성인간호학 학습목표 개발을 위한 간호실무 조사연구 (A Survey of the Scope of Nursing Competency for Developing Learning Objectives In Adult Health Nursing)

  • Nurses in today's challenging health care settings need to be skilled critical thinkers and clinical experts. The nurse must be able to use a broad knowledge base to mobilize resources, coordinate actions and evaluate outcomes in complex new situations. So the national licensing examination for registered nurses is change to improve the quality of professional competency of nurses in Korea. Prior to this, learning objectives should be developed and improved periodically. The purpose of this study is to describe the nursing competency to provide base line data for developing learning objectives in adult health nursing. This study was conducted by means of a questionnaire which was developed by the researcher after reviewing the literature. The questionnaire was based on learning objectives which were developed by a nation-wide nursing faculty majoring in adult health nursing. The subjects were 45 nurses in a middle level hospital. The collected data were treated using SPSS Win 7.5 Statistical Package so as to obtain such descriptive statistics as mean score, frequency, and to test reliability test, nonpar-Friedman test. To summarize the major findings in this study, it showed the scope of nursing competency and can guide the direction of study and methodological criteria to develop learning objectives. Recommendations for further research are: firstly, it is necessary to state learning objectives with learners' behavioral terminology; secondly, to overcome locality in scope of this study, there is a need to analyze with nation-wide sampling by an in-depth statistical analysis; thirdly, because the subjects of this study are mostly three-year graduate nurses, there is a need to compare this study with other studies of different subjects.

기혼간호사의 일-가정양립경험: 가족확대기 간호사를 중심으로 (Work-Family Compatibility Experience of Married Nurse: Focusing on the Expanding Stage of the Family Life Cycle)

  • 본 연구의 목적은 가족확대기의 기혼간호사들의 일과 가정의 양립의 경험의 본질을 탐색하고자 가족확대기의 기혼간호사 7명의 참여로 2015년 6월 10일부터 11월 30일까지 자료를 수집하여 Colaizzi의 현상학적 연구방법을 적용한 질적 연구이다. 연구결과 가족확대기의 기혼간호사들의 일과 가정의 양립의 경험은 29개의 주제와 12개의 주제모음, 4개의 범주로 도출되었다. 최종 도출된 4개의 범주는 '일과 양육 병행에 대한 현실', '일과 양육 양립의 대한 외적 내적 갈등', '양립의 어려움을 해결하기 위한 현실과의 타협', '양립을 위한 성장 욕구와 노력'으로 분석되었다. 기혼간호사의 일과 가정 양립의 원동력은 전문직 여성으로써의 가치감과 아이 양육에서의 행복감에 의미를 두고 있었다. 따라서 기혼간호사의 일과 가정 병행을 위한 기관 및 가족의 지속적인 지원이 필요할 것으로 사료된다.

  • Purpose: The purpose for this study was to identify the present working status and tasks of nurse administrators in response to requirements of expanding nurses' roles in hospitals. Methods: The design of the study was descriptive-exploratory. The participants were 338 registered nurses who were working in hospital administrative departments. The data were collected from January 17, to March 20, 2011, and analyzed with descriptive statistics. Results: The majority of nurse administrators worked in quality assurance departments while no nurse administrators assigned in finance/taxation departments. Duties of nurse administrators were developed as 117 tasks in 11 divisions. Conclusion: These results suggest that expanding nurses' roles require developing job descriptions, nursing curricula emphasizing hospital administration, positive relationships with other health professions and a supportive system.

비판적 사고와 간호근무환경이 간호사의 임상적 의사결정에 미치는 영향 (Effects of critical thinking and nursing practice environment on clinical decision making among nurses)

  • 본 연구는 간호사의 비판적 사고 및 간호근무환경과 임상적 의사결정의 관련성을 확인하고 임상적 의사결정에 미치는 관련요인을 확인하기 위한 서술적 조사연구이다. 대상자는 서울시내 일개 상급 종합병원에서 근무하는 간호사 261명으로 자가보고식 설문지를 이용하여 자료수집 하였다. 자료분석은 SPSS 21.0 프로그램으로 기술적 통계, independent t-test, one-way ANOVA, Pearson's correlation coefficients, 위계적 회귀분석을 시행하였다. 임상적 의사결정은 연령, 학력, 직위, 교대근무, 임상경력에 따라 통계적으로 유의한 차이를 보였다. 임상적 의사결정은 비판적 사고 및 간호근무환경과 정의 상관 관계를 보였다. 임상적 의사결정에 대해 영향을 미치는 요인은 직위, 비판적 사고 중 임상적 의사결정, 간호근무환경 중 양질의 간호를 위한 기반 및 인적 물적 자원의 적절성으로 이를 통해 임상적 의사결정의 35.8%를 설명하였다. 그러므로 비판적 사고 향상과 더불어 간호의 전문성 향상을 위한 질 관리, 효과적 인적 물적 자원 관리를 위한 행정지원은 간호사의 임상적 의사결정능력을 향상시킨다.

가정전문간호사의 의사소통 유형에 따른 주돌봄자의 자기효능감과 스트레스 대처방식 (Primary Caregivers' Self-Efficacy and Stress Coping Strategy According to Home Care Nurses' Communication Styles)

  • Purpose: This study aimed to investigate the difference between primary caregivers' self-efficacy and coping strategy according to the communication styles of home care nurses. Methods: Data were collected from 123 primary caregivers of patients who were registered at a home care nursing center in D city and who had been receiving home care for more than 3 months from January 1 to February 27, 2018. The questionnaire included items on communication style, self-efficacy, and stress coping strategy. The data were analyzed using descriptive statistics, t-test, and ANOVA. Results: Regarding primary caregivers' self-efficacy in terms of communication style, the caregivers showed higher efficacy in providing informative and friendly communication (F=14.07, p=.001). Regarding home care nurses' communication style and the stress coping strategy of the primary caregivers, the informative-friendly communication style was adopted the most for the problem-solving coping strategy (F=7.17, p=.001). Regarding the social support-seeking coping, home care nurses' friendly communication style was the most adopted (F=4.40, p=.014). Conclusion: This study suggests that home care nurses will plan to provide informative and friendly communication-oriented nursing care, and to improve self-efficacy and positively influence the coping method by using the communication styles appropriate to the state of the primary caregiver.

코로나 전담병원 간호사의 사회적 고립감과 우울의 관계에서 사회적 지지의 매개효과 (The Mediating Effect of Social Support on the Relationship between Social Isolation and Depression of Nurses in The COVID-19 Specialized Hospitals)

  • 본 연구는 간호사의 사회적 고립감과 우울의 관계에서 사회적 지지의 매개효과를 규명하고자 하였다. 연구의 대상자는 3개 코로나 전담병원에서 근무하는 간호사 135명이었다. 수집된 자료는 IBM SPSS Statistics 25.0을 이용하여 기술통계, 상관관계 및 매개 분석하였다. 매개효과는 PROCESS macro for SPSS 4.0 프로그램을 사용하여 분석하였다. 연구 결과, 사회적 고립감은 사회적 지지(𝛽=-.62, p<.001)와 우울(𝛽=.49, p<.001), 사회적 지지는 우울(𝛽=-.19, p=.028)에 직접적인 영향을 나타냈다. 사회적 고립감과 우울의 관계에서 사회적 지지는 매개효과가 있는 것으로 나타났다((indirect effect=.12, 95% CI=0.02~0.29). 그러므로 간호사의 우울을 경감시키기 위해서는 사회적 고립을 감소시키는 중재가 요구되고, 사회적 고립감을 지각하는 간호사의 우울을 완화하기 위해 적절한 개입을 고안할 때 사회적 지지를 강화하는 전략을 개발할 필요가 있다.

  • 본 연구는 병원 간호사를 대상으로 감염관리 조직문화, 자원의 지원, 코로나19 지식, 회복탄력성이 COVID-19 감염관리 수행에 영향을 미치는 요인을 파악하고자 시도되었다. 2022년 7월 27일부터 8월 10일까지 자가보고식 설문지를 통해 총 150명의 간호사에게 데이터를 수집하였으며, 그 중 147명의 자료가 연구에 활용되었다. 다중회귀분석에서 자원의 지원과 코로나19 지식, 회복탄력성이 코로나19 감염관리 수행의 의미 있는 요인으로 분석되었다. 지속되는 코로나19 팬데믹 상황에서 간호사의 코로나19 감염관리 수행을 향상시키기 위해 조직 차원의 지원과 개인의 감염관리 역량을 증가시킬 수 있는 전략을 위한 프로그램 개발 및 평가가 필요하다.

간호사 국가고시문제의 내용분석 (A Content Analysis of the test of the National Examination for Registration Nurses in Korea over 3 years)

  • This study aimed to analyse the test contents of the national examination for the registered nurses (NERN) over 3 years from 1991 to 1993 in Korea. In recent years in Korea, the MCQ(multiple choice question) has been showing to be a highly recognized method for assessing the qualification of registered nurses. Unfortunately, nursing faculties have found NERN had some bad MCQs through having evaluation workshop for Some MCQs often provide so many unwriting clues which become a bias of the results, and some items fell into the category of the lower level of educational taxonomy such as isolated recall a fact or data. Frequently the stems of the questions are ambigous, unclear, disputable, esoterical or trivial. Considering those fallacies of the national examination, it is very critical to review the test items to see whether it is of high quality, is more fair, reliable and objective in depth. Therefore, this study was done to provide data for the improvement of the test contents as well as the teachers's assessment skill. For this study, the ad hoc committee was composed of 16 members, including 5 education board members of Korean Academic Nurses Association and 11 nursing faculty members. This committee had one day panel discussion and filled the checklist for this study. The process of analysing data was held over 10 times during 1992-1994. The analysis focussed on educational taxonomy such as cognitive domain(knowledge), psychmotor domain (skill), affective domain(attitude) and the level of learning such as recall, understanding, problems solving, and learning area of theory and practice, and the learning content categorised by nursing process and disease process. The test analysed using difficulty index and the structure of the test items was analysed. The conclusions and suggestion as follows : 1. In learning area, the average ratio of the theory and practice was 1 : 1.1 which was less than 1 : 2 suggested by Korean National Health Institute, and the ratio was different by the 8 leaning subjects of nursing. 2. In category of the educational taxonomy, the knowledge domain was emphasized mostly(79. 7%), the skill domain was 14.9%, and the attitude domain was 5.4% only. 3. In the level of learning, generally, the test items of the level of recall(45.5%) and the understanding(46.3%) were covered almost and the problem solving was 8.1%. 4. In the learning contents, generally, the test items related to nursing process was 67.2% and that of disease process was 32.8%. However, this proportion was different by the 8 leaning subjects. Even though the nursing diagnosis has been emphasized in nursing curricula recently, the test items of this was identified very few. 5. In the structure of the test item, some were not clear, incorrect grammar, unclear description and some have clues to answer. 6. In the item analysis, the non-acceptable level of the difficulty index (means too easy) was 65.7%, and the acceptable level was 33.9%. Considering the results we would like to suggest the followings, 1. Since the test items of knowledge domain was dominant, the test items of the practice domain and attitude domain should be emphasized more. 2. The regular review and analysis of NERN should be arranged in order to improve the quality of the test items which will give influence to the nursing education positively.
