폐 aspergillosis의 외과적 치료 - 5예 보고 - (Surgical Treatment of Pulmonary Aspergillosis - 5 Cases -)

  • Pulmonary aspergillosis is a rare disease, most commonly presenting pre-existing cavitary disease. In Department of thoracic and cardiovascular surgery, Chonbuk National University, 5 patients have been recognized as having this disorder from 1988 to 1990, June. Of the 5 patients, three were female and two were male. Age ranged from 30 to 53 years, the average age was 38.4 years. The most common presenting symptoms were blood-tinged sputum, hemoptysis, coughing, and chest pain Pulmonary tuberculosis occupied 8-% of underlying pulmonary disease. The locations of lesion were right upper lobe in 3 cases, left upper lobe in 1, and right lower lobe in l. All of these patients were treated by surgical resection. The operative procedures were as follows: lobectomy, 3 cases; segmentectomy, 1 case; lobectomy and segmentectomy, 1 case. There was no death in early and late postoperative period. Empyema and dead space developed in two cases, respectively. The postoperative empyema was treated with open thoracostomy and the dead space was carefully observed. During follow-up, there was no recurrence.

식도게실의 외과적 치료 (Surgical Treatment of Esophageal Diverticulum)

  • Sixteen patients with esophageal diverticulum operated on between July 1979 and September 1988 at the Department of Thoracic Surgery of National University Hospital have been studied. There were 2 cases of the pharyngoesophageal diverticula, 12 cases of the midesophageal diverticula, and 2 cases of the epiphrenic diverticula. Twelve cases of midesophageal diverticula consisted of 9 cases of pulsion type and 3 cases of traction type. There were 13 women and three men, whose ages ranged from 25 to 65 years with an average age of 45.5 years. Diverticulectomy alone in three cases, diverticulopexy with myotomy in two cases, and diverticulectomy with myotomy in 11 cases were performed. There were no deaths or morbidity and all patients have achieved marked improvement of their symptoms except four patients who had a concomitant hypopharyngeal carcinoma, had a postoperative recurrence in epiphrenic diverticulum, and had two cases of postoperative transient regurgitation which subsided spontaneously one and one and half year later.

Round Acupuncture for the Treatment of Recurrent Carpal Tunnel Syndrome

  • Objectives: Round Acupuncture having blunt end has developed from acupotomy. This case report is to find out that Round Acupuncture is effective in treating patients with recurrent carpal tunnel syndrome (CTS), which has not improved by steroid injection or acupotomy. Methods: Round Acupuncture was inserted into the distal fibers of transverse carpal ligament and released toward the proximal fibers. Treatment was performed three times in total. T ingling, numbn ess, night pain and swelling sensation were assessed, and provocative maneuvers were also used. Results: After treat ment, all symptoms completely disappeared and the patient had no recurrence until 3 months after treatment. Conclusion: Round Acupuncture co uld be an effective treatment for recurrent CTS.

  • The diagnosis of isolated tuberculosis of the brachioradialis muscle without osseous involvement was made by excision of the lesion and histologic examination in a 72-year-old woman who was in a poor general state. The patient was free of other systemic symptoms or other foci of infection. The wound healed without complication with the aid of antituberculous chemotherapy. Two years later, the patient remains symptom-free with no signs of recurrence.

제 2단계 견봉하 충돌증후군에서 관절경적 감압술(5년에서 12년의 장기추시관찰 결과) (Arthroscopic Decompression in Stage II Subacromial Impingement - Five to Twelve Years Follow up -)

  • We investigated the effectiveness of arthroscopic decompression in stage Ⅱ subacromial impingement after long term follow up. Arthroscopic subacromial decompression was done in 104 consecutive patients who had stage Ⅱ sub­acromial impingement. After average of 8.4 years follow up, the final results were as following; 57 shoulders(55%) in excellent, 25(24%) in good, 16(15%) in fair and 6(6%) in poor. All parameters-pain, function, muscle strength and motion-were improved significantly(p<0.00l). Rotator cuff tear was developed in 10 shoulders after decompression. Among them, 8 shoulders had unfavorable results including two poor. In 6 failures, two had rotator cuff tear, three had recurrence of impingement with degenerative change and reflex sympathetic dystrophy was developed in one. Reoperations were done in 4 shoulders. Improvement of impingement symptoms was maintained in the most of patients(79%) after long term follow up. Arthroscopic decompression surgery was very effective means for stage Ⅱ impingement syndrome.

Arthroscopic Excision of Intra-articular Osteochondroma of the Elbow: A Case Report

  • Osteochondromas are one of the most common benign bone tumors usually involving extraarticular metaphysis of long bone. Solitary intra-articular osteochondroma arising from the elbow joint has rarely been reported. We present a case of 23-year-old female who had pain and limited motion of the left elbow as a result of intraarticular osteochondroma of the distal humerus. Arthroscopic excision of the osteochondroma yielded complete relief of symptoms. Absence of recurrence was confirmed radiographically at two years after surgery. To the best of our knowledge, this is the first report of osteochondroma of the elbow successfully treated arthroscopically.

  • Pneumothorax-either spontaneous or iatrogenic-is commonly encountered in pulmonary medicine. While secondary pneumothorax is caused by an underlying pulmonary disease, the spontaneous type occurs in healthy individuals without obvious cause. The British Thoracic Society (BTS, 2010) and the American College of Chest Physicians (ACCP, 2001) published the guidelines for pneumothorax management. This review compares the diagnostic and management recommendations between the two societies. Patients diagnosed with primary spontaneous pneumothorax (PSP) may be observed without intervention if the pneumothorax is small and there are no symptoms. Oxygen therapy is only discussed in the BTS guidelines. If intervention is needed, BTS recommends a simple aspiration in all spontaneous and some secondary pneumothorax cases, whereas ACCP suggests a chest tube insertion rather than a simple aspiration. BTS and ACCP both recommend surgery for patients with a recurrent pneumothorax and persistent air leak. For patients who decline surgery or are poor surgical candidates, pleurodesis is an alternative recommended by both BTS and ACCP guidelines. Treatment strategies of iatrogenic pneumothorax are very similar to PSP. However, recurrence is not a consideration in iatrogenic pneumothorax.

  • Dyshidrotic eczema is a chronic vesicular skin disease due to stress, sweating and nickel dermatitis. It has the characteristic symptoms of a cluster of vesicles and frequent recurrence. We diagnosed a 63-year old male patient case with hyperkeratosis, itching and vesicles at his right foot as dyshidrotic eczema and treated him with application of herbal medicine(bo-jung-ik-ki-tang) and acupunture at Dong-su Oriental-medicine hospital in 2003. We had a good result, so we report this case.

  • Pelvic inflammatory disease (PID) is a general term that refers to infection of the internal reproductive organs such as the female uterus, fallopian tubes, and ovaries. PID is caused by germs that invade the genital organs, causing the acute inflammatory reaction, and chronic PID is to have a recurrence of PID because of the lack of effective treatment of acute PID. Symptoms of PID include pelvic pain, fever, leukorrhea, infertility, and fatigue. Treatments for PID can be improved by combining herb medicine therapy, intestinal irrigation, or the therapy that put herb medicine on the abdominal region with antibiotics, or analgesic drugs.

Herniated Lumbar Disc Combined with Spinal Intradural Extramedullary Cysticercosis

  • Spinal cysticercosis is a very uncommon manifestation of neurocysticercosis which is caused by the larvae of Taenia solium. However, it can develop as a primary infection through blood stream or direct larval migration. It can result in high recurrence and severe neurologic morbidity if not treated appropriately. We report the case of a 43-year-old woman who presented with severe lower back pain and left leg radiating pain in recent 2 weeks. Magnetic resonance image (MRI) of lumbar spine demonstrated extruded disc at the L5-S1 level combined with intradural extramedullary cystic lesion. We performed the open lumbar microdiscectomy (OLM) at L5-S1 on the left with total excision of cystic mass. After surgery, the patient showed an improvement of previous symptoms. Diagnosis was confirmed by histopathological examination as intradural extramedullary cysticercosis. We discuss clinical features, diagnostic screening, and treatment options of spinal cysticercosis.
