A Study on the Establishment & Functional Characteristics of Health Facilities for the Aged in Japan

  • Various housing measures are needed for the rapidly aging society of Korea. In particular, the welfare policy for the elderly has changed towards the community care. Taking this fact into consideration, it is necessary to have the establishment of a system that offers the elderly appropriate welfare services at their appropriate residence (ageing in place) for the effectiveness of the community care. In this aspect, there are a number of implications to Korea to study merits and demerits of the Health Facilities for the Aged (HFA) in Japan. The society of Japan has been rapidly aging since 1970, and Korea is to face the same situation. As for the data of this study, a total of 2,393 facilities (as of November 1999) mentioned in the annual report of the Japanese Ministry of Health, Labor and Welfare were classified based on types of their establishment: (1) free-standing structures (603 facilities); (2) annexes to hospitals (981 facilities); (3) annexes to welfare facilities (511 facilities); and (4) annexes to clinics (298 facilities). Next, 239 facilities were selected through taking a sample of 10 percent from each type of the HFA mentioned above. This was done through the random sampling method with the computer program of MS EXCEL. The Implications of the results of analyses are as follows. First, most of the health facilities were planned with the scale that was larger than the scale of standard special nursing homes in terms of the total floor area. Precise equations that were to obtain precise results of the scale of the HFA and the appropriate number of residents were obtained through the method of the regression analysis. Korea and Japan have similarities in terms of culture, society and family relations; however, the two countries also have differences in terms of the application of laws on the establishment of houses, hospitals, and welfare facilities. As for planning the scale of the HFA, the realities of Korea should be considered. Second, as for the functional aspect of the HFA with a condition of returning home, the place before and after the HFA showed the pattern of 'from a residential place to a residential place' and 'from a hospital to a hospital.' This reveals a close correlation with the types of the HFAs and operational ways of the facilities. Its cause is considered to be the aspect of the operation and management of the HFA rather than the aspect of its function of providing services in association with medical and health facilities. Therefore, when intermediate welfare facilities are considered in Korea, it is strongly advised to consider the problem of annexes to other facilities and efficiency of sharing of the facilities in terms of its operation and management.

Epidemiological and Clinicopathological Characteristics of Lung Cancer in a Teaching Hospital in Iran

  • Background: Lung cancer is one of the most commonly diagnosed cancers and the most frequent cause of cancer-related death worldwide. In Iran, it ranks second and third as the cancer-causing death in men and women, respectively. We carried out this study to find out the demographic, clinical, and histological characteristics and risk factors of lung cancer in a referral tertiary center in Iran. Materials and Methods: A retrospective study was conducted on cases of primary lung cancer based on the results of registered cancer reports of cytological and pathological specimens between March 2001 and March 2012. Demographic variables, clinical manifestations, histology and location of tumors were determined based on the data found in the medical records of each patient. Definite or probable etiologic factors were identified. Data analysis was performed with SPSS version 16 and a p-value ${\leq}0.05$ was considered as significant. Results: A total of 203 patients, with a mean age of 65.7 years (SD=11.2), with primary lung cancer were identified, 81.3% being men. Of the total, 110 cases (54.2%) were living in urban areas. In 53.2% of cases, the site of tumor was on the right side and in 72.9% of cases the lesion was centrally located. The histological types of lung cancer were squamous cell carcinoma (SCC) in 107 cases (52.7%), adenocarcinoma (AC) in 30 cases (14.8%), and small cell carcinoma (SC) in 27 cases (13.3%). Significant correlations between the gender and residence, smoking, and the histological type and location (central or peripheral) of tumor were found. The percentage of smokers was 75.2% in men and 15.8% in women. Conclusions: Smoking was the most important risk factor and squamous cell carcinoma the most common histological type of lung cancer in our study. Male sex and being a smoker was associated with histological types of SCC while being nonsmoker had relationship with adenocarcinoma.

삼육대 및 서울여대 재학 중인 일부 여대생의 거주형태에 따른 영양섭취상태 및 신체계측 비교 연구 (Comparison of Nutritional and Physical Status according to the Residental Type among College Women in Seoul Women's University and Sahmyook University)

  • This study was conducted to investigate the effects of residence types ((1) parent house, (2) dormitory or boarding home, (3) boarding home w/o food) on the dietary habits and health status of college women (20-23 y). Overall, 46.1% of college women reported that they were economically middle class and spent 200,000-390,000 won. Additionally, 17.4% of college women considered themselves healthy, while 33.8% reported that they were not healthy. Furthermore, 32.0% of college women had experience to control their weight and 39.8% reported that they exercised to control their weight. There were no significant differences in the height and weight of the subjects according to residential type, but the amount of skeletal muscles mass ($21.0\;{\pm}\;2.6\;kg$) of the subjects that lived in dormitory or boarding home was significantly higher (p < 0.05) than that of women who lived at home. Additionally, 30.8% of students that lived in private residences, 25.0% of students that dwelled in dormitory or boarding homes and 27.7% of students that boarding home w/o food had three regular meals every day (p < 0.05). It has been reported that 18.3% of college students eat processed and instant foods due to their convenience. In the present study, intakes of energy, protein, fat, vitamin A, vitamin $B_1$, vitamin $B_2$, vitamin $B_6$, niacin, vitamin C, vitamin E and folic acid for the subjects who lived in dormitory or boarding homes was significantly higher than those of students who lived in private residences (p < 0.05). Additionally, intakes of iron, calcium, and zinc were lower in the subjects that boarding home w/o food (p < 0.05). Finally, the blood glucose level was $84.7\;{\pm}\;13.0\;mg/dL$, and differed significantly by residential types (p < 0.05); however, the average glucose levels of all subjects were within the normal range (90-110 mg/dL).

인터넷 기반의 아파트 리모델링 및 자동 내역산출을 위한 시뮬레이터 디자인 연구 (The Study on the Internet-based Virtual Apartment Remodeling and Auto Estimation Simulator)

  • 최근 다양한 가족유형이 일반화되어가면서 현재 웹상의 주거공간 설계에 대한 전반적인 재검토와 함께 다양하게 표출되는 사용자 요구에 충족할 수 있도록 새로운 형식의 주거 공간 리모델링 제안이 이루어져야 할 시점에 이르렀다. 이에 거주자 생활 패턴 변화에 대응하는 주거 공간의 제안과 웹 구조상치 설계 및 리모델링의 과정이 요구되고 있는 실정이다. 따라서 본 연구에서는 -인터넷 기반 가상아파트 리모델링 지능형 서비스 시스템- 사용 시나리오와 프로토타입을 제시하여 리모델링 자동 내역 산출을 위한 시뮬레이터 디자인을 연구하였다. 연구는 크게 네 부분으로 구성되어진다. 먼저, 문헌 연구를 통하여 리모델링의 범위 및 유형 분류로 가능하게 된, 전반적인 리모델링 개념과 기존 범람되어지고 있는 용어들을 정리하였다. 두 번째로 본 연구의 기본모델인 아파트의 공간 가변화를 고정 요소와 가변 요소로 분류하여 실제적 가변벽체의 적응성과 유연성을 살펴보았다. 세 번째로 현장 공사단계의 시뮬레이션을 위한 상담, 마감재, 가견적, 실견적의 데이터베이스화 과정이다. 그 데이터베이스는 다이아그램 계층구조와, 모델을 리모델링 할때 생성되는 일위대가 및 물량 산출 내역 근거표에 기초하여 소개되었다. 마지막으로 기 개발된 시스템을 활용하여 리모델링 시뮬레이터의 시스템 구조도와 사용 시나리오를 제시하였다. 그리고 그 시스템의 디자인 대안을 탐색하고 표현하기 위한 수단으로 프로토 타입을 제작하여 기존 온라인 프로그램 인터페이스의 효율성을 증가시키고 정보 누락의 문제점을 개선하였다.

입원환자의 의료보장형태에 따른 가정간호 이용의사에 대한 연구 (A Study on Hospitalized Patients' Intent to Use Home Care Nursing According to the Types of Medical Security)

  • Purpose: This study is a descriptive research which is designed to investigate hospitalized patients' intent to use home care nursing according to the types of medical security. Method: This researcher surveyed 236 patients who were hospitalized at B medical center located in Busan,. Data were collected from Sep. 1 to Nov. 30, 2005 using a questionnaire survey, medical records, face-to-face interviews and observations. Collected data were analyzed in terms of frequency, percentage, mean and standard deviation through $x^2$-test and t-test under SPSS WIN 10.0 Program. Result: Out of the total subjects, 59.3% were medical aid clients and the remaining 40.7%, health insurance ones. The hospitalized period and frequency of the former group were 38.0 days and 4.0 times, respectively, while those of the latter, 37.7 and 3.4. When home care nursing clients were examined using a given classification device, it was found that out of the total 236 subjects, 205(86.9%) were needed to receive home care nursing, 121, medical aid and the other 84, health insurance. 24.0% of medical aid clients heard about home care nursing ever before, lower than 39.3% of health insurance clients. 43.8% of the former clients said cost for home care nursing was high while, 47.6% of the latter group responded expense for the nursing intervention was low. 30.6% of medical aid clients had intent to use home care nursing, lower than 47.6% of health insurance clients. 71.7% of those patients whose monthly income was 99 million won or below had no intent to use home care nursing, higher than 62.5% of those who were 100 million or over in monthly income(p<.05). 76.4% of those clients who had no nursing provider intented to use home care nursing, higher than those who had nursing provider(p<.05). Concerning contents of home care nursing, 85.1% of medical aid clients needed education, training and counseling while, 77.4% of health insurance aids wanted medication and injection. Conclusion: In conclusion, the use of home care nursing by medical aid clients should be promoted through improving conditions for home care nursing in terms of expense, family and residence and making public relations about activities and contents of the home care nursing.

주거이동을 통한 주거 불안정성 변화에 관한 연구 -저소득층을 대상으로 하여- (An Analysis of the Changes in the Housing Instability by the Residential Mobility of Low-Income Households)

  • 본 연구는 저소득층을 대상으로 주거이동을 통한 주거 불안정성의 변화를 분석하는데 목적을 두었다. 주거 불안정성은 주택의 노후도, 이사 횟수, 무주택 기간, 주거비 지불능력 등의 지표를 통해 측정하였다. 분석 결과 빈곤층 가구 중 자가 가구는 노후화된 주택의 물리적인 측면에서, 차가 가구는 주거비 부담 측면에서 주거 불안정성이 매우 심각한 것으로 나타났다. 주거이동을 통한 주거 불안정성 변화 요인을 다항로짓모형을 이용하여 추정한 결과 차가 가구의 경우 여성 가구주에 비해 남성 가구주일수록, 소득이 높을수록, 연령이 높을수록 상향이동할 확률이 높아지는 것으로 나타났다. 거주지별로 보면 비수도권 시 도에 비해 광역시 및 수도권에서 자가 가구의 하향이동 비율이 높아지고 차가 가구의 상향이동 비율이 낮아지는 것으로 나타나 대도시에 거주하고 있는 빈곤층 가구들이 자력으로 주거 불안정성을 개선하는데 더 많은 어려움을 겪는 것으로 나타났다. 또한 이전 거주지의 일반주택비율이 높을수록 빈곤층 자가 가구와 차가 가구 모두 하향이동 확률이 낮아지며, 특히 차가 가구의 상향이동 비율이 높아지는 것으로 나타났다. 이는 빈곤층의 주거 불안정성 개선을 위해서는 아파트 위주의 주택공급보다 저소득층이 지불가능한 저렴한 주택 재고량을 유지시키는 정책이 더 효과적일 수 있음을 시사 해준다.

몽골 울란바토르시 게르촌 주택의 겨울철 실내 초미세먼지(PM2.5) 농도의 시간적 변이 (Temporal Variation of Winter Indoor PM2.5 Concentrations in Dwellings in Ger Town of Ulaanbaatar, Mongolia)

  • Objectives: In Mongolian housing, they use coal as a fuel for indoor heating and cooking. The combustion of coal releases particulate matter, which can affect indoor air quality. The purpose of this study was to analyze the concentrations of indoor $PM_{2.5}$ in winter time dwellings in ger town. Methods: In this study, indoor $PM_{2.5}$ concentrations, temperature and humidity in houses were measured by a real-time PM monitor, while the time activity patterns of the residents were also observed. Results: The correlation between factors that may affect the indoor air quality was analyzed.The indoor $PM_{2.5}$ concentrations were $178.4{\pm}152.7{\mu}g/m^3$ (n=37). Five types of indoor $PM_{2.5}$ concentrations have been classified, which were associated with indoor activity. The stove type, fuel types and indoor activities such as cleaning, cooking and opening the stoves were not significantly associated with indoor $PM_{2.5}$ levels. Conclusions: Further study is needed to determine the effect of stove type through 24hours of indoor air quality monitoring.

재난취약계층의 위험이슈분석 -어린이, 임산부 사례를 중심으로- (Risk Issue Analysis of Disaster Vulnerable Groups -Focusing on Cases of Children and Pregnant Women)

  • 현대사회에서 노인, 장애인, 외국인, 어린이 등 재난취약계층의 수가 빠르게 증가하고 있다. 이러한 재난에 취약한 계층의 일반적인 특성은 경제적 어려움으로 인하여 주거형태가 재난에 취약하고, 재난에 노출되면 회복력이 더디다는 것이다. 본 연구는 재난취약계층에 대한 대상자 위험이슈분석을 통해 새롭게 도출되는 위험이슈를 파악하고, 정책개발의 기초자료를 제공하는 데 그 목적이 있다. 연구방법은 재난취약계층 중에서 어린이와 임산부의 사례를 중심으로 최근 10년간('10-19) 소셜미디어의 이슈 데이터를 중심으로 소셜네트워크 분석을 수행하였고, 연구결과 첫째, 이슈의 추이를 살펴보면 특정 사건의 발생과 연관성을 갖는다. 둘째, 위기관리의 유형, 대상, 관리방식에 대한 인식이 분석되었다. 셋째, 위험이슈의 해결방안이나 대상의 특성을 반영한 감정어가 분석되었고, 부정적인 감성을 유발하는 단어도 분석되었다. 따라서 본 기초데이터를 바탕으로 정부와 지자체는 국민의 정서분석과 대중의 인식을 토대로 급변하는 재난환경의 효율적인 위기관리체계를 구축하기를 바란다.

거주자 참여형 공동주거의 평면계획에 적용된 가변성의 특성 - 유럽의 거주자 참여형 다층 공동주거를 중심으로 - (The Characteristics of Flexibility applied to Unit Plan of Housing by Residents Participation - focusing on European Multi-story Housing applying Residents Participation -)

  • First of all, the multi-story Housing applying resident's participation in europe was classified by the menu selection method, the two-step supply method and the cooperative method. And then I analyzed flexible unit plan of cases for deriving the planning methode and the characteristics of flexibility. First, I analyzed the area and form of the unit plan, structure and Installation, fixed and variable elements to derive the planning method. The area of units are distributed from a minimum of $35m^2$ to a maximum of $150m^2$, and many of the unit planes have a narrow front and a deep depth. The structure is a long-span wall-structure or a skeleton structure, and is designed without any columns and bearing walls in the interior space for flexibility in spatial composition. The vertical shafts are located in the center of the unit in a box-form or in the corner at the unit dividing wall for free placement of interior wall. Fixed elements are framework and facility systems. Most of the future residents in the two-steps supply method and the cooperative method were able to freely design the internal space within the zoning concept proposed by the architect and change the location of the facade element within module system proposed by the architect. Second, the characteristics of the flexibility applied to the unit plan were divided in integrated flexibility, functional flexibility, construction flexibility, and supply flexibility. The integrated flexibility enables residents to give the variable space combination based on the complex structure of the inner space for providing various living experiences. Regarding functional flexibility, the three-dimensional spatial structure with neutral space has multi-functionality according to the needs of residents and easily accepts mixing of hybrid programs such as work and residence. Constructive flexibility allows residents to create identity by freely planning interior space and changing the size or location of facade components in a determined system of architects. Finally, various types of size and space composition are proposed and realized in the whole building applying menu selection method, so that flexibility in the offer can accommodate and integrate various types of living.

입지특성에 따른 문화재 유형별 홍수 민감성 기초연구 (A Study on Flood Susceptibility of Heritage Sites by Heritage Type Depending on Locational Characteristics)

  • 본 연구는 서울특별시 문화재의 입지특성을 분석하여 문화재 유형별 홍수 민감성을 파악하고자 하였다. 홍수 민감성 관련 입지요인으로 고도, 경사, 최소 수계거리, 지형입지를 분석하고, 문화재가 현 지점에 위치하게 된 역사·인문적 입지환경을 문헌고찰하였으며, 연구결과는 다음과 같다. 첫째, 서울특별시 문화재는 특히 한양도성 내에 집중 분포하고 있다. 계량적 공간분석 결과에서도 중구-종로구-은평구 진관동-강북구 우이동을 중심으로 문화재가 밀집하고 있음이 확인되었다. 둘째, 홍수 민감성이 높은 입지특성을 지닌 문화재 유형은 상업·유통, 교통, 근대교통·통신, 지구과학기념물, 주거, 관청, 궁궐로 나타났다. 셋째, 홍수 민감성이 낮은 입지특성을 지닌 유형은 자연명승, 통신, 요업, 불교, 무덤, 능묘조각 문화재로 나타났다. 자연환경의 변화와 더불어 인위적인 영향으로 인해 문화재의 가치를 손상시킬 수 있는 위험 요인들이 점차 증가하는 작금에, 본 연구는 문화재 고유의 특성을 반영한 취약성 분석을 위한 기초자료를 제공한다. 이는 자연적 요인과 더불어 인문·사회과학적 자료를 분석에 함께 포함하여 문화재 관리 및 보호를 위한 좀 더 포괄적이고 종합적인 통찰에 기여할 수 있을 것이다.
