Massive pulmonary hemorrhage in enterovirus 71-infected hand, foot, and mouth disease

  • Hand, foot, and mouth disease (HFMD) is an acute, mostly self-limiting infection. Patients usually recover without any sequelae. However, a few cases are life threatening, especially those caused by enterovirus 71 (EV71). A 12-month-old boy was admitted to a primary hospital with high fever and vesicular lesions of the mouth, hands, and feet. After 3 days, he experienced 3 seizure episodes and was referred to our hospital. On admission, he was conscious and his chest radiograph was normal. However, 6 hours later, he suddenly lost consciousness and had developed a massive pulmonary hemorrhage that continued until his death. He experienced several more intermittent seizures, and diffuse infiltration of both lung fields was observed on chest radiography. Intravenous immunoglobulin, dexamethasone, cefotaxime, leukocyte-depleted red blood cells, fresh frozen plasma, inotropics, vitamin K, and endotracheal epinephrine were administered. The patient died 9 hours after intubation, within 3 days from fever onset. EV71 subgenotype C4a was isolated retrospectively from serum and nasopharyngeal swab by real-time reverse transcription-polymerase chain reaction. Here, we report a fatal case of EV71-associated HFMD with sudden-onset massive pulmonary hemorrhage and suspected encephalitis.

The Opinions of Some Dental Hygienists about Improvement of Korean Dental Hygiene Education

  • This study's aim was to investigate their opinions to improve the educational system for dental hygienists on the basis of their duties as dental hygienists. The qualitative study was conducted among 19 dental hygienists at Daejeon, Korea from July to August 2016. Two researchers followed each subject for working hours in a day and recorded all the tasks and time to take for each task. After one day, the researchers met each subject and conducted face-to-face interviews to investigate the opinions about the dental hygiene curriculum and national board examination. The main duties recognized by more than half of the subjects were oral disease prevention including scaling and dental treatment assistance. The subjects' minority opinion about the main duties included radiography, impression taking, dental implant surgery assistance, orthodontic treatment, patient counseling, dental management and staff management, and oral health education. The most important tasks perceived by the subjects were prosthetic and implant impressions, scaling and implant surgery assistance. The subjects' minority opinion about the most important duties included patient counseling and making temporary crowns. The most difficult tasks answered by the subjects were prosthetic and implant impressions and dental implant surgery assistance. The subjects' minority opinion about it included patient counseling, scaling, and making temporary crowns. They mentioned that their curriculum in college was different from the actual work and the national board examination was not reflective of their real duties. We found out Korean dental hygienists had a lot of roles as dental assistants, dental business managers, and so on. We suggest that the law, curriculum and national board examination for dental hygienists should be revised to be able to reflect the reality of the clinical field.

  • 새 구조의 액정 엑스선 감지기를 만들었다. 이것은 액정판을 만들고 유리판을 얇게 식각한 다음, 그 유리판 위에 반사막과 광전도층을 연속하여 입힌 구조이다. 새 구조의 액정엑스선 감지기는 공정의 안정성, 대면적화, 감도 등에서 이미 상품화된 엑스선 감지기와도 충분히 경쟁할 수 있으며, 따라서 성공적으로 상용화 할 수 있음을 확인했다.

Ante-Mortem Diagnosis of an Atypical Mixed Form of Feline Infectious Peritonitis through Pericardial Effusion Analysis in a Cat

  • A 1-year-old castrated male Korean Shorthair cat presented with dyspnea, anorexia, lethargy, and seizures. Physical examination revealed salivation, right forelimb hemiparesis, and rapid breathing. No abnormalities were detected on auscultation. Laboratory findings revealed increased levels of bilirubin, aspartate aminotransferase (AST), globulin, glucose, and a decreased albumin-to-globulin (A:G) ratio. Both N-terminal pro-B-type natriuretic peptide (NT-proBNP) and feline serum amyloid A (fSAA) levels were significantly elevated. Thoracic radiography revealed mild cardiomegaly and diffuse increased interstitial infiltration with soft tissue opacity in the periphery of the right caudal pleural space. Echocardiography and lung ultrasonography were performed to investigate the cause of mild cardiomegaly and soft tissue opacity in the pleural space. Echocardiography revealed a mild amount of echogenic pericardial effusion, and lung ultrasonography showed an echogenic soft tissue mass with no blood signal in the right caudal pleural space, suggestive of a granulomatous lesion. After obtaining 5 mL of pericardial fluid through pericardiocentesis, cytology of the pericardial effusion sample revealed marked neutrophils and macrophages with no bacteria. IDEXX feline infectious peritonitis (FIP) virus real-time reverse transcriptase polymerase chain reaction (RT-PCR) confirmed the presence of the FIP virus biotype in the sample. This case presents a rarely reported atypical mixed form of FIP in a cat diagnosed ante-mortem using pericardial effusion analysis. In this case, ultrasound examination played a crucial role in the definitive diagnosis of FIP by PCR biotyping through pericardiocentesis. Ultrasonography can be highly beneficial in guiding the diagnosis and evaluation of cats with suspected FIP.

  • Breast cancer is the most dangerous and deadly form of cancer. Initial detection of breast cancer can significantly improve treatment effectiveness. The second most common cancer among Indian women in rural areas. Early detection of symptoms and signs is the most important technique to effectively treat breast cancer, as it enhances the odds of receiving an earlier, more specialist care. As a result, it has the possible to significantly improve survival odds by delaying or entirely eliminating cancer. Mammography is a high-resolution radiography technique that is an important factor in avoiding and diagnosing cancer at an early stage. Automatic segmentation of the breast part using Mammography pictures can help reduce the area available for cancer search while also saving time and effort compared to manual segmentation. Autoencoder-like convolutional and deconvolutional neural networks (CN-DCNN) were utilised in previous studies to automatically segment the breast area in Mammography pictures. We present Automatic SegmenAN, a unique end-to-end adversarial neural network for the job of medical image segmentation, in this paper. Because image segmentation necessitates extensive, pixel-level labelling, a standard GAN's discriminator's single scalar real/fake output may be inefficient in providing steady and appropriate gradient feedback to the networks. Instead of utilising a fully convolutional neural network as the segmentor, we suggested a new adversarial critic network with a multi-scale L1 loss function to force the critic and segmentor to learn both global and local attributes that collect long- and short-range spatial relations among pixels. We demonstrate that an Automatic SegmenAN perspective is more up to date and reliable for segmentation tasks than the state-of-the-art U-net segmentation technique.

Implementation of a Deep Learning-Based Computer-Aided Detection System for the Interpretation of Chest Radiographs in Patients Suspected for COVID-19

  • Objective: To describe the experience of implementing a deep learning-based computer-aided detection (CAD) system for the interpretation of chest X-ray radiographs (CXR) of suspected coronavirus disease (COVID-19) patients and investigate the diagnostic performance of CXR interpretation with CAD assistance. Materials and Methods: In this single-center retrospective study, initial CXR of patients with suspected or confirmed COVID-19 were investigated. A commercialized deep learning-based CAD system that can identify various abnormalities on CXR was implemented for the interpretation of CXR in daily practice. The diagnostic performance of radiologists with CAD assistance were evaluated based on two different reference standards: 1) real-time reverse transcriptase-polymerase chain reaction (rRT-PCR) results for COVID-19 and 2) pulmonary abnormality suggesting pneumonia on chest CT. The turnaround times (TATs) of radiology reports for CXR and rRT-PCR results were also evaluated. Results: Among 332 patients (male:female, 173:159; mean age, 57 years) with available rRT-PCR results, 16 patients (4.8%) were diagnosed with COVID-19. Using CXR, radiologists with CAD assistance identified rRT-PCR positive COVID-19 patients with sensitivity and specificity of 68.8% and 66.7%, respectively. Among 119 patients (male:female, 75:44; mean age, 69 years) with available chest CTs, radiologists assisted by CAD reported pneumonia on CXR with a sensitivity of 81.5% and a specificity of 72.3%. The TATs of CXR reports were significantly shorter than those of rRT-PCR results (median 51 vs. 507 minutes; p < 0.001). Conclusion: Radiologists with CAD assistance could identify patients with rRT-PCR-positive COVID-19 or pneumonia on CXR with a reasonably acceptable performance. In patients suspected with COVID-19, CXR had much faster TATs than rRT-PCRs.

  • 호흡운동 조절 방사선치료 시 환자체표면 움직임을 추적하여 실시간 보정하고자 한다. 본 연구에서 사용한 시스템은 치료테이블에 기반을 둔 동 팬텀(CBMP, couch based computer-controlled motion phantom), 초음파 센서 및 제어, 구동, 분석 프로그램 등으로 구성하였다 동물실험 결과 호흡주기는 2.9초이었고, 호흡진폭은 6mm이었다. 실시간 체표면 추적시스템의 유용성 평가에 중요한 항목인 호흡운동 획득-보정간의 지연시간은 $2.34{\times}10^{-4}sec$ 초이어서 호흡운동 조절 방사선치료 시 사용할 수 있는 새로운 실시간 체표면 추적 기술의 임상적용에의 가능성을 확인할 수 있었다.

소켓 용접부 결함 검사용 초음파 자동 검사 장비 개발 (Development of the Automated Ultrasonic Testing System for Inspection of the flaw in the Socket Weldment)

  • 대구경 배관에서 소구경 배관으로 연결하는 부위 및 배관의 방향 전환을 위해 사용하는 소켓용접부는 용입불량 및 사용시간이 경과되면서 내부로부터 진전될 수 있는 균열 등의 결함을 가질 수 있다. 그러나 지금까지 적용하고 있는 비파괴검사법인 액체침투탐상과 방사선투과검사로는 내부에 존재하는 균열성 결함의 검출이 어렵다. 본 연구에서는 소켓용접부 내부의 균열성 결함 검출을 위한 초음파 검사 기법을 확립하였고, 검사를 수행할 수 있는 초음파 탐촉자를 설계 제작하였으며, 자동으로 검사할 수 있는 검사 장비와 제어용 운영 프로그램을 개발하였다. 개발된 장비는 컴퓨터를 기반으로 하고 있으며, pulser/receiver를 내장하고 100 MHz 고속 A/D board를 사용하여 초음파 탐상기 역할을 프로그램으로 구현하였으며, ISA interface type으로 4축 제어용 motion controller를 개발하여 자동 검사를 수행하는 scanner를 제어하도록 하였으며, 검사 결과는 소켓용접부 단면을 실시간 영상으로 나타내도록 되어 있다. 인공결함 시험편의 결함을 평가한 결과 결함의 깊이가 1mm인 결함의 평가 길이는 실제 크기보다 작게 나타내고 있으며, 결함의 깊이가 증가할수록 결함의 길이가 더 크게 평가되었다. 본 연구로 개발한 장비는 원자력 발전소나 화학플랜트에 많이 널려 있는 소켓용접부 내의 용입불량, 피로 균열 등을 검출하여 객관적인 검사 결과를 제시할 수 있으므로 설비 안전 관리 및 보수 부위 결정에 기여할 수 있을 것으로 기대된다.

방사선치료 시 환자자세 검증을 위한 분석용 도구 개발 (Patients Setup Verification Tool for RT (PSVTS) : DRR, Simulation, Portal and Digital images)

  • 목적 : 네트워크를 이용해 영상수집을 하여 방사선치료를 수행하는데 요구되는 기계중심점과 종양중심점의 일치여부 및 환자자세의 재현성을 확인하는 기능 등을 수행할 수 있는 환자자세검증도구를 개발하고자 한다. 대상 및 방법 : 전산화단층촬영 영상으로부터 재구성한 디지털화재구성사진(digitally reconstructed radiography, DRR) 영상과 투과 영상인 모의치료영상, 상용전자포탈 영상장치에서 수집한 포탈영상 밑 CCD 카메라로부터 획득한 디지털영상 등을 네트워크를 통해 수집하였고, 이를 조작할 수 있는 분석용 프로그램을 프로그램 제작툴인 IDL(IDL 5.4, Research systems, USA)을 이용해 작성하였다. 이를 환자자세 검증도구(patients setup verification tool for RT, PSVTS)라 명명하였다. 기계중심점과 종양중심점의 일치여부 확인은 전산화단층촬영영상으로부터 재구성한 DRR 영상과 모의치료영상을 이용하여 위치변위를 분석함으로써 치료조준오차를 구하였다. 환자자세의 재현성 확인은 첫 치료 시에 얻은 포탈 영상과 매 치료 시마다 얻는 포탈영상을 비교, 분석하는 방법과 디지털영상을 이용하는 방법을 사용하였다. 결과 : 방사선치료 시 요구되는 기계 중심점과 종양중심점의 일치여부 및 환자자세의 재현성을 확인할 수 있는 사용자편의방식의 소프트웨어와 환자자세검증도구를 개발하였다. 종양중심점과 기계중심점의 일치여부 확인 결과 치료조준오차는 두경부에서 전후, 좌우 각각 0.8$\pm$0.2 mm, 1.0$\pm$0.3 mm이었고, 골반부에서 1.1$\pm$0.5 mm, 1.0$\pm$0.6mm이었다. 환자자세 재현성은 영상을 실시간으로 획득하여 환자 자세 위치 변화를 시각적으로 평가할 수 있어실용성 있음을 알 수 있었다. 결론 : 환자자세의 재현성 및 종양중심점과 기계중심점의 일치여부를 파악할 수 있는 환자자세검증도구를 개발하였고, DRR 영상의 질 증진, 사용자편의방식의 소프트웨어를 보완한다면 임상응용에 가능할 것으로 사료된다.

  • Engineers have developed new instruments that aid in diagnosis and therapy Ultrasonic imaging has provided a nondamaging method of imaging internal organs. A complex transducer emits ultrasonic waves at many angles and reconstructs a map of internal anatomy and also velocities of blood in vessels. Fast computed tomography permits reconstruction of the 3-dimensional anatomy and perfusion of the heart at 20-Hz rates. Positron emission tomography uses certain isotopes that produce positrons that react with electrons to simultaneously emit two gamma rays in opposite directions. It locates the region of origin by using a ring of discrete scintillation detectors, each in electronic coincidence with an opposing detector. In magnetic resonance imaging, the patient is placed in a very strong magnetic field. The precessing of the hydrogen atoms is perturbed by an interrogating field to yield two-dimensional images of soft tissue having exceptional clarity. As an alternative to radiology image processing, film archiving, and retrieval, picture archiving and communication systems (PACS) are being implemented. Images from computed radiography, magnetic resonance imaging (MRI), nuclear medicine, and ultrasound are digitized, transmitted, and stored in computers for retrieval at distributed work stations. In electrical impedance tomography, electrodes are placed around the thorax. 50-kHz current is injected between two electrodes and voltages are measured on all other electrodes. A computer processes the data to yield an image of the resistivity of a 2-dimensional slice of the thorax. During fetal monitoring, a corkscrew electrode is screwed into the fetal scalp to measure the fetal electrocardiogram. Correlations with uterine contractions yield information on the status of the fetus during delivery To measure cardiac output by thermodilution, cold saline is injected into the right atrium. A thermistor in the right pulmonary artery yields temperature measurements, from which we can calculate cardiac output. In impedance cardiography, we measure the changes in electrical impedance as the heart ejects blood into the arteries. Motion artifacts are large, so signal averaging is useful during monitoring. An intraarterial blood gas monitoring system permits monitoring in real time. Light is sent down optical fibers inserted into the radial artery, where it is absorbed by dyes, which reemit the light at a different wavelength. The emitted light travels up optical fibers where an external instrument determines O2, CO2, and pH. Therapeutic devices include the electrosurgical unit. A high-frequency electric arc is drawn between the knife and the tissue. The arc cuts and the heat coagulates, thus preventing blood loss. Hyperthermia has demonstrated antitumor effects in patients in whom all conventional modes of therapy have failed. Methods of raising tumor temperature include focused ultrasound, radio-frequency power through needles, or microwaves. When the heart stops pumping, we use the defibrillator to restore normal pumping. A brief, high-current pulse through the heart synchronizes all cardiac fibers to restore normal rhythm. When the cardiac rhythm is too slow, we implant the cardiac pacemaker. An electrode within the heart stimulates the cardiac muscle to contract at the normal rate. When the cardiac valves are narrowed or leak, we implant an artificial valve. Silicone rubber and Teflon are used for biocompatibility. Artificial hearts powered by pneumatic hoses have been implanted in humans. However, the quality of life gradually degrades, and death ensues. When kidney stones develop, lithotripsy is used. A spark creates a pressure wave, which is focused on the stone and fragments it. The pieces pass out normally. When kidneys fail, the blood is cleansed during hemodialysis. Urea passes through a porous membrane to a dialysate bath to lower its concentration in the blood. The blind are able to read by scanning the Optacon with their fingertips. A camera scans letters and converts them to an array of vibrating pins. The deaf are able to hear using a cochlear implant. A microphone detects sound and divides it into frequency bands. 22 electrodes within the cochlea stimulate the acoustic the acoustic nerve to provide sound patterns. For those who have lost muscle function in the limbs, researchers are implanting electrodes to stimulate the muscle. Sensors in the legs and arms feed back signals to a computer that coordinates the stimulators to provide limb motion. For those with high spinal cord injury, a puff and sip switch can control a computer and permit the disabled person operate the computer and communicate with the outside world.
