Image and Exposure Dose in Accordance with Radiation Quality on Plain Chest Radiography (흉부촬영(胸部撮影)에서 증감지(增感紙)-필름계의 선질변화(線質變化)에 따른 감도(感度)와 화질에 관(關)한 연구(硏究))

  • Routine chest radiography is generally imaged by high voltage technique but some radiological technologists use low voltage for imaging. High voltage is usually said between $120\;kV{\sim}140\;kV$. Some RTs like using heavy filtration but others seldom like using it. However which is better for use calcium tungustate film screen system or ortho system and high contrast film or wide latitude c-type film for the exculusive use of chest radiography. We could not make a decision which is ideal method for use. In my opinion any method is not always exellent for chest radiography. In my experiments that I had at Kaken hospital in Japan last year I expect to keep the balance between image quality and diagnostic range and to reduce radiation dose for patients. My experiments are as follows. 1. We have looked into system characteristics(speed and contrast) in accordance with kVp($80{\sim}140$) and added filter($no{\sim}1/16\;VL$) in three screen film systems(BX3+CRONEX4, SRO750+MGH, SRO750+MGL). 2. We have looked into skin dose and film dose with same D=1.8 lung field density in accordance with kVp($80{\sim}140$) and added filter($no{\sim}1/16\;VL$) in three screen film systems. 3. We have compared with the evaluation between correlation of physical image quality(MTF) and optical diagnostic capability. Result are follows. 1. Speed of BX3+CRONEX4 became higher in accodance with kVp and thickness of filter but speed of ortho system was not as like regular system. Thicker filter diminished the speed over 100 kV range in SRO750+MGL. In case of SRO750+MGH speed of 1/16VL filter was looked into lower than speed of 1/4VL filter. Sensitivity of ortho system depends on tube voltage and added filter. 2. Skin dose has been detected $225\;{\mu}Gy{\sim}66\;{\mu}Gy$ in BX3+CRONEX4 from 80 kV, no filter to 140 kV, 1/16VL filter. SRO750+MGH could reduce the patient dose $1/2{\sim}1/3$ level in comparison to that of BX3+CRONEX4. 3. The higher kV was the worse MTF became the thicker filter was the worse MTF became too. MTF of BX3+CRONEX4 was detected better than MTF of SRO750+MGH but SRO750+MGH's optical detectability of small lesion in lung field came out better than that of BX3+CRONEX4. Conclusion Recently routine chest radiography is generally imaged by high voltage but it seems to be there are some questions in using of film screen combination. In high voltage chest radiography the subject contrast will come down that means latitude become wider. In this case if we select the low contrast film screen system(C or L type) the film contrast will fall down extremly and detectability of small lesion will be deteriorated. Wide latitude C, L type film has a merit of high detectability on mediastinum. Furthermore high contrast film screen system has the advantage to keep the high contrast in low density region as like mediastinum and heart shadow. Therefore in low subject contrast high voltage chest radiography we would rather choose the high contrast film screen system(H type) I think. From a view point of patient dose detectability of mediastinum and lung field. The optimum technical facter was found out 120 kV, 1/16VL filter : BX3+CRONEX4, 140 kV, 1/4VL filter : SRO750+MGH, 100 kV, 1/4VL filter : SRO750+MGL.

  • Neutropenia is a major dose-limiting factor in cancer chemotherapy diminishing its usefulness and increase patient's susceptibility to infectious disease. Some recombinant human granulocyte colony stimulating factors(rhG-CSFs) are in use to reduce the risk of this serious side effect. In this study, we examined the pharmacological properties of DA-3030, a rhG-CSF expressed in E. coli. DA-3030 100 and $\mu\textrm{g}$/kg, i. v., had no significant effect on the central nervous, gastrointestinal system in mice and cardiovascular system in rabbits, but it slightly inhibited the spontaneous motility of isolated nonpregnant uterus in rats. It also had no influence on excretion of urinary electrolytes. DA-3030 administered for successive 3 days increased the blood WBC count in zymosan air pouch inflammed rats and in normal rats. These results indicate that DA-3030 has little side effects in animals.

Are dental radiographs dangerous? (치과방사선영상 검사는 위험한 술식인가?)

  • Radiographs can help in the diagnosis and treatment planning, but the exposure to ionizing radiation may elevate the risk of developing cancer in a person's lifetime. The objective of this review is to briefly summarize 1) radiation risk, especially cancer risks associated with diagnostic imaging, 2) linear, non-threshold (LNT) hypothesis, 3) the risks of radiation exposure to a fetus, and 4) the campaign of Image Gently. The individual risk of radiation-related cancer from any single medical imaging procedure is extremely small and it is not likely to be cancer risk at doses lower than 100 mGy, but patients may be harmed by avoiding diagnostic imaging due to fear of radiation hazard. Dentists need to understand the radiation doses delivered by various radiographic techniques and the acceptable exposure thresholds to effectively advise the patient and to reduce the unnecessary radiation

  • Cerebral palsy is a nonprogressive brain disorder occurring during gestation, parturition or neonatal period with resultant abnormality of posture or other deficits. Recently the early diagnosis and early treatment has been emphasized in management of cerebral palsy. We studied 1 cerebral palsied child that having been treated anticonvulsive therapy in Sangge-Baek Hospital(상계 백병원) for 1 year. We improved the patient's general condition through dispensing Herb-Med(養胃湯 加味), thus we could reduce the antiepileptic & Sedative drug dose simultaneously controlling convulsion. In this point, we could find the possibility of east-west integrated medicine's cooperation and the fact that Herb-Med could be helpful to controlling convulsion.

  • We describe a backprojection filtering method for limited angle tomography. In order to reduce blur artifacts originated from the out-of-planes and control high-frequency noise, we employed two band-limited window functions, and which were realized by Hann filters in this study. Based on the experimentally measured data, the performance of the developed method is demonstrated by comparing with the conventional shift-and-add and filtered backprojection methods. Application of the developed method to the dental imaging has a potential to be used for the preoperative evaluation of the jaw for dental implants with much reduced patient dose instead of the conventional dental computed tomography.

  • Recently, digital tomosynthesis system (DTS) has been developed to reduce overlap using conventional X-ray and to overcome high patient dose problem using computed tomography (CT). The purpose of this study was to develop image reconstruction algorithm and to evaluate image characteristics and dose with chest digital tomosynthesis (CDT) system. Image reconstruction was used for filtered back-projection (FBP) methods and system geometry was constructed ${\pm}10^{\circ}$, ${\pm}15^{\circ}$, ${\pm}20^{\circ}$, and ${\pm}30^{\circ}$ angular range for acquiring phantom images. Image characteristics carried out root mean square error (RMSE) and signal difference-to-noise ratio (SDNR), and dose is evaluated effective dose with ${\pm}20^{\circ}$ angular range. According to the results, the phantom image with slice thickness filter has superb RMSE and SDNR, and effective dose was 0.166 mSv. In conclusion, we demonstrated usefulness of developed CDT image reconstruction algorithm and we constructed CDT basic output data with measuring effective dose.

Dose Reduction Effect by using Compression Band during Chest CT Examination in Female Patients (여성의 흉부 CT 검사 시 압박밴드 사용에 따른 선량 감소효과)

  • CT scan is reported to have a high risk of cancer due to a relatively high dose among medical radiological examinations. In particular, exposure to radiation to the breast, which is sensitive to radiation, is inevitable during a chest CT scan for female patient. In this study, the dose reduction effect of wearing a compression band during chest CT scans in women was evaluated, and the lifetime attributable risk due to the effective dose exposed during the CT scan was estimated. As a result, when the compression band was used, the effective tube current decreased as the outer perimeter of the chest became smaller, and it was analyzed that the CT dose index and effective dose were also reduced. In addition, the lifetime attributable risk by chest CT scan was found to reduce the cancer risk by 3.2 per 100,000 for all cancers, 0.2 per 100,000 for solid cancer, and 0.8 per 100,000 for breast cancer, based on women in their 30s when using a compression band. It is judged that the risk of cancer can be reduced through the use of appropriate scan parameters and dose optimization measures such as compression bands for future CT examinations.

A Study on the Reduction of Absorbed Dose through the Insertion of a Shielding Material in the Intraoralsensor of Dental Radiography (치과 방사선촬영 시 구내 센서 내 차폐체 삽입을 통한 피폭선량 감소 연구)

  • In order to reduce the absorbed dose given to the patient during dental radiography, a sensor that inserts a shield into the intraoralsensor was designed. Using the designed sensor, the change in absorbed dose depending on whether or not a shield was used was evaluated. The system used to evaluate the absorbed dose is VEX-S300C from Vatech, and the energy spectrum of X-rays was obtained through SPEKTR simulation based on the irradiation conditions of 65 kV, 3 mA, and 0.15 sec, and the number of photons for each energy was derived. After designing the system through Genat4 Application for Tomographic Emission(GATE) simulation, the energy spectrum obtained was used as a radiation source to calculate the absorbed dose. Lead was used for the shield, and simulations were performed at 0.1 mm thickness intervals from 0.1 mm to 0.5 mm was evaluated. In the case of using an X-ray field with a diameter of 60 mm, the decrease in absorbed dose according to the presence or absence of a shield decreased exponentially as the thickness of the shield increased. In addition, when a 20 mm × 30 mm field was used, the absorbed dose was significantly reduced even when no shield was used, and it was confirmed that the absorbed dose was further reduced when a shield was used.

A study on the usefulness of a fusion model designed cloak shield to reduce the radiation exposure of the assistant during CT of severely injured patient (중증외상환자 CT 검사 시 검사보조자의 방사선피폭 경감을 위한 융합적 망토 차폐체의 유용성 연구)

  • The purpose of this study is to evaluate the usefulness of a newly fusion model designed cloak shield to reduce the radiation exposure of the assistant during CT(computed tomography) of severely injured patient. Radiation dose was measured in the heart, both axillary and thyroid areas using newly designed cloak shield and existing shield with head phantom and human phantom under the same conditions as brain vascular CT scan. The newly designed cloak shield was measured higher for radiation shielding rate than the existing shields, 61.9 % for heart, 46.2 % for left axillary, 69.8 % for right axillary and 71.1 % for thyroid gland, respectively. a newly developed fusion model of cloak shields are useful for reducing radiation exposure. It is expected to make a significant contribution to reduction of radiation exposure.

The Evaluation of Hybrid-Volumetric Modulated Arc Therapy for Lung Cancer Radiation Therapy (폐암 방사선 치료 시 Hybrid-Volumetric Modulated Arc Therapy의 유용성 평가)

  • Objectives: In the Lung, the VMAT rotates continuously and examines radiation. That increases the low doses to normal lung. Due to that, the incidence of radiation pneumonia among radiation side effects may increase. The cause of radiation pneumonia is the lower dose area of the lungs. The H-VMAT was applied to patients who applied to reduce radiation in the lower doses of the lungs. We wanted to assess the usefulness of the H-VMAT by comparing the radiation doses to the low dose areas of the lungs and the normal organs. Materials and Methods: A total of 26 patients who applied for a H-VMAT procedure were applied to the patient. The prescription dose applied to total dose 44 Gy from 22 divisions. For each patient, a plan was implemented with Conventional RT, VMAT and H-VMAT. Conventional RT was carried out in four to five fields each, considering the size, location, shape, and location of the PTV. In the case of a VMAT plan, the two Half ARC, three Half ARC method and the two Full ARC were planned. The H-VMAT was planned by adding two Static fields in the VMAT, taking into account the dose of the lung and the tolerance dose of the organs. Results: In the NSCLC, the lung doses $V_5$ and $V_{10}$ of the lungs except for the treatment plan volume were the lowest with $55.40{\pm}13.39%$ and $32.05{\pm}11.37%$ of H-VMAT. And, in the SCLC, the lung doses of V5 and V10 were the lowest at $64.32{\pm}16.15%$ and $35.50{\pm}9.91%$, respectively. The spinal dose of VMAT in NSCLC was $21.15{\pm}4.02Gy$, which was 7.94 Gy lower than other treatment methods. The lowest spinal dose was delivered at $19.72{\pm}1.82Gy$ for SCLC. The mean dose delivered to the esophagus was also $17.44{\pm}2.04Gy$ and $17.84{\pm}9.20Gy$ in SCLC and NSCLC, respectively. Conclusion: When comparing the value of the surrounding normal organ dose, the VMAT showed that less doses were transmitted from the heart, esophagus and spinal cord than the rest of the treatment plan. However, it was similar to VMAT in normal organs except for the spinal cord. VMAT has increased doses of some normal organs but did not exceed the tolerance dose. It showed a low value in $V_5$, $V_{10}$. When comparing Conventional RT, VMAT, and H-VMAT, If the dose to the heart, esophagus and spinal cord is lower than the tolerance dose, it is thought to reduce the incidence of radiation pneumonia by applying H-VMAT that show the benefits of low doses of the lungs.
