antibiotic resistance for H. pylori eradication are rare worldwide. The present study was designed to evaluate efficacy of 10-day sequential therapy (SQT) and 14-day standard triple therapy (STT) with four- times-daily dosing of amoxicillin for H. pylori eradication related to these important host and bacterial factors in Thailand. Materials and Methods: This prospective randomized study was performed during March 2015 to January 2016. H. pylori infected gastritis patients were randomized to receive 10-day sequential therapy and 14-day standard triple therapy. CYP2C19 genotyping, IL1 polymorphism (IL-1B and IL-1RN genotypes) and antibiotic susceptibility tests were performed in all patients. 13C-UBT was conducted to confirm H. pylori eradication at least 4 weeks after treatment. Results: A total of 100 patients (33 males and 67 females, mean age=51.1 years) were enrolled. Eradication rate by PP analysis was 97.9% (47/48) with the 10-day SQT regimen and 87.8% (43/49) with 14-day STT regimen (97.9% vs 87.8%; p-value=0.053). Antibiotic susceptibility testing demonstrated 45% resistance to metronidazole, 14.8% to clarithromycin, and 24.1% to levofloxacin. CYP2C19 genotyping revealed 44.9% RM, 49% IM and 6.1% PM. IL-1B and IL-1RN genotypes were demonstrated as 21.4% for CC, 48.1% for TC, 36.8% for TT, 72.7% for 1/1, and 21.2% for 1/2 genotypes, respectively. The 10-day SQT regimen provided 100% eradication in patients with clarithromycin or dual clarithromycin and levofloxacin H. pylori resistant strains. Moreover, the 10-day SQT regimen resulted in a 100% eradication rate in all patients with CYP2C19 genotype RM and almost type of IL-1B (TC and TT) and IL1-RN genotypes ( 1/2 and other). Conclusions: Treatment with 10-day sequential therapy is highly effective for H. pylori eradication regardless of the effects of clarithromycin resistance, dual clarithromycin and levofloxacin resistance, CYP2C19 genotype, IL-1B and IL1-RN genetic polymorphisms and can be used as effective first line therapy in Thailand.

Application of tylosin antibiotics to olive flounder (Paralichthys olivaceus) infected with Streptococcus parauberis

  • Background: Olive flounder, Paralichthys olivaceus, is an economically important aquaculture species in Korea. Olive flounders have been heavily damaged by streptococcal infections every year and are treated with antibiotics. However, antibiotic abuse is causing the emergence of resistant strains, and to overcome this, research has shown that new antibiotics must be applied. Tylosin is a relatively safe antibiotic and has good activity against Gram-positive bacteria and mycoplasma. We studied the therapeutic effects and side effects of tylosin on Streptococcus parauberis-infected olive flounder. Methods: After artificial infection of olive flounder with S. parauberis SPOF18J3, an appropriate dose of tylosin was confirmed by intramuscular injection (I.M.) at 2.5, 5, 10, and 15 mg/kg, and oral administration at 10 and 20 mg/kg. After I.M. and oral administration dosing of tylosin, side effects were confirmed by serological analysis, histopathological analysis, and median lethal dose (LD50) analysis at both an appropriate concentration and a high concentration. Statistical analysis was performed using one-way analysis of variance (ANOVA) and Tukey's test (p < 0.05). Results: The appropriate I.M. and oral administration concentration of tylosin administered to olive flounder infected with S. parauberis SPOF18J3 was found to be 10 mg/kg. Alanine aminotransferase (ALT) and aspartate aminotransferase (AST) levels were showed not significantly different between the control group and the experimental groups. The histopathologic results showed mild inflammatory responses in muscle and tubular vacuolization and tubular atrophy appeared, but there were no significant differences between the groups. The LD50 was confirmed to be 461 mg/kg. Conclusion: In this study, an effective treatment method was provided by verifying the treatment effects and side effects of tylosin in olive flounder infected with S. parauberis, which can be applied directly to aquaculture sites. In addition, these results may be used as a reference for evaluation required upon request to obtain approval for tylosin antibiotics as fishery antibiotics in Korea. After approval, it is possible that a fishery disease manager will be able to prescribe and sell the antibiotic tylosin.

Antibiotic Resistance and Plasmid Profile of Vibrio parahaemolyticus Strains Isolated from Kyunggi-Incheon Coastal Area (경기인천 연안에서 분리된 장염비브리오균의 항생제 내성 및 플라스미드 보유 현황)

  • Vibrio parahaemolyticus is one of the major agents responsible for food poisoning during summer in Korea, which is transmitted via seawater or seafoods. Recently, distribution of the bacteria in the marine environment has been increased due to global warming. Great concern also has been raised regarding public hygiene as well as marine culture by the emergence of pathogens with antibiotic resistance. Therefore, distribution of V. parahaemolyticus and antibiotic resistance of the isolates were monitored in 7 coastal areas of Kyonggi Province and Incheon by sampling seawater, fishes and clams monthly. V. parahaemolyticus was detected from 47.7% of 966 samples (seawater 61.9%, seafoods 41.8%) analyzed using $CHROMagar^{TM}$ and TCBS agar plates as well as multiplex PCR. Among 13 antibiotics tested, resistance to vancomycin and ampicillin was observed in 97.3% and 87.3% of the isolates, respectively, and the ratios of them resistant to cephalothin (48.8%) and rifampin (46.1%) were also high. The isolates were most highly sensitive to chloramphenicol (91.7%) and trimethoprim-sulfamethoxazole (91.8%). The ratio of sensitivity for other antibiotics was also high in the descending order of gentamycin (82.3%), tobramycin (74.8%), nalidixic acid (71.6%), tetracyclin (69.4%), cefotaxime (63.0%). About 69% of the isolates showed multiple drug resistance toward 3 antibiotics including vancomycin and ampicillin. Two of them exhibited resistance for 11 antibiotics used in this study. Plasmid profile analysis of the isolates with antibiotic resistance revealed that 55.1% of them retained plasmids of 24 different types. However, no clear inter-relationship between the resistance and the plasmid profile has been observed.

Effect of Organic Acids Derived from Black Liquor on Growth of Selected Escherichia coli MG 1655 (흑액 유래 유기산의 Escherichia coli MG 1655 성장에 미치는 영향 탐색)

  • In this study, we selected an Escherichia coli strain (E. coli MG 1655) metabolizing arabinose derived from acid hydrolyzed black liquor as a carbon source and investigated effect of organic acids (acetic acid, formic acid, and lactic acid) presented in black liquor on growth of the E. coli MG 1655. We measured growth of E. coli MG 1655 under various concentration of each and combined three kinds of organic acids. The E. coli MG 1655 shows tolerance to acetic acid, lactic acid and formic acid at these concentrations ($1.0g/{\ell}$ acetic acid, $1.2g/{\ell}$ lactic acid and $0.8g/{\ell}$ formic acid, respectively), but displays some growth retardation over $1.5g/{\ell}$ acetic acid, lactic acid $2.0g/{\ell}$, and formic acid $1.2g/{\ell}$, respectively. In addition, formic acid was shown to be a critical factor affecting growth of the E. coli MG 1655 in the presence of three kinds of organic acids. These results indicate that the inhibitors should be removed at least $1.0g/{\ell}$ of acetic acid, $1.2g/{\ell}$ of lactic acid, $0.8g/{\ell}$ of formic acid for normal cell growth required for high yield fermentation. In addition, there is a need to construct recombinant strains that may be resistant to the same or higher organic acids concentration (> $1.2g/{\ell}$) in the growth.

Antibiotic Susceptibility of Vibrio parahaemolyticus Isolated from Commercial Marine Products (시판 어패류에서 분리한 장염 비브리오균의 항생제 감수성 특성)

  • Ninety three strains (4.2%) of Vibrio parahaemolyticus were isolated from 2,239 samples of commercial marine products during 2006-2008 in Seoul, Korea. We examined 16 antimicrobial susceptibilities of 93 V. parahaemolyticus isolates. Antibiotic resistance of V. parahaemolyticus was most frequently observed to ampicillin (93.5%), followed by cephalothin (90.3%), streptomycin (87.1%), ticarcillin (55.9%), and amikacin (40.9%). Antibiotic susceptibility was most frequently observed to nalidixic acid, chloramphenicol and ampicillin/sulbactam (100%), followed by trimethoprim/sulfamethoxazole (98.9%), gentamicin and tetracycline (82.8%), and ceftriaxone (63.4%). In addition, the isolates also displayed intermediate resistance to kanamycin (79.6%), ciprofloxacin (64.5%), amikacin (47.3%) and cefoxitin (43.0%). All isolates were resistant to more than two drugs. The most prominent multiple drug resistance was 3 drug resistance (37.6%), followed by 4 drug (24.7%), 5 drug (17.2%), and 6 drug resistance (11.8%). The most prominent multiple drug resistance pattern was the cephalothin-streptomycin-ampicillin resistance pattern (22.6%), followed by cephalothinticarcillin-streptomycin-ampicillin (18.3%) and cephalothin-ticarcillin-streptomycin-ampicillin-amikacin (9.7%). Multiple drug resistance patterns of V. parahaemolyticus from marine products require continuous monitoring.

Occurrence of Leaf Mold Pathogen Fulvia fulva Isolates Infecting Tomato Cf-9 Cultivars in Korea (토마토 Cf-9 저항성 품종에 잎곰팡이병을 일으키는 Fulvia fulva 균주의 국내 발생)

  • Leaf mold symptoms were found on commercial tomato cultivars carrying the Cf-9, a resistance gene to leaf mold pathogen Fulvia fulva in 2012 at Buyeo, Chungnam in Korea. Fifteen-fungal isolates were obtained from four Cf-9 cultivars of tomato including 'Cutie', 'otaerangdia', 'Unicorn' and 'Rapito'. Due to their same morphological appearances and colony color, nine isolates were selected and identified as F. fulva based on molecular analysis of the internal transcribed spacer rDNA sequence. Pathogenicity of the 15 isolates on five commercial cultivars carrying Cf-4, Cf-5, and Cf-9 were tested. All the isolates showed strong pathogenicity on Cf-9 cultivars, 'Cutie' and 'Dotaerangdia', and Cf-5 cultivar, 'Yoyocaptain'. In contrast, on Cf-4 cultivar, 'Superdotaerang', five isolates were virulent and the other isolates were not. In addition, two fungal isolates, infecting Cf-9 cultivar and non-infecting Cf-4 cultivar, were selected and their pathogenicity was tested on 17 commercial cultivars reported as tomato having Cf-9 resistance gene. Among them, 15 cultivars were susceptible and 2 cultivars were resistant. It is likely that the two cultivars include other resistance gene. To our knowledge, this is the first report on the occurrence of Cf-9 infecting F. fulfva strains in Korea.

Characterization of Plasmid-Mediated SHV-11 β-lactamase Gene of Klebsiella pneumoniae Isolated from the Clinical Specimens (임상검체로부터 분리한 플라스미드 매개성 SHV-11 β-lactamase 유전자의 특성)

  • In this study, we characterized extended-spectrum $\beta$-lactamase (ESBL)-producing Enterobacteriaceae isolated from clinical specimens in Korea and found two strains harboring plasmid-mediated $bla_{SHV-11}$, Klebsiella pneumoniae. First, the isolates were detected using the Vitek system and confirmed by the double-disk synergy test. The classification of gene coding for ESBL was also performed by polymerase chain reactions and followed by DNA sequencing. The transmission of genes was confirmed by transconjugation and transformation. Resistant expression of transformants was determined by broth microdilution minimal inhibitory concentration test. Genotypic analysis revealed that one strain harbored the $bla_{TEM-1}$, $bla_{SHV-11}$ and $bla_{CTX-M-15}$ and the other strain harbored the $bla_{SHV-11}$ and $bla_{CTX-M-15}$. They showed high resistance to oxyiminocephalosphorins (3rd-generation cephalosporins), while the transformant containing only $bla_{SHV-11}$ did not show any resistance to the antibiotics.

In vivo Evaluation of a Novel ${\beta}-Lactam$ Antibiotics, YH-487 (새로운 ${\beta}-Lactam$계 항생물질 YH-487의 in vivo 항균활성)

  • A novel compound, named YH-487, was synthesized by attaching the thiol and aminothiazole residue to $C_3$ and $C_7$ position of 7-aminocephalosporanic acid (7-ACA). The therapeutic efficacy on infected animals, pharmacokinetics in vivo and the effect on intestinal microflora of YH-487 were examined. The pharmacokinetics of YH-487 were similar to that of cefotaxime, a third generation ${\beta}-lactam$ antibiotics, in rat. Upon in vivo administration, YH-487 was predominantly delivered to kidney, and mostly excreted through kidney without making any metabolites. The therapeutic efficacy of YH-487 to animal infected with E. coli was three times and twenty times higher than that of cefotaxime and cefotiam, respectively, In vivo administration of YH-487 to Sprague-Dawley rats significantly decreased the population of intestinal gram negative species such as Enterobacteria and Barteroides. However, no significant changes were obseved in gram positive species such as Lactobacillus, Bifidobacteria and Staphylococcus. In addition, continuous administration of YH-487 did not increase the possibility to induce resistant strains in intestinal microflora.

Differences in the Antibiotic Resistance Pattern of Staphylococcus aureus Isolated by Clinical Specimens in a University Hospital in South Korea (일개 대학병원의 임상 검체에서 분리한 포도알균 항생제 내성 경향 차이)

  • Information on the prevalence of S. aureus and the current antimicrobial resistance profile is necessary in selecting the appropriate treatment of S. aureus infections in any part of the world. This study examined the frequency and antibiotic resistance list of S. aureus isolates obtained from clinical specimens at one hospital in Korea. A total of 1,746 gram positive cocci collected were identified as S. aureus. S. aureus isolates were obtained from different samples including sputum (N=565; 32.4%), endotracheal aspirate (358; 20.5%), wounds (329; 18.8%), blood (137; 7.8%), urine (67; 3.8%), and pus (59; 3.4%). All 1,545 S. aureus (100%) strains screened from sputum (565; 36.6%), endotracheal aspirate (388; 25.1%), wounds (329; 21.3%), blood (137; 8.9%), urine (67; 4.3%), and pus (59; 3.8%) were sensitive to glycopeptide (vancomycin, teicoplanin), oxazolidinone (linezolid) and stretogramin (quinupristin/dalfopristin). The prevalence of resistant S. aureus was significantly (P<0.01) lower in urine, blood, pus, wounds, and sputum than in endotracheal aspirates. As a result, there was a significant difference in the antibiotic resistance of S. aureus according to the clinical specimens.

Physico-Chemical Properties and Antimicrobial Activity of Pyocyanine Produced by Pseudomonase aeruginosa KLP-2 (Pseudomonas aeruginosa KLP-2가 생산한 Pyocyanine의 항균활성 및 생리화학적 성상)

  • The antimicrobial substance produced by Pseudomonas aeruginosa KLP-2 strain was purified and identified. The substance was identified as a pyocyanine by the fast atom bombardment mass(FAB-MS). In physic-chemical properties, the pyocyanine was dark blue needles, and was soluble in various organic solvents such as chlorogorm, methanol, ethanol and ethyl acetae. The pyocyanine possessed a ultraviolet absorbance spectrum in methanol, 0.1 M HCl, and chlorogorm. The maximum absorption peak of the pyocyanine showed at 318 mm in methanol. The molecular formula of the pyocyanine was determined to the $C_{13}$ H$_{10}$ N$_{2}$O and protonate molecular ion species (M+H)$^{+}$ was observed at m/z 211 by FAB-MS. The pyocyanine showed antimicrobial against Bacillus cereus, Bacillus subtilis, Micrococcus luteus, Rodococcus equi, Staphylococcus aureus, Streptococcus faecalis, E. col, Legionella pneumophila, Shigella flexneri Shigella boydii, shgella sonnei, NAG Vibrio cholerae, Vibrio parahaemolyticus, Vibro vulnificus, Yersinia enterocolitica, and Saccharomyces cerevisiae. However, Salmonella spp. Shigela dysenteriae, 3 strains of Pseudomonas aeruginosa, Klebsiela pneumoniae, and Aspergillus niger were resistant to the pyocyanine. The pyocyanine showed the highest antimicrobial activity aganist Legionella pneumophila based on the size of inhibition zone by the disk contained 0.5 $\mu\textrm{g}$ of the pyocyanine.e.
