SURGICAL REPOSITIONING OF AN INTRUDED PERMANENT MAXILLARY INCISOR IN A CEREBRAL PALSY PATIENT: A CASE REPORT (뇌성마비 환자에서 함입된 상악 중절치의 외과적 재위치: 증례보고)

  • Patients with cerebral palsy have higher risk of traumatic dental injuries because of clinical characteristics, such as, ataxia, large overjet and lip incompetency. Especially, intrusive luxation has rare occurrence but higher incidence of complications. It can be treated by expecting re-eruption, orthodontic reposition, and surgical reposition. Clinicians should be aware of management and follow-up in dealing with cerebral palsy patients who are exposed by intrusive luxation, due to their involuntary movement. This case report describes a 9-year-old male patient with cerebral palsy and epilepsy who experienced intrusion of maxillary permanent central incisor. After one-month follow-up, waiting for spontaneous eruption, pulp necrosis on maxillary permanent central incisor had proceeded. Therefore, surgical reposition with resin wire splint and apexification was performed under conscious sedation with midazolam. After two months, removal of resin wire splint was done. Gutta percha filling and composite resin restoration were performed after sixteen months. During five-year follow-up ankylosis and partial root resorption were observed. But there was no significant complications.

Effect of Motor Functions of Ipsilateral Upper Limb Induced by Long-Term Cane Usage in Chronic Stroke Patients (장기간 지팡이의 사용이 뇌졸중 환자의 건측 상지 기능에 미치는 영향)

  • Purpose: The purpose of this study was to investigate the adverse effects of sensorimotor function at the shoulder joint according to long-term cane usage in stroke patients without apraxic behavior, in terms of the presence of shoulder joint pain, accuracy of tracking task, proprioceptive joint position sense, and nine-hole pegboard. Methods: Nineteen stroke patients with long-term cane usage (cane usage group) and nineteen stroke patients without cane usage (non-cane usage group) were recruited. All subjects were tested in pain presence, a tracking task for visuomotor function, joint reposition, and nine-hole pegboard in the shoulder joint regarding the non-affected side. Results: In the accuracy index for tracking task and the nine-hole pegboard test, significant differences were observed between the cane usage group and the non-cane usage group. However, although a higher emergence of shoulder pain and a lower accuracy for joint reposition sense were detected in the cane usage group in comparison to the non-cane usage group, there were no significant differences between the two groups. Conclusion: Our findings suggest that long-term cane usage could induce to decrease in delicate movement and coordination in the non-affected upper arm in stroke patients. In addition, they could experience high frequency of shoulder pain and poor joint reposition sense. Therefore, careful evaluation and observation will be required concerning stroke patients with long-term cane usage.

The Adverse Effect of Proprioceptive Sense in Head-Neck according to Smartphone Usage

  • Purpose: Most studies have reported pain in the head-neck and upper-limbs according to smartphone usage, which is related to the proprioception sense in the head and neck, but there have been few studies. Therefore, the aim of this study was identify the adverse effects of the proprioceptive sense in the head-neck according to smartphone usage. Methods: Twenty-seven young adults (male: 9, female: 18) were enrolled in this study. The proprioceptive sense was measured through the joint reposition sense error and neural positon error in the head-neck during smartphone usage for 0, 5, and 20 minutes. The Noraxon MyoMotion system was used to record the joint position angle and neutral positon in the head-neck. One-way repeated ANOVA was used to identify the differences between the three smartphone use durations and the least-squares difference was used as a post hoc test. The data were analyzed using SPSS 18.0 software. Results: The joint reposition sense error and neural positon error in the head-neck were significantly different among the 0, 5, and 20 minutes of smartphone usage (p<0.05). In the post hoc test, the joint reposition sense error and neural positon error showed a significant difference between smartphone use for 0 minute and 5 minute, and between smartphone use for 0 minute and 20 minutes. Conclusion: This study suggests that smartphone use within 5 minutes can have adverse effects on the proprioceptive sense. Therefore, it is necessary to consider the appropriate use time and break time when using smart phones.

  • Objective : We report a case of BPPV responded to the Guibitang, Spleen Tonifying Sa-Am Acupuncture and Canalith Reposition Therapy. Methods : 1. Diagnosis and Evaulation: Bithermal Caloric test, Video Nystagmography, Dix-Hallpike test, VAS 2. Treatment: Guibitang, Acupuncture, Canalith Reposition Therapy. Results : We can observe remarkable improvement evaulated by VAS. Conclusion : It is considered that Guibitang and Acupuncture can be applied to the treatment of BPPV.

  • H maritime company in Korea had a long history of difficulty in obtaining enough empty containers to transport its commodities. Despite efforts to resolve the problem, the repositioning of empty containers between depots was still planned manually, which consumed much worker time. To automate the generation of reposition plans, we developed a planning system to optimize the company's reposition of empty containers between global depots with a linear programming model functioning as a key decision engine in the system. The model determines the route, volume and mode of transportation for repositioning empty containers from depots with surplus containers to those requiring containers, as well as the empty space of ships generated from the unloading or loading of full and empty containers. The system has been in operation in the company's global network since March 2008.

The Knee Joint Position Sense in Healthy Women (정상 성인 여성의 무릎관절 위치감각)

  • Purpose : The purpose of this study is to ascertain whether age, body mass index(BMI) and exercise frequency(EF) are correlated with knee joint position sense in healthy women. Methods : Healthy women of 150 who participated in this study were tested knee joint position sense; each reposition error was analyzed according to the age, BMI and EF. Reposition error was measured with a Myrin goniometer. Results : The age groups, BMI groups and EF groups demonstrated significant differences of the knee joint position sense. The older the healthy women are, the higher BMI is, and the lower EF is, the more decreased knee joint position sense has become. Conclusion : The older the women are, the higher BMI is, and the lower EF is, the more decreased the knee joint position sense in healthy women is. Therefore it needs to be considered to require management of obesity and regular exercise for prevention of knee injuries due to decreased joint position sense.

Anatomical repair of a bilateral Tessier No. 3 cleft by midfacial advancement

  • Background: Bilateral Tessier number 3 clefts are extremely rare, and their surgical treatments have not been well established. Case presentation: The authors describe the case of a patient with a right Tessier number 3, 11 facial cleft with microphthalmia, a left Tessier number 3 facial cleft with anophthalmia, and cleft palate. We repaired simultaneously the bilateral soft tissue clefts by premaxillary repositioning, cleft lip repair, facial cleft repair by nasal lengthening, midfacial advancement, and an upper eyelid transposition flap with repositioning both the medial canthi. Postoperatively, the patient showed an esthetically acceptable face without unnatural scars. Conclusions: We achieved good results functionally and esthetically by midfacial advancement with facial muscle reposition instead of traditional interdigitating Z-plasties. The surgical modality of our anatomical repair and 3 months follow-up results are presented.

Comparison of Lumbar Joint Reposition Error When Sitting in Upright and Slouched Positions for Five Minutes (5분 동안 바로 앉은 자세와 구부린 앉은 자세 시 허리 관절재위치 오류의 비교)

  • The purpose of this study was to compare the slouched and upright sitting positions on lumbar joint reposition error (LJRE). Twenty subjects without low back pain were recruited for this study and, using a random number table, were randomly assigned to two groups; the upright sitting position group (UP group) and the slouched sitting position group (SP group). UP group was first asked to sit in an upright position and the SP group to sit in a slouched position as an intervention, and then the LJRE of both groups was measured at the neutral sitting position (lumbar flexion $0^{\circ}$). The measurement of the LJRE was repeated after one day. The sitting positions were performed for five minutes each and the LJRE was measured using an electronic goniometer. An independent t-test was used to compare the LJRE of both groups after each sitting position and after one day. The results of this study showed that the LJRE after an intervention in the UP group was lower than in the SP group (p<.05) and the LJRE after one day in the UP group was lower than in the SP group (p<.05). The findings of this study indicate that the upright sitting position can be applied to decrease LJRE, compared with the slouched sitting position. These findings also support that the upright sitting position reduces the potential for proprioceptive loss.

The Effects of Lumbar Repositioning Sense and Muscle Fatigue after Stabilization Exercise Program in Disc Disease Patients (허리 디스크탈출증 환자의 재위치 감각과 근 피로도에 미치는 안정화운동 프로그램의 영향)

  • Background: The purpose of this study was designed to find out the effectiveness of reposition sense, muscle fatigue response on lumbar spine after apply lumbosacral stabilization exercise program to 4 patients with chronic low back pain and for 12 weeks. Method: In this study the reposition sense was measured in 3 angle(60, 30, 12) of the lumbar spine motion with blind by MedX test machine and the difference of instability to lumbar vertebra segments in flexion, extension test of standing position and spinal load test Mattress Test by Spinal Mouse. The stabilization exercise program was applied 2 times a week for 12 weeks in hospital and 2 times a day for 20 minutes at home. Result: The results of the present study were that the repositioning sense was appeared the most error in 12 angles of lumbar flexion and Men was appeared to decrease an error more than female in average value of 4 angles after 12 weeks. And average error of male was decrease more than female. Thus the effects of lumbosacral stabilization exercise was improved repositioning sense of prorioceptor. Fatigue response test(FRT) results, in male, was raised muscle fatigue rate during increase weight, on the other hand female appeared lower than male. Conclusion: As a results, lumbosacral stabilization exercise was aided to improvement of lumbar spine repositioning sense and vertebra segments stabilization. It was showed the rate of decrease in typically 12 degree angle point of each 3 angle(60, 36, 12). Especially, that spine instability patients will have a risk when in lifting a load or working with slight flexion posture around 12 degree during the daily of living life and it is probably to increase recurrence rate. Thus, not only lumbar extension muscle strength but also stability of vertebra segments in lumbar spine may be very important.

  • Intrusive luxation that takes approximately three percent of permanent teeth commonly occures at anterior teeth. This intrusion frequently leads to pulp necrosis, root resorption, marginal bone loss and these complications are influenced by depth of intrusion and stage of root development. Various treatment approaches have been suggested to manage of intrusive luxation. Techniques aiming to reposition the intruded tooth include an observation for spontaneous re-eruption, surgical or orthodontic repositioning. We report two cases with clinically satisfactory results for traumatically intruded maxillary central incisor. In one case which has a large open apex and mild intrusion depth, we observed for spontaneous eruption and then repositioning by forced eruption method. In other case, which has been completely intruded, was repositioned by surgical extrusion and followed by apexification.
