산성음료가 치아색 수복재의 표면에 미치는 영향 (The effect of acidic drinks on the surface of tooth-colored restorative materials)

  • 법랑질 침식의 병인 중에서 식이습관은 많은 관심을 받아왔고 특히 산성음료의 음용이 많은 주의를 끌어왔다. 산성음료의 급속한 소비량 증가는 치아뿐만 아니라 치아색 수복재료의 표면에도 많은 영향을 미칠 것으로 생각된다. 따라서 본 연구는 어린이들이 비교적 일상적으로 접하는 산성음료 중 가장 보편화된 3종을 선택하여 치아색 수복재료의 노출시간에 따른 표면의 변화를 조사할 목적으로 시행되었다. Resin-modified glass-ionomer, polyacid-modified composite resin, composite resin을 0.9% NaCl, 콜라, 오렌지 쥬스, 스포츠 음료의 각 4개 군으로 나누어 저장하였다. 저장 전과 저장 후 1일, 1-,2-,3-,4-주에 표면강도 및 표면조도를 측정하였고 다음과 같은 결론을 얻었다. 1. 산성음료에 저장했을 경우 composite resin을 제외한 치아색 수복재들은 통계학적으로 유의한 표면강도와 표면조도의 변화를 보였다(p<0.05). 2. 표면강도 및 표면조도의 변화는 resin-modified glass-ionomer, polyacid-modified composite resin composite resin 순으로 크게 나타났다. 3. 콜라나 오렌지 쥬스보다 스포츠 음료에서 표면변화는 더 크게 나타났다. 4. 산성음료에 저장한 군의 SEM 촬영에서 resin-modified glass-ionomer, polyacid-modified composite resin의 경우 충전제의 상실이 관찰되었으며 특히 resin-modified glass-ionomer의 경우 심한 균열이 관찰되었다.

다양한 치은 연하 수복물에 대한 치은 섬유아 세포 부착 연구 (Attachment of Human Gingival Fibroblast to Various Subgingival Restorations;A Comparative Study in Vitro)

  • When mucoperiosteal flaps are positioned and sutured to desirable position, the wound contains several interface between tissues which differ fundamentally in composition & biological reaction. Thus the C-T surface of the flap will, on one hand, oppose another vascularized surface, and on the other, the avascular dental material for example, when root resoptions, fractured root, endodontic perforation, deep root carious lesions were filled with amalgam, glass ionomer, resin etc. Recently, a number of case report described the successful treatment of a subgingival root lesion with restorative material & free gingival graft, open flap surgery, but more objective research was needed . Most of study on restorative materials were concerned for cytotoxicity not for actual healing event on that materials and its influencing factors such as biocompatibility, surface wettability, surface topography . The aim of this in vitro study was to evaluate the effect of amalgam, resin modified glass ionomer, composite resin per se, and their surface roughness on the growth of human gingival fibroblast. The cells were obtained and placed on culture flask and incubated for 3 days with the prepared test materials. Then count the attached cell number with hemocytometer,(n=12) and 2 samples were examined with SEM about attachment cell morphology . Another 4 samples were evaluated on their surface roughness with Talysurf and average surface roughness value(Ra) were obtained. Statistical difference in attached cell number, roughness value were analyzed using ANOVA. The number of attached cell was as follows, for root dentin specimen 16.7${\pm}$4.41, resin modified glass ionomer 14.0${\pm}$4.15, resin 8.13${\pm}$3.63, amalgam 0.72${\pm}$3.33(${\times}10^3$). Between root dentin and resin-modified glass ionomer, no significant difference was observed, but resin, amalgam showed a significant less cell numbers than for root dentin, resin modified glass ionomer cement. SEM examination expressed many cell surface attachment apparatus in root dentin and resin modified glass ionomer specimens. For resin specimen, cell attachment was observed but exposed less appratus. The average surface roughness value are following results. Dentin specimen 0.6972${\pm}$ 0.104, resin modified glass ionomer 0.0822${\pm}$0.009, resin 0.0875${\pm}$0.005, amalgam 4.2145${\pm}$0.985(${\mu}m$). Between root dentin, resin-modified glass ionomer, and resin, no significant difference was observed, but amalgam showed a significant more rough surface than other groups. When evlauated the interrelationship between cell attachment and surface roughness, therefore, there was weak reverse correlation.(pearson correlation : - 0.593) These results suggest that resin modified glass ionomer have the favorable healing potential when used for subgingival restoration. And for relationship between cell attachment and surface characteristics, further investigations were needed.

상아질에 대한 광중합형 글라스 아이오노머 시멘트의 접합도 (ADAPTATION OF RESIN-MODIFIED GLASS IONOMER CEMENT TO DENTIN)

  • The purpose of this study was to evaluate the adaptation of self-cured glass ionomer cement and resin-modified glass ionomer cement and polyacid-modified resin composite, which are light-cured giass ionomer cements, to dentin surface. Twelve extracted human maxillary and mandibular molar teeth were used in this study. The entire occlusal dentin surfaces of teeth were exposed with Diamond Wheel Saw and smoothed with sand papers (300, 600, 1200grits). They were randomly assigned into 3 groups according to glass ionomer cements used; Control group- Fuji II, Expeimental group 1 - Fuji II LC, Expeimental group 2 - Dyract. According to the manufacturer's directions, three glass ionomer cements were bonded to exposed dentin surfaces of the tooth crown and cured. Crowns and glass ionomers were trimmed after 24hrs and sectioned horizontally and vertically with diamond saw. The interface of glass ionomer cements and dentin was examined under SEM. The results were as follows : 1. Good adatation between glass ionomer cement and dentin on the horizontal section was showed in control and experimental group 1, but the gap of $20{\mu}m$, which was observed distinct separation between glass ionomer cement and dentin, was showed in experimental group 2. 2. Good adatation between glass ionomer cements and dentin on the vertical section was showed in control and experimental group 1, but the gap of 80-$100{\mu}m$ was showed in experimental group 2. 3. Cohesive fracture within glass ionomer cements in control and experimental group 1 was showed, but no cohesive fracture was showed in experimental group 2.

불소 제재가 심미 수복 재료의 표면 구조에 미치는 영향 (THE EFFECT OF TOPICAL FLUORIDES ON SURFACE STRUCTURES OF VARIOUS ESTHETIC RESTORATIVE MATERIALS)

  • Topical fluoride application for children is a widely performed procedure in the field of Pediatric Dentistry for its dental caries prevention effects. However, it is recently recognized as having some unwanted effects on several esthetic restorative materials as it roughens the surfaces of the restorative materials. In order to evaluate the surface changes in esthetic restorative materials, the author immersed composite resin, glass ionomer cement, and resin-modified glass ionomer cement specimens in various topical fluoride agents and measured the weight loss and also, examined the specimens under the scanning electron microscope. The followings are the results : 1. All the specimens immersed in APF gel for 4 minutes showed statistically significant weight loss. (paired t-test, P<0.05). 2. There was no statistically significant weight loss for the resin-modified glass ionomer cement and composite resin groups immersed in sodium fluoride solution (paired t-test, P>0.05). 3. When the glass ionomer cement group was immersed in APF gel for 1 and 4 minutes, there was a statistically significant weight loss compare to other esthetic restorative materials (ANOVA, P<0.05). 4. In the resin-modified glass ionomer cement group and the composite resin group, weight loss in the APF gel 4 minutes immersion group was greater than the 1 minute immersion group, and it was statistically significant (ANOVA, P<0.05). 5. When the specimens were examined under scanning electron microscope, the surface changes were greatest in the order of glass ionomer cement, resin-modified glass ionomer cement, composite resin and also in the order of APF gel 4 minute immersion group, 1 minute immersion group, sodium fluoride immersion group, and control group.

Thermal cycling과 시효처리가 Glass-Ionomer 수복재의 인장강도에 미치는 영향 (EFFECT OF THERMAL CYCLING AND AGING ON THE TENSILE STRENGTH OF GLASS-IONOMER RESTORATIVE MATERIALS)

  • 본 연구에서는 구강환경과 유사한 액상의 조건하에서의 온도변화가 광중합형 글래스아이오노머계 수복재의 인장강도에 미치는 영향을 평가하기 위해 대조군으로 2종의 재래형 glass ionomer를 선택하고 실험군으로 2종의 광중합형의 resin-modified glass ionomer와 2종의 polyacid-modified resin composite을 선택한 다음 수중에서의 thermal cycling과 시효처리를 행하였으며, 다음과 같은 결론을 얻었다. 1. 글래스아이오노머 수복재의 인장강도를 측정한 결과, polyacid-modified resin composite, resin-modified glass ionomer 그리고 재래형 glass ionomer 의 순으로 나타났다. 2. 인장강도는 30일간의 시효처리로 증가되는 경향을 보였다. 3. 재래형 glass ionomer 수복재의 인장강도는 thermal cycling 처리로 증가되는 경향을 보였으며, $37^{\circ}C$의 증류수 중에 1시간 침적한 군과 10,000회의 thermal cycling을 비교한 결과 유의한 차이로서 강도의 증가를 나타냈다(P<0.01). 4. thermal cycling 군의 인장강도는 DR군이 45.4MPa로 최대치를, FL군이 13.4MPa로 최소치를 나타냈으며, 각 군간의 통계적 유의성을 검증한 결과 polyacid-modified resin composite의 인장강도가 나머지 군과 유의한 차이를 보였다(p<0.05). 5. 특성강도는 DR군이 48.6MPa로 가장 높은 강도치를 보였으나 Weibull 계수는 CG군이 8.9로 가장 높은 값을 보여 시험재료 중에서 가장 작은 강도의 분산을 나타냈다.

Bonding of a resin-modified glass ionomer cement to dentin using universal adhesives

  • Objectives: This study aims to assess the effect of universal adhesives pretreatment on the bond strength of resin-modified glass ionomer cement to dentin. Materials and Methods: Fifty caries-free human third molars were employed. The teeth were randomly assigned into five groups (n = 10) based on dentin surface pretreatments: Single Bond Universal (3M Oral Care), Gluma Bond Universal (Heraeus Kulzer), Prime&Bond Elect (Dentsply), Cavity Conditioner (GC) and control (no surface treatment). After Fuji II LC (GC) was bonded to the dentin surfaces, the specimens were stored for 7 days at 37℃. The specimens were segmented into microspecimens, and the microspecimens were subjugated to microtensile bond strength testing (1.0 mm/min). The modes of failure analyzed using a stereomicroscope and scanning electron microscopy. Data were statistically analyzed with one-way analysis of variance and Duncan tests (p = 0.05). Results: The surface pretreatments with the universal adhesives and conditioner increased the bond strength of Fuji II LC to dentin (p < 0.05). Single Bond Universal and Gluma Bond Universal provided higher bond strength to Fuji II LC than Cavity Conditioner (p < 0.05). The bond strengths obtained from Prime&Bond Elect and Cavity Conditioner were not statistically different (p > 0.05). Conclusions: The universal adhesives and polyacrylic acid conditioner could increase the bond strength of resin-modified glass ionomer cement (RMGIC) to dentin. The use of universal adhesives before the application of RMGIC may be more beneficial in improving bond strength.

공초점레이저주사현미경을 이용한 심미수복재와 상아질의 접착계면에 관한 연구 (A CONFOCAL LASER SCANNING MICROSCOPIC STUDY ON THE INTERFACE BETWEEN TOOTH COLORED RESTORATIVE MATERIALS AND DENTIN)

  • The purpose of this study was to evaluate on the interfacial morphology between dentin and restorative materials. In this in vitro study, the cavity wall restorated with 3 different kinds of tooth colored restorative materials [resin-modified Glass Ionomer cement (Fuji II LC), composite resin (Z-100), compomer (Dyract)]. The thirty extracted human molar teeth without caries and/or restorations are used. The experimental teeth were randomly divided into three groups of ten teeth each. In each group, Wedge shaped cavities (width: 3mm, length: 2mm, depth: 1.5mm) were prepared at the cementoenamel junction on buccal and lingual surfaces. The adhesive of composite resin were mixed with rhodamine B. Primer of composite resin, Prime & Bond 2.1 of Dyract and liquid of Fuji II LC were mixed with fluorescein. In group 1, the cavity wall was treatment with dentin conditioner, and then restorated with Fuji II LC. In group 2, the cavity wall was treatment with Prime & Bond 2.1 and then restorated with Dyract. In group 3, the cavity wall was etching with 10% maleic acid, applied with primer and bonding agent and then restorated with Z-100. The interface between dentin and restorative materials was observed by fluoresence imaging with a confocal laser scanning microscope. The results were as follows : 1. In Glass ionomer group, adaptation of resin modified Glass-ionomer restoration against cavity wall is tight, but the crack formed inside of restoration were observed. 2. In Dyract group, the penetration of resin tag is shorter and the width of hybrid layer is narrower than composite resin group. 3. In Z-100 group, primer penetrated deeply through dentinal tubule. Also bonding agent was penetrated along the primer, but the penetration length is shorter than primer part, and in 3-D image, the resin tag is conical shape and lateral branch is observed.

Comparison of effect of desensitizing agents on the retention of crowns cemented with luting agents: an in vitro study

  • PURPOSE. Many dentists use desensitizing agents to prevent hypersensitivity. This study compared and evaluated the effect of two desensitizing agents on the retention of cast crowns when cemented with various luting agents. MATERIALS AND METHODS. Ninety freshly extracted human molars were prepared with flat occlusal surface, 6 degree taper and approximately 4 mm axial length. The prepared specimens were divided into 3 groups and each group is further divided into 3 subgroups. Desensitizing agents used were GC Tooth Mousse and $GLUMA^{(R)}$ desensitizer. Cementing agents used were zinc phosphate, glass ionomer and resin modified glass ionomer cement. Individual crowns with loop were made from base metal alloy. Desensitizing agents were applied before cementation of crowns except for control group. Under tensional force the crowns were removed using an automated universal testing machine. Statistical analysis included one-way ANOVA followed by Turkey-Kramer post hoc test at a preset alpha of 0.05. RESULTS. Resin modified glass ionomer cement exhibited the highest retentive strength and all dentin treatments resulted in significantly different retentive values (In Kg.): GLUMA ($49.02{\pm}3.32$) > Control ($48.61{\pm}3.54$) > Tooth mousse ($48.34{\pm}2.94$). Retentive strength for glass ionomer cement were GLUMA ($41.14{\pm}2.42$) > Tooth mousse ($40.32{\pm}3.89$) > Control ($39.09{\pm}2.80$). For zinc phosphate cement the retentive strength were lowest GLUMA ($27.92{\pm}3.20$) > Control ($27.69{\pm}3.39$) > Tooth mousse ($25.27{\pm}4.60$). CONCLUSION. The use of $GLUMA^{(R)}$ desensitizer has no effect on crown retention. GC Tooth Mousse does not affect the retentive ability of glass ionomer and resin modified glass ionomer cement, but it decreases the retentive ability of zinc phosphate cement.

Do conventional glass ionomer cements release more fluoride than resin-modified glass ionomer cements?

  • Objectives: The aim of this study was to evaluate the fluoride release of conventional glass ionomer cements (GICs) and resin-modified GICs. Materials and Methods: The cements were grouped as follows: G1 (Vidrion R, SS White), G2 (Vitro Fil, DFL), G3 (Vitro Molar, DFL), G4 (Bioglass R, Biodinamica), and G5 (Ketac Fil, 3M ESPE), as conventional GICs, and G6 (Vitremer, 3M ESPE), G7 (Vitro Fil LC, DFL), and G8 (Resiglass, Biodinamica) as resin-modified GICs. Six specimens (8.60 mm in diameter; 1.65 mm in thickness) of each material were prepared using a stainless steel mold. The specimens were immersed in a demineralizing solution (pH 4.3) for 6 hr and a remineralizing solution (pH 7.0) for 18 hr a day. The fluoride ions were measured for 15 days. Analysis of variance (ANOVA) and Tukey's test with 5% significance were applied. Results: The highest amounts of fluoride release were found during the first 24 hr for all cements, decreasing abruptly on day 2, and reaching gradually decreasing levels on day 7. Based on these results, the decreasing scale of fluoride release was as follows: G2 > G3 > G8 = G4 = G7 > G6 = G1 > G5 (p < 0.05). Conclusions: There were wide variations among the materials in terms of the cumulative amount of fluoride ion released, and the amount of fluoride release could not be attributed to the category of cement, that is, conventional GICs or resin-modified GICs.

상아질 위치에 따른 접착성 수복재의 미세전단결합강도에 관한 연구 (Micro-shear bond strength of resin-modified glass ionomer and resin-based adhesives to dentin)

  • 이 연구의 목적은 수종 상아질 접착 시스템과 복합 레진 및 resin-modified glass ionomer를 상아질 표면에 접착하여 재료 및 상아질내 위치에 따른 미세전단결합강도를 측정, 비교하는 것이다. 상아질 접착 시스템으로는, 3-step인 Scotchbond Multi-Purpose Plus, 2-step인 Single Bond와 자가 부식형 시스템인 Clearfil SE Bond를, 1-step인 Prompt L-Pop을 사용하였다. 이와 함께 hybrid type의 복합 레진인 Clearfil AP-X와 2250을 사용하였으며 resin-modified glass ionomer로는 Fuji II LC를 사용하였다. 상악 소구치를 치아의 근원심 중앙부를 절단하여 상아질면을 노출시켰다. 5개 실험군으로 분류하고 상아질면을 위치에 따라 치관부의 occlusal $\frac{1}{3}$, middle $\frac{1}{3}$, cervical $\frac{1}{3}$과 치근부로 구분지어 시편을 부착하였다. 미세전단결합강도측정는 Universal testing machine(EZ-test; Shimadzu, Japan)에서 측정하였다. Occlusal 1/3부위에서는 SE가 가장 높은 값을, SM과 SB간에는 유의차가 없었으며, PL, GI순으로, Middle 1/3부위에서는 $SM{\;}{\geq}{\;}SE{\;}{\geq}{\;}SB{\;}{\geq}{\;}PL{\;}{\geq}{\;}GI$순으로, cervical 1/3부위에서는 SM, SE, SB간에 유의차가 없었다. Root dentin에서는 SM이 가장 높은 값을 보였으며 SE, PL, GI간에 유의차가 없었다. SE만이 치관부 상아질에 비해 치근부에서 유의할만한 결합강도의 감소를 나타냈다(p<0.05). GI는 치관부 상아질에서는 다른 군에 비해 유의성 있게 낮은 결합강도를 보였으나 치근부에서는 SE, PL과 유의차가 없었다.
