Root Causes and Characteristics of Occupational Incidents by Cause Investigation - Focusing on Maintenance and Repair Work - (사고 원인조사를 통한 농작업 안전사고 특성 연구 - 유지, 보수 작업을 중심으로 -)

  • This study aims to identify the types of safety accidents that occur during maintenance and repair operations and analyze the root causes. We used the logic tree diagramming to extract the root causes of 65 safety incidents specifically related to maintenance and repair work out of a total of 1,024 agricultural safety incidents that occurred between 2007 and 2020 collected by investigation with interviews. The extracted root causes were analyzed by categorizing them into six different safety system classifications. The results of the analysis and suggestions are as follows. Incidents related to maintenance and repair work, which can lead to irreversible safety hazards, have occurred frequently. These incidents were found to be occurring due to the overlapping of various safety system errors such as personal protective system and safety operation procedure. In the short term, it is necessary to emphasize compliance with the use of personal protective equipment, and enhancement of maintenance training. In the long term, it is necessary to establish a legal distinction for maintenance and repair work and clarify the responsible parties. Introducing a maintenance system is also crucial to prevent occupational injuries during maintenance and repair in agriculture.

  • A minicomputer system with the purpose of automations the operations of a Repair Service Bureau is described. SLMOS automates three major functional areas-administration, testing, and report senerations In the administration of repair sevice SLMOS performs a database maintenance functions alons with trouble collection and tracking. Trough ALT, automated testing of customer lines is performed, and the results of the test are utilized to dertermine the action taken to effect a repair. Certain problem areas may be detected and corrected by using the reports senerated by Trouble Report statics and Analysis System.

  • To systematic maintenance and record 10,000 port structures, under Ministry of Maritime and Fisheries, data base management system is required. In this study, POMIS(Port Maintenance and Information System) program is developed for this Purpose. In this program, records for inspection and repair for the various type of port structures can be maintained and operated through internet. Thus ministry can efficiently maintenance and repair port structures and systematically manage computerized maintenance and repair data.

  • Waterproofing is one of the important factor to improve performance of building elements. But existing repair system for waterproofing have many problems like a damage of waterproof layer due to movement of concrete substrate, blistering effect and moisture condition. So waterproofing layer can not play an important part to improve waterproofing function and performance of building.   • Based on real jobs, this study was carried out the Railway maintenance works in Taiwan. It hopes to use Geographic Information System(GIS) and Building Information Modeling(BIM) for changing the maintenance model from "post-damage repair" to "preventive maintenance". Accordingly, this research will build a Railway Repairing Database that installs all repair data. This database will also connect to GIS so that can be the source of analysis and statistic for repair paln. The main results of research are as follows: I. All data stored on cloud space that raises the ability to manage and plan on repair. II. Simplify the file work and increase the quantity of case data to the manager who can get more reliable source. III. The data offer objective judgment to avoid subjective argument. IV. Add the GIS information to improve the maintenance because of good analisis.

  • We have developed an automated circuit defect repair method. We focused on the resist patterns on the circuit material layer of TFT substrates before the etching process. In this paper, we report on the repair method that utilizes the syringe system and the stability of the open defect repair process.

The Internet-based Composite Repair (인터넷 기반 복합재 보수)

  • As composite materials are gaining wide acceptance in aircraft structure, repair of damaged composite is becoming an important issue. The issues in composite repair include high cost, material interchangeability, water ingression, and structural integrity. To address these problems, researchers have studied on the composite repair in various aspects. In this paper, an Internet-based advisory service (called Repair Advisory Service, RAS) for composite repair is proposed to increase efficiency for repair process. In the RAS system the web browser is used as its user interface, which provides easy access to the service. The RAS server provides web-based tools for failure prediction, Structural Repair Manual (SRM), automated prepreg cutting process, material properties, inventory and knowledge base. The computer codes implemented for repair design estimate the tensile failure and shear failure of repaired structures. The prediction of failure is based on the maximum strain criterion for tensile failure while elastic-perfect plastic shear failure model is applied for interfacial failure. The OEM's SRM is provided in the PDF format for viewing and searching by web browsers instead of looking up paper version SRM. The knowledge base in this site offers a room to share and distribute ideas, memos, publications, or suggestions from the repair engineers. The fabrication tool of RAS reads repair geometry from engineers then generates a CNC toolpath to cut prepreg patches. The RAS service is open to public and available at http://nano.gsnu.ac.kr/. Broad feedback from field technicians and engineers is welcome to improve the usefulness of RAS.

Preventive Policy With Minor Failure Under Age and Periodic Replacement (경미한 고장을 수반하는 시스템에 대한 노화 및 예방적 교체 정책)

  • The purpose of this study was to propose useful suggestion by analyzing preventive replacement policy under which there are minor and major failure. Here, major failure is defined as the failure of system which causes the system to stop working, however, the minor failure is defined as the situation in which the system is working but there exists inconvenience for the user to experience the degradation of performance. For this purpose, we formulated an expected cost rate as a function of periodic replacement time and the number of system update cycles. Then, using the probability and differentiation theory, we analyzed the cost rate function to find the optimal points for periodic replacement time and the number of system update cycles. Also, we present a numerical example to show how to apply our model to the real and practical situation in which even under the minor failure, the user of system is not willing to replace or repair the system immediately, instead he/she is willing to defer the repair or replacement until the periodic or preventive replacement time. Optimal preventive replacement timing using two variables, which are periodic replacement time and the number of system update cycles, is provided and the effects of those variables on the cost are analyzed.

Recent Evidence and Initial Experiences of Transcatheter Edge-to-Edge Repair of the Mitral Valve in South Korea

  • As a percutaneous technique for the reduction of mitral regurgitation, the MitraClip system (Abbott Vascular, Abbott Park, IL, USA) for transcatheter edge-to-edge repair of the mitral valve was developed in 1998 and first used in 2003. Its main advantage is being less invasive than surgery, because it can be performed through a transcatheter approach without any hemodynamic compromise. Recent studies have shown that this procedure reduces symptoms and improves functional capacity with low complication rates. Two randomized clinical trials have investigated the use of this technique for functional mitral regurgitation. The Korean Ministry of Food and Drug Safety approved its use for degenerative mitral regurgitation in 2019, and this procedure started to be performed in Korea in January 2020. Its use for functional mitral regurgitation was also approved in Korea in 2020. In this article, recent evidence on transcatheter edge-to-edge repair of the mitral valve and our initial experiences in Korea will be reviewed.

  • Laser cladding technology was studied as a method for upgrading the present repair procedures of damaged tubes in a nuclear steam generator and Doosan subsequently developed and designed a new Laser Cladding Repair System. One of the important features of this newly developed Laser Cladding Repair System is that molten metal can be deposited on damaged tube surfaces using a laser beam and filler wire without the need to install sleeves inside the tube. Laser cladding qualification tests on the steam generator tube material, Alloy 600, were performed according to ASME Section IX. Residual stress analyses were performed for weld metal and heat affected zone of as-welded and PWHT with SYSWELD software.
