사용 방법에 따른 임플란트 토크 조절기의 정확성 비교 분석 연구 (Comparative analysis of accuracy of how to use the implant torque controllers)

  • 연구 목적: 임상에서 가장 보편적으로 사용되는 기계식 토크 조절기를 선택하여 토크 조절기를 사용하는 방법에 따라서 제조사에서 권장하는 토크를 적용하고 유지하는데 정확도의 차이가 있는지를 비교, 분석하고자 하였다. 연구 재료 및 방법: 36개월을 사용한 3개 제조회사 제품의 토크 최대값 제한 장치 형태의 기계식 토크 조절기를 대상으로 하였다. 토크 조절기를 사용할 때마다 핸들 부위를 조이고 사용 후에는 핸들 부위를 풀어서 토크 조절기 내부의 스프링이 압축력에 의해 변형되는 것을 막고자 한 그룹과 토크 조절기를 사용 후에도 핸들 부위를 풀어내지 않고 토크 조절기 내부의 스프링이 압축력 하에 유지되게 사용한 그룹으로 나누어, 총 6개의 토크 조절기를 이용하여 풀림 토크를 측정하였다. 반복 측정에 따른 풀림 토크값의 차이를 비교하기 위해 repeated measures of ANOVA test (${\alpha}=.05$)를 사용하였고, 사용 방법의 차이에 따른 두 그룹간의 비교를 위해 독립표본 t-검정(Independent sample t-test)을 시행하였다. 결과:모든 실험군에서 반복 측정에 따라 풀림 토크값이 유의성 있게 감소하는 것으로 나타났고(P<.05), 사용 방법에 따른 두 그룹 간에 풀림 토크값은 유의한 차이가 있었다. 결론: 토크 조절기를 사용한 뒤 다시 핸들부분을 풀어서 토크 조절기 내부의 스프링이 압축력에 의해 변형되는 것을 막고 스프링의 복원력을 회복한 그룹은 그렇게 사용하지 않은 그룹과 통계적으로 유의한 차이가 관찰됐고, 기준 토크값(30 Ncm)에 좀 더 가까운 풀림 토크값을 보였다.

교점용 미니 임플랜트의 cutting flute의 길이 및 형태에 따른 식립 및 제거 토크의 비교 (Effect of cutting flute length and shape on insertion and removal torque of orthodontic mini-implants)

  • 본 연구는 self-tapping screw의 특징적 구조인 cutting flute의 길이 및 형태 변화에 따른 미니 임플랜트의 식립 및 제거 토크를 비교해보기 위해 시행되었다. 외경, 내경, 길이 및 pitch 등 다른 조건이 모두 동일하고 cutting flute의 형태만 다른 세 종류의 미니 임플랜트를 사용하였다(A군; 2.6 mm 길이의 flute가 형성된 그룹, B군; 3.9 mm의 길이이면서 직선형의 flute를 갖는 그룹, C군; 3.9 mm의 길이이면서 나선형의 flute를 갖는 그룹). 골밀도를 균일하게 하기 위해 실험용 인공골(Sawbones Inc., USA)을 사용하였으며, 수직력 및 회전속도를 일정하게 하기 위해 구동식 토크시험기를 이용하였다. 그 결과 다음과 같은 결론을 얻었다. 총 삽입 시간은 2 mm 두께의 피질골 시편에서는 세 군간에 차이를 보이지 않으나, 4 mm 두께의 피질골 시편에서는 C군 > B군 > A군 순으로 짧은 삽입 시간을 보였다. 최대 삽입 토크는 2 mm 두께의 시편에서는 A군이 다른 두 군에 비해 높은 값을 보이나, 4 mm 두께의 피질골 시편에서는 C군 > A군 > B군 순으로 높은 값을 보였다. 최대 제거 토크는 2 mm 두께의 피질골 시편에서는 세 군 사이에 차이가 없으나, 4 mm 두께의 피질골 시편에서는 C군이 다른 두 군에 비해 높은 값을 보여주었다. 이상의 결과로 보아 피질골이 두꺼운 부위에 미니 임플랜트를 식립할 경우 cutting flute의 길이 및 형태 또한 고려하여 미니 임플랜트를 선택하는 것이 바람직하다.

가토 경골에 식립된 Ca-P 박막코팅 임프란트의 뒤틀림 제거력 및 조직형태학적 평가 (THE EVALUATION OF THE REMOVAL TORQUE AND THE HISTOMORPHOMETRY OF THE CA-P COATING SURFACE IN RABBIT TIBIA)

  • Statement of problem: Surface texture of the implant is one of the important factors of the implant success, especially in the immediate implant loading. Many methods of the surface treatment of implant have developed and introduced. Purpose : This study was to evaluate the effects of the Ca-P coating implant crystallized the hydroxyapatite on the surface by the removal torque test and the histomorphometric analysis in vivo. Material and methods: 135 screw type implants, 4.0mm in length and 3.75mm in diameter were used in this study. Implants were divided into 3 groups and treated in the different mothods. Group I was not treated, Group II was treated in the SLA method, and Group III was treated in the Ca-P coating with the anodizing method and the hydroxyapatite was crystallized on the surface with the hydrothermal treatment. Firstly, the surface roughness of each group was measured, 45 rabbits were used in this experiment. Two implants were inserted on right tibial metaphysis and one implant was inserted on left side with the alternating order. After the healing periods of 3, 5, and 12 weeks, the rabbits were sacrificed to evaluate the osseointergration by the removal torque test and the histomorphometric analysis. Results : 1. In the analysis for the surface roughness, Group II showed the highest roughness. And Group III showed higher secondly. There was a significant difference one another statistically 2. In the removal torque test, Group III and II were significantly higher than Group I. There was no statistical difference between Group III and Group II. 3. For all Groups, the removal torque values at 12th week were significantly higher than at 3rd and 5th week. 4. In histomorphometric analysis, the bone implant contact rates of Group III and II were higher than that of Group I at 3rd and 5th week. There was a significant difference at 5th week. 5. In histomorphometric analysis, the bone implant contact rate of Group III and II increased from 3rd week to 5th week, but decreased at 12th week. In Group I, the contact rate at 12th week was significantly higher than at 3rd week and 5th week.

The influence of systemically administered oxytocin on the implant-bone interface area: an experimental study in the rabbit

  • PURPOSE. The purpose of this study was to assess the effect of systemically administered oxytocin (OT) on the implant-bone interface by using histomorphometric analysis and the removal torque test. MATERIALS AND METHODS. A total of 10 adult, New Zealand white, female rabbits were used in this experiment. We placed 2 implants (CSM; CSM Implant, Daegu, South Korea) in each distal femoral metaphysis on both the right and left sides; the implants on both sides were placed 10 mm apart. In each rabbit, 1 implant was prepared for histomorphometric analysis and the other 3 were prepared for the removal torque test (RT). The animals received intramuscular injections of either saline (control group; 0.15 M NaCl) or OT (experimental group; $200{\mu}g/rabbit$). The injections were initiated on Day 3 following the implant surgery and were continued for 4 subsequent weeks; the injections were administered twice per day (at a 12-h interval), for 2 days per week. RESULTS. While no statistically significant difference was observed between the two groups (P=.787), the control group had stronger removal torque values. The serum OT concentration (ELISA value) was higher in the OT-treated group, although no statistically significant difference was found. Further, the histomorphometric parameter (bone-to-implant contact [BIC], inter-thread bone, and peri-implant bone) values were higher in the experimental group, but the differences were not significant. CONCLUSION. We postulate that OT supplementation via intramuscular injection weakly contributes to the bone response at the implant-bone interface in rabbits. Therefore, higher concentrations or more frequent administration of OT may be required for a greater bone response to the implant. Further studies analyzing these aspects are needed.

Influence of the implant abutment types and the dynamic loading on initial screw loosening

  • PURPOSE. This study examined the effects of the abutment types and dynamic loading on the stability of implant prostheses with three types of implant abutments prepared using different fabrication methods by measuring removal torque both before and after dynamic loading. MATERIALS AND METHODS. Three groups of abutments were produced using different types of fabrication methods; stock abutment, gold cast abutment, and CAD/CAM custom abutment. A customized jig was fabricated to apply the load at $30^{\circ}$ to the long axis. The implant fixtures were fixed to the jig, and connected to the abutments with a 30 Ncm tightening torque. A sine curved dynamic load was applied for $10^5$ cycles between 25 and 250 N at 14 Hz. Removal torque before loading and after loading were evaluated. The SPSS was used for statistical analysis of the results. A Kruskal-Wallis test was performed to compare screw loosening between the abutment systems. A Wilcoxon signed-rank test was performed to compare screw loosening between before and after loading in each group (${\alpha}$=0.05). RESULTS. Removal torque value before loading and after loading was the highest in stock abutment, which was then followed by gold cast abutment and CAD/CAM custom abutment, but there were no significant differences. CONCLUSION. The abutment types did not have a significant influence on short term screw loosening. On the other hand, after $10^5$ cycles dynamic loading, CAD/CAM custom abutment affected the initial screw loosening, but stock abutment and gold cast abutment did not.

임플란트 고정체의 인위적 비틀림 후 시간 경과에 따른 골재유착 반응에 관한 연구 (Investigation of osseointegration according to the healing time after having iatrogenic mobility of implant fixtures)

  • 연구 목적: 본 연구의 목적은 골 유착된 임플란트에 인위적 비틀림을 가하여 골 유착을 파괴시킨 뒤 시간 경과에 따른 골 재유착력의 변화와 임플란트와 골과의 계면에서의 골 반응을 조직형태학적으로 분석하는 것이다. 연구 재료 및 방법: 표면처리 하지 않은 실험용 임플란트 (cp titanium, ${\phi}3.75\;mm{\times}4\;mm$)를 제작하였다. 3.5 kg이상의 뉴질랜드산 흰색 암컷 토끼 12마리의 좌우 경골에 제작한 임플란트를 2 - 3개씩 식립한 후, 6주의 골유착 유도한 기간을 부여한 후 비틀림 제거력을 측정하여 1차 측정치로 기록한 다음, 골 재유착을 얻기 위하여 다시 침하시켰다. 시간 경과에 따른 골유착의 변화를 살펴보기 위해 각각 4일 (I군), 1주 (II군), 2주 (III군), 3주 (IV군), 4주 (V군), 및 5주 (VI군)의 치유기간 후에 2차 비틀림 제거력을 측정하였다. 형광 조사 하에서 골 재형성여부를 검사하기 위해서 각군의 토끼에게 테트라사이클린 15 mg/kg을 근육 주사하였다. 2차 비틀림 제거력을 측정한 후 토끼를 희생시키고 각 실험군당 2 - 3개씩 총 16개를 제작하였다. 광학 현미경을 이용하여 형광 조사 하에서 골재형성여부를 관찰하였다. 측정된 각 실험군의 1차 비틀림 제거력과 2차 비틀림 제거력의 증감율의 평균을 구하였다. 그리고 각 실험군에서 그리고 실험군간에서 2차 비틀림 제거력이 1차 비틀림 제거력에 대하여 유의한 변화가 있는지 알아보기 위하여 paired t-test, one-way ANOVA, 그리고 DUNCAN 다중비교법으로 통계처리하였다. 결과: 실험 I군과 II군에서는 2차 비틀림 제거력이 감소하였으며 특히 실험 I군은 유의성 있게 감소하였다. 실험 III, IV, V, 그리고 VI군에서는 2차 비틀림 제거력이 유의성있게 증가하였다. 실험군 간 차이 비교에서, 실험 I, II군과 실험 III, IV, V, VI군 사이에 유의성있는 차이를 보였다. 형광 조사 검사에서 1주째 시편부터는 임플란트와 골과의 계면 부위에서 형광띠로 나타나는 광물화가 시작되는 부분이 관찰되었으며, 2주째 시편 이후부터는 더욱 분명하게 골생성이 이루어짐을 볼 수 있었다. 결론: 임플란트에 의원성 동요가 발생된 후 힘을 가하지 않은 상태로 일정시간 유지하게 되면 임플란트와 골 사이 계면에 다시 골유착이 발생되며 토끼의 경골에서는 높은 골유착력을 2주 만에 얻을 수 있었다는 것을 알 수 있었다.

Effect of microthreads on removal torque and bone-to-implant contact: an experimental study in miniature pigs

  • Purpose: The objective of this study was to evaluate the effect of microthreads on removal torque and bone-to-implant contact (BIC). Methods: Twelve miniature pigs for each experiment, a total of 24 animals, were used. In the removal torque analysis, each animal received 2 types of implants in each tibia, which were treated with sandblasting and acid etching but with or without microthreads at the marginal portion. The animals were sacrificed after 4, 8, or 12 weeks of healing. Each subgroup consisted of 4 animals, and the tibias were extracted and removal torque was measured. In the BIC analysis, each animal received 3 types of implants. Two types of implants were used for the removal torque test and another type of implant served as the control. The BIC experiment was conducted in the mandible of the animals. The $P_1-M_1$ teeth were extracted, and after a 4-month healing period, 3 each of the 2 types of implants were placed, with one type on each side of the mandible, for a total of 6 implants per animal. The animals were sacrificed after a 2-, 4-, or 8-week healing period. Each subgroup consisted of 4 animals. The mandibles were extracted, specimens were processed, and BIC was analyzed. Results: No significant difference in removal torque value or BIC was found between implants with and without microthreads. The removal torque value increased between 4 and 8 weeks of healing for both types of implants, but there was no significant difference between 8 and 12 weeks. The percentage of BIC increased between 2 and 4 weeks for all types of implants, but there was no significant difference between 4 and 8 weeks. Conclusions: The existence of microthreads was not a significant factor in mechanical and histological stability.

Impact on Retrievability by Cement Variety for Implant Restorations Equipped with a Lingual Slot

  • Purpose: The purpose of this study is to measure and compare the removal torques of different cements applied in attachments of zirconia restorations on titanium (Ti) abutments fitted with retrievable cement-type slot (RCS) on the lingual side for the better retrievablity by use of a slot driver. Materials and Methods: Three types of cements were used in the experiment: two permanent cements in $RelyX^{TM}$ U200 (RU) (3M ESPE) which is a resin cement and $FujiCem^{TM}$ (FC) (GC) which is a resin-modified glass ionomer cement, and a temporary cement in $Freegenol^{TM}$ temporary cement (TC) (GC). Measurements of removal torques were conducted as follows; an attached sample was fixed on the equipment customized for the experiment; a slot driver was connected to a MGT12 (Mark-10 Corp.), a torque measurement instrument; the sample had the driver fitted to its RCS and then was rotated until the it was removed; and finally, the maximum torque value was recorded. Result: As for the removal torque measurement results, the average values were $47.9{\pm}2.6Ncm$ for RU, $43.4{\pm}1.5Ncm$ for FC, and $20.9{\pm}1.0Ncm$ for TC. The statistical analysis using Kruskal-Wallis test yielded the significance probability of P<0.05 (P=0.002), which confirmed the presence of significant differences between the three groups. Conclusion: All three cements exhibit clinically acceptable levels of removal torque when applied to an upper zirconia implant restoration fitted with a lingual slot, with RU and FC, the two permanent cements, having the significantly higher values than that of TC, the temporary cement.

타이태늄 합금의 생체적합도에 관한 연구 (Quantitative investigations of titanium alloy implants)

  • Screw shaped implants of Titanium-13Zirconium-6Niobium(newly developed), Titanium-6Zirconium-6Sn-6Niobium(newly developed) and Titanium-6Aluminum-4Vanadium were machined with square top and inserted in rabbit bone for 3 months. Biomechanical tests(removal torque) showed Titanium-13Zirconium-6Niobium and Titanium-6Zirconium-6Sn-6Niobium to be more stable in the bone bed than those of Titanium-6Aluminum-4Vanadium. Titanium-13Zirconium-6Niobium implants demonstrated a mean removal torque of 31.59Ncm while Titanium-6Aluminum-4Vanadium demonstrated a mean removal torque of 25.27Ncm and Titanium-6Zirconium-6Sn-6Niobium revealed a mean removal torque of 37.44Ncm and were statistically significance in Wilcoxon Signed Rank test(P<0.05). Histomorphometrical comparisons were performed on $10\;{\mu}m$ thick undecalcified ground sections in the light microscope and Titanium-13Zirconium-6Niobium showed more mean bone-tometal contact ratio than to other twotitanium alloys but had no statistically significant differences were found among the three materials(P>0.01).

임플란트 직경이 골유착에 미치는 영향에 관한 연구 (INFLUENCE OF IMPLANT DIAMETER ON THE OSSEOINTEGRATION OF IMPLANTS : AN EXPERIMENTAL STUDY IN RABBITS)

  • Statement of problem : The survival rate of wide diameter implants was lower than of 3.75-mm implants in some clinical researches. Purpose : The purpose of this study was to investigate the influence of implant diameter on the osseointegration of implants in the rabbit femoral condyle and tibial metaphyses by means of removal torque measurements and histomorphometric analysis. Material and Method : Ten adult New Zealand White rabbits were used in this study Two 3.75-mm diameter implants were inserted through one cortical layer in the tibial metaphyses and one 3.75-mm diameter implant was inserted in the femoral condyle. 5.0-mm diameter implants were inserted in the other leg in the same manner. A total of 60 implants (3.75-mm diameter implants:30 : 5.0-mm diameter implants:30) were installed. After a healing time of 4 and 12 weeks, the peak removal torque values required to shear off the implants were recorded. From the removal torque values (Ncm) obtained, the mean shear stress ($N/mm^2$) was calculated. And the percentage of direct bone-to-implant contact and the percentage of bone area inside the thread were measured by Kappa Image Base-metreo. The Student's t-test was undertaken for statistical analysis (p<0.05). Results : The removal torque value of 5.0-mm diameter implants was higher than of 3.75-mm diameter implants (p<0.05). The difference of shear stress value between 3.75-mm and 5.0-mm diameter implants was not statistically significant (p>0.05). The percentage direct bone-to-implant contact had no statistical difference between two groups (p>0.05). The percentage of bone area inside the thread had no statistical difference between two groups (p>0.05). Conclusion It is concluded that the quality f osseointegration is not influenced by increasing implant diameter.
