Sweet's Syndrome Associated with Graves' Disease

  • Sweet's syndrome, or acute febrile neutrophilic dermatosis, occurs in association with autoimmune diseases such as Hashimoto's thyroiditis but is rare in Graves' disease, in which all cases are induced by propylthiouracil (PTU). We report a case of Sweet's syndrome in a patient with Graves' disease treated with methimazole (MMI) during three weeks. A 34-year-old man presented with the acute onset of high fever, skin rashes on the whole body, arthralgia, and acroparesthesia. Laboratory results showed leukocytosis and elevated C-reactive protein. MMI first stopped and antibiotics and antihistamine therapy started, but his symptoms dramatically improved after oral prednisolone. Graves' disease has again been treated by MMI because of his aggravated ophthalmopathy. After one year of retreatment with MMI, there has been no recurrence of Sweet's syndrome, supporting that Sweet's syndrome in this case was not related to MMI exposure. To our knowledge, this is the first report of Sweet's syndrome associated with Graves' disease per se but not PTU or MMI use.

Polydeoxyribonucleotide, as a Novel Approach for the Management of Medication-Related Osteonecrosis of the Jaw: A Preliminary Observational Study

  • Purpose: Polydeoxyribonucleotide (PDRN), consisting of a mixture of deoxyribonucleotide polymers, has been suggested to have anti-inflammatory effects and enhance angiogenesis as an adenosine $A_{2A}$ receptor agonist. The aim of this study was to report the effectiveness of PDRN as an adjuvant therapy after surgical debridement in MRONJ (medication-related osteonecrosis of the jaw) patients. Materials and Methods: Five patients (1 male, 4 females, age 65~79 years) who were diagnosed with MRONJ stage 2 or 3 underwent surgical debridement and PDRN mucosal injection. After surgical debridement, patients were subject to daily injection with 1 ml of PDRN around the surgical wound for 14 days. Result: The patients' symptoms gradually disappeared. The surgical wound uneventfully healed, and no recurrence was observed during the follow-up period. Conclusion: Although further studies are required, the present study first describes the possibility of PDRN as a useful option for MRONJ treatment.

한의학적 치료를 활용한 손톱을 침범한 사마귀 환자 11례 증례보고 (11 Cases of Periungual Warts Treated by Korean Medicine)

  • Objectives : The purpose of this study is to report the effect of the Korean medical treatment for periungual warts. Methods : From December 2016 to March 2019, all patients were treated by herbal medicine, acupuncture, sweet bee venom acupuncture and moxibustion. Photographs of lesions were used to evaluate the changes in symptoms. Results : After Korean medical treatment, the periungual warts of all patients were disappeared remarkably and there were no adverse effects such as nail dystropy, nail loss and no recurrence at all. Conclusions : This study shows that Korean Medicine such as herbal medicine, acupuncture and moxibustion was effective to treat periungual warts.

A Case of Long-term Survival in a Patient with Primary Primitive Neuroectodermal Tumor of the Lung

  • Primitive neuroectodermal tumor (PNET) arising primarily in the lung is an extremely rare and aggressive malignancy with poor chances of patient survival. We present a case of long-term survival by a 29-year-old woman with PNET diagnosed after a hertological and immunohertochemical examination of a biopsy specimen obtained by performing video-assisted thoracic surgery. The patient underwent a left lower lung lobe lobectomy and 6 cycles of adjuvant chemotherapy. The patient has been free of any symptoms of the recurrence of the disease for 6 years after treatment completion.

Isolated calcific tendinitis at the posterosuperior labrum: a rare case study

  • Calcific tendinitis of the shoulder joint, also known as chemical furuncle of the shoulder, causes intense shoulder pain and usually occurs within 1-2 cm from the insertion of the rotator cuff. We experienced a rare case of calcific tendinitis in the posterosuperior labrum of the shoulder joint in a 39-year-old male patient who presented with severe pain and weakness in the right shoulder. Radiographs and magnetic resonance imaging (MRI) findings showed calcific tendinitis in the posterosuperior labrum of the shoulder joint. A 1-week attempt at conservative treatment failed, so the calcified deposit in the posterosuperior labrum was arthroscopically removed. The patient's symptoms were completely relieved, and satisfactory clinical outcomes were achieved. Postoperative follow-up X-ray and MRI showed no recurrence of calcific tendinitis.

Successful Treatment of Post-operative Keloid with Combined Cryotherapy and Ablative Fractional CO2 Laser

  • Keloids are pathologic fibroproliferative conditions characterized by excessive collagen deposition during wound healing. The pathogenesis of keloids is not fully understood, and current treatment options show variable results. In this case report, the patient developed a keloid after bilateral total thyroidectomy, and was treated with a combination approach using fractional ablative laser systems along with cryotherapy and triamcinolone injection. After seven monthly sessions of combination treatment, the patient presented marked improvement of the scar texture and symptoms. Furthermore, there was no recurrence for up to 2 years. We propose this combination as a safe and effective treatment option for keloid patients.

  • A 38-year-old man visited the hospital for a voice change that began two months previously. There were no other accompanying symptoms; the patient had a history of 20 years of smoking. Carbon dioxide (CO2) laser cordectomy was performed on the left side of the vocal cord, and the biopsy results showed squamous cell carcinoma. Radiation therapy was not performed, and CO2 laser cordectomy was performed again because of a recurrence of left vocal cord mass three years after a progress observation.

Periantral fungal abscess after zygoma reduction: a case report

  • This case report describes our treatment of a persistent periantral abscess in a 35-year-old woman. The abscess developed following a zygoma-reduction surgery, during which a preexisting fungal ball had not been addressed. Our comprehensive treatment approach included functional endoscopic sinus surgery, fungal ball removal, abscess drainage, and debridement. Two weeks postoperatively, the patient's symptoms had resolved. A 6-month postoperative follow-up revealed no signs of recurrence or complications, and the patient reported satisfactory functional and aesthetic results. This case underscores the importance of thorough preoperative evaluations and raises awareness about the potential risks of untreated asymptomatic pathologies, which can potentially progress and lead to further complications.

Simple bone cyst recurred in adjacent areas: A case report

  • Simple bone cyst is a pseudocyst that typically occurs in patients during their second and third decades of life. This benign entity is an empty or fluid-filled cavity that lacks a true epithelial lining. Simple bone cysts are often asymptomatic and are commonly found in mandibular body, predominantly in the posterior region. The treatment of simple bone cysts can be influenced by factors such as the patient's age, the size of the lesion, and the presence or absence of symptoms. In the case of a simple bone cyst in the mandible that is small and symptomless, a watchful waiting approach may be appropriate. However, if the cyst is large and symptomatic, surgical treatment is recommended. This report presents a radiological examination of a simple bone cyst that developed around the root of the mandibular first molar in a 36-year-old female patient. The cyst recurred in adjacent areas despite surgical treatment.

Auricular conchal cartilage for treatment of empty nose syndrome

  • Empty nose syndrome is an uncommon complication of turbinoplasty and other nasal operations. It presents with various manifestations, with the most common symptom being paradoxical nasal obstruction. Here, we report the case of a 67-year-old male patient diagnosed with empty nose syndrome 20 years after turbinoplasty. Autologous auricular conchal cartilage was harvested and implanted into the lateral wall of the inferior turbinate to reduce nasal volume. The patient experienced significant relief from subjective nasal obstruction symptoms immediately after surgery. Over a 23-month follow-up period, no side effects or symptom recurrence was reported.
